SMARCB1 (INI1)-deficient malignant melanocytic neoplasm with histologically confirmed vertebral metastases in a young adult: a case report.

BACKGROUND: SMARCB1 (INI1)-deficient tumors are rare aggressive neoplasms characterized by biallelic inactivation of the SMARCB1 gene and loss of nuclear INI1 protein expression. Although melanocytic differentiation has occasionally been reported in SMARCB1-deficient tumors, intraocular presentations with histologically confirmed vertebral metastases in adults remain exceptionally uncommon and diagnostically challenging. CASE PRESENTATION: A 21-year-old woman presented with progressive left retro-orbital pain and visual impairment. Ophthalmologic imaging revealed a choroidal lesion initially

Retinoblastoma, microphthalmia, coloboma, and neuroepithelioma of the pineal body.

A one-month-old infant boy was examined early in life because his mother had bilateral retinoblastoma and his father had bilateral microphthalmia. The ophthalmologist found his right eye was normal size with a coloboma of the iris, choroid, and retina. The left eye was microphthalmic with a coloboma of the uveal tract and retina. A vascularized fluffy white mass in the posterior pole was diagnosed clinically as a retinoblastoma. The tumor regressed with radiation. When the patient was four years of age, a large tumor was found in the region of the pineal recess, causing hydrocephalus and seizu

Uveal melanoma in association with multiple malignancies. A case report and review.

A 66-year-old man had a pigmented choroidal lesion in his right eye. Over a subsequent period of 8 1/2 years, the lesion showed slight growth. At autopsy, the diagnosis was spindle B melanoma of the choroid without evidence of systemic metastasis. A very unusual feature of this case was the presence in this patient of seven primary malignant tumors and five benign neoplasms. In addition to the choroidal melanoma, these included three distinct pulmonary carcinomas, leiomyosarcoma of the stomach, adenocarcinoma of the sigmoid colon, and basal cell carcinoma. The literature on multiple malignanci

Effect of the method of enucleation on the prognosis of choroidal melanoma.

The prognosis following treatment of uveal melanomata depends not only on factors such as cell type and degree of mitotic activity, size and location of the tumour, and the age of the patient, but also on the method of enucleation, as surgical manipulation of the neoplasm may potentiate the shedding of malignant cells into the blood stream. One hundred and thirty-one patients treated by different methods of enucleation were reviewed and the mortality patterns assessed. The numbers of deaths over the first 5 years differed significantly between the different groups. A specific method of enuclea

Conjunctival Melanocytic Neoplasia.

This article explores pigmented conjunctival lesions, emphasizing their clinical and histopathologic features. It highlights the diagnostic challenges posed by the overlapping clinical morphologies of benign, premalignant, and malignant melanocytic proliferations; biopsies of such lesions may be evaluated by dermatopathologists. This review discusses the distinct origins and molecular genetics of conjunctival melanocytic neoplasms, especially in sun-exposed areas like the bulbar conjunctiva, which share similarities with their cutaneous counterparts. Uveal melanoma, which is not the topic of t

How we treat patients with metastatic uveal melanoma.

Uveal melanoma is the most prevalent and aggressive intraocular malignancy affecting adults. Compared with cutaneous melanoma, uveal melanoma has distinct pathogenesis and molecular characteristics. Not surprisingly, it derives limited benefits from checkpoint inhibitors. Until recently, no systemic therapy had impacted survival outcomes for this patient population. Tebentafusp, a T-cell receptor-based molecule, is the first US Food and Drug Administration/European Medicines Agency-approved systemic therapy to improve the survival outcomes for uveal melanoma patients expressing HLA-A∗02:01. Clinical Management of Uveal and Conjunctival Melanoma.

Ocular melanoma is a rare but potentially devastating malignancy arising from the melanocytes of the uveal tract, conjunctiva, or orbit; it represents less than 5% of all melanoma cases in the United States. The management of ocular melanoma varies depending on its anatomic origin, since uveal and conjunctival melanoma have distinct biologies and thus different treatment strategies. Uveal melanoma is the most common type of ocular melanoma and is characterized by activation of the mitogen-activated protein kinase (MAPK) pathway (among other signaling pathways) via mutations in GNAQ or GNA11. The pathology of ocular cancer.

Primary cancers of the eye are rare. These include uveal melanoma, a tumor that preferentially affects the choroid of light-eyed, fair-skinned Europeans, and the pediatric retinal neoplasm retinoblastoma, which is slightly more common worldwide. Uveal melanoma kills about half of affected patients. Most succumb to hepatic metastases, which are unresponsive to current therapy. Factors indicative of poor prognosis include tumor size, ciliary body involvement, epithelioid cells, extraocular extension, lymphocytic and melanophagic infiltration, mitotic activity, vascular mimicry patterns, and most

Genetic Alterations among Korean Melanoma Patients Showing Tumor Heterogeneity: A Comparison between Primary Tumors and Corresponding Metastatic Lesions.

PURPOSE: Melanoma is a highly heterogeneous neoplasm, composed of subpopulations of tumor cells with distinct molecular and biological phenotypes and genotypes. In this study, to determine the genetic heterogeneity between primary and metastatic melanoma in Korean melanoma patients, we evaluated several well-known genetic alterations of melanoma. In addition, to elucidate the clinical relevance of each genetic alteration and heterogeneity between primary and metastatic lesions, clinical features and patient outcome were collected. MATERIALS AND METHODS: In addition to clinical data, BRAF, NRA

A modern review of uveal melanoma: molecular insights, clinical management, and emerging therapies.

Uveal melanoma is the most frequent primary intraocular malignancy in adults and is characterized by an aggressive clinical course, high propensity for hepatic metastasis, and poor survival after metastatic progression. Improvements in ocular imaging and local therapies have improved primary tumor control, but approximately half of patients will ultimately develop metastatic disease with poor treatment options. Recent advances in genomics and molecular biology have dramatically improved our understanding of uveal melanoma pathogenesis, with the discovery of activating mutations in GNAQ , GNA11

An Evolutionary Patient Journey in Uveal Melanoma.

Patients who receive a rare cancer diagnosis begin a diagnostic and treatment journey which is complicated by personal fear of the unknown and the reality of a scientific and clinical research community which may not have many insights, options, and definitive answers to enable long-term survival. Patients and families may initially struggle to find clinicians well-informed in the rare cancer and clinical trials of emerging therapies. Patient advocacy groups focused on specific rare cancers have established an important role and point of contact for connecting patients who share the same diagn

A Case of Metastatic Uveal Melanoma: From Diagnosis to Survival Four Years Later.

Uveal melanoma includes melanoma of the choroid, ciliary body or the iris of the eye and is a rare malignancy that accounts for about 1,500 new cases in the U.S. every year. Of the choroid, ciliary body or the iris, the choroid is most often affected. Local treatment is well-studied; however, this cancer tends to metastasize in nearly 50% of patients, even if the primary melanoma is treated appropriately. There are limited approved treatments for metastatic uveal melanoma and thus, survival rates are low. However, emerging clinical trials demonstrate promising results and play a prominent role

Estimation of DNA content in uveal melanomas by flow cytometry.

Flow cytometry was used to evaluate ploidy and tumour cycle kinetics in fresh tissue samples obtained from 19 uveal melanomas. The results were compared with other parameters including, histological cell type, tumour size and anatomical location. Three tumours (15.8%) were aneuploid (two mixed cell, one epithelioid cell). Cell turnover was estimated in the 16 diploid tumours by summating the total percentage of cells in S and G2/M phases. We found the mean percentage of cells in G2/M/S to be 5.96% (range 2.2-9.8%). Spindle cell neoplasms appeared to have lower cell turnover rates (4.5 +/- 1.2%

Ophthalmopathologic characterization of multicentric or metastatic neoplasms with an extraocular origin in dogs and cats.

OBJECTIVE: To characterize the frequency and distribution of secondary ocular neoplasms in dogs and cats identified during necropsy. METHODS: A retrospective analysis of necropsy records of dogs and cats was conducted, and cases with metastatic/multicentric neoplasms with an extraocular origin, involving the eyes, were selected. RESULTS: From January 2015 to January 2019, we identified a total of 233 dogs and 100 cats with metastatic disease. Of these, 11.6% (27/233) of the dogs and 13% (13/100) of the cats had ocular metastases. Lymphoma was the most common multicentric neoplasm involving t

Management of liver metastases from uveal melanoma.

Uveal melanoma is the most common primary intraocular malignancy in adults, with distinct genetic and clinical characteristics compared with cutaneous melanoma. Despite improvements in the treatment of the primary tumour, nearly half of the patients will develop distant metastases, most commonly in the liver. Once metastases are detected, the median overall survival is approximately 1 year, with a 2-year survival rate of only 8%. Systemic treatment, including chemotherapy, immunotherapy, and targeted therapy, has historically shown limited efficacy. The first (and so far only) systemic treatme

Ipsilateral orbital metastasis following enucleation of intraocular ciliochoroidal melanoma: a rare occurrence.

This case report demostrates an unusual occurence of orbital metastasis along with liver metastasis 5 years following enucleation for ciliochoroidal melanoma with no extraocular extension. It exemplifies that metastasis of the tumour can occur in the ipsilateral orbit after enucleation. Importance of close clinical follow-up after enucleation for choroidal melanoma, careful socket examination, and regular systemic metastatic workup is demonstrated. To the best of our knowledge, this is the first case report of uveal melanoma with ipsilateral orbital metastasis.
