Predicting clinical visual acuity of presbyopia treatments.

PURPOSE: To predict the potential clinical visual acuity of different presbyopia treatments. METHODS: The defocus transfer function (DTF) is a mathematical technique for simultaneously illustrating the optical transfer function for all levels of defocus. Through-focus clinical visual acuity measurements were superimposed onto the DTF to determine the corresponding retinal contrast and map out a modulation threshold curve. The process is performed for an apodized diffractive and a zonal refractive intraocular lens, as well as an opaque annular ring implant that extends depth of focus. RESULTS

Contrast visual acuity with bifocal contact lenses.

PURPOSE: To compare the quality of vision of a bifocal rigid gas-permeable contact lens versus a bifocal soft contact lens in subjects with presbyopia. METHODS: Sixteen healthy presbyopic subjects with no ocular disease but experienced with contact lens wear were enrolled in the study. Subjects randomly wore both types of bifocal lenses for 30 minutes with a washout period of 30 minutes each in a crossover manner. The primary outcome measure was the difference in contrast visual acuity. Distance and near contrast visual acuities with the two bifocal lens types were measured in different light

Longitudinal changes in visual acuity in keratoconus.

PURPOSE: The present investigation aimed to identify factors that predict reduced visual acuity in keratoconus from a prospective, longitudinal study. METHODS: This report from the Collaborative Longitudinal Evaluation of Keratoconus (CLEK) Study used 7 years of follow-up data from 953 CLEK subjects who did not have penetrating keratoplasty in either eye at baseline and who provided enough data to compute the slope of the change over time in high- or low-contrast best-corrected visual acuity (BCVA). Outcome measures included these slopes and whether the number of letters correctly read decrea

Visual acuity and X-linked color blindness.

PURPOSE: Optimal sampling for visual acuity requires a fine array of cones with identical sensitivity. Thus, dichromats, whose inner fovea is made up of cones having the same spectral sensitivity, may have better than normal visual acuity. We investigated this by comparing the visual acuities of trichromats and X-linked dichromats, while taking into account the different molecular genetics underlying the disorder. METHODS: Our subjects were age- and refraction-matched groups of normals (n=8) and X-linked dichromats (n=13). The dichromats (four protanopes and nine deuteranopes) were genotyped

Visual acuity in children with colobomatous defects.

Examining a child with a colobomatous defect can be challenging. The most important predictor of visual outcome is identification of normal foveal anatomy. The clinician should first evaluate the child for the associated systemic conditions. The next step is to identify the presence of normal foveal structures in the involved eye(s). If present, a trial of amblyopia treatment should be initiated. Anisometropic amblyopia is common and should be addressed. Retinal detachments are common and add uncertainty to predicting visual acuity.

Components of visual acuity loss in strabismus.

Strabismus, the misalignment of the visual axis of one eye relative to that of the other eye, reduces visual acuity in the affected eye. Several processes contributing to that loss are: amblyopia, which results in a chronic acuity loss whether or not the fellow eye is viewing; strabismic deviation, which shifts the image of an acuity target onto more peripheral, and therefore less acute, retina when the fellow eye fixates; interocular suppression and binocular masking, which reduce visibility in the strabismic eye due to neural influences from the other eye. We measured the losses due to these

Sinusitis associated with disorders of visual acuity.

We report a case of sinusitis associated with disorders of visual acuity. The patient's vision was dramatically improved by sinus surgery despite the fact that the changes in the ethmoidal cells were slight and had no direct observable relation to the optic nerve canal. Possible mechanisms involved in nasal optic neuritis are discussed.

Progression of visual acuity after penetrating keratoplasty.

A consecutive series of 721 eyes was followed for visual acuity changes after keratoplasty in four groups: keratoconus, Fuchs' dystrophy, pseudophakic bullous keratopathy with retained intraocular lenses, and aphakic/pseudophakic bullous keratopathy with secondary implants during keratoplasty. Follow-up ranged from 12 to 84 months. Keratoconus eyes showed the quickest recovery of visual acuity: by 12 months, 91% attained a best-corrected vision of 20/40, and the mean lines of visual acuity for the group plateaued thereafter. The other three groups showed continuing improvement in vision throug

Decreased visual acuity from optic disc drusen.

Although visual field defects occur commonly from optic disc drusen, reduction in visual acuity is rare. We describe four patients with reduced visual acuity and one with visual field constriction to the point of legal blindness that resulted from optic disc drusen. The acute episodic and stepwise nature of visual loss in some of the cases in our series suggests a vascular mechanism. Reduced visual acuity from drusen remains a diagnosis of exclusion. It should be considered, however, when visual loss has been acute and nonprogressive or stepwise in a nerve fiber bundle distribution and when vi

Visual acuity of infants with ocular diseases.

We tested the visual acuities of 110 infants with ocular disorders by a preferential-looking technique that provides fast, accurate measurements for preverbal children. The results were used to confirm initial clinical evaluations and to prescribe and monitor treatment. The test was particularly helpful in assessing the effects of occlusion therapy. The child sat on a parent's lap 50 cm away from a black partition containing two circular screens. A grating was projected on one screen and a homogeneous field was projected on the other. A hidden observer watched the infant's eye and head movemen

THE RELATION BETWEEN VISUAL ACUITY AND ILLUMINATION.

1. An apparatus for measuring the visual acuity of the eye at different illuminations is described. The test object is continuously variable in size and is presented at a fixed distance from the eye in the center of a 30 degrees field. Observation of the field is through an artificial pupil. The maximum intensity obtainable is more than enough to cover the complete physiological range for the eye with white light though only 110 watts are consumed by the source. Means for varying the intensity over a range of 1:10(10) in small steps are provided. 2. The relation of visual acuity and illuminati

[Subjective decrease of visual acuity after photodynamic therapy: documentation by optical coherence tomography and its correlation with visual acuity].

CASE REPORT: A 68-year-old patient complained of decreased visual acuity (VA) two days after photodynamic therapy (PDT) had been performed. VA decreased from the pre-operative value of 0.4 to 0.1 on the second postoperative day. With additional + 1.75 diopters, VA was 0.32. Optical coherence tomography (OCT) disclosed a subneuroretinal fluid accumulation causing a serous retinal detachment 600 microm in height. One week postoperatively, neither decreased vision nor retinal elevation were noted. DISCUSSION: The temporary subjective decreased VA could be predominantly reduced by adequate refrac
