Are cervical spine radiographs of value in elderly patients with vertebrobasilar insufficiency?

Radiographs of the cervical spine of 32 elderly patients clinically diagnosed as having vertebrobasilar insufficiency secondary to cervical spondylosis were compared with 32 age- and sex-matched controls. The mean age was 77.6 years. There was no significant difference in the severity of the radiological changes between the two groups as judged by the narrowing of disc space and marginal osteophyte formation. It is concluded that there is no place for routine examination of the cervical spine in patients thought to have vertebrobasilar insufficiency secondary to cervical spondylosis.

Signs and symptoms of vertebrobasilar insufficiency secondary to atherosclerosis: a systematic review.

CONTEXT: Clinicians face a difficult challenge in identifying vertebrobasilar insufficiency (VBI) resulting from atherosclerosis. VBI is a term utilized to describe a reduction in blood flow to the vertebral and basilar arteries that supply the posterior cerebral system. For musculoskeletal clinicians, diagnostic differentiation of VBI is essential, because its presence directly impacts the clinical use of manual treatment interventions. Clinical guidelines provide a set of cardinal symptoms (inclusive of Coman's 5D's) in which VBI may manifest, the accuracy of which is under contestation beca

Treatment of vertebrobasilar insufficiency due to stenosis and aneurysm of the vertebral artery.

A dissecting aneurysm of the vertebral artery in the extracranial portion is a rare pathology. It may either have a symptom-free course or induce a clinical picture of vertebrobasilar insufficiency. To the main methods of surgical treatment belong endovascular techniques and resection of an aneurysm with shunting of the V3 segment of the vertebral artery. Presented in the article is a clinical case report regarding successful surgical management of a dissecting aneurysm of the extracranial portion in a young woman presenting with a clinical course of vertebrobasilar insufficiency and treated b

Noninvasive imaging of the cervical vertebral artery in the diagnosis of vertebrobasilar insufficiency.

The cervical vertebral arteries were visualized by real-time B-mode ultrasonography with duplex Doppler in 595 consecutive patients. Comparison with angiography in 40 patients showed a sensitivity of 84% and a specificity of 97% in identifying vertebral artery lesions. The incidence of vertebral artery lesions in patients with symptoms of ischemia referable to the vertebrobasilar distribution was 28% (24/84), significantly higher (p = 0.05) than the incidence of 13% (7/54) in patients with carotid artery insufficiency. Patients with completed strokes in the vertebrobasilar territory had an eve

Anastomosis of the superior thyroid artery to the vertebral artery for vertebrobasilar insufficiency.

Anastomosis of the superior thyroid artery to the cervical vertebral artery was performed with good results in a case of vertebrobasilar insufficiency caused by proximal occlusion of the vertebral artery. As a donor artery the superior thyroid artery is moderate in size, long, and easy to anastomose. It is emphasized that this cervical procedure is simpler and safer than the intracranial procedures. The authors believe this case to be the first in which the superior thyroid artery was used as a donor artery for anastomosis to the vertebral artery.

Basilar Artery Fenestration Versus Hypoglycemia-Induced Vertebrobasilar Insufficiency: A Case Report.

PURPOSE: Vertebrobasilar insufficiency (VBI) is a common transient neurological condition related to posterior circulation hemodynamic insufficiency. However, it is rarely seen as an initial presentation in basilar artery (BA) fenestration or hypoglycemia. We present this case to further clarify the association between BA fenestration and hypoglycemia-induced VBI, as well as the difficulty in diagnosis, especially in acute clinical settings. CASE REPORT: Herein, we report a case with BA fenestration, in which the patient suffered from transient episodes of focal neurological deficits, includi

Isolated dizziness in vertebrobasilar insufficiency: Clinical features, angiography, and follow-up.

Dizziness is the most common manifestation of vertebrobasilar insufficiency (VBI) and may occur as an isolated symptom in some patients. In order to determine if there is a characteristic pattern of vascular occlusion associated with this symptom, we reviewed the cerebral angiograms of seven patients (average age, 62 years) who presented with isolated episodes of dizziness. Six of seven had atherosclerotic occlusion of the vertebrobasilar junction between the takeoff of the posterior and anterior inferior cerebellar arteries. The other patient had basilar dolichoectasia without luminal narrowi

Vertebrobasilar insufficiency: evaluation by quantitative duplex flow measurements. A preliminary report.

With the use of duplex sonography, quantitative vertebral artery flow measurements have been made in 283 patients who had nonlocalizing symptoms of cerebral ischemia suggestive of vertebrobasilar insufficiency. Net vertebral artery flow was calculated by adding flows from the right and left sides. When net vertebral flow was greater than 200 ml/min, 89 of 148 patients (60%) were found to have a significant stenosis (greater than 50% diameter reduction) in one or both internal carotid arteries. Conversely, when net flow was less than 200 ml/min, 101 of 135 patients (75%) had normal or only mild

[Electronystagmography study in patients with the insufficiency of vertebrobasilar artery after betahistine treatment].

The aim of the study was objective assessment of the results of betahistyne treatment in patients with chronic insufficiency of the vertebrobasilar artery system by means of elektronystagmography (ENG). The patients had mostly vertigo and/or dizziness, headache, ear buzzing and disequilibrium sensations. In 46 patients clinical, audiological and ENG examinations were carried out. All symptoms decreased in frequency after the treatment. Vertigo and/or dizziness improvement was noted in 58.5% patients of cases, headache decreased in 56.5% of cases, ear buzzing decreased in 42.8% and hearing impr

Carotid-vertebral artery bypass with saphenous vein graft for symptomatic vertebrobasilar insufficiency.

OBJECTIVESymptomatic steno-occlusion of the proximal vertebral artery (VA) or subclavian artery (ScA) heralds a poor prognosis and high risk of stroke recurrence despite medical therapy, including antiplatelet or anticoagulant drugs. In some cases, the V2 segment of the cervical VA is patent and perfused via collateral vessels. The authors describe 7 patients who were successfully treated by external carotid artery (ECA)-saphenous vein (SV)-VA bypass.METHODSSeven cases involving symptomatic patients were retrospectively studied: 3 cases of V1 segment occlusion, 2 cases of severe in-stent reste

Hemodynamic vertebrobasilar insufficiency caused by multiple arterial lesions: results of surgical treatment.

From 1982 to 1990, 111 of 1013 patients undergoing cerebral artery reconstruction presented with signs of vertebrobasilar insufficiency associated with hemodynamically significant lesions of at least three cerebral arteries. There were 71 men and 40 women whose mean age was 70.3 +/- 8.4 years. Forty patients also had hemispheric symptoms, whereas three had ophthalmic symptoms as well. A total of 191 arteries were reconstructed in 139 procedures. During the first 30 postoperative days there were nine deaths (8.1%) attributable to four neurologic events--one myocardial infarction, two hemorrhage

Hemodynamic outcomes of stenting for vertebrobasilar insufficiency in patients with a low flow state.

IntroductionThe Vertebrobasilar Flow Evaluation and Risk of Transient Ischemic Attack and Stroke (VERiTAS) study determined patients with low flow in their vertebrobasilar (VB) system are at increased risk of recurrent stroke. Endovascular interventions such as angioplasty and stenting are reserved for patients with refractory symptoms; however, few series to date have demonstrated either hemodynamic or clinical outcomes in this high-risk patient group. We present our combined institutional series of patients with symptomatic VB atherosclerotic disease and low-flow state who underwent angiopla

Sudden bilateral simultaneous deafness with vertigo as a sole manifestation of vertebrobasilar insufficiency.

A 68 year old woman presented with bilateral sudden simultaneous hearing loss and transient spontaneous vertigo as a sole manifestation of vertebrobasilar insufficiency. Extensive investigation to exclude other causes was unremarkable. Magnetic resonance imaging of the brain, including diffusion images, showed no abnormalities. A magnetic resonance angiogram showed severe stenosis of the middle third of the basilar artery. A pure tone audiogram showed moderate sensorineural-type hearing loss bilaterally. The localisation and mechanism of an isolated cochleovestibular dysfunction are discussed.

Endovascular treatment of supra-aortic extracranial stenoses in patients with vertebrobasilar insufficiency symptoms.

PURPOSE: Stenoses and thromboses of vessels feeding the vertebrobasilar territory can evoke serious disturbances including ischemic stroke. We present our experience of endovascular interventions for patients with signs of vertebrobasilar insufficiency (VBI) resulted from subclavian, vertebral and brachiocephalic arterial stenoses. METHODS: Twenty-one patients (10 men) aged from 35 to 84 years (mean 64.3 years) with symptoms compatible with VBI underwent balloon angioplasty and stenting of subclavian (SA), innominate (IA) and vertebral (VA) arteries. Procedures were done by radiologists exper

Homonymous quadrantic visual field defect resulting from vertebrobasilar insufficiency: report of a case.

A patient showing a congruous homonymous superior quadrantanopsia was found at autopsy to have infarction of the striate cortex. The lesion involved the inferior bank of the calcarine fissure almost entirely and approximately one fifth of the superior bank. Additionally, infarctions of the visual association cortex and the pontine tegmentum were found. No lesions in the corresponding temporal lobe were present. Two types of visual field defects have been described in association with occipital lobe lesions: a monocular temporal crescentic defect if the lesion is anterior and a congruous homony

Posterior communicating artery visualization in predicting results of carotid endarterectomy for vertebrobasilar insufficiency.

Retrospectively, 51 patients who related two or more signs or symptoms of vertebrobasilar insufficiency and possessed concomitant carotid arterial disease were identified. The patient population was separated into two groups based on the presence or absence of an angiographically visualized posterior communicating artery during selective carotid artery injection. All patients had undergone unilateral or staged bilateral carotid endarterectomy. Comparison of postoperative clinical outcome with intracerebral angiographic findings exhibited statistically significant improvement in favor of those

[Differential diagnosis of vertigo in functional cervical vertebrae joint syndromes and vertebrobasilar insufficiency].

The disequilibrium caused by a functional disturbance of the upper cervical spine is compared with the disequilibrium caused by the vertebrobasilar insufficiency. Anamnesis, otoneurological and manual examination enable the differentiation of the cervical vertigo, what is not only of theoretical interest but above all has therapeutic consequences.
