Sex disparities in STEMI and stroke outcomes persist despite universal health care: a nationwide study in Spain (2007-2022).

INTRODUCTION AND OBJECTIVES: Sex disparities in cardiovascular disease (CVD) outcomes persist despite advances in prevention and care. We assessed sex-specific trends in CVD hospitalizations, in-hospital mortality and length of stay in Spain, a high-income country with universal health care, from 2007 to 2022. METHODS: We conducted a nationwide, retrospective, population-based study including all adults (≥ 18 years) hospitalized with a primary CVD diagnosis. Analyses focused on ST-elevation myocardial infarction (STEMI), non-ST-elevation myocardial infarction, stroke, heart failure (HF), and

Return on Investment of HIV Care Within Universal Health Coverage in Low- and Middle-Income Countries: Evidence From Morocco.

HIV is a serious disease requiring long-term care and incurring high economic costs, particularly in low- and middle-income countries (LMICs). In Morocco, since 1998, the Ministry of Health has provided free care for people living with HIV, with financial support from the Global Fund. The study assessed the return on investment of providing HIV care as part of universal health coverage (UHC) financed by national health insurance in order to reduce dependence on international aid. We measured the benefit-cost ratio of the direct benefits and expected economic costs of HIV care under UHC between

OBJECTIVES: Considering knowledge gaps regarding the legal aspects of PC in Uganda, this study sought to assess the legal needs and challenges faced by persons receiving PC in the country. DESIGN: Cross-sectional design utilizing both quantitative and qualitative meth

Regional Virtual Acute Care Helpline in Singapore at a National University Health System Virtual Care Centre: Retrospective Study.

BACKGROUND: Emergency department (ED) overcrowding and delayed access to care are ongoing challenges in Singapore. The COVID-19 pandemic further underscored the need for scalable virtual care models that go beyond traditional hospital settings, allowing patients to access acute specialist care quickly and efficiently. OBJECTIVE: This study describes the design, implementation, and early outcomes of the National University Health System (NUHS) Virtual Care Centre (VCC), a clinician-led helpline aimed at reducing unnecessary ED visits and supporting community-based acute care. METHODS: In 2020

Delay in health-care-seeking treatment among tuberculosis patients in Japan: what are the implications for control in the era of universal health coverage?

OBJECTIVE: To study the trends in and risk factors for patient delay (the time from the onset of symptoms to the initial doctor visit) in pulmonary tuberculosis (PTB) using three temporal categories - short (2 weeks to < 2 months), medium (2 months to < 6 months) and long (≥ 6 months) - and discuss implications for social protection measures. METHODS: A descriptive cross-sectional study was conducted by analysing Japanese TB surveillance data from patients with symptomatic PTB registered between 2007 and 2017 (n = 88 351). RESULTS: While the proportion of patients with short delay has decrea

COVID-19 in health care workers in a university hospital during the quarantine in São Paulo city.

Health care workers (HCW) are at a higher risk of being infected in their workplace. Out of a total of 466 HCW of Hospital São Paulo with influenza-like illnesses or any clinical suspicion of COVID-19 were tested for COVID-19 by RT-PCR for SARS-CoV-2 169 (36%) turned out positive and were analyzed by type of exposure and hospital occupation. Data of HCW household locations were also obtained. Logistic workers had the highest positivity rate for SARS-CoV-2 (p = 0.002), while nurse technicians had the highest rate among those reporting routine contacts with patients (p = 0.001). Physicians prese

Pharyngeal carriage rates of Neisseria meningitidis in health care professionals at a tertiary university pediatric hospital.

Pharyngeal carriage is the reservoir for Neisseria meningitidis in the population and the first step in disease transmission. Especially in young infants and adolescents, N. meningitidis can cause serious invasive infection with high fatality rates and high rates of long-term sequelae among survivors. The aim of this study was to determine N. meningitidis colonization rates in asymptomatic health care professionals at a tertiary university pediatric hospital and to identify risk factors for carriage. This cross-sectional meningococcal carriage survey was conducted between April and October 201

Universality in daily primary care practice: a qualitative study of mental health management in Italy and Spain.

AIM: This article examines how general practitioners (GPs) in Italy and Spain manage patients with mental health needs in daily primary care practice, and how their discretionary choices shape the substantive reach of universal access to care. BACKGROUND: Modern healthcare systems in Italy and Spain formally guarantee universal access to primary care. However, mental health services in primary care reveals persistent gaps between this normative principle and everyday practice. Drawing on Lipsky's Street-Level Bureaucracy (SLB) theory, GPs are understood as frontline professionals whose discre

Tuberculin reactivity among health care workers at King Abdulaziz University Hospital, Saudi Arabia.

Tuberculosis is both a nosocomial and an occupational infection. The prevalence of positive tuberculin skin reactions at King Abdulaziz University Hospital, Saudi Arabia, was investigated by testing 298 health care workers. Conventional tuberculin skin testing was performed using 0.1 mL of purified protein derivative injected intracutaneously. After 48-72 hours, induration size was recorded. The prevalence of positive tests (induration > or = 10 mm) was 78.9% overall, 60.0% for Saudi Arabians compared with 81.8% for non-Saudi Arabians (P < 0.01). The mean response size (8.9 +/- 7 mm) for Saudi

Socioeconomic disparities in long-term outcomes after pediatric liver transplantation in a universal health care system: a population-based study.

We conducted a population-based cohort study of pediatric liver transplantation (LT) in Ontario, Canada, a universal health care setting, to quantify survivorship burden relative to matched general-population controls and assess whether socioeconomic status modified these differences. First, isolated pediatric LTs (1991-2021) were linked to provincial administrative data and matched 1:5 on sex and birth year to general-population controls. Outcomes were mortality, chronic conditions, de novo cancers, and live births, with prespecified socioeconomic status interaction analyses. Among 449 LT rec

The emergence of a global right to health norm--the unresolved case of universal access to quality emergency obstetric care.

BACKGROUND: The global response to HIV suggests the potential of an emergent global right to health norm, embracing shared global responsibility for health, to assist policy communities in framing the obligations of the domestic state and the international community. Our research explores the extent to which this global right to health norm has influenced the global policy process around maternal health rights, with a focus on universal access to emergency obstetric care. METHODS: In examining the extent to which arguments stemming from a global right to health norm have been successful in ad
