Vascular remodeling in autogenous arterio-venous fistulas by MRI and CFD.

Hemodynamic parameters play an important role in regulating vascular remodeling in arterio-venous fistula (AVF) maturation. Investigating the changes in hemodynamic parameters during AVF maturation is expected to improve our understanding of fistula failure, but very little data on actual temporal changes in human AVFs is available. The present study aimed to assess the feasibility of using a noncontrast-enhanced MRI protocol combined with CFD modeling to relate hemodynamic changes to vascular remodeling following native AVF placement. MR angiography (MRA) and MR velocimetry (MRV) data was acq

Percutaneous embolization of a caroticoazygous fistula with the Amplatzer Vascular Plug 1.

We present the case of a five-year-old boy with a caroticoazygous fistula, which is an extremely rare congenital vascular malformation. The patient also had patent ductus arteriosus, a common cause of continuous murmur. The murmur continued despite successful coil embolization of the ductus. Repeated catheter-angiography revealed a large caroticoazygous fistula. The fistula was successfully embolized using the Amplatzer(®) Vascular Plug 1. Transcatheter occlusion of caroticoazygous fistula was performed easily, safely, and efficiently with a vascular plug.

Treatment of ureteroarterial fistulae with covered vascular endoprostheses and ureteral occlusion.

BACKGROUND: Ureteroarterial fistulae (UAFs) are a rare entity, often difficult to identify, and associated with a high mortality rate. This fact has been attributed to a delay in diagnosis and treatment. Five conditions that can predispose to the development of this uncommon entity have been described: prior pelvic surgery, prolonged ureteral stenting, radiation therapy, previous vascular surgery and vascular pathology. METHODS: We present 4 patients with UAFs and at least three of the above-mentioned conditions. Ureteral ischemia and subsequent necrosis promote the formation of these fistula

Balloon Anchor Technique to Aid Closure of Coronary Fistula Using Vascular Plug.

Coronary arteriovenous fistulas (CAFs) are abnormal communications between coronary arteries and cardiac chambers or great vessels.1 They are usually congenital but may be acquired after trauma, surgery, or percutaneous procedures. Although often incidental in childhood, hemodynamically significant CAFs in adults can cause myocardial ischemia, arrhythmia, heart failure, or endarteritis.2 Evidence to guide management is limited, and multidisciplinary evaluation at centers experienced in both percutaneous and surgical approaches is recommended.3 A 60-year-old man with hypertension and prior left

A Rare Vascular Tangle Unraveled: Multimodal Imaging in Giant Coronary Arteriovenous Fistula.

A 63-year-old woman presented with intermittent palpitations. Transoesophageal echocardiography (TEE) identified a high-velocity shunt from a tortuous left circumflex artery into an enlarged coronary sinus, with a 5-mm fistulous orifice. Coronary CT angiography delineated the full course of a congenital coronary arteriovenous fistula (CAVF), associated circumflex aneurysm, and coronary sinus dilatation. Surgical closure was successfully performed, with intraoperative confirmation of shunt resolution. This case highlights the synergistic utility of multimodal imaging: echocardiography for real-

Percutaneous vascular plug in management of an acquired broncho pleural cutaneous fistula.

BACKGROUND: Bronchopleural fistula (BPF) / broncho pleural cutaneous fistula is an abnormal communication between the peripheral bronchial tree and pleural space which can further also open to the skin surface. It is associated with significant morbidity and mortality in addition to poor quality of life. Management requires a multidisciplinary approach with careful evaluation to choose the best approach to treatment. CASE PRESENTATION: A 36-year-old male presented with a left chest wall tumor with multiple surgeries and CT revealing a left apico-posterior segment broncho pleural cutaneous fis

Effects of a More Selective Arteriovenous Fistula Strategy on Vascular Access Outcomes.

SIGNIFICANCE STATEMENT: The optimal choice of vascular access for patients undergoing hemodialysis-arteriovenous fistula (AVF) or arteriovenous graft (AVG)-remains controversial. In a pragmatic observational study of 692 patients, the authors found that among patients who initiated hemodialysis with a central vein catheter (CVC), a strategy that maximized AVF placement resulted in a higher frequency of access procedures and greater access management costs for patients who initially received an AVF than an AVG. A more selective policy that avoided AVF placement if an AVF was predicted to be at

Percutaneous embolization of idiopathic renal arteriovenous fistula using Amplatzer vascular plug II.

Renal arteriovenous fistula (AVF) presents a rare but important condition that can be acquired, congenital, or idiopathic. Embolization techniques have recently been considered the first-line therapy in the treatment of these lesions, but they carry a possible high risk of distal migration of embolic agents into the venous and pulmonary circulation. This risk is especially important with large, high-flow fistulas. In this report, we present a case in which a 31-year-old man with a symptomatic idiopathic AVF was treated with embolization using the Amplatzer vascular plug II after unsuccessful s

Secondary aorto-enteric fistula--still a devastating complication of major vascular surgery.

BACKGROUND: Despite advances in vascular surgery and intensive care, secondary aorto-enteric fistulas (AEF) remain a serious threat of major vascular interventions. This report summarizes our experience in the treatment of this condition. PATIENTS AND METHODS: Nineteen patients with secondary AEF admitted between Jan 2004 and Dec 2008 were prospectively enrolled into this cohort observational study. Nine of them presented with massive gastrointestinal bleeding and hemorrhagic shock. RESULTS: None of the patients died preoperatively. Graft in situ replacement was performed in eight patients (

Intra vascular ultra sound: one more tool to diagnose aorto-duodenal fistula.

The incidence of aorto-enteric fistula in the first 5 years after abdominal aortic replacement ranges from 0.3 to 2%. We present a clinical case in which all conventional diagnostic tools failed to demonstrate the aorto-enteric fistula. A 73 year-old male suffering intermittent episodes of melena without signs and symptoms of infection was repeatedly admitted at our institution. All conventional diagnostic tools failed to show the bleeding source. Precise diagnosis was obtained using intra vascular ultrasound (IVUS). IVUS allowed prompt diagnosis of the aorto-duodenal fistula and opened the wa

Transposed basilic vein-brachial arteriovenous fistula: an alternative vascular access for hemodialysis.

Twenty-five brachial-basilic arteriovenous (AV) fistulas with transposed basilic vein for alternative vascular access were created in 22 chronic hemodialysis patients. This surgical procedure was performed under brachial block or general anesthesia. After a longitudinal skin incision that was made in the inner side of the arm, the basilic vein was exposed, transposed subcutaneously, and anastomosed end-to-side to the brachial artery. The follow-up was between 7 and 24 months. Early complications were hemorrhage, thrombosis, steal syndrome, and swelling of the arm. Among the late complications

Vascular Ehlers-Danlos syndrome with ruptured thoracic aortic aneurysm complicated by vascular graft and endograft infection and aortobronchial fistula.

The combination of vascular graft or endograft infection and aortic hereditary disease is extremely rare and just as challenging. We report the case of a 40-year-old man presenting with a ruptured saccular aortic isthmus aneurysm treated with thoracic endovascular aortic repair, complicated by vascular graft or endograft infection with an aortobronchial fistula and multiple pseudoaneurysms. Despite complete graft explantation, prolonged antibiotic therapy, and additional endovascular and open interventions, he developed recurrent pseudoaneurysms with an ultimately fatal outcome. Effect of adventitial dissection in brachiobasilic arteriovenous fistulae opened in children as vascular access for hemodialysis on patency and maturation of fistulae.

BACKGROUND: Recently, therapy of pediatric patients with chronic renal failure has been carried out using hemodialysis, peritoneal dialysis, or renal transplant. In this study, we prospectively investigated the role of adventitial dissection (performed on brachiobasilic arteriovenous fistulae in the antecubital area) in the patency rate, maturation, and early initiation of dialysis among pediatric cases undergoing chronic hemodialysis. METHODS: Thirty children (7 male and 23 females) were included in this study. They were operated on at the Department of Cardiovascular Surgery of Alsancak Sta

Use of an amplatzer vascular plug for transcatheter embolization of a renal arteriovenous fistula.

Renal arteriovenous fistulae (AVF) are a rare condition that can be acquired, congenital, or idiopathic. Idiopathic AVF are the rarest type of renal AVF. Traditionally, renal AVF were treated with surgery, but the mainstay of treatment has shifted to an endovascular approach by coil embolization. Embolization procedures are often difficult secondary to the "high flow" of the AVF, and there is the substantial risk of distal embolization of coils. We present a case in which an idiopathic renal AVF was successfully embolized with an Amplatzer vascular plug.

Vascular Rupture After Banding for High-Flow Arteriovenous Fistula: A Multicenter Case Series.

High-flow arteriovenous fistula often goes unnoticed during long-term dialysis in patients with chronic kidney failure until complications arise. The banding procedure is a commonly used method to reduce flow currently. However, data on rare but serious adverse events remain limited. This multicenter case series describes 6 patients who developed vascular rupture following banding for high-flow access, including 5 early and 1 late event. A suture-related cutting effect on dilated, high-tension vessels may be an explanation, although this remains speculative. Although previous studies have repo

Interventional closure of artificial vascular anastomotic fistula after aortic replacement: a case report.

Two years ago, the patient suffered from type A aortic dissection. As a result, partial aortic dissection artificial vascular replacement and partial aortic arch artificial vascular replacement were performed. Six months after the operation, an anastomotic fistula in the ascending aorta was detected, which subsequently progressed to chronic heart failure of New York Heart Association (NYHA grade) class III. After eliminating the operation - related contraindications, the patient successfully had the fistula occluded through transcatheter ascending aorta - right atrial fistula in our hospital.
