Rectoprostatic fistula is a rare complication after transurethral resection of the prostate or prostatectomy for benign and malignant neoplasms of the prostate. Repair of these fistulas is difficult, especially when previous treatment includes radiation therapy to the prostate. Various operative approaches have been described to close these inaccessible fistulas. Because of their location near the outlet of the pelvis, access to or exposure of these fistulas is quite limited. These fistulas can be easily exposed and repaired through the posterior wall of the rectum (transsphincteric approach).

A 56-year-old Japanese man consulted a urologist because of urethral bleeding. He had been undergoing hemodialysis for the past 15 years due to polycystic kidney disease. Computed tomography revealed an irregular cyst wall in the left kidney. Cystitis cystica (CC) is aproliferative disorder of bladder urothelium and usually subsides with medical therapy. However, this is not true for severe CC where surgical intervention is required to control breakthrough urinary tract infection (UTI). It may be mistaken as bladder neoplasm or posterior urethral valve, especially in children. Here, we report a case of CC in a 2-year-old boy where we had to excise the large pedunculated intravesical lesion to control breakthrough UTI and ongoing renal damage.

Uroradiological examinations on 33 patients subjected to abdomino-perineal resection were analyzed. The most common finding was medial displacement of both ureters, with the right one frequently crossing the midline. Bladder deformity, sometimes severe, was present in all patients. Localized ureteral narrowing due to stricture or recurring neoplasm was found in 30% of the patients examined. Other common complications included retrovesical abscesses, periurethral abscesses, urethral fistulas and strictures, and bladder retention. All these findings were more prominent in men.

Primary urethral adenocarcinomas are very rare neoplasms accounting for <10% of all urethral carcinomas. Site of their origin is unclear, but they seem to arise from Skene's paraurethral glands, which is the female homologue of the male prostate. When certain strictures which have been treated for a long time start to bleed, the possibility of carcinoma should be considered. However, cancer may present in the form of more recent and deteriorating dysuria. The diagnosis is based on urethrography which shows a narrow and tortuous pass. Biopsy should be performed under urethroscopic guidance. The cancer is usually in the bulbomembranous segment. Treatment is disappointing. Wide excision is very badly disabling.

Nineteen clear cell adenocarcinomas of the urethra, 18 from women and one from a man, are described. The patients ranged from 35 to 80 years of age (average, 58 yrs). The clinical presentation and gross findings were similar to those of urethral carcinomas, except that 12 tumors, all from women, arose within a urethral diverticulum. Paraurethral leiomyoma is a rare, benign, hormone-dependent neoplasm of mesenchymal origin affecting women. The clinical evidence varies, but it tends to be asymptomatic or associated with the sensation of a foreign body; urinary symptoms are rarely described. A 64-year-old man underwent initial F-FDG PET/CT staging for gastric cardia adenocarcinoma, which showed the primary gastric tumor, several bone metastases, and high uptake in a periurethral mass mimicking pelvic neoplasm developed after injection of polydimethylsiloxane (Macroplastique) urethral bulking agent 6 years ago for postprostatectomy stress urinary incontinence. Retroperitoneal fibrosis is an inflammatory disease, which is either idiopathic or secondary to infection, neoplasm, hemorrhage, aortic aneurysm or drugs. Primary cancer of urethra (PCU) is one of rarest malignancies of the urinary tract. In early stages this type of cancer presents non specific symptoms which can be mistaken with more common urethral strictures. That is why the PCU is frequently recognize in a locally advanced stage. The basic tool used in the diagnosis is MRI, but ultrasonography can be also used at the beginning of diagnosis. We present the case of 66-year old patient with PCU, initially diagnosed due to urethral stricture. We report probably the first case of well documented sonourethrography findings in PCU.
