Choices of treatment for patients with estrogen receptor (ER) positive metastatic breast cancer refractory to first-line endocrine therapies include progestins, antiadrenal drugs, and androgens. Vincristine-sulfate-related vocal cord paralysis has been reported infrequently in the literature. Vindesine is a semisynthetic vinca alkaloïd. The short plasma half-life suggested that continuous infusion can improve the therapeutic results by maintaining a constant plasma level. Hyponatremia is a common complication among cancer patients. Certain antineoplastic agents have been associated with syndrome of inappropriate secretion of anti-diuretic hormone-induced hyponatremia. The most common agents associated with secretion of anti-diuretic hormone are vinca alkaloids, platinum compounds, and alkylating agents. We report a case of secretion of anti-diuretic hormone associated with tyrosine kinase inhibitors.

We present an elderly patient with metastatic breast cancer and Parkinson's disease (PD) of moderate severity who developed paralytic ileus of protracted course after a low dose of 0.16 mg/m(2) intravenous vincristine. Since colonic myenteric plexus involvement occurs in PD, we suggest that it may predispose to vinca alkaloids-induced autonomic neuropathy. Vincristine should be used with caution in this clinical setup.

The neurotoxicity of the vincristine is well known, however, cranial neuropathy is not widely recognized. We describe a child with acute lymphoblastic leukemia who developed vincristine-induced bilateral vocal cord paralysis. Vocal cord paralysis resolved spontaneously upon withdrawal of the vincristine. Vinca-alkaloid-induced vocal cord paralysis is a potentially dangerous but reversible condition.

The advantages and limitations, theory, and factors in the use of electric current to deliver drugs across the skin are discussed. Application of the technique in the delivery of local anaesthetics, antibiotics/antibacterials, mydriatics, vinca alkaloids, non-electrolytes, biogenic amines, peptides, and other agents is reviewed. The enzymatic basis for quinine 1 biosynthesis was investigated. Transcriptomic data from the producing plant led to the discovery of three enzymes involved in the early and late steps of the pathway. A medium-chain alcohol dehydrogenase (CpDCS) and an esterase (CpDCE) yielded the biosynthetic intermediate dihydrocorynantheal 2 from strictosidine aglycone 3. Additionally, the discovery of an O-methyltransferase specific for 6'-hydroxycinchoninone 4 suggested the final step order to be cinchoninone 16/17 hydroxylation, methylation, and keto-reduction.

Cardiac toxicity following the administration of chemotherapeutic agents is well documented. Vinca alkaloids, as well as high-dose cyclophosphamide, have been associated with myocardial ischemia. The present report describes a case of acute myocardial infarction occurring in a patient with no antecedent cardiac history who received both vincristine and conventional chemotherapeutic doses of cyclophosphamide for the treatment of chronic lymphocytic leukemia. Physicians should possess a heightened awareness of this potentially serious complication.
