Pulmonary vascular diseases.

Diseases of the pulmonary vasculature are a cause of increased pulmonary vascular resistance (PVR) in pulmonary embolism, chronic thromboembolic pulmonary hypertension (CTEPH), and pulmonary arterial hypertension or decreased PVR in pulmonary arteriovenous malformations on hereditary hemorrhagic telangiectasia, portal hypertension, or cavopulmonary anastomosis. All these conditions are associated with a decrease in both arterial PO2 and PCO2. Gas exchange in pulmonary vascular diseases with increased PVR is characterized by a shift of ventilation and perfusion to high ventilation-perfusion rat

Acute severe asthma: pathophysiology and pathobiology of gas exchange abnormalities.

Acute severe asthma, or "status asthmaticus", is a devastating clinical condition ultimately resulting in life-threatening hypoxaemia. The pivotal intrapulmonary mechanism of this condition is profound ventilation/perfusion (V'A/Q') mismatch, characterized by a predominant bimodal blood flow pattern reflecting a marked deterioration (increase) of the dispersion of pulmonary blood flow. This V'A/Q' profile is consistent with the presence of numerous alveolar units with low V'A/Q' ratios, in which ventilation is markedly reduced, although never abolished, but perfusion is maintained. Further V'A

Visualization and functional consequence of pulmonary vascular impairment in patients with rheumatic diseases.

STUDY OBJECTIVES: Systemic rheumatic diseases impair the vasculature in lungs. The aim of this study was to visualize vascular impairment and determine its consequence on lung function. PATIENTS AND MEASUREMENTS: Seventy-two patients with rheumatic diseases were evaluated by pulmonary function tests, ventilation-perfusion scintigraphy, and thermography of the hands. The ventilation-perfusion mismatch of the lungs was visualized and quantified by calculating the ventilation-perfusion ratio (/) at each pixel. The microvascular impairment in fingers was assessed by the temperature difference bet

Early hypoxemia detection with minimal FiO₂: Putting routine hyperoxygenation in anesthesia practice to the test.

BACKGROUND: High inspired oxygen fractions (FiO₂) are routinely administered during mechanical ventilation in anesthesia in order to prevent hypoxemia. However, liberal oxygen use often exceeds physiological requirements, increasing the risk of oxygen toxicity and absorption atelectasis. Moreover, by sustaining normal-appearing oxygen saturation, high FiO₂ may obscure the presence of pulmonary congestion, edema, acute respiratory distress syndrome (ARDS), inadvertent endobronchial intubation, pneumothorax, and other conditions associated with impaired gas exchange. This approach may postpone t

Unilateral pulmonary oedema/atelectasis in the lateral decubitus position.

An obese male patient developed hypoxia, hyercarbia and radiological signs of pulmonary oedema/atelectasis in the dependent lung after surgery in the lateral decubitus position. This appears to have been due to ventilation-perfusion mismatch, although other factors were considered. The patient recovered following 36 hours of intermittent positive pressure ventilation of the lungs.

Does impaired diffusion for oxygen exist in diseased lungs?

The historical development of ideas concerning impairment of diffusion in the lungs (alveolar-capillary block) is described. The major publications by Riley and Cournand, and others, which showed that impairment of diffusion exists in some diseased lungs were based on careful measurements of the arterial oxygen tension or saturation of patients. These papers described work carried out in the State of New York. Other workers claim to have shown that impairment of diffusion does not exist in diseased lungs: the impairment of oxygen transfer in alveolar-capillary block is said to be due to uneven

Blood gas in exercise-induced bronchospasm: a review.

Exercise-induced bronchospasm (EIB) in some cases of asthma is related to hypocapnia, hypoxemia, and acidosis, but studies have shown that children do not develop as abnormal PCO2, PO2, or pH levels with the induction of EIB. Gradient changes of alveolar-arterial oxygen differences reveal ventilation perfusion abnormalities existing at the onset of exercise in those patients who do develop EIB.

Radiation dosimetry and safety issues in the investigation of pulmonary embolism.

When considering the investigation of the patient with possible pulmonary embolism, one needs to balance the likelihood of disease and the diagnostic utility of the test against the risks associated with the investigation. Both computed tomography pulmonary angiography (CTPA) and the ventilation/perfusion (V/Q) scan involve exposure to ionizing radiation. The effect of low-level ionizing radiation remains an issue of some controversy. CTPA delivers a greater effective dose and, in particular, greater doses to breast tissue, than the V/Q scan (typically 10-70 mGy for CTPA vs <1.5 mGy for V/Q to

[Mechanism of hypoxemia after methacholine challenge].

Hypoxemia after the treatment of asthmatic attack continues for a while, and can even become worse after the treatment. This hypoxemia has been attributed to inhomogeneity of ventilation-to-perfusion ratios in the lungs of asthmatic patients. We studied post-hyperventilation hypoxemia in subjects with hypoxemia after a methacholine challenge test. Nineteen subjects who were hyperresponsive and hypoxemic after a methacholine challenge were studied. The methacholine challenge with incremental doses was stopped when the respiratory resistance doubled. Thereafter, oxygen and carbon dioxide concent

Should inverse ratio ventilation be used in adult respiratory distress syndrome?

IRV-induced increases in MAP are clearly associated with shunt reduction, but we find no studies that show shunt reduction without increased end-expiratory alveolar pressure. On the other hand, various studies in humans with ARDS and hyaline membrane disease and animal models of acute lung injury indicate that shunt reduction does not occur with IRV if there is no increase in end-expiratory alveolar pressure (21), that shunt reduction is the same with IRV as with conventional ventilation with PEEP when there are comparable levels of end-expiratory volume or alveolar pressure (16, 32), and that

The pattern of breathing and the ventilatory response to breathing through a tube and to physical exercise in sport divers.

Well-trained divers can be expected to differ from healthy controls in their ventilatory response to breathing through a tube and to physical exercise. Therefore, we measured their minute ventilation (VE) at rest and during breathing through a tube combined with two levels of physical exercise (1 or 2 W.kg body weight-1). For breathing through a tube an additional dead space of 600 ml was used. All divers were trained in the breath-hold technique and in the use of the breathing apparatus. Their mean period of training as divers was 9 +/- 6 years. The approximate age of the subjects was 25 year

A 60-Year-Old Woman with a 6-Week History of Shortness of Breath and Intermittent Chest Pain Due to Chronic Thromboembolic Pulmonary Disease Undetected by Computed Tomography Pulmonary Angiography (CTPA) and Diagnosed by Ventilation-Perfusion Imaging.

BACKGROUND Chronic thromboembolic pulmonary disease (CTEPD) is the persistent occlusion of pulmonary arteries resulting from 1 or more thrombo-emboli. Its presentation is often non-specific, with exertional dyspnea and fatigue, yet if left undiagnosed risks of chronic thromboembolic pulmonary hypertension and right-sided cardiac failure can ensue. Computed tomography pulmonary angiography (CTPA) and ventilation/perfusion (V/Q) imaging are most commonly utilized for investigating CTEPD. This report is of a 60-year-old woman with a 6-week history of breathlessness and intermittent chest pain due
