Integrating High-Value Care and Environmental Sustainability to Reduce Unnecessary Laboratory Testing by Residents: An Interventional Pilot.

BACKGROUND: Routine lab testing of stable inpatients represents low-value care and contributes toward healthcare's significant carbon footprint. If the US healthcare system was its own nation, it would be the 13th largest emitter of greenhouse gases. Integrating the concepts of high-value care and environmental sustainability represents a potentially new approach to reduce unnecessary lab testing, low-value care, and carbon emissions. OBJECTIVE: To assess if an intervention integrating concepts of environmental sustainability and high-value care can influence resident lab ordering practices.

The connotations of Value-Based Healthcare: interpretive perspectives and differences in perceptions.

OBJECTIVE: This study aims to elucidate the connotations of Value-Based Healthcare (VBHC), correct misconceptions, and explore perceptual differences among distinct groups. METHODS: An analytical framework for VBHC connotations was first constructed from five perspectives: subject-object and subjective-objective relations, the individual and the collective, the refined value equation, conceptual categories and scope, and value implications. A questionnaire was then developed, and statistical analyses, including analysis of variance (ANOVA) and chi-square tests, were conducted on the collected

Wound care and the NHS 10-year health plan: aligning clinical innovation with system value.

The NHS Fit for the Future 10 Year Health Plan published in July 2025, represents the most ambitious structural redesign of the health service for a generation. For wound care, this creates both significant challenges and considerable opportunities. This article examines how wound care products and services can be aligned with the strategic priorities of the 10-year plan, including three systemic shifts, in addition to value-based procurement reform and drug tariff changes. These changes will affect how wound care products already listed on drug tariff are assessed through a renewal process. T

When Outcomes Become Currency: Reconsidering Value in Interventional Pain Management Under Emerging Episode-Based Payment Models.

The transition toward value-based care in the United States has introduced episode-based payment models that increasingly tie physician reimbursement to longitudinal costs and standardized outcomes. The Centers for Medicare & Medicaid Services (CMS) Ambulatory Specialty Model (ASM), targeting chronic low back pain (cLBP), represents a pivotal extension of this framework into interventional pain management. While intended to reduce low-value utilization, such models risk redefining clinical success in ways that may not align with the heterogeneous and biopsychosocial nature of chronic pain. Thi

Initiating value-based healthcare in psoriasis: Proposing a value-based outcome set for daily clinical practice.

BACKGROUND: With the current trend in healthcare moving towards a more value-based approach, it is essential to understand what value encompasses. OBJECTIVES: To develop an actionable value-based outcome set (VOS) for daily practice. METHODS: A mixed method approach was used consisting of four phases. Formerly, a systematic review was conducted, providing an overview of all patient-relevant outcomes defined in current literature. These 23 outcomes were then presented to a group of patients, using a modified nominal group technique (NGT), to establish whether these results represented all of

Transforming long-term post-acute care for the aging population through home infusion therapy in China: an assessment of need and demand (Part 1).

BACKGROUND: China's healthcare system is confronting a rising burden of antimicrobial resistance, chronic complex diseases, and an aging population requiring long-term, post-acute care. As a value-based care model in the USA for over 50 years, home infusion therapy (HIT), combined with home health services (HHS), presents a sustainable alternative to prolonged hospitalization for intravenous medication administration. In this two-part narrative review, Part 1 provides a needs assessment describing the demographic, clinical, and public health factors driving the demand for HIT and HHS in China.

Study on the reform effect of payment based on curative effect value for TCM dominant diseases in China: an interrupted time series analysis.

BACKGROUND: Anorectal diseases, as prevalent and recurrent conditions for which Traditional Chinese Medicine (TCM) demonstrates distinct advantages, are closely associated with patients' quality of life. The current Diagnosis-Related Group (DRG) payment system struggles to fully capture the unique value of TCM, whereas Value-Based payment models grounded in therapeutic efficacy offered a viable pathway for reforming payment mechanisms for TCM-dominant diseases. METHODS: This study included a total of 5,304 patients diagnosed with hemorrhoids, anal fistulas, and other anorectal diseases who we

Rethinking Blood Pressure Quality Measurement: Accounting for Seasonal Variation in Value-Based Care.

Blood pressure (BP) control is a core quality metric in value-based care (VBC). The Healthcare Effectiveness Data and Information Set (HEDIS) Controlling High Blood Pressure (CBP) measure uses the final BP reading of the calendar year, a method that overlooks well-documented seasonal variation-BP declines in summer and rises in winter. We evaluated the impact of this pattern on measured performance in a large health system. We conducted a retrospective analysis of monthly BP control rates (July 2019 to December 2025) across the Jefferson Health-Lehigh ambulatory network, totaling 78 months and

Effect of Implementing a Bundled-Payment Program for Heart Failure at a Safety-Net Community Hospital.

Safety net community hospitals face unique challenges when entering risk-based contracts. The financial viability of such programs in these settings has not been well studied. This study analyzed a bundled-payment program for congestive heart failure at one such facility. To assess financial performance, the authors calculated the net patient payment by quarter after bundle implementation, and also compared the leading cost drivers before and after bundle implementation, specifically the next site of care and readmission rate. After 21 months of participating in the bundle, the program has sav

Development of a Comprehensive, Nonsurgical Joint Health Program for People With Osteoarthritis: A Case Report.

BACKGROUND AND PURPOSE: Existing osteoarthritis (OA) care models often fall short in addressing the many biological, psychological, social, and behavioral characteristics that contribute to disability. As US health care shifts towards value-based payment, there is an increasing need to develop and test scalable, cost-effective, and multi-modal OA care models. This administrative case report will describe the development and pilot of a new, value-based comprehensive care model for OA. CASE DESCRIPTION: The Joint Health Program (JHP) is a physical therapist-led conservative care model for indiv

The Population Health Value Framework: Creating Value by Reducing Costs of Care for Patient Subpopulations With Chronic Conditions.

PROBLEM: With the growth in risk-based and accountable care organization contracts, creating value by redesigning care to reduce costs and improve outcomes and the patient experience has become an urgent priority for health care systems. APPROACH: In 2016, UCLA (University of California, Los Angeles) Health implemented a system-wide population health approach to identify patient populations with high expenses and promote proactive, value-based care. The authors created the Patient Health Value framework to guide value creation: (1) identify patient populations with high expenses and reasons f

Early Impacts of a Medicaid Value-Based Payment Policy on Quality of Care in a Large Population with Serious Mental Illness.

BACKGROUND: In the US, several states including New York State (NYS) have adopted Medicaid value-based payment (VBP) policies to improve healthcare quality and reduce spending. However, the impacts of these policies on vulnerable Medicaid beneficiaries with high behavioral health needs including those with serious mental illness (SMI) are unknown. AIMS OF THE STUDY: To inform policy efforts to improve the value of Medicaid-funded care by examining the early impacts of NYS's VBP policy implemented in 2017 on the quality of healthcare received by adult beneficiaries with SMI. METHODS: We condu

An Optimized Heart Failure Triaging Protocol to Reduce Avoidable Hospitalizations and Total Costs of Care.

Heart failure remains a leading cause of potentially avoidable hospital admissions throughout the United States and a significant driver of unnecessary, avoidable costs within value-based care programs. In a baseline population of 3233 hospital emergency department (ED) visits for heart failure, which occurred over 12 months ending in June 2023 across six WellSpan Health acute care hospitals, 2868 (88.7%) resulted in inpatient admissions. Among those admissions, 2535 (96.0%) were deemed potentially avoidable by the U.S. Centers for Medicare and Medicaid Services (CMS) Prevention Quality Indica

Designing an Indicator‑Driven, Value‑Based Architecture for Pneumonia Prevention in Japan: A Formative Policy Viewpoint on Adult Vaccination and Oral Care.

Japan's aging society concentrates pneumonia burden across communities, long‑term care, perioperative pathways, and hospitals. Adult pneumococcal and influenza vaccination are supported by trials and meta‑analyses and are often cost‑effective; yet, realized value depends on targeting, measurement, financing, and pairing with bedside prevention such as oral care and hospital‑acquired pneumonia bundles. This viewpoint proposes a formative, indicator‑driven architecture linking cost‑effectiveness to operations by aligning vaccination with complementary oral‑care prevention and value‑based payment

A public health value-based healthcare paradigm for HIV.

BACKGROUND: HIV patients face considerable acute and chronic healthcare needs and battling the HIV epidemic remains of the utmost importance. By focusing on health outcomes in relation to the cost of care, value-based healthcare (VBHC) proposes a strategy to optimize quality of care and cost-efficiency. Its implementation may provide an answer to the increasing pressure to optimize spending in healthcare while improving patient outcomes. This paper describes a pragmatic value-based healthcare framework for HIV care. METHODS: A value-based HIV healthcare framework was developed during a series

Improving Musculoskeletal Costs of Care Using a Virtual Integrated Practice Unit: A Claims-Based Analysis.

Musculoskeletal (MSK) conditions are among the leading drivers of health care spending for risk-bearing organizations, including health insurance plans and self-funded employers. Costs are often amplified in Medicare Advantage populations owing to the high prevalence of MSK conditions and overutilization of expensive health care services. At Health Alliance Plan, a Michigan-based regional health insurance plan and subsidiary of Henry Ford Health, MSK costs increased following the coronavirus 2019 pandemic despite already being a top cost-of-care driver. This case study describes the implementa

Advocacy 101 for the Early-Career Arthroplasty Surgeon: Payment, Policy, and Professional Sustainability.

Modern hip and knee arthroplasty surgeons graduate prepared to manage complex deformity, revision surgery, complications, and high-volume clinical practice but often receive little formal preparation for the payment and policy systems that determine whether that care remains accessible and sustainable. For early-career surgeons, this gap between clinical work and reimbursement reality can feel confusing, frustrating, and outside their control. Yet, payment policy directly shapes patient access, practice viability, workforce morale, and professional autonomy. This primer provides an "Advocacy 1
