Long-term results of the artificial urinary sphincter in male patients with neurogenic bladder.

The records of patients who received an artificial urinary sphincter at the University of Minnesota were reviewed. A total of 19 patients fulfilled the following criteria: male gender, neurogenic sphincteric incompetence, no previous surgery on the bladder neck, myelodysplasia and more than 5 years of followup. Mean age at the time of implantation was 8 years, mean age at the last followup was 16.5 years and mean followup was 8 years. At the last followup 16 patients (84.2%) were continent and 3 were expected to regain continence after revision or bladder augmentation. At the last followup 78.

Save the Bladder: Continent Urinary Diversion as Best Practice for Patients With Refractory Neurogenic Lower Urinary Tract Dysfunction.

INTRODUCTION: Refractory neurogenic lower urinary tract dysfunction presents an obstinate clinical problem. For patients who fail nonsurgical measures, we advocate for bladder-sparing, continent urinary diversion as the optimal approach to management. METHODS: The arguments presented herein were informed by a review of the contemporary literature and our longitudinal institutional experience caring for patients with neurogenic lower urinary tract dysfunction. RESULTS: Bladder-sparing, continent urinary diversion comprises procedures such as bladder augmentation, creation of a continent cathe

Long-term follow-up of bladder outlet procedures in children with neurogenic urinary incontinence.

INTRODUCTION: Achieving continence in children with neurogenic sphincteric incompetence is a challenge. Awareness of the long-term outcome in this young patient population is important. In the past 25 years, the study institution has built experience in bladder outlet procedures such as bladder neck sling and bladder neck reconstructions. OBJECTIVE: The objective of this study was to evaluate the long-term outcome on continence and re-intervention rate of bladder outlet procedures in children with neurogenic urinary incontinence at the study institution. DESIGN: All children who underwent a

Parkinsonian GM2 synthase knockout mice lacking mature gangliosides develop urinary dysfunction and neurogenic bladder.

Parkinson's disease is a neurodegenerative disorder that reduces a patients' quality of life by the relentless progression of motor and non-motor symptoms. Among the non-motor symptoms is a condition called neurogenic bladder that is associated with detrusor muscle underactivity or overactivity occurring from neurologic damage. In Parkinson's disease, Lewy-body-like protein aggregation inside neurons typically contributes to pathology. This is associated with dopaminergic neuron loss in substantia nigra pars compacta (SNc) and in ventral tegmental area (VTA), both of which play a role in mictu

Low-frequency bladder vibration for the treatment of urinary tract infections in spinal cord injury patients with neurogenic bladder.

PURPOSE: Evaluate if low-frequency bladder vibration (LFBV) combined with clean intermittent catheterization (CIC) alleviated urinary tract infections (UTIs) in spinal cord injury (SCI) patients with neurogenic bladder (NB). METHODS: A single-blind randomized controlled trial was conducted. Patients who developed UTIs while rehabilitating from SCI were enrolled continuously between January 2021 and March 2023 and randomly assigned to the control or the intervention group. Patients in the intervention group received LFBV twice daily (5-10 Hz, 5-10 min each time) for 10 days. The primary endpoi

A Framework for Addressing Health Disparities in Adult Neurogenic Lower Urinary Tract Dysfunction-Systematic Review and Neurogenic Bladder Research Group Recommendations.

The systematic review and workshop recommendations by the Neurogenic Bladder Research Group offer a comprehensive framework for evaluating health disparities in adult neurogenic lower urinary tract dysfunction (NLUTD). The study acknowledges the multifaceted nature of health, highlighting that medical care, though critical, is not the sole determinant of health outcomes. Social determinants of health significantly influence the disparities seen in NLUTD. This report calls for a shift in focus from traditional urologic care to a broader, more inclusive perspective that accounts for the complex

Analysis of Urinary Incontinence in the Neurogenic Bladder and Its Relationship with the Satisfaction and Lifestyle of People with SCI.

One of the most common complications of neurogenic bladder secondary to spinal cord injury (SCI) is urinary incontinence, which is possibly related to bladder-emptying methods and changes in quality of life. This study aimed to identify the occurrence of this complication in adults with SCI and analyze its relationship with bladder-emptying methods, satisfaction, and lifestyle. This is a quantitative, exploratory, and cross-sectional study. The variables were collected using the Bowel and Bladder Treatment Index during a telephone interview with 290 participants from February to November 2021.

Enhanced susceptibility to urinary tract infection in the spinal cord-injured host with neurogenic bladder.

Neurogenic bladder predisposes to recurrent urinary tract infections (UTI) and renal failure, and susceptibility is commonly ascribed to urinary stasis from elevated residual urine volumes. Escherichia coli UTI was modeled in the spinal cord-injured (SCI) rat with the hypothesis that SCI animals would require fewer bacteria to establish infection, have an exaggerated inflammatory response, and have delayed clearance of infection compared to normal-voiding controls. T10 SCI rats and controls had median infectious doses (ID50) of 10(2) and 10(5) CFU, respectively. Mean residual volumes in the SC

Reconstruction of the lower urinary tract for neurogenic bladder: lessons from the adolescent age group.

Of 42 adolescents with neurogenic bladder who presented for lower urinary tract reconstruction, 4 patients with severe spina bifida were unfit for major surgery. Five patients had a simple undiversion; 4 of these had a poor outcome because of unpredicted sphincter incontinence (1 patient) or instability (3). A total of 26 patients had an enterocystoplasty. All 7 patients who had a substitution cystoplasty had a good result, as did 3 males with a clam cystoplasty. Nine of 16 females had a poor result from a clam procedure. In 3 the problem lay with self-catheterisation difficulties. One each ha

The effect of an antibacterial drug on urinary calcium, magnesium and phosphorus excretion in patients with neurogenic bladder.

The effect of the simultaneous administration of hexamine mandelate and methionine on the urinary excretion of calcium, magnesium and phosphate is reported in 3 groups of paraplegic patients. Long-term therapy with G 500 increases bone loss in paralysed patients, and is contra-indicated when there is infection of the urinary tract by urea-splitting organisms.

First reported urinary tract infection caused by Siccibacter turicensis in an adolescent with neurogenic bladder.

Siccibacter turicensis is a rare environmental member of the Enterobacteriaceae family that has seldom been implicated in human infection. We report what is, to our knowledge, the first case of S. turicensis-associated urinary tract infection (UTI). A 17-year-old male with traumatic spinal cord injury and catheter-dependent neurogenic bladder presented with fever and dysuria three months after a motorcycle accident. Urine culture grew a Gram-negative bacillus identified as S. turicensis by MALDI-TOF mass spectrometry and confirmed by 16 S rRNA Sanger sequencing. The isolate was susceptible to

Comparison of artificial sphincter implantation and bladder neck reconstruction in patients with neurogenic urinary incontinence.

The results of artificial urinary sphincter implantation and Young-Dees-Leadbetter bladder neck reconstruction were compared in 27 patients with neurogenic urinary incontinence. Although an equal degree of continence was achieved with both procedures the reoperation rate was significantly higher in patients who underwent artificial sphincter implantation. The advantages and disadvantages of each procedure are discussed.
