Adenomatous metaplasia (nephrogenic adenoma) of urothelium. An analysis of 70 cases.

A retrospective analysis of 70 cases of adenomatous metaplasia seen in the St Peter's Hospitals over a 15-year period has been carried out. The lesions occurred at all ages (7-81 years) and in both sexes (51 males, 19 females) and were found throughout the urinary tract from pelvis to urethra. In the pelvis and ureter the finding was usually an incidental one in association with stones or chronic inflammation; ulceration was a frequent accompaniment. Most vesical and urethral lesions followed a surgical procedure months or years previously, the patients re-presenting with haematuria or irritat

Attenuated crosstalk between urothelium and fibroblasts promotes ureteral stricture development.

BACKGROUND: Ureteral stricture (US), characterized by fibrotic remodeling of the ureteral wall, represents an obstructive urological disorder with incompletely characterized pathophysiological mechanisms. This study integrates single-cell RNA sequencing (scRNA-seq) with immunohistochemical validation in human tissues to investigate the molecular and cellular mechanisms underlying US pathogenesis. METHODS: Specimens of US (n = 7) and normal ureters (n = 8) were collected from patients prospectively. Single-cell RNA sequencing was performed to dissect the transcriptomic landscape of US, with su

Is electrolyte transfer across the urothelium important?: ICI-RS 2015.

AIMS: This article summarizes discussion at the International Consultation on Incontinence Research Society (ICI-RS) 2015 meeting of urine modification in the urinary tract by the urothelium. It considers the literature and proposes pertinent questions that need to be addressed to understand this phenomenon within a physiological context. METHODS: Following the ICI-RS meeting, publications in PubMed relating to urine modification in the renal pelvis, ureter, and bladder were reviewed. RESULTS: Historically, the urothelium has been simply considered as a passive, impermeable barrier, preventi

Differentiation potential of urothelium from patients with benign bladder dysfunction.

OBJECTIVE: To develop a novel in vitro approach to test the hypothesis that failure of urothelial differentiation underlies the aetiopathology of interstitial cystitis (IC), where there is evidence of compromised urinary barrier function, as benign dysfunctional bladder disease encompass several poorly understood clinically defined conditions, including IC, idiopathic detrusor overactivity (IDO) and stress urinary incontinence (SUI). MATERIALS AND METHODS: Biopsy-derived urothelial cells from dysfunctional bladder biopsies were propagated as finite cell lines and examined for their capacity t

Nectin-4 immunohistochemistry distinguishes urothelial carcinoma in situ from reactive urothelium.

Nectin-4 has been reported to be frequently overexpressed in urothelial carcinoma; however, its expression and diagnostic utility in urothelial carcinoma in situ (CIS) have not been systematically evaluated. In this study, we compared Nectin-4 expression in 50 cases of urothelial CIS and 50 cases of normal or reactive urothelium. Whole sections were immunohistochemically stained for Nectin-4, CK20 and p53, and staining results were evaluated using the H-score. Membranous Nectin-4 expression was significantly higher in urothelial CIS than in normal or reactive urothelium (p < 0.001) (24/50). A

Expression of Transgenes in Native Bladder Urothelium using Adenovirus-Mediated Transduction.

In addition to forming a high-resistance barrier, the urothelium lining the renal pelvis, ureters, bladder, and proximal urethra is hypothesized to sense and transmit information about its environment to the underlying tissues, promoting voiding function and behavior. Disruption of the urothelial barrier, or its sensory/transducer function, can lead to disease. Studying these complex events is hampered by lack of simple strategies to alter gene and protein expression in the urothelium. Methods are described here that allow investigators to generate large amounts of high-titer adenovirus, which

Improving the barrier function of damaged cultured urothelium using chondroitin sulfate.

AIMS: To determine whether glycosaminoglycan (GAG) replenishment is able to improve recovery of a deficient urothelial barrier, chondroitin sulfate (CS) instillations were tested using an in vitro model. Porcine urothelial cells (Ucells) were terminally differentiated in culture conditions to construct a urothelial layer with a functional barrier. This layer was damaged to compromise barrier function to simulate a key characteristic of bladder pain syndrome/interstitial cystitis. The functional effect of subsequent treatment with CS was evaluated. METHODS: Primary porcine Ucells were isolated

Immune snapshots along the inflammation-to-cancer road in bladder urothelium.

The inflammatory microenvironment formed by chronic inflammation is not only a major risk factor for cancer but also a well-recognized precursor to bladder cancer. However, the immunological transitions that occur along the inflammation-to-cancer continuum remain incompletely understood. This mini-review synthesizes recent advances in understanding how the immune microenvironment evolves from an inflamed yet non-malignant urothelium to invasive carcinoma. First, we discuss how persistent stimuli-such as chronic infection or exposure to carcinogens-disrupt immune homeostasis, leading to sustain

Hypoxic changes to the urothelium as a bystander of end-stage bladder disease.

INTRODUCTION: Urothelial cells harvested from benign diseased bladders have a compromised capacity to propagate or differentiate in vitro, potentially limiting their application in autologous tissue engineering approaches. The causative pathways behind this altered phenotype are unknown. The hypothesis is that hypoxic damage to the urothelium occurs as a bystander to chronic or recurrent episodes of infection and inflammation. OBJECTIVE: The aim of this study was to assess immunohistochemically detected nuclear hypoxia-inducible factor 1 alpha (HIF-1α) and vascular endothelial growth factor i

Stress-induced autonomic dysregulation of mitochondrial function in the rat urothelium.

AIM: Chronic stress exacerbates the symptoms of most pain disorders including interstitial cystitis/bladder pain syndrome (IC/BPS). Abnormalities in urothelial cells (UTC) occur in this debilitating bladder condition. The sequence of events that might link stress (presumably through increased sympathetic nervous system-SNS activity) to urothelial dysfunction are unknown. Since autonomic dysregulation, mitochondrial dysfunction, and oxidative stress all occur in chronic pain, we investigated whether chronic psychological stress initiated a cascade linking these three dysfunctions. METHODS: Adu

[Endourologic treatment of tumor of the upper urothelium: results and clinical course].

Presentation of 31 patients with suspected diagnosis of upper urothelium tumour seen between 1986 and 1991. In agreement with our study protocol and the endourological treatment followed in this type of tumours, there were eight false positive, which were excluded. Five cases were treated by retrograde ureterorenoscopy while in the remaining 18 cases the anterograde percutaneous approach was used. A review of both the aspects of endourological treatment and the evolution of these patients during that period is included.

Cell cycle of normal bladder urothelium in developing and adult mice.

The present research has employed a novel, nonradioactive technique to quantitatively study normal urothelial proliferation in foetal, neonatal, juvenile and adult mouse bladder. Using whole mount histological preparations, the total number of urothelial nuclei per mouse bladder, and per given urothelial cell layer, have been assessed to provide data of the (unstimulated) kinetic behaviour of basal urothelial cells (the proliferative population), to analyse characteristics of the normal urothelial cell cycle. The urothelial cell cycle time increases from 30.6 h (foetal) to 40 weeks (adult), th

Microbiome-urothelium crosstalk in bladder cancer: From dysbiosis to clinical translation.

Bladder cancer (BCa) ranks among the most frequently diagnosed malignancies worldwide. The advent of high-throughput sequencing technologies has revealed distinct urinary microbial signatures that differentiate healthy individuals from BCa patients. While growing evidence implicates microbiota dysbiosis in the pathogenesis, progression, and therapeutic response of BCa, establishing a direct causal relationship remains challenging due to methodological heterogeneity across studies. This review summarizes the current knowledge of the human urinary microbiome (urobiome) in both health and disease

Species differences in regenerative hyperplasia of bladder urothelium after transurethral cauterization.

The early histological changes in bladder urothelium following transurethral cauterization were sequentially studied in female rats, mice, hamsters and guinea pigs. The changes differed markedly between species: papillary hyperplasia or papilloma in the rats and simple hyperplasia in the mice, hamsters and guinea pigs. The regenerative response to the fulgurative ulcer within a given species was similar to the early response previously observed by others following administration of known bladder carcinogens. This suggests that there may be a regulatory mechanism in the urothelium which is comm

STAG2 cohesin cooperates with DREAM to maintain quiescence and suppress tumourigenesis in the urothelium.

The maintenance of quiescence is essential for tissue homeostasis. STAG2 is one of the few genes mutated in the normal urothelium of organ donors, with mutant cells undergoing positive selection 1 . STAG2 is also a major tumour suppressor gene 2-4 and its inactivation is an early event in bladder carcinogenesis 1,3 . However, how STAG2, a cohesin component, regulates urothelial homeostasis remains largely unknown. Here, we demonstrate that Stag2 inactivation in normal murine urothelial cells interferes with differentiation programs, triggers transient cell cycle entry, and primes cells for clo

Deficiency of Pdcd10 causes urothelium hypertrophy and vesicle trafficking defects in ureter.

The scaffolding protein programmed cell death protein 10 (Pdcd10) has been demonstrated to play a critical role in renal epithelial cell homeostasis and function by maintaining appropriate water reabsorption in collecting ducts. Both ureter and kidney collecting duct systems are derived from the ureter bud during development. Here, we report that cadherin-16 (Cdh16)-cre drives gene recombination with high specificity in the ureter, but not the bladder, urothelium. The consequences of Pdcd10 deletion on the stratified ureter urothelium were investigated using an integrated approach including me

Involvement of mitochondrial pathway of apoptosis in urothelium in ketamine-associated urinary dysfunction.

BACKGROUND: Long-term use of ketamine results in ketamine-associated urinary dysfunction (KAUD), which is characterized by frequent micturition, urgent urination, urine pain, hematuria, dysuria and urge incontinence. This study aims to examine the effect of ketamine on the urothelium and investigate the underlying mechanisms responsible for KAUD. METHODS: A rat model of KAUD by ketamine injection was developed. Histological changes in bladder tissues were measured by hematoxylin and eosin staining, and apoptosis by the TUNEL assay. Primary bladder epithelial cells were treated with ketamine,

Differential expression of functional cannabinoid receptors in human bladder detrusor and urothelium.

PURPOSE: Although cannabinoid receptor expression has been demonstrated in human brain and other peripheral neuronal tissues, definitive expression of these receptors in the human bladder has not been reported. Consequently we investigated the expression of functional cannabinoid 1 and 2 receptors in human bladder detrusor and urothelium. MATERIALS AND METHODS: Human bladders were micro-dissected for detrusor (6) and urothelium (8), and analyzed for cannabinoid 1 and 2 mRNA expression using real-time quantitative polymerase chain reaction, and for protein expression using immunohistochemistry

Dietary effects of ortho-phenylphenol and sodium ortho-phenylphenate on rat urothelium.

Ortho-phenylphenol (OPP) and sodium ortho-phenylphenate (NaOPP) are pesticides used commercially in the food industry that have been shown to be carcinogenic to rat urothelium. Dietary administration of 1.25% OPP or 2.0% NaOPP caused increased incidences of urothelial hyperplasia and eventually caused tumors in male F344 rats, with NaOPP apparently having a more potent effect. In other studies, various sodium salts such as saccharin and ascorbate enhanced bladder carcinogenesis, although the acid forms of these salts did not. In studies with high dietary doses of these sodium salts, an amorpho
