BACKGROUND: One way to achieve universal health coverage (UHC) in low- and middle-income countries (LMIC) is the implementation of health insurance schemes. A robust and up to date overview of empirical evidence assessing and substantiating health equity impact of health insurance schemes among specific vulnerable populations in LMICs beyond the more common parameters, such as income level, is lacking. We fill this gap by conducting a systematic review of how social inclusion affects access to equitable health financing arrangements in LMIC.
METHODS: We searched 11 databases to identify peer-

BACKGROUND: Universal Health Coverage has been openly recognized in the United Nations health-related Sustainable Development Goals by 2030, though missing under the Millennium Development Goals. Ghana implemented the National Health Insurance Scheme programme in 2004 to improve financial access to healthcare for its citizens. This programme targeting low-income individuals and households includes an Exempt policy for older persons and indigents. Despite population ageing, evidence of the participation and perceptions of older persons in the scheme in cash grant communities is unknown. Hence,

BACKGROUND: In efforts to advance universal healthcare coverage (UHC) in Nigeria, the federal government empowered sub-national entities such as states to develop and implement their respective state-supported health insurance schemes (SSHIS). This study assessed the coverage and predictors of enrollment in the SSHIS in six Nigerian states.
METHODS: This study used a quantitative cross-sectional design. Respondents were recruited across six Nigerian states with operational SSHIS, corresponding to the country's six geopolitical zones. These include the Cross River, Enugu, Oyo, Kwara, Sokoto, a

Mobile health units (MHUs) are an important source of health care for the uninsured; however, it is unclear what role these units play in Canada, where a universal health insurance system exists. The purpose of this study was to understand why individuals who live in a country with universal health insurance seek care at an MHU and to determine whether MHUs are used in addition to or in place of the client's usual source of care. This study investigated the use of the Rotary Club of Toronto Health Bus among 150 homeless and marginally housed adults in Toronto, Ontario, over a 3-month period. D

Based on the described prevalence and size of the population, approximately 300 to 500 patients with epidermolysis bullosa (EB) reside in Canada. There are specific challenges faced by patients and families as well as the practitioners looking after patients with EB.

The world is off track six years to the 2030 deadline for attaining the sustainable development goals and universal health coverage. This is particularly evident in Africa's armed conflict-affected and humanitarian settings, where pervasively weak health systems, extreme poverty and inequitable access to the social dimensions and other determinants of health continue to pose significant challenges to universal health coverage. In this article, we review the key issues and main barriers to universal health coverage in such settings. While our review shows that the current health service deliver

BACKGROUND: A service coverage index has recently been proposed to monitor progress towards universal health coverage (UHC), and baseline results for 2015 are available. However, evidence on equity in that progress is scarce. The service coverage index did not consider services for neglected tropical diseases (NTDs), a group of preventable diseases defined by WHO member states on the basis of the disproportionate burden they place on their poorest, remotest, and otherwise most marginalised communities. Because of the much-needed equity lens that it could provide, NTD service coverage should no

BACKGROUND: Universal health coverage (UHC) is a concept that includes-protection from financial risks for all; access to quality primary health services; and, access to essential medicines and vaccines that are safe, effective, high quality, and inexpensive. Public health expenditure (PHE) is expected to improve healthcare delivery in terms of access, quality and financial risk protection. To this end, this study aims to explore the relationship between progress in UHC and PHE across various countries.
METHODS: We analyzed the trends in UHC and PHE between 2000 and 2021 in 187 countries, usi

The history of Korean health security is usually told as a compressed achievement, from workplace insurance in 1977 to universal coverage in 1989 and unified National Health Insurance in 2000. Departing from that framework, this article places the Rural Health Insurance of 1988, which extended coverage to farming and fishing villages, within the longer history of rural medical marginalization since liberation, in which the equitable distribution of medical care remained an unmet task across regimes. It asks how farmers' health briefly emerged as a social category and was then reduced to a disp

UN member states have committed to universal health coverage (UHC) to ensure all individuals and communities receive the health services they need without suffering financial hardship. Although the pursuit of UHC should unify disparate global health challenges, it is too commonly seen as another standalone initiative with a singular focus on the health sector. Despite constituting the cornerstone of the health-related Sustainable Development Goals, UHC-related commitments, actions, and metrics do not engage with the major drivers and determinants of health, such as poverty, gender inequality,

INTRODUCTION: Universal Health Coverage (UHC) has been a global concern for a long time and even more nowadays. While a number of publications are almost unanimous that Rwanda is not far from UHC, very few have focused on its financial sustainability and on its extreme external financial dependency. The objectives of this study are: (i) To assess Rwanda UHC based mainly on Community-Based Health Insurance (CBHI) from 2000 to 2012; (ii) to inform policy makers about observed gaps for a better way forward.
METHODS: A retrospective (2000-2012) SWOT analysis was applied to six metrics as key indi

BACKGROUND: The international community's health focus is shifting from achieving disease-specific targets towards aiming for universal health coverage. Integrating the global HIV/AIDS response into universal health coverage may be inevitable to secure its achievements in the long run, and for expanding these achievements beyond addressing a single disease. However, this integration comes at a time when international financial support for the global HIV/AIDS response is declining, while political support for universal health coverage is not translated into financial support. To assess the risk

Universal health coverage requires simultaneous progress in service coverage and financial protection, yet these dimensions are commonly assessed separately. We conducted a secondary ecological panel analysis using official World Health Organization/World Bank indicators. A document-oriented workflow preserved irregular subgroup structures before conversion to an unbalanced country-year panel comprising 981 observations from 159 countries/economies between 2000 and 2023. The primary inferential specification was a two-way fixed-effects model with country and year effects and country-clustered

Financial protection is a key goal for universal health coverage, but globally, this indicator has been lagging, especially for low-income households. Understanding how to improve it is urgent. This rapid evidence review examined the effectiveness of interventions, which affected financial protection in health across low- and middle-income countries (LMICs). A systematic search of published literature identified empirical studies reporting on outcomes such as out-of-pocket expenditure (OOPE), catastrophic spending, impoverishment, and care foregone for financial reasons, with data extracted an

This review summarises empirical studies on the progressivity and redistributive effects of healthcare financing mechanisms. The evidence varies significantly across countries and financing sources. Tax-based systems exhibit high progressivity, as direct taxes contribute to a favourable redistribution toward low-income households, often offsetting the regressive nature of indirect taxes. Social insurance systems are found to be progressive but may be regressive in practice due to contribution ceilings and exemptions for high-income earners. This creates disparities where high-income taxpayers

Providing health services to sparsely populated, geographically isolated areas was a major challenge in many Canadian provinces before the advent of universal hospital insurance in the late 1950s, and universal physician insurance in the late 1960s. The task was especially daunting during the interwar depression years. Nonetheless, from 1924 to 1942, Alberta's Department of Public Health sponsored an itinerant Travelling Clinic during the summer which conducted physical examinations, dental examinations and treatment, tonsillectomies and other minor surgery, and vaccination against smallpox to

The Mexican health system is historically segmented, being based on social insurance for formal workers and public health actions for the general population. This study characterizes health reforms in Mexico from 2000 to 2024 considering the trajectory of health policies, coverage, and use of the health system. This research involved a literature review, documentary analysis, secondary data and interviews with key informants. We observed moments of reform under the influence of various groups and government political orientations. From 2003 to 2018, the implementation of a Popular Health Insur

BACKGROUND: In 2010, the islands of Bonaire, Sint Eustatius and Saba became part of the Netherlands, one of the four constituent countries within the Kingdom of the Netherlands. Public administration, including the health system, was reformed, with the Dutch Government aiming for a "level of facilities acceptable within the Netherlands, taking into account the specific circumstances on the islands".
REFORM CONTENT: The Dutch Health Ministry became responsible for legislating, financing and policy in a health system with limited stakeholders. The health policy agenda focused on building a publ

BACKGROUND: Demographic transitions, societal changes, and evolving population health needs are placing increasing pressure on healthcare systems, necessitating ongoing reforms. Primary health care (PHC) is a foundational component of Universal Health Coverage (UHC) and sustainable health systems. Many countries have undertaken PHC reforms aimed at improving population health. This review explores the objectives, implementation mechanisms, challenges, and outcomes of these reforms.

Although many sub-Saharan African countries have made efforts to provide universal health coverage (UHC) for their citizens, several of these initiatives have achieved little success. This study aims to review the challenges facing UHC in Ghana, Kenya, Nigeria, and Tanzania, and to suggest program or policy changes that might bolster UHC. Routine data reported by the World Bank and World Health Organization, as well as annual reports of the national health insurance schemes of Ghana, Kenya, Nigeria, and Tanzania, were analyzed. The data were supplemented by a review of published and gray liter
