A variation on the introducer technique for unlimited access to the subclavian vein.

An adaptation of the sheath set approach for permanent transvenous electrode placement has been carried out in fifteen patients. At the time of lead insertion, the sheath guide wire is left in place. If a second lead is desired or lead exchange becomes necessary, a second sheath is applied to the retained guide wire and a new lead inserted, thus avoiding a second subclavian puncture or new venous cutdown. This in effect offers a limitless access to the central venous system as long as the guide wire is left in place. This adaptation is especially suited for dual chamber pacing, where multiple

Use of an intercostal vein for central venous access in home parenteral nutrition: a case report.

A key factor in the success of a home parenteral nutrition program is prolonged and safe access to the central venous system. There are some patients, however, in whom the cephalic, external jugular, internal jugular, subclavian, and saphenous veins cannot be used. In these patients, cannulation of an intercostal vein can be useful for central venous access. Such a patient was recently presented to our medical care facility with bilateral subclavian vein thrombosis, left external iliac vein thrombosis, and superior vena cava obstruction above the level of the azygous vein. A Broviac catheter w

Pulmonary artery catheter placement with two-dimensional echocardiographic guidance: a technique applicable in critically ill neonates.

In nine critically ill neonates with persistent fetal circulation, femoral venous catheters were inserted at the bedside to initiate treatment and provide venous access. After femoral vein puncture or cutdown, a 5-F sheath was placed in the inferior vena cava through the femoral vein. With use of two-dimensional echocardiographic guidance, a 5-F balloon angiographic catheter was advanced through the inferior vena cava into the right atrium and subsequently manipulated through the tricuspid valve and into the main pulmonary artery. No major complications were attributable to the procedure. When

Vasoconstrictive responses of the cephalic vein during first-time cardiac implantable electronic device placement.

BACKGROUND: During cardiac implantable electronic device (CIED) implantation procedures cardiac leads have been mostly introduced transvenously. The associated injury to the selected vessel and adjacent tissues may induce reflex vasoconstriction. The aim of the study was to assess the incidence of cephalic vein (CV) vasoconstriction during first-time CIED implantation. MATERIALS AND METHODS: Of the 146 evaluated first-time CIED implantation procedures conducted in our centre in 2016, we selected those during which CV vasoconstriction was recorded. We focused on the stage of the procedure invo

[Diaphragmatic paralysis caused by a phrenic nerve lesion secondary to internal jugular vein cutdown].

We discuss the case of a premature baby weighing 1,200 g who developed right diaphragmatic paralysis when a venous cutdown was performed on his internal jugular vein. Acquired unilateral diaphragmatic paralysis has been associated with obstetric trauma of the cervical plexus and surgical procedures of the neck and thorax, when the phrenic nerve is injured. We suggest fluoroscopy as the best diagnostic method; early diaphragmatic plication reduces the morbimortality of these patients.

Insertion of totally implantable venous access devices: an expertise-based, randomized, controlled trial (NCT00600444).

OBJECTIVE: Comparison of two different insertion techniques for implantation of totally implantable access ports (TIAP). BACKGROUND: TIAP are introduced through different open and closed cannulation strategies and by various medical experts. The aim of this expertise-based randomized trial was to compare venous cutdown approach with puncture of subclavian vein. METHODS: One hundred and ten patients scheduled for primary implantation of a TIAP were randomly assigned to either open insertion technique performed by surgeons or puncture of the subclavian vein under fluoroscopic guidance by radio

Haemothorax--a source of error in emergency central vein cannulation.

Central venous cannulation is often performed in patients with penetrating wounds of the chest as an aid to diagnosis, and is sometimes also used as a volume infusion resuscitation line in an emergency. The traditional signs of successful central venous cannulation--free aspiration of blood from the exploring needle and inserted cannula, free infusion flow and backflow of blood when the set is lowered below the patient--may all give a false impression of correct placement if there is a significant haemothorax on the same side. Two cases are described where the cannula was sited in a pleural ca

Use of a split-sheath vein introducer for subclavian venipuncture in the placement of silicone catheters for chronic venous access.

The technique of venous cutdown for placement of silicone catheters can be difficult, uncomfortable for the patient, and very time-consuming. Adapting a technique used for the placement of permanent transvenous pacemaker leads, subclavian venotomy and a split-sheath dilator can be used for direct placement of large-bore catheters into the superior vena cava. This approach decreases patient discomfort and the likelihood of infectious or hemorrhagic complications of silicone catheter placement, and has greatly simplified and shortened this procedure.

Unusual Venous Access for Device Implantation.

BACKGROUND Cardiac implantable electronic devices (CIED) are mainstay therapy for a variety of patients with bradyarrhythmia as well as those at risk of sudden cardiac death and heart failure. At present, commonly used venous access are axillary, cephalic cutdown, and subclavian puncture. However, there are situations when these approaches cannot be employed because cannulation is not possible due to small size, spasm, absence, or occlusion of the vein. One of the alternative approaches is through an internal or external jugular vein. A jugular vein approach can be also used for upgrading CIED

Ultrasound-Guided Percutaneous Vein Access for Placement of Broviac Catheters in Extremely Low Birth Weight Neonates: A Series of 3 Successful Cases.

As medical and surgical interventions to support premature infants have evolved, the need for long-term vascular access in extremely low birth weight infants has increased. The classic approach to Broviac(®) (C.R. Bard, Covington, GA) catheter placement in very small neonates has been through an open surgical cutdown technique. Ultrasound guidance has emerged as a potentially beneficial method for obtaining central venous access in children and is being applied to smaller and smaller infants. This case series reports the feasibility of using ultrasound-guided percutaneous vein access to obtain

Cephalic Vein Cut-Down Method for Totally Implantable Venous Access Ports: A Single-Institution Experience.

BACKGROUND: The widespread use of chemotherapies has increased the need for totally implantable venous access ports (TIVAPs). Previously, the subclavian puncture approach with the landmark technique was the most used implantation method; however, it has been related to early complications such as pneumothorax, hemothorax, and arterial puncture. Therefore, a safer implantation method is required. This study aimed to assess the safety and efficacy of the cephalic vein cut-down method used in our institution. METHODS: Patients who underwent TIVAPs implantation using the cephalic vein cut-down me

Permanent central venous access devices.

The development of permanent central venous access devices in the last 20 years has been accompanied by decreased complication rates, improved patient comfort, and increased cost-effectiveness. Subclavian venous access, first applied to infants and small children in the early 1970s, has given way to the Silastic permanent right atrial catheter by both cutdown and percutaneous techniques. The disadvantages of an occlusive aseptic dressing, frequent catheter irrigation, and disturbance of body image led to development of the totally implantable venous access system, which has been successfully u

A cephalic vein cutdown and venography technique to facilitate pacemaker and defibrillator lead implantation.

The aim of this study was to assess the feasibility of a cephalic vein cutdown and venography technique for implantation of a pacemaker or ICD and to determine the causes of failure of cephalic vein cutdown. In consecutive patients who underwent pacemaker or ICD implants, a modified cephalic vein guidewire technique was performed. This technique was attempted in 289 pacemaker implants and 26 ICD implants (155 men, 160 women; mean age 74 +/- 10 years). The success rate for implantation of a single chamber and a dual chamber device by using this technique alone was 84% (54/64) and 74% (185/251),

[Cryoanesthesia by freon spray for venepuncture in children].

This prospective study was designed to assess the quality of skin analgesia provided by cryoanesthesia induced by a spray of freon (dichloro-tetrafluoro-ethane) for venous cannulation (22 or 24 Gauge cannulas). Eighty children between the ages of 5 and 15 years were allocated to two groups: 40 children had a conventional venepuncture, 40 others had a venepuncture under cryoanesthesia. The spray of freon was applied for ten seconds on the area of skin to be anaesthetised. The venous cannulation was carried out by an anaesthetist of the paediatric surgical unit. The intensity of pain at venepunc

Long-term central venous catheters in patients with acquired immunodeficiency syndrome.

BACKGROUND: As long-term vascular access becomes more prevalent among patients with AIDS, it is becoming more important to consider their potential complications. METHODS: One hundred two central venous access devices placed in 84 patients with AIDS were reviewed for septic and mechanical complications. Catheters were inserted by one surgeon by means of the cephalic vein cutdown technique. The sample included 88 implanted venous reservoir catheters (86.3%) and 14 tunneled central venous catheters (13.7%). RESULTS: Mean catheter life was 141 +/- 15 days. Total number of catheter days was 14,3

Placement of 10-gauge catheter by cutdown for rapid fluid replacement.

The establishment of immediate venous access and rapid fluid administration remains of paramount importance in the treatment of hypovolemic shock. We describe a technique for placement of a recently available 10-gauge catheter via venous cutdown. This technique is simpler and quicker than placing intravenous tubing directly into the vein, and we show that flow rates through the catheter with both saline and blood are equivalent to rates obtained through intravenous extension tubing. In addition, our studies show that the use of wide-bore intravenous tubing (urology irrigating tubing) instead o

External jugular vein cutdown approach, as a useful alternative, supports the choice of the cephalic vein for totally implantable access device placement.

BACKGROUND: Cephalic vein (CV) cut down for totally implantable venous access device (TIVAD) placement has been accepted as an alternative to the percutaneous subclavian vein approach. The aim of this retrospective study was to validate the external jugular vein (EJV) cut-down approach when the CV is not feasible. METHODS: Patients receiving a TIVAD from January 1995 to December 2003 were included in this study. Age, sex, surgical technique, disease, device used, length of the procedure, and morbidity were considered. RESULTS: A total of 427 TIVADs were placed in 425 patients: 253 men (59.5%

Cephalic vein cutdown for inserting indwelling subclavian vein catheters in gynecologic oncology patients.

To decide on an alternate procedure for placement of central vein catheters other than percutaneous access of the subclavian vein, we conducted a study of 46 women who underwent placement of a central venous catheter utilizing the cephalic vein procedure at Ruby Memorial Hospital at West Virginia University in Morgantown. Of these patients, 96% had a successful placement of a catheter into the subclavian vein following dissection of the cephalic vein within the deltopectoral groove. There were no cases of pneumothorax or injury to the great vessels during the surgical procedure, which is a sig

A prospective randomized trial of ultrasound- vs landmark-guided central venous access in the pediatric population.

BACKGROUND: The purpose of this prospective randomized study was to compare landmark- to ultrasound-guided central venous access when performed by pediatric surgeons. The American College of Surgeons advocates for use of ultrasound in central venous catheter placement; however, this is not universally embraced by pediatric surgeons. Complication risk correlates positively with number of venous cannulation attempts. STUDY DESIGN: With IRB approval, a randomized prospective study of children under 18 years of age undergoing tunneled central venous catheter placement was performed. Patient accru
