Identifying gaps in women's preventive health services for women living with HIV in Vermont.

BACKGROUND: Women living with HIV (WLHIV) in the United States experience higher rates of death from cervical and breast cancer and lower screening rates for both compared to women without HIV. These disparities highlight the need to better understand access to and uptake of women's preventive health services-particularly cervical cancer screening, breast cancer screening, and HPV vaccination. Most existing data on these services among WLHIV come from urban settings; therefore, the purpose of this study was to investigate factors affecting adherence to these preventive health measures among WL

Geography but not alternative host species explain the spread of raccoon rabies virus in Vermont.

In North America, the raccoon-associated variant of rabies virus (RRV) is of special concern, given its relatively rapid spread throughout the eastern USA and its potential public health impact due to high raccoon host densities in urban areas. Northward expansion of this epizootic included an outbreak in the Canadian province of Quebec in 2006-2009 due to trans-border spread from the State of Vermont. To inform a more proactive approach to future control efforts, this study uses phylogenetic analyses to explore the role of geography and alternative carnivore hosts in the dynamics of RRV sprea

Caring for pregnant opioid abusers in Vermont: A potential model for non-urban areas.

Opioid addiction is no longer a primarily urban problem. As dependence on heroin and prescription pain relievers has become a significant issue in rural areas, the need for effective treatment of opioid-dependent pregnant women and their neonates has grown accordingly. In addition to the adverse perinatal outcomes associated with opioid addiction in pregnant women, the high costs of caring for these mothers and their babies motivate efforts to develop appropriate treatment models. We found that integration and coordination of services that promote maternal recovery and ability to parent are ke

The pathology of Legionnaires' disease. Fourteen fatal cases from the 1977 outbreak in Vermont.

Fourteen fatal cases from the 1977 Vermont outbreak of Legionnaires' disease have been analyzed. Serious underlying diseases were present in all patients. The only consistent lesions were in the lungs. Bronchopneumonia was present in all cases and was confluent in most. No lobe of the lung was preferentially involved and consolidation was usually bilateral. Abscesses were evident macroscopically in only two cases. Microscopically, there was an extensive alveolar infiltrate of polymorphonuclear neutrophils and macrophages. Lysis of the inflammatory cells was frequently present and was associate

Overcoming barriers to HPV vaccination in rural Vermont through a multicomponent peer-based approach.

Human Papillomavirus (HPV) causes almost all cervical cancers and many cancers of the anus, vagina, vulva, penis, and oropharynx. The HPV vaccine provides protection to all adolescents from a broad spectrum of cancers, yet HPV vaccination rates remain lower than those of other routine vaccines. Developing effective HPV vaccine interventions is particularly important in rural areas, whose residents have lower rates of HPV vaccination and higher cervical cancer incidence and mortality; however, interventional research in these populations is relatively limited. Furthermore, though rural areas ar

Notes from the Field: Increase in Eastern Equine Encephalitis Virus Activity - Vermont, 2023-2024.

Eastern equine encephalitis (EEE) virus (EEEV) is a mosquitoborne alphavirus maintained in an enzootic cycle with mosquitoes and birds. EEEV can be transmitted to humans and susceptible animals by mosquitoes that bite both mammals and birds. EEEV causes severe neuroinvasive disease in humans; although an EEE vaccine is available for horses, no human vaccine is currently licensed, and treatment is supportive. Approximately one third of human cases are fatal, and many survivors experience long-term neurologic sequelae (1). In the United States, a majority of EEE cases occur in states along the A

Patient centered medication treatment for opioid use disorder in rural Vermont: a qualitative study.

BACKGROUND: Opioid-related fatal overdoses are occurring at historically high levels and increasing each year. Accessible social and financial support are imperative to the initiation and success of treatment for Opioid Use Disorder (OUD). Medications for Opioid Use Disorder (MOUD) offer effective treatment but there are many more people with untreated OUD than receiving evidence-based medication. Patient-centered care is associated with increased care utilization for substance use disorders. This qualitative study explored the patient perspective of OUD care through a Patient-Centered Care (P

Cardiopulmonary resuscitation in the very low birth weight infant: the Vermont Oxford Network experience.

OBJECTIVE: The limited literature available to date suggests that the use of delivery room cardiopulmonary resuscitation (DR-CPR) is associated with very poor outcomes, especially for extremely low birth weight infants. We reviewed the cumulative experience of the Vermont Oxford Network to determine the actual utilization of DR-CPR and the neonatal outcomes of such infants. METHODS: A retrospective review of information available in the Vermont Oxford Network Database for the years 1994 to 1996. The data set was collected from 196 neonatal units who participate in the Network (data for infant

Geospatial Analysis of Rural-Urban Access of Otolaryngology-Head and Neck Surgeons and Clinics in Vermont.

OBJECTIVES: This study examines the distribution of Otolaryngology-Head and Neck Surgeons (OHNS) in Vermont to identify gaps in OHNS clinic accessibility between rural and urban areas and better understand the disparities faced by Vermonters. METHODS: A geospatial, population-based cross-sectional study was conducted, and the OHNS workforce was assessed using public databases. The ArcGIS Pro Near Analysis tool was used to assess the travel burden to OHNS clinics. Wilcoxon rank sum tests were used to compare the average distances to Comprehensive Otolaryngology Care Centers (COCC) and General

Resuscitation Opportunities for Fellows of Very Low Birth Weight Infants in the Vermont Oxford Network.

BACKGROUND: Neonatal-perinatal medicine (NPM) fellowship programs must provide adequate delivery room (DR) experience to ensure that physicians can independently provide neonatal resuscitation to very low birth weight (VLBW) infants. The availability of learning opportunities is unknown. METHODS: The number of VLBW (≤1500 g) and extremely low birth weight (ELBW) (<1000 g) deliveries, uses of continuous positive airway pressure, intubation, chest compressions, and epinephrine over 3 years at accredited civilian NPM fellowship program delivery hospitals were determined from the Vermont Oxford N

Assessment of medication for opioid use disorder as delivered within the Vermont hub and spoke system.

Opioid overdose deaths in the United States have risen dramatically in the past decade. In response to this public health crisis, Vermont created an innovative system called the "hub-and-spoke" (H & S) system, initiated in January 2013. The H & S system has 7 regional "hubs" that offer methadone and buprenorphine, as well as intensive support, and 77 local "spokes" (primary care settings) that offer buprenorphine (and naltrexone to a much lesser extent). Questionnaires were administered to 80 participants in the H & S system (stratified by geographic region, treatment site, and gender) and 20

Increased mammography use and its impact on earlier breast cancer detection in Vermont, 1975-1999.

BACKGROUND: A trend toward earlier breast carcinoma detection in the United States has been attributed to screening mammography, although direct evidence linking this trend to the increased use of mammography in a general population is lacking. This study examined the effects of mammography on tumor size and axillary lymph node metastasis in Vermont over 25 years. METHODS: Pathology and mammography data from 3499 Vermont women who were diagnosed with invasive breast carcinoma during 1975-1984, 1989-1990, and 1995-1999 were compared. Logistic regression analysis was used to estimate the effect

Longitudinal Analysis of a Statewide, Social Ecological Approach to Child Sexual Abuse Prevention in Vermont.

In 2009, Vermont became the first state to enact a comprehensive health education policy that included child sexual abuse (CSA) prevention. This study describes the implementation efforts of CSA prevention programming led by Prevent Child Abuse Vermont, both one year prior to the policy enactment and ten years later (i.e., 2008-2019). We used data from the National Child Abuse and Neglect Data System (NCANDS) to obtain rates of substantiated CSA reports and examined their association with program implementation. Findings from longitudinal hierarchical linear modeling indicated that, in seven V

Identifying Barriers to Access and Utilization of Preventive Health-Care Services by Young Adults in Vermont.

PURPOSE: The objective of this study was to examine barriers to accessing and utilizing routine preventive health-care checkups for Vermont young adults. METHODS: A population-based analysis was conducted using aggregated data from the 2011-2014 Behavioral Risk Factor Surveillance System (BRFSS) surveys of Vermont young adults aged 18-25 years (N = 1,329). Predictors analyzed as barriers were classified county of residence, health-care coverage, and annual household income level, as well as covariates, with the outcome of the length of time since the last routine checkup. RESULTS: A total of

Reduction of elevated blood lead levels in children in North Carolina and Vermont, 1996-1999.

BACKGROUND: Few studies have examined factors related to the time required for children's blood lead levels (BLLs) > or = 10 microg/dL to decline to < 10 microg/dL. OBJECTIVES: We used routinely collected surveillance data to determine the length of time and risk factors associated with reducing elevated BLLs in children below the level of concern of 10 microg/dL. METHODS: From the North Carolina and Vermont state surveillance databases, we identified a retrospective cohort of 996 children < 6 years of age whose first two blood lead tests produced levels > or = 10 microg/dL during 1996-1999.

Ultrastructure of lung in Legionnaires' disease. Observations of three biopsies done during the Vermont epidemic.

Open lung biopsies from three patients with Legionnaires' disease were examined by light and transmission electron microscopy. The patients had serious underlying disease. All developed a rapidly progressive pneumonia unresponsive to penicillin, oxacillin, and gentamicin. One patient, who received erythromycin, survived. Light microscopy in all three showed severe acute bronchopneumonia. The Legionnaires' disease bacterium was seen in tissue sections and confirmed by direct immunofluorescence. Transmission electron microscopy showed numerous rod-shaped intracellular organisms that were morphol

Substance use patterns, sociodemographics, and health profiles of harm reduction service recipients in Burlington, Vermont.

BACKGROUND: Understanding current substance use practices is critical to reduce and prevent overdose deaths among individuals at increased risk including persons who use and inject drugs. Because individuals participating in harm reduction and syringe service programs are actively using drugs and vary in treatment participation, information on their current drug use and preferred drugs provides a unique window into the drug use ecology of communities that can inform future intervention services and treatment provision. METHODS: Between March and June 2023, 150 participants in a harm reduction

Neurodevelopmental outcome of extremely low birth weight infants from the Vermont Oxford network: 1998-2003.

BACKGROUND: Physicians and parents face significant uncertainties when making care decisions for extremely low birth weight (ELBW) infants. Many published estimates of death and developmental outcome are from well-funded university programs and may not reflect outcomes of infants from a variety of settings. The best estimates of the probabilities of death and severe disability combine local experience and published data. OBJECTIVE: To describe the neurodevelopmental outcome of ELBW infants from centers of the ELBW Infant Follow-Up Group of the Vermont Oxford Network (VON) and to identify char

Analysis of Historical Worker Exposures to Respirable Dust from Talc Mining and Milling Operations in Vermont.

OBJECTIVES: Talc is mined and milled throughout the world for use in a variety of industrial and consumer products. Although prior studies have evaluated workplace exposures or health effects from talc operations, the primary emphasis of these investigations has been on certain mineral contaminants (e.g. crystalline silica and asbestos) rather than talc itself. The purpose of this analysis is to evaluate historical worker exposures to respirable dust (as a measure of talc exposures) in the Vermont talc mines and mills, which involved a relatively pure form of talc (i.e. no asbestos and <0.25%
