Anterior uveitis and meningococcemia: a case report.

PURPOSE: To describe a case of meningococcemia with anterior uveitis. METHODS: Observational case report. RESULTS: A 38-year-old woman developed meningococcal septicemia caused by Neisseria meningitidis type B. During her admission, she had pain in her left eye, inflammatory cells, and a fibrinous exudate in the anterior chamber and multiple posterior synechiae, all in the context of an anterior uveitis. She was treated with topical steroids and mydriatics with resolution of ocular inflammation. CONCLUSIONS: This case illustrates the possible association between anterior uveitis and a menin

[Approach to the patient with acute anterior uveitis].

Uveitis refers to inflammation of the uveal tract, which includes the iris, ciliary body, and choroid. Common usage has expanded this definition, however, to include intraocular inflammation of any type. Hence, retinitis, retinal vasculitis, and papillitis are also grouped under uveitis, even in the absence of significant uveal tract inflammation. Uveitis can also be secondary to corneal, scleral, or orbital inflammation. Experience has shown that identifying the primary focus of intraocular inflammation is important, both for establishing a diagnosis and as a predictor of natural history and

Anterior uveitis and its relation to stress.

BACKGROUND/AIMS: Idiopathic recurrent acute anterior uveitis (RAAU) is a common reason for attendance at ophthalmic casualty departments. Patients who suffer with this condition may have multiple recurrent episodes which are often debilitating and necessitate time off work. If recurrences are identified early and treatment initiated rapidly, the inflammation can be minimised and settles quickly on topical treatment with a speedy functional recovery by the patient. It has been our clinical impression that some patients with RAAU relate recurrences of inflammation to psychological factors. There

HL-A 27 and acute anterior uveitis.

51 (30 men and 21 women) of 90 consecutive patients with acute non-granulomatous anterior uveitis were HL-A 27 positive. This frequency of 55-7 per cent compares with 8-2 per cent in controls. Twenty-three patients (18 men and 5 women) had in addition evidence of systemic disease, including ankylosing spondylitis, sacroiliitis and Reiter's syndrome, sometimes associated with psoriasis. Twenty-eight of 63 patients without evidence of systemic disease were HL-A 27 positive, suggesting that the uveitis in many of these cases has a similar aetiology to those with rheumatic disease. The uveitis ass

Genome-wide association study of anterior uveitis.

BACKGROUND/AIMS: The purpose of this study is to define genetic factors associated with anterior uveitis through genome-wide association study (GWAS). METHODS: In this GWAS meta-analysis, we combined data from the FinnGen, Estonian Biobank and UK Biobank with a total of 12 205 anterior uveitis cases and 917 145 controls. We performed a phenome-wide association study (PheWAS) to investigate associations across phenotypes and traits. We also evaluated genetic correlations of anterior uveitis. RESULTS: We identified six anterior uveitis-associated loci. Genome-wide significant (p<5 × 10-8) asso

Ophthalmic anterior segment metastasis masquerading as uveitis.

Ocular malignancies are rare, with many cancers of the eye being the result of metastases, the most common of which result from primary tumours of breast, lung and gastrointestinal tract. Diagnosis is often made at a late stage in the disease course, leading to poorer outcomes. Disease-directed therapy in the presence of ocular metastases varies based on the primary tumour and patient performance status but generally involves systemic treatment, either with chemotherapy or involved-field radiation. We herein present an interesting case of ocular malignancy with neuroendocrine small cell featur

Recent Developments in HLA B27 Anterior Uveitis.

There has been steady progress in understanding the pathogenesis, clinical features, and effective treatment of acute anterior uveitis (AU) over the past 5 years. Large gene wide association studies have confirmed that AU is a polygenic disease, with overlaps with the seronegative arthropathies and inflammatory bowel diseases, associations that have been repeatedly confirmed in clinical studies. The role of the microbiome in AU has received increased research attention, with recent evidence indicating that human leukocyte antigen B27 (HLA B27) may influence the composition of the gut microbiom

Differential Diagnosis of Viral-Induced Anterior Uveitis.

Diagnosis of uveitis is often challenging, but can be easy in typical viral-induced anterior uveitis (VIAU). Associated symptoms and signs are an important source of information. Certain classical clinical features such as keratic precipitates (KPs) distribution, iris atrophy, elevated intraocular pressure (IOP), and unilaterality are commonly used to support the diagnosis of VIAU. However, many etiologies of anterior uveitis may to a certain extent mimic VIAU, especially the ones with unilateral granulomatous KPs and elevated IOP. This review begins with how the clinician can differentiate vi

HLA-B27 Anterior Uveitis: Immunology and Immunopathology.

Acute anterior uveitis (AAU) is the commonest type of uveitis and HLA-B27 AAU is the most frequently recognized type of acute anterior uveitis and anterior uveitis overall. Recent evidence indicates that acute anterior uveitis is a heterogenous disease, is polygenic and is frequently associated with the spondyloarthropathies (SpA). Studies of patients with AAU and animal models of disease indicate a role for innate immunity, the IL-23 cytokine pathway and exogenous factors, in the pathogenesis of both SpA and acute anterior uveitis. Recently described genetic associations cluster around immuno

Recurrent anterior uveitis in Henoch Schonlein's vasculitis.

Uveitis is an important component of many rheumatic diseases. The main causes of recurrent uveitis are seronegative spondylarthropathies and Behçet's disease. We describe a rare case of Henoch Schönlein vasculitis (HSV) along with multiple recurrences of acute anterior uveitis. In cases of skin rash and recurrent anterior uveitis, HSV should be considered in the differential diagnosis.

Acute anterior uveitis: clinical and experimental aspects.

Acute anterior uveitis (AAU) or iritis is an inflammatory disorder of the anterior structures of the eye that may be associated with a number of disease entities. A significant proportion of patients will have no evidence of an underlying disorder and are labeled as idiopathic. Within this group approximately 50% will possess the human leukocyte antigen, HLA-B27, and some will have an associated spondyloarthropathy such as ankylosing spondylitis or Reiter's syndrome. Nevertheless, a number of HLA-B27-positive patients have no apparent underlying rheumatic disorder. The potential interplay of H

Clinically silent anterior uveitis in secondary syphilis.

1 In a series of twenty patients with secondary syphilis, ten showed signs of overt or subclinical iritis alone. Nine showed signs of purely subclinical iritis. 2 Nineteen were Caucasian men and the one woman was an unmarried Caribbean nurse. 3 Of the nineteen men, fourteen were homosexuals. 4 All received adequate anti-syphilitic treatment as soon as the diagnosis was made, and all ocular signs of inflammation disappeared within 14 days.

Secukinumab in refractory non-infectious anterior uveitis.

BACKGROUND: Secukinumab is a monoclonal antibody that selectively neutralizes interleukin-17A and has shown efficacy in the treatment of psoriatic arthritis, psoriasis, and axial spondyloarthritis. Its use in non-anterior non-infectious uveitis is controversial, with evidence generally not supporting its effectiveness in these conditions. However, the role of secukinumab in anterior non-infectious uveitis remains unclear. METHODS: Case series. Five patients with biological therapy-refractory non-infectious anterior uveitis who were treated with secukinumab were included. RESULTS: All 5 patie

Stromal Keratitis Associated With Cytomegalovirus Anterior Uveitis.

PURPOSE: Human cytomegalovirus (CMV) has commonly been reported as a cause of anterior uveitis and corneal endotheliitis. Unlike its other herpetic family members, herpes simplex virus and varicella zoster virus, involvement of the corneal stroma in CMV is uncommon. In this case series, we describe patients with CMV stromal keratitis. METHODS: This was a retrospective chart review of patients seen at a tertiary referral center from 1999 to 2023 with stromal keratitis who tested positive for CMV by directed polymerase chain reaction of aqueous fluid or corneal tissue. RESULTS: This series des

Clinical Features of CMV-Associated Anterior Uveitis.

Cytomegalovirus (CMV) anterior uveitis is the most common ocular manifestation of CMV disease in immunocompetent individuals. It is thought to be due to a local reactivation of latent CMV and is usually unilateral. The acute form presents as Posner-Schlossman Syndrome, a recurrent hypertensive anterior uveitis with few granulomatous keratic precipitates. There are geographic differences in the chronic form of CMV anterior uveitis. Asian patients commonly present as Fuchs Uveitis Syndrome with diffuse stellate keratic precipitates, while the European patients present with a chronic hypertensive

Chronic anterior uveitis in common variable immunodeficiency.

PURPOSE: To describe patients with uveitis and common variable immunodeficiency (CVID). DESIGN: Retrospective observational case series. METHODS: Retrospective review of patients with uveitis and CVID, specifically focusing on clinical presentation and treatment. RESULTS: Three patients with CVID and uveitis were identified. All patients had chronic anterior uveitis. Two required systemic immunosuppression to control uveitis, with one of these patients relapsing after discontinuation of immunoglobulin therapy. One improved on topical steroid therapy; however, follow-up on this patient did n
