Clinical comparison of two optional vena cava filters.

PURPOSE: To compare the clinical safety and efficiency of two optional inferior vena cava (IVC) filters. MATERIAL AND METHODS: Ninety-three consecutive Günther Tulip filters (92 patients) were compared with 83 consecutive OptEase filters (80 patients). Filters were placed at the same institution in patients with high-risk multiple trauma or those undergoing neurosurgery with contraindications to primary prophylaxis (70 patients in the Günther Tulip group and 44 in the OptEase group) and in patients with venous thromboembolism and contraindications to anticoagulation (22 patients in the Günthe

Optional vena cava filters: what, why, and when.

Optional vena cava filters can used to provide either short-term or permanent protection from pulmonary embolism. These devices have recently become available for clinical use in the United States. However, there remains a paucity of data about these devices and their outcomes. This article reviews current and future devices, the rationale behind non-permanent caval filtration, and the generally accepted guidelines for their clinical application.

Retrieval analysis of clinical explanted vena cava filters.

Vena cava filters are the most commonly used mechanical devices to prevent pulmonary embolism. A retrievable permanent filter has been available since 1999. That has allowed the direct study of thrombi captured in humans and the punctual interaction of blood and device at long and short term. Through traditional histologic methods, captured thrombi and the tissues formed around the filter were observed. An innovative environmental scanning electron microscopy technique allowed detection of micro- and nanosized foreign bodies inside thrombi and tissues, and chemical analysis could be carried ou

Retrievable inferior vena cava filters: initial clinical results.

Anticoagulation is the accepted therapy for patients with thromboembolic disease. When contraindications to anticoagulant therapy are present, however, interruption of the inferior vena cava (IVC) may prevent pulmonary embolism (PE). The objective of this study was to report our early technical and clinical results with retrievable IVC filters (IVCFs) for the prevention of PE. One hundred and twenty-seven multitrauma patients between December 1, 2002, and December 31, 2004, underwent placement of Gunther-Tulip (n = 49), Recovery (n = 41), or OptEase (n = 37) retrievable IVCFs under real-time i

Are temporary inferior vena cava filters really temporary?

BACKGROUND: Despite significant risk for venous thromboembolism, severely injured trauma patients often are not candidates for prophylaxis or treatment with anticoagulation. Long-term inferior vena cava (IVC) filters are associated with increased risk of postphlebitic syndrome. Retrievable IVC filters potentially offer a better solution, but only if the filter is removed; our hypothesis is that the most of them are not. METHODS: This retrospective study queried a level I trauma registry for IVC filter insertion from September 1997 through June 2004. RESULTS: One IVC filter was placed before

Prophylactic indications for vena cava filters: critical appraisal.

Vena caval filters (VCFs) were developed and initially used for therapeutic indications, primarily to prevent recurrence of pulmonary embolism (PE) or its occurrence in selected cases of deep venous thrombosis (DVT), where risk of PE was very high and anticoagulant therapy (AC Rx) was deemed ineffective, contraindicated by concurrent disease, or had to be discontinued because of complications. Prophylactic indications-where there was no DVT or PE but the risk of them was considered very high and AC Rx was contraindicated or considered ineffective-were invoked relatively infrequently at first,

Retrievable inferior vena cava filters: early clinical experience.

AIM: Multiple-trauma patients often have injuries that prevent the use of anticoagulant or sequential compression device prophylaxis. Temporary inferior vena cava filters (IVCFs) offer protection against pulmonary embolism (PE) during the early, highest-risk perioperative and immediate injury period, while avoiding potential long-term sequelae of a permanent IVCF. The objective of this study was to evaluate the efficacy of prophylactic, temporary IVCF placement at the intensive care unit (ICU) bedside under real-time intravascular ultrasound (IVUS) guidance in multiple-trauma patients. Recurrent thromboembolism in patients with vena cava filters.

BACKGROUND: Patients with venous thromboembolic disease are treated with anticoagulation or vena cava filter placement to prevent pulmonary embolism. A recent report suggested that filter placement may increase the risk of recurrent deep venous thrombosis (DVT) and prompted a review of our experience. METHODS: Prospectively collected data on 2109 consecutive patients receiving filters were evaluated for recurrent thromboembolism, vena cava occlusion, or venous stasis ulceration. Outcomes were stratified and analyzed according to the use of anticoagulants at the time of insertion and at follow

Thromboembolism: prevention and treatment with vena cava filters.

Mechanical approaches to the prevention of pulmonary thromboembolism have evolved from direct operative procedures to the percutaneous insertion of a variety of filter devices. The longest follow-up experience is with the Greenfield vena caval filter (Medi-Tech, Inc, Watertown, MA), which has a long-term vena caval patency rate of 98% and a recurrent embolism rate of 4%. Alternative filters, such as the Bird's Nest (Cook, Inc, Bloomington, IN), VenaTech (VenaTech, Evanston, IL), and Simon Nitinol (Nitinol Medical Technologies, Woburn, MA), are effective in the prevention of embolism but have a

[Vena cava filter -- is there an indication for implantation?].

The indication of vena cava filter implantation is controversially discussed. On the basis of the available literature and our own results a critical analysis of this issue is given. Between 1994 and 2003, we inserted a total of 29 vena cava filters; 24 temporary and 5 permanent filters. In twelve patients, the placement of the filter was indicated due to pulmonary embolism and a contra-indication to dose adjusted heparin therapy. Seven additional patients experienced a recurrent pulmonary embolism despite adequate heparin therapy. An additional prophylactic filter insertion was carried out in

Translumbar high inferior vena cava access placement in patients with thrombosed inferior vena cava filters.

Venous access is a dire necessity in some patients such as those with end-stage renal disease or short gut syndrome. The right internal jugular vein is the preferred entry site for tunneled central venous catheters. Alternatively, the left internal jugular is considered next, with the external jugular and subclavian veins being considered later. Catheter-related venous stenosis approaches 40% in certain sites, resulting in loss of access sites. As sites are lost, insertion of functional long-term central venous catheters becomes challenging. Translumbar inferior vena cava (IVC) access created

Inferior vena cava filter from left-sided superior vena cava.

We describe the unusual case of a 71-year-old male with a history of deep vein thrombosis and recurrent multiple pulmonary embolism (PE) despite adequate anticoagulation. Computed tomography (CT) and brachiocephalic venography revealed a left-sided superior vena cava. We describe successful placement of an inferior vena cava filter via a left-sided superior vena cava.

[Thrombosed temporary vena cava filter].

The application of temporary vena cava filters for the treatment of deep venous thrombosis of the lower extremity has become increasingly important in recent years. The filters are supposed to guarantee temporary protection from more extensive pulmonary embolism. Occlusion of the filter system by a larger embolus as well as vena cava thrombosis including the filter struts present major therapeutic problems. We report on one patient in whom the temporarily inserted filter was trapped in a large vena cava thrombus and had to be removed surgically by caval thrombectomy. Because of possible compli

Misplacement of a vena cava filter in the retroperitoneum.

An inferior vena cava filter was misplaced into the pericaval retroperitoneum. This was probably secondary to placement into and avulsion of the right spermatic vein. The filter delivery system was modified, and such misplacement should now be impossible.

Inferior Vena Cava Filter Litigation Review: An Analysis of Medicolegal Cases Pertaining to Inferior Vena Cava Filters.

PURPOSE: To identify and analyze all medical malpractice and product liability lawsuits pertaining to inferior vena cava (IVC) filters published within a well-recognized legal research database. MATERIALS AND METHODS: LexisNexis, a legal research database, was used to retrieve cases that mentioned harm from IVC filters, or lack thereof, as the cause for legal action. A total of 672 cases were analyzed for type of case (medical malpractice or product liability), filter model implanted, filter complications, court decisions, and settlement payments if any. RESULTS: Of 95 analyzed cases, 20 (21

Inferior Vena Cava Filters in Pregnancy: A Systematic Review.

During pregnancy, patients have an increased risk of venous thromboembolism (VTE). This is an important cause of maternal mortality. Inferior vena cava (IVC) filters can be used to prevent pulmonary embolism in complicated cases of VTE during pregnancy. The present systematic review includes all patients reported in the literature who had an IVC filter placed during pregnancy. The indications for IVC filters are discussed, along with practical considerations for placement during pregnancy, filter effectiveness, and maternal and fetal mortality and morbidity. IVC filters can be used safely when

Optional inferior vena cava filters in the trauma patient.

Trauma patients are at exceedingly high risk of development of venous thromboembolism (VTE) including deep venous thrombosis and pulmonary embolism (PE). The epidemiology of VTE in trauma patients is reviewed. PE is thought to be the third major cause of death after trauma in those patients who survive longer than 24 hours after onset of injury. In fact, patients recovering from trauma have the highest rate of VTE among all subgroups of hospitalized patients. Various prophylactic and surveillance methods have been evaluated and found helpful in certain situations, but VTE complications can occ
