Management of senescent gynecomastia in the Veterans Health Administration.

Senescent gynecomastia is common in Veterans Health Administration patients because of clinical characteristics in the population, mainly obesity and the use of numerous etiologic drugs. To assess management of the disease, we studied patients treated between January 1, 2003 and December 31, 2007, in the Veterans Affairs-Tennessee Valley Healthcare System. Four hundred and fifty-four cases of senescent gynecomastia were retrieved from 670 charts addressing breast complaints (positive predictive value = 68%). Average patient age was 68.5 years. Mean body mass index was 29.7. Ninety-two percent

Positive Mental Health Screens in the Veterans Health Administration, 2021 to 2024.

IMPORTANCE: Prevalence of mental health conditions is rising, but there have been few national reports in recent years. The Veterans Health Administration (VA) is the nation's largest institution providing mental health services and provides a fertile ground for assessing rates of mental health conditions. OBJECTIVE: To examine the trends in the risk of 4 mental health conditions in the VA using mandated mental health screens. DESIGN, SETTING, AND PARTICIPANTS: In this cohort study, unadjusted and adjusted rates of positive screens from fiscal year (FY) 2021 to FY 2024 were examined for depr

The influence of physical and mental health symptoms on Veterans' functional health status.

Veterans who have been deployed to combat often have complex medical histories including some combination of traumatic brain injury (TBI); mental health problems; and other chronic, medically unexplained symptoms (i.e., chronic multisymptom illness [CMI] clusters). How these multiple pathologies relate to functional health is unclear. In the current study, 120 Veterans (across multiple combat cohorts) underwent comprehensive clinical evaluations and completed self-report assessments of mental health symptoms (Patient Health Questionnaire-2 [PHQ-2], PTSD Checklist-Civilian Version [PCL-C]) and

Increasing Goals of Care Discussions Among Veterans With Cirrhosis: A Veterans Health Administration Quality Improvement Initiative.

INTRODUCTION: Goals of care discussions (GCDs) are recommended for patients with cirrhosis but rarely conducted. Since 2017, the Veterans Health Administration has provided a standardized note template that clinicians can use to document GCDs for Veterans with serious illness. We aimed to quantify the frequency and timing of GCDs in this population, along with the factors associated with GCDs and their content. METHODS: This retrospective cohort study identified Veterans with cirrhosis from January 2017 to July 2022 and captured GCDs until July 2023. We assessed the association between demogr

Posttraumatic Growth in U.S. Military Veterans: Results from the National Health and Resilience in Veterans Study.

Despite increasing recognition that positive psychological changes or posttraumatic growth (PTG) may develop after highly stressful or traumatic events, contemporary population-based data on the epidemiology of PTG in high-risk samples such as U.S. military veterans are lacking. Additionally, in light of emerging evidence suggesting an 8-factor model of posttraumatic stress disorder (PTSD) symptoms, an up-to-date characterization of how these symptom clusters relate to PTG can help inform efforts to help promote PTG. Data were analyzed from the 2019-2020 National Health and Resilience in Veter

Psychological well-being and alcohol misuse among community-based veterans: results from the Veterans' Health Study.

BACKGROUND: Maladaptive drinking is an increasing concern among military policy makers and healthcare providers. The goal of this study was to assess how social and psychological factors relate to alcohol problems among post-deployed US veterans and how problematic drinking is associated with well-being. METHODS: Data were collected via a telephone survey from a random sample of veterans receiving their healthcare from a large non-VA hospital system in central Pennsylvania (N = 1730). Interviewers inquired about participants' current alcohol consumption, using the CAGE and AUDIT-C scales, and

Antipsychotic Medication Prescriptions for Homeless and Unstably Housed Veterans in the Veterans Affairs Health Care System.

OBJECTIVE: Many homeless and unstably housed (HUH) adults have severe mental illnesses that are managed with antipsychotic medications. The US Department of Veterans Affairs (VA) is the largest provider of homeless services, but there has been little study of psychotropic medication prescriptions for HUH veterans. METHODS: Using national VA administrative data in 2017, rates and characteristics associated with prescriptions for antipsychotic medications for veterans using VA health care services (N = 2,882,993), including HUH veterans (n = 266,855), were analyzed. RESULTS: Among HUH veterans

Utilization of Care Outside the Veterans Affairs Health Care System by US Veterans With Rheumatoid Arthritis.

OBJECTIVE: Many veterans enrolled in Veterans Affairs (VA) health care systems also receive care through other health care systems. Both VA and non-VA health care use must therefore be considered when conducting research in this population. This study characterized dual-care utilization in veterans with rheumatoid arthritis (RA) and explored associations with RA disease activity. METHODS: Through a questionnaire mailed to RA patients at 3 VA sites, veterans reported medical services by non-VA primary care and subspecialty providers, comorbidities, non-VA medications, and hospitalizations. Examining the Interfacility Variation of Social Determinants of Health in the Veterans Health Administration.

INTRODUCTION: Recently, numerous studies have linked social determinants of health (SDoH) with clinical outcomes. While this association is well known, the interfacility variability of these risk favors within the Veterans Health Administration (VHA) is not known. Such information could be useful to the VHA for resource and funding allocation. The aim of this study is to explore the interfacility variability of 5 SDoH within the VHA. METHODS: In a cohort of patients (aged ≥ 65 years) hospitalized at VHA acute care facilities with either acute myocardial infarction (AMI), heart failure (HF), o
