[Vestibular and auditory functions in Ménière's disease].

This paper is, aiming at understanding disturbances of and correllation between colchear and vestibular functions in Meniere's disease. Auditory and vestibular functions in 121 Meniere cases (70 males and 51 females, age ranging from 19 to 63 years), diagnosed according to the AAO-HNS criteria, were analysed. The results were as follows: 1)There was positive correlation between semicircular canal paresis and hearing impairment. 2) There was statistically significant positive correlation between hearing impairment and the -SP/AP ratio. 3) There was statistically significant correlation between

Diagnosis of vestibular Menière's disease with electrocochleography.

The existence of vestibular Menière's disease, once considered a variant of classic Menière's disease, but without hearing loss, has been questioned because of lack of objective evidence that endolymphatic hydrops is involved with the disease process. Transtympanic electrocochleography (TT ECoG) has emerged as a useful tool for electrophysiologic monitoring of the inner ear, and is especially valuable in assessing endolymphatic hydrops. A retrospective chart review was performed to identify those patients with a diagnosis consistent with vestibular Menière's disease in order to determine the p

Vestibular autorotation testing of Menière's disease.

Ten patients with previously confirmed diagnosis of acute-stage Meniere's disease were tested with the Vestibular Autorotation Test (VAT), a portable computerized test that measures the higher-frequency vestibulo-ocular reflex (VOR) to compute gain and phase. Patients were asked to fixate on a target and move their heads in synchrony with a computer-generated, sweep-frequency audible cue (0.5 to 6.0 Hz) through an 18-second test epoch. Head movements in both horizontal (yaw) and vertical (pitch) planes were used separately to test horizontal and vertical VORs, respectively. All subjects easily

[Histopathology of the vestibular nerve in Menière's disease].

Histological and morphometrical investigations of the vestibular nerve were performed on surgical specimens from 19 patients suffering from Ménière's disease and on 5 autopsy preparations from patients without inner ear disease. The number of lipofuscin granules and vacuoles in the ganglion cells of specimens from Ménière patients was not increased compared with control specimens of a similar age, nor did the amount of endoneuronal connective tissue in the vestibular nerve specimens differ between the two groups. The density of capillaries in Scarpa's ganglion and vestibular nerve was signific

Vestibular studies in Pelizaeus-Merzbacher disease.

Pelizaeus-Merzbacher disease is a rare sex-linked recessive neurological disorder. It presents with a slowly progressive neurological deterioration. It is liable to be misdiagnosed as cerebral palsy. A family history of similar disease is a clue to the diagnosis. A patient with this condition has been followed over many years. Nystagmus was noted to be present shortly after birth. Audiovestibular investigations were performed. All tests of peripheral hearing were normal. ABR indicated abnormal brainstem conduction. Several electronystagmographic abnormalities were present. These were of a cent

[Auditory and vestibular lesions in Behcet's disease].

Auditory and vestibular function tests were conducted in 16 patients with Behcet's disease. Vestibular lesions included spontaneous nystagmus in 5 cases, and a bilateral vestibular syndrome after provoked tests in 13 cases (7 of the deficiency type and 6 irritative in nature). Findings after auditory tests were bilateral symmetric deafness in 2 cases; one of the perception type and the other showing mixed features. Various etiopathogenic theories are discussed. The possibility of a bilateral central lesion of the VIIIth nerve was also considered, 8 patients in the series having cerebral manife

Cochleo-vestibular correlations in Meniere's disease.

The cochlear and vestibular functions were investigated in a sample of 36 patients with unilateral Meniere's disease. Caloric reactions and hearing thresholds were compared separately at several frequencies. A topodiagnostic relationship between the cochlear and vestibular function was discovered. Using four qualitative categories, a significantly high correlation was obtained between the basal hearing loss (4000 Hz) and unilateral weakness, whereas no correlation was obtained when evaluating the more apical hearing loss (lower frequencies 250-1 000 Hz). A normal caloric reaction can be reason

Tomography of the vestibular aqueduct in ear disease.

A controversy exists concerning whether or not roentgenographic narrowing or nonvisualization of the vestibular aqueduct is a specific sign for Meniere's disease. Of 190 ears that were evaluated, abnormal aqueducts were seen in 42.9% of ears with Meniere's disease, 45.4% of contralateral, noninvolved ears from patients with Meniere's disease, 41.3% of ears with diseases other than Meniere's disease, 30.4% of ears with no disease, and 51.6% of normal ears. Narrowing or nonvisualization of the vestibular aqueduct is a nonspecific roentgenographic sign that is seen in diseased, as well as in norm

[Vestibular schwannoma. Incidence of the disease and the consequences].

During the period 1977-1996 142 vestibular schwannomas in 138 patients in a well-defined population in the County of Aarhus, Denmark were registered. The incidence of vestibular schwannomas changed from approximately six tumours pr. million inhabitants in the years 1977-1981 to 18 tumours pr. million in the period 1992-1996. Most tumours were operated, a smaller group of vestibular schwannomas were observed by repetitive tomographic imaging, due to small tumour size, advanced age, poor health conditions or the patient's refusal of operation. Whether the strategy for the treatment of vestibular

Vestibular rehabilitation strategies in Meniere's disease.

Once the episodic spells of vertigo associated with Meniere's disease have abated, vestibular rehabilitation exercises play an important role in promoting adaptation to the decreased vestibular input. Outlines of different treatment approaches for three types of patients with Meniere's disease are presented. The exercise approaches advocated here consist of exercises to foster adaptation of the vestibular system in cases of unilateral loss or hypofunction and exercises to promote the substitution of alternative strategies and to enhance remaining function in cases of bilateral vestibular hypof

Development of the vestibular aqueduct in Menière's disease.

The external aperture width of the vestibular aqueduct was measured in living subjects of various ages, with normal and diseased ears. Measurements were made from 3-dimensional reconstruction of CT images, and the developmental process was studied. The following results were obtained. i) In Menière's disease the external aperture was hypoplastic and its width significantly smaller than that in normal individuals and those with chronic otitis media, including children. ii) There were no significant differences between normal ears and ears with chronic otitis media in either adults or children.

[A vestibular model of the course of Menière's disease].

Based on directional preponderance (DP) of the caloric nystagmus of 36 patients with Ménière's disease (MD), 57 calorigrams are divided in three groups: with ipsilateral, contralateral and without DP (neutral). Each calorigram is an unique moment in the course of MD. The age of the patient at that moment, the time since the first symptoms and since the last vertiginous spell, as well as the percentages of caloric response reduction and of DP are recorded. There are significant (Mann-Whitney U-test) differences between duration of the MD, time since the last spell and DP of the ipsilateral grou
