Forced vital capacity (FVC) as an indicator of survival and disease progression in an ALS clinic population.

In a large cohort of 1034 patients with the diagnosis of definite or probable amyotrophic lateral sclerosis (ALS), the association of forced vital capacity (FVC) at baseline with (a) time to progression of 20 points in Appel ALS (AALS) score or (b) tracheostomy free survival was investigated. The median survival of ALS patients with baseline FVC <75% was 2.91 years, compared with 4.08 years for patients with baseline FVC >75% (p<0.001). Patients with baseline FVC <75% progressed more rapidly (taking 8.0 months to progress 20 AALS points) compared with patients with baseline FVC >75% (10.0 mont

Comparison of vital capacity induction with sevoflurane to intravenous induction with propofol for adult ambulatory anesthesia.

UNLABELLED: We compared vital capacity inhaled induction (VC) with sevoflurane with i.v. induction with propofol for adult ambulatory anesthesia. Patients were randomly assigned to receive either 8% sevoflurane in 75% N2O/O2 from a primed circuit (VC, 32 patients) or propofol 2-mg/kg bolus (i.v., 24 patients). Times to loss of consciousness (response to command) and induction side effects (airway, hemodynamic, motor) were assessed. Anesthesia was maintained with sevoflurane/N2O via a face mask for both groups. At the end of surgery, recovery times were measured and psychomotor function tests w

The repeated measurement of vital capacity is a poor predictor of the need for mechanical ventilation in myasthenia gravis.

OBJECTIVE: Testing the hypothesis that, in myasthenia gravis (MG), repeated measurements of vital capacity (VC) and various parameters derived from this measurement [median or lowest value of measured VCs during hospitalization, VC values < 20 ml/kg body weight (BW) or < 13 ml/kg BW, or an index assessing the variability of VC values during the whole ICU stay] could predict the need for intubation and mechanical ventilation (MV), as has been shown in other neuromuscular diseases with respiratory failure. DESIGN: Retrospective study with medical chart revision of all the patients with MG and r

Clinical features of idiopathic pleuroparenchymal fibroelastosis with progressive phenotype showing a decline in forced vital capacity.

BACKGROUND: Idiopathic pleuroparenchymal fibroelastosis (IPPFE) is heterogeneous, with some patients showing a progressive decline in forced vital capacity (FVC). However, the clinical features of these cases with progressive phenotypes remain unknown. METHODS: This retrospective study included 48 patients diagnosed with IPPFE who underwent longitudinal pulmonary function tests at our institution from 2005 to 2021. The progressive phenotype was defined as a relative decline of ≥10% in %FVC within two years from diagnosis of IPPFE, and its clinical features were evaluated. RESULTS: Of the 48

Successful surgery for a neuromuscular scoliosis patient by pulmonary rehabilitation with forced vital capacity below 30.

A rare case of a 15-year-old male patient with neuromuscular scoliosis with forced vital capacity (FVC) below 30%, who went through a successful surgery without any pulmonary complications, is reported herein. The patient had obvious asymmetric shoulders and poor exercise tolerance. The Cobb's angle of the main thoracic curve was 62.8°, and FVC in sitting position was 18% of predictive value. After skull traction and pulmonary rehabilitation, the FVC was still below 30%, and he finally went through surgery under this serious condition. By early pulmonary rehabilitation using home ventilator, h

Vital Capacity Impairment due to Neuromuscular Disease and its Correlation with Diaphragmatic Ultrasound: A Preliminary Study.

The purpose of this pilot study was to evaluate the correlation between diaphragmatic excursion measured by a right sub-costal ultrasound approach and forced vital capacity in patients with amyotrophic lateral sclerosis (ALS) or myotonic dystrophy (MD). All patients referred for pulmonary function testing underwent ultrasonic measurement of diaphragmatic excursion during quiet breathing, voluntary sniffing (Esniff) and forced breathing (EDEmax). Forty-five patients were included, mainly for amyotrophic lateral sclerosis or myotonic dystrophy. There was a significant correlation between EDEmax

Changes in forced vital capacity over time in systemic sclerosis: application of group-based trajectory modelling.

OBJECTIVE: An accelerated rate of decline in forced vital capacity (FVC) affects >50% of patients with SSc but data on the variability and determinants of this change are scarce. We sought to identify trajectories of FVC and their associated variables in SSc patients over a 12-year period. METHODS: Clinical and pulmonary function data were retrospectively collected. SSc patients with three or more FVC values were included. Group-based modelling was used to cluster similar FVC patterns into trajectories. Baseline variables were associated with the trajectories using multinomial logistic regres

Recovery of Percent Vital Capacity by Breathing Training in Patients With Panic Disorder and Impaired Diaphragmatic Breathing.

Slow diaphragmatic breathing is one of the therapeutic methods used in behavioral therapy for panic disorder. In practice, we have noticed that some of these patients could not perform diaphragmatic breathing and their percent vital capacity was initially reduced but could be recovered through breathing training. We conducted a comparative study with healthy controls to investigate the relationship between diaphragmatic breathing ability and percent vital capacity in patients with panic disorder. Our findings suggest that percent vital capacity in patients with impaired diaphragmatic breathing

Acute Exacerbation and Decline in Forced Vital Capacity Are Associated with Increased Mortality in Idiopathic Pulmonary Fibrosis.

RATIONALE: Exploration of FVC as it relates to mortality in idiopathic pulmonary fibrosis (IPF), a chronic, progressive, and ultimately fatal parenchymal lung disease, is important both clinically and to the current drug development paradigm. We evaluated the association between FVC decline and mortality in what is to our knowledge the largest well-characterized placebo cohort to date. Additionally, we sought to explore the risk of death caused by acute exacerbations and to further validate previously identified baseline predictors of mortality. OBJECTIVES: To validate and further characteriz

Contribution of the diaphragmaticus muscle to vital capacity in fasting and post-prandial American alligators (Alligator mississippiensis).

The importance of the diaphragmaticus muscle to vital capacity was investigated in juvenile American alligators by transection of this muscle. In both fasting and post-prandial animals a pneumotach was used to study vital capacity that was stimulated by either a hypercapnic-anoxic gas mixture or a hypercapnic-normoxic gas mixture in two types of control groups of animals (a shamoperated group and a group receiving no treatment) and in the experimental (transected) group. Transection did not significantly reduce vital capacity or affect time of inspiration or expiration in fasted animals. For b

Baseline disease activity index and forced vital capacity predict clinically meaningful complications and mortality in systemic sclerosis.

OBJECTIVE: The objective of this study was to evaluate whether the modified disease activity index (mDAI) predicts subsequent disease progression and organ damage in SSc. METHODS: We analysed 227 patients diagnosed with SSc who were enrolled in a prospective SSc registry. Baseline disease activity was assessed using the modified EUSTAR disease activity index (mDAI), with active disease being defined as an mDAI of ≥2.5. The primary outcome was a composite of disease-related clinically meaningful complications-based on revised CRISS step 1 events (interstitial lung disease progression, precapil

Prognostic significance of forced vital capacity decline prior to and following antifibrotic therapy in idiopathic pulmonary fibrosis.

BACKGROUND: Idiopathic pulmonary fibrosis (IPF) is a progressive and fatal interstitial lung disease (ILD). Currently, two antifibrotic drugs are available for reducing forced vital capacity (FVC) decline in IPF. However, many pulmonologists wait before initiating treatment, especially when IPF patients have stable disease. This study aimed to investigate the impact on survival outcome of FVC decline and a slow rate of FVC decline prior to and following treatment with these two antifibrotic drugs. METHODS: Out of the 235 IPF patients treated with antifibrotic therapy that were screened, 105 c

Effect of graded water immersion on vital capacity and plasma volume in patients with cervical spinal cord injury.

STUDY DESIGN: A case-control study was conducted. OBJECTIVE: The aim of the study was to test the hypothesis that the increase in hydrostatic pressure on the abdominal wall may be the major determinant of vital capacity (VC) improvement in tetraplegic subjects during water immersion, despite the blood volume shifts from the legs and abdomen to the thorax. SETTING: The study was carried out in the Rehabilitation Center, Brasília/DF, Brazil. METHODS: In total, 11 men with tetraplegia (complete motor lesion, C4-7, 30.4 years old) and 12 healthy controls were studied. Hematocrit level and spiro

Safeguarding vitality and cognition: The role of sarcopenia in intrinsic capacity decline among octogenarians from multiple cohorts.

BACKGROUND: Sarcopenia and intrinsic capacity (IC) declines pose significant challenges to healthy aging, particularly in the rapidly growing octogenarian population. This study aimed to elucidate the relationship between sarcopenia and declines in IC across multiple cohorts of community-dwelling older adults. METHODS: Data from four Taiwanese cohorts were analyzed. Sarcopenia was diagnosed based on the Asian Working Group for Sarcopenia (AWGS) 2019 criteria (algorithm 1: categorized as either having possible sarcopenia or not (robust); algorithm 2: categorized as robust, possible sarcopenia

Effect of chest and abdominal wall mobility and respiratory muscle strength on forced vital capacity in older adults.

This study investigated by sex the effect of chest and abdominal wall mobility and respiratory muscle strength on forced vital capacity (FVC) in older adults. FVC, respiratory muscle strength, and chest and abdominal wall mobility were assessed in 154 community-dwelling older adults. As an index of the chest and abdominal wall mobility, chest (upper and lower chest) and total (upper chest, lower chest, and abdomen) scale values were measured using the breathing movement scale. Subjects with a restrictive spirometric pattern showed significantly lower chest and total scale values in both sexes,

Percent vital capacity predicts postoperative sarcopenia after esophagectomy in initially nonsarcopenic esophageal cancer patients: a retrospective cohort study.

PURPOSE: The development of sarcopenia after esophagectomy is reported to affect the outcomes of patients with esophageal cancer (EC); however, the characteristics of patients likely to be predisposed to postoperative sarcopenia have not been defined. This study explores the associations between preoperative respiratory function and surgery-induced sarcopenia in EC patients confirmed as nonsarcopenic preoperatively. METHODS: The subjects of this retrospective review were 128 nonsarcopenic patients who underwent esophagectomy for EC. We took body composition measurements and performed physical

A Machine-Learning Method of Predicting Vital Capacity Plateau Value for Ventilatory Pump Failure Based on Data Mining.

Ventilatory pump failure is a common cause of death for patients with neuromuscular diseases. The vital capacity plateau value (VCPLAT) is an important indicator to judge the status of ventilatory pump failure for patients with congenital myopathy, Duchenne muscular dystrophy and spinal muscular atrophy. Due to the complex relationship between VCPLAT and the patient's own condition, it is difficult to predict the VCPLAT for pediatric disease from a medical perspective. We established a VCPLAT prediction model based on data mining and machine learning. We first performed the correlation analysi
