Obesity and pelvic organ prolapse: Review.

INTRODUCTION: Obesity is a major public health challenge with substantial effects on women's health and quality of life. Pelvic organ prolapse (POP), characterized by the descent of pelvic organs into the vaginal canal, is influenced by both obesity-related mechanical forces and molecular alterations. This review aimed to evaluate the relationship between obesity and POP, with particular emphasis on etiopathogenesis, clinical consequences, and implications for prevention and treatment strategies. METHODS: This was a narrative review of the literature examining the relationship between obesity

Gamma-irradiated amniotic membrane graft as an adjuvant to posterior colporrhaphy: an exploratory prospective single-arm clinical trial.

BACKGROUND: Posterior compartment prolapse is a frequent pelvic floor disorder. While conventional posterior colporrhaphy is widely used, the recurrence rates and limitations in terms of tissue support remain concerns. Human amniotic membrane represents a promising biological scaffold with potent regenerative properties. This study aimed to evaluate the feasibility, safety and effectiveness of using amniotic membrane as a graft in posterior colporrhaphy. METHODS: This pilot one-arm clinical trial was conducted on women with posterior compartment prolapse at Ain Shams University Hospital durin

[The surgical treatment of urinary stress incontinence following hysterectomy or operation for genital prolapse (author's transl)].

Thirty patients suffering from stress incontinence of urine following hysterectomy or operation for genital prolapse underwent operation after study of the colpocystogram. This examination, practically painless and free of risk, visualises pelvic visceral kinetics and is felt by the author to be essential. It confirms the existence of stress incontinence, reveals the usual cause, detects associated abnormalities and indicates the appropriate surgical technique. The method of treatment was paramedial or medial fixation of the uterus, vagina or residual cervix to the public promontory, complemen

Anatomical conditions for pelvic floor reconstruction with polypropylene implant and its application for the treatment of vaginal prolapse.

OBJECTIVE: The purpose of the surgical treatment of vaginal prolapse is not only the restoration of the anatomy but also of the visceral functioning. To maintain the quality of life for patients with recurrent vaginal prolapse, to reduce the failure rates of operations and to avoid a colpectomy or a colpocleisis at the same time, synthetic materials have been introduced in transvaginal reconstructive surgery of the pelvic floor. The TVM Group from France described the reconstruction of the pelvic floor with polypropylene implants in 2004. The aim of this study is to determine the anatomical po

Normal vulvovaginal, perineal, and pelvic anatomy with reconstructive considerations.

A thorough insight into the female genital anatomy is crucial for understanding and performing pelvic reconstructive procedures. The intimate relationship between the genitalia and the muscles, ligaments, and fascia that provide support is complex, but critical to restore during surgery for correction of prolapse or aesthetic reasons. The external female genitalia include the mons pubis, labia majora and minora, clitoris, vestibule with glands, perineal body, and the muscles and fascia surrounding these structures. Through the perineal membrane and the perineal body, these superficial vulvar s

Role of perioperative low dose vaginal oestrogens in improving the outcomes of pelvic organ prolapse surgery.

Pelvic organ prolapse (POP) is the most common gynaecological disorder requiring surgical treatment in postmenopausal women. Surgical treatment of POP might include anterior or posterior vaginal repair, vaginal hysterectomy, vault fixation procedures like sacrocolpopexy and sacrospinous fixation. Complications of POP surgery include excessive bleeding, visceral injuries, postoperative wound infection, urinary tract infection (UTI), sexual dysfunction secondary to vaginal scarring and recurrence. Postmenopausal vaginal atrophy may increase the risk of visceral injuries due to thinning of vagina

Colonic and anorectal dysfunction in a patient with the tethered cord syndrome.

Although visceral dysfunction is commonly encountered in the tethered cord syndrome, alteration of intestinal motor and sensory function has not yet been specifically addressed in this neurological disorder. We report the case of a 54-yr-old woman with a rare form of adult onset tethered cord syndrome who presented to us for preoperative assessment of marked constipation, urinary and fecal incontinence, and a rectal prolapse. It is shown that the tethered cord syndrome causes complex clinical patterns of bowel dysfunction and that careful efforts are mandatory to differentiate their underlying

Laparoscopic assisted proctosigmoidectomy with extracorporeal transanal anastomosis. A pilot study.

Laparoscopic colon resections have often required an abdominal incision to remove the specimen and perform the anastomosis. Our aim was to mobilize the left colon and rectum using the laparoscope and perform a perineal proctosigmoidectomy with a primary end-to-end anastomosis. In eight pigs we used the operating laparoscope to mobilize the left colon, to ligate the inferior mesenteric artery at its origin, to ligate the inferior mesenteric vein as it crossed the left colic artery, and to fully mobilize the rectum. The rectum and sigmoid colon were then prolapsed through the anal canal, transec

Primary soft tissue hydatidosis: aspiration cytological diagnosis in two cases.

The most common sites of cysts caused by hydatid disease are liver and lung. In the absence of visceral involvement, the disease rarely affects musculoskeletal and soft tissues. In such locations, hydatid cyst mimics commoner soft tissue tumors. A 12-year-old girl presented with a gradually increasing painless right shoulder swelling. With a clinical diagnosis of lipoma, fine needle aspiration cytology was performed, which showed cytomorphological features of a hydatid cyst. Another patient, a 20-year-old man, presented with a right paravertebral soft tissue swelling. Radiological investigatio

Hepatocolonic vagrancy: wandering liver with colonic abnormalities.

Abnormalities of hepatic fixation resulting in excessive mobility in a transverse plane are uncommonly encountered. The unusual incidental finding of a freely mobile liver and spleen in a patient presenting with sigmoid volvulus is reported. At laparotomy, the inferior aspect of the right hemidiaphragm was smoothly peritonealized, without evidence of coronary or triangular ligaments. It is postulated that this abnormal hepatic mobility reflects persistence of the primitive ventral mesogastrium. To the authors' knowledge, this unusual condition has not previously been recognized. The literature

[Chest pain: differential diagnosis in general practice].

In patients with chest pain somatic pain (thoracic wall pain) has to be differentiated from visceral pain (organ pain). History and careful physical examination are diagnostic in most cases. Presented are rare and not well-known diseases like valvular aortic stenosis, idiopathic hypertrophic subaortic stenosis and the mitral valve prolapse syndrome. Not seldom they are masked by angina pectoris-like symptoms, although in general the coronary arteries are normal. In acute chest pain differential diagnostic considerations have to include lung embolism, acute pericarditis, spontaneous pneumothora

Concomitant Hysterectomy and vNOTES-Assisted Sacrocolpopexy: A Feasible and Safe Scarless Approach for Apical Prolapse Repair.

Background/Objectives: Durable apical support after hysterectomy is crucial to prevent subsequent vaginal vault prolapse. Abdominal sacrocolpopexy remains the gold standard but carries risks of visceral injury and wound morbidity. The vaginal natural orifice transluminal endoscopic surgery (vNOTES) approach provides a scarless, minimally invasive alternative, but data on vNOTES-assisted sacrocolpopexy (vNOTES-SC) performed concurrently with hysterectomy remain limited. Methods: A retrospective cohort of 30 women with stage II uterine prolapse underwent concomitant hysterectomy and vNOTES-assis

The Posterior Fornix Syndrome Is an Important Differential Diagnosis for Interstitial Cystitis/Bladder Pain Syndrome.

BACKGROUND: The catalyst for this analysis was a statement by Dr. Magnus Fall at a meeting of invited experts in 2020 of IC/BPS (interstitial cystitis/bladder pain syndrome): "A paradigm shift in the understanding of IC/BPS is urgent." This review analyses such a paradigm shift. SUMMARY: The paradigm shift began with the serendipitous cure of Hunner's lesion in a woman with a diagnosis of posterior fornix syndrome (PFS) and uterine prolapse. Retrospective analysis of surgical data from eight PFS studies reporting pain and urge cure found PFS was consistent with IC/BPS definitions. No Hunner's

Pain, distress and joint hyperlaxity.

Pain dominates the lives of many patients with hyperlaxity syndromes, most commonly the Benign Joint Hypermobility Syndrome (BJHS/EDS). As a result they may experience psychosocial problems, which in many cases severely affects their healthy functioning. Above all is the overriding chronic pain in joints, muscles and ligaments, which arises from an inherent predisposition to the effects of everyday trauma, but other factors such as associated osteoarthritis or fibromyalgia are also important. There may also be neurophysiological factors at play producing nociceptive enhancement. Pain and distr

Vaginal vascular malformation mimicking pelvic organ prolapse requiring serial embolizations.

Vaginal vascular malformation (VVM) is rare. There are, in fact, less than ten cases reported to date. VVM often presents as a mass protruding from the vagina, mimicking pelvic organ prolapse (POP). It can coexist with POP, thereby usually exaggerating the severity of POP. We report a case of VVM in a premenopausal woman who presented as severe POP and urinary incontinence. The diagnosis was confirmed with computed tomography (CT) scan and angiography. The patient underwent conservative management with embolization. These procedures had to be repeated three times in 1.5 years due to lesion rec

Laparoscopic apical mesh excision for deep dyspareunia caused by mesh banding in the vaginal apex.

BACKGROUND: Gynecare Prolift has been successfully used for pelvic floor repair with favorable objective and subjective outcomes. There have been, however, increasing reports of significant rates of postoperative dyspareunia and impairment of sexual function. MATERIALS AND METHODS: We are presenting two cases of post Prolift dyspareunia. The patients underwent several vaginal revisions with excision of the apical bands. However, they returned soon with the same complaint. Ultimately, laparoscopic excision of the apical mesh was performed. RESULTS: Patients reported significant improvement in
