PURPOSE OF REVIEW: In patients with symptoms of stress urinary incontinence, there is still a debate regarding the benefit of a multichannel urodynamic investigation prior to surgical management. The purpose of this article is to review recent evidence on this topic.
RECENT FINDINGS: Results of two large randomized controlled trials provided evidence that preoperative urodynamics do not improve outcome of incontinence surgery in women with uncomplicated stress urinary incontinence. Furthermore, in this selected group of women, urodynamics hardly lead to deviation of surgery as a primary treat

AIM: We review the literature regarding recurrent urinary stress incontinence (USI) following various anti-incontinence procedures and analyze possible risk factors for failure of different anti-incontinence procedures. Management options for recurrent USI and future trends are also covered.
METHODS: A PubMed search was undertaken for English language literature using the MeSH terms 'stress urinary incontinence', 'recurrence', 'treatment', and 'failure'. Literature from the year 2001 to 2010 was selected. We only selected those studies including randomized control trials, meta-analysis, and p

PURPOSE OF REVIEW: Vaginal childbirth is a significant risk factor for stress urinary incontinence (SUI). Women with SUI demonstrate dysfunction of the pelvic floor and pudendal nerve. Animal models of SUI have been developed to investigate its pathophysiology and for preclinical testing of potential treatments.
RECENT FINDINGS: Vaginal distension, a method of simulating childbirth injury in animals, produces a reliable decrease in leak point pressure (LPP), a measure of urethral resistance to leakage and quantification of SUI severity in animals. In addition to ischemia and direct tissue dam

The prevalence of stress urinary incontinence is quite significant with large numbers of women affected. Many of these women will not seek medical help in the belief that they will not be cured or improved. With the increasing numbers of procedures now available we present a review of both established and novel surgical techniques that is intended to help both urologists and primary care physicians in the counselling of patients complaining of stress incontinence. We compare the "gold standards" of surgical treatment for all types of stress incontinence with the newer techniques that have rece

Urinary incontinence after prostate surgery is a rare, but potentially devastating problem. Guidelines for the evaluation and treatment of male stress urinary incontinence have evolved over the years. A review of current behavioral, medical and surgical approaches (male sling, artificial urinary sphincter) for post prostatectomy incontinence is reviewed in this manuscript.

Treatment options for stress urinary incontinence (SUI) in women are designed to prevent the involuntary loss of urine from the urethra during increases in intraabdominal pressure that occur during physical activity, coughing, or sneezing. Effective nonsurgical therapies include behavioral therapy (eg, bladder training, fluid and dietary modification) and drug therapy. Surgical therapy for this condition has existed for well over 100 years. Currently, approximately 200 different surgical procedures have been described. Because of the physiologic risks inherent in surgical procedures, the cost

The wide use of retropubic TVT has been associated with various complications. To avoid these, alternative procedures have been developed and continence rates obtained with these new routes have been quite similar to those after classic TVT. In the transobsturator technique (TOT) described by Delorme and colleagues in 2001, the tape is inserted through the obturator foramens from outside to inside and is positioned without tension under the urethra. Another surgical technique allows the passage of a tape from inside to outside. The aim of this paper is to describe a new, simple surgical techni

The treatment of stress urinary incontinence (SUI) is one of more controversial aspects of pelvic floor surgery. The indications for the surgical approach are related to the international classification as: Type 1 and Type 2 (Anatomical Incontinence), and Type 3 urinary incontinence (Intrinsic Sphincteric Dysfunction). The procedure of choice for Type 1 and Type 2 is Bladder Neck Suspension (BNS) that create a strong hammock against which the urethra can be compressed with sudden changes of abdominal pressure. Type 3 has to be treated by coaptation or compression of the deficient sphinteric un

Retropubic bladder neck suspension can easily be performed laparoscopically giving the same therapeutic result as open surgery. Laparoscopy enables better visibility of the operative field, exact placement of paraurethral sutures and acceptable hemostasis. Patients who undergo laparoscopic retropubic suspension meet with less postoperative discomfort, fewer operative complications, a shorter hospital stay and less postoperative scars. The patients are allowed to drive and return to work 1-2 weeks after surgery.

OBJECTIVE: A new concept is put forward to explain stress urinary incontinence (SUI). Urine continence depends on an intact strong internal sphincter and a reactive sympathetic activity.
STUDY DESIGN: Thirty patients suffering from SUI were evaluated clinically and by urodynamic studies. During surgical repair of SUI, minute tissues were taken from the bladder neck and were examined microscopically after preparation and staining. Furthermore, examination of 15 postmortem specimens was done. Also a clinical trial was done to demonstrate the effects of sympathomimetics and alpha-blockers on the

Almost half of all women who exercise regularly experience some degree of urinary stress incontinence. The cause is often multifactorial, but physical activity often aggravates it. Because denial is commonplace, at each yearly gynecologic exam primary care physicians should ask patients about urologic problems. If a problem exists, a urinary questionnaire and voiding diary can help determine the nature of the condition. In addition, a thorough medical, urologic, and gynecologic history can help pinpoint the cause. Physical exam can reveal such conditions as neurologic deficits and pelvic floor

With the advent of magnetic resonance imaging, the treatment of female incontinence has undergone a renaissance. This change has primarily been due to superior understanding of anatomy and function of the supports of the urethra, bladder neck and bladder base in the female pelvis.

Genuine SUI is defined as that associated with hypermobility of the urethra and bladder neck. Accurate history-taking and physical examination allows for proper diagnoses and subsequent therapy in the majority of cases. Patients in whom bladder instability or intrinsic sphincteric deficiency are suspected may benefit from urodynamic testing. Regardless of method, all surgical procedures used to treat genuine SUI aim at supporting the bladder within the pelvic cavity. This allows for adequate compression of the urethra by those intra-abdominal forces that would otherwise only act to expel urine

This article reviews traditional vaginal and suprapubic procedures as well as more recent advances such as urethral bulking agents and artificial sphincters. It illustrates how the chosen operation must be tailored to the individual patient and how the surgeon treating urinary incontinence requires a variety of procedures in his repertoire.

To establish the incidence of surgical failure and misdiagnosis, we reviewed the post-surgical cystometries and clinical data of 33 patients with recurrent stress urinary incontinence following surgical management of this condition. The results of the study show that 63.6% were due to failure of the surgical technique. However, recurrence after a second procedure is likely due to bladder instability. The literature is reviewed and the data compared.

Retropubic bladder neck suspension has been used to treat 352 women presenting with the primary complaint of urinary stress incontinence. The author's concept of the proper assessment and management of such patients, in whom previous surgical treatment has failed, is stated. A plea is made for the abandonment of traumatic surgical methods and the confusing multiplicity of names that accompanies them.
