Vital signs during complication-free recovery after major surgery: a retrospective analysis.

Continuous vital sign monitoring (CVSM) is superior to intermittent manual vital sign assessment in detecting deviations, but alert thresholds have limited evidence, as a strong definition of physiologically normal variations are lacking. Consequences of too few alerts are missed detections of oncoming complications, and too many alerts result in over-alerting and over-treatment. This study aimed to describe duration and deviation of vital signs in postoperative patients without complications when monitored continuously. Adult patients admitted to the general ward after major surgery were incl

Promoting Sleep for Neurology Inpatients: The Value of Routine Overnight Vital Signs.

Background and objectivesSleep is a critical component of health and recovery. Awakening sleeping patients for routine vital sign monitoring overnight is a common inpatient practice, but its benefits are uncertain. This study asked 3 questions: (1) How often are routine overnight vital signs abnormal? (2) How often are those abnormal vital signs clinically actionable? (3) Are there patients who may not need routine overnight vital sign monitoring?MethodsWe analyzed observational retrospective data extracted from electronic health records of patients admitted to inpatient neurology floors at ou

Vital signs preceding in-hospital cardiac arrest: a matched case-control study.

BACKGROUND: Hospitalised patients undergo routine vital sign measurements to detect clinical deterioration. Research into vital signs prior to in-hospital cardiac arrest is crucial, as early detection of deterioration can enable timely intervention and potentially prevent cardiac arrest. This study aimed to investigate the association between vital signs and the risk of in-hospital cardiac arrest. METHODS: We conducted a registry-based, matched case-control study, including cases with in-hospital cardiac arrest and hospitalised patients as controls, in the Central Denmark Region from 2017 to

Establishing outcome-driven vital signs ranges for children in the prehospital setting.

BACKGROUND: Vital signs are frequently used in pediatric prehospital assessments and guide protocol utilization. Common pediatric vital sign classification criteria identify >80% of children in the prehospital setting as having abnormal vital signs, though few receive lifesaving interventions (LSIs). We sought to identify data-driven thresholds for abnormal vital signs by evaluating their association with prehospital LSIs. METHODS: We evaluated prehospital care records for children (<18 years) transported to the hospital during 2022 from a large, national repository of emergency medical servi

Development and validation of novel sepsis subphenotypes using trajectories of vital signs.

PURPOSE: Sepsis is a heterogeneous syndrome and identification of sub-phenotypes is essential. This study used trajectories of vital signs to develop and validate sub-phenotypes and investigated the interaction of sub-phenotypes with treatment using randomized controlled trial data. METHODS: All patients with suspected infection admitted to four academic hospitals in Emory Healthcare between 2014-2017 (training cohort) and 2018-2019 (validation cohort) were included. Group-based trajectory modeling was applied to vital signs from the first 8 h of hospitalization to develop and validate vitals

Vital Signs: Hepatitis C Treatment Among Insured Adults - United States, 2019-2020.

INTRODUCTION: Over 2 million adults in the United States have hepatitis C virus (HCV) infection, and it contributes to approximately 14,000 deaths a year. Eight to 12 weeks of highly effective direct-acting antiviral (DAA) treatment, which can cure ≥95% of cases, is recommended for persons with hepatitis C. METHODS: Data from HealthVerity, an administrative claims and encounters database, were used to construct a cohort of adults aged 18-69 years with HCV infection diagnosed during January 30, 2019-October 31, 2020, who were continuously enrolled in insurance for ≥60 days before and ≥360 days

A Novel Photoplethysmography Sensor for Vital Signs Monitoring from the Human Trachea.

Current pulse oximeter sensors can be challenged in working accurately and continuously in situations of reduced periphery perfusion, especially among anaesthetised patients. A novel tracheal photoplethysmography (PPG) sensor has been developed in an effort to address the limitations of current pulse oximeters. The sensor has been designed to estimate oxygen saturation (SpO2) and pulse rate, and has been manufactured on a flexible printed circuit board (PCB) that can adhere to a standard endotracheal (ET) tube. A pilot clinical trial was carried out as a feasibility study on 10 anaesthetised p

Vital Signs: HIV Transmission Along the Continuum of Care - United States, 2016.

BACKGROUND: In 2016, an estimated 1.1 million persons had human immunodeficiency virus (HIV) infection in the United States; 38,700 were new infections. Knowledge of HIV infection status, behavior change, and antiretroviral therapy (ART) all prevent HIV transmission. Persons who achieve and maintain viral suppression (achieved by most persons within 6 months of starting ART) can live long, healthy lives and pose effectively no risk of HIV transmission to their sexual partners. METHODS: A model was used to estimate transmission rates in 2016 along the HIV continuum of care. Data for sexual and

Prehospital continuous vital signs predict need for resuscitative endovascular balloon occlusion of the aorta and resuscitative thoracotomy prehospital continuous vital signs predict resuscitative endovascular balloon occlusion of the aorta.

BACKGROUND: Rapid triage and intervention to control hemorrhage are key to survival following traumatic injury. Patients presenting in hemorrhagic shock may undergo resuscitative thoracotomy (RT) or resuscitative endovascular balloon occlusion of the aorta (REBOA) as adjuncts to rapidly control bleeding. We hypothesized that machine learning along with automated calculation of continuously measured vital signs in the prehospital setting would accurately predict need for REBOA/RT and inform rapid lifesaving decisions. METHODS: Prehospital and admission data from 1,396 patients transported from

A case of hemorrhage at the junctions of the posterior intercostal arteries-a vital sign?

The authors present the case of a 58-year-old man found hanging from a radiator by his shoelaces. The time of death was approximately 6 h before the body was discovered. An autopsy was performed approximately 24 h after the body was found, which revealed hemorrhages in the thoracic aorta at the junctions of the posterior intercostal arteries. Before autopsy, a routine whole-body CT scan was performed. Histologic examination of the aorta and the posterior intercostal arteries revealed a fresh hemorrhage into the tunica adventitia of the aorta. To our knowledge, there is no case description of s

Plethysmographic Peripheral Perfusion Index: Could It Be a New Vital Sign?

The plethysmographic peripheral perfusion index (PPI) is a very useful parameter with various emerging utilities in medical practice. The PPI represents the ratio between pulsatile and non-pulsatile portions in peripheral circulation and is mainly affected by two main determinants: cardiac output and balance between sympathetic and parasympathetic nervous systems. The PPI decreases in cases of sympathetic predominance and/or low cardiac output states; therefore, it is a useful predictor of patient outcomes in critical care units. The PPI could be a surrogate for cardiac output in tests for flu

Inhaled cyanide poisoning as a vital sign in a room fire victim.

The corpse of a 71-year-old woman was found on the floor of her smoke-filled room. The source of the fire was the mattress of a double bed on which newspapers had apparently been set aflame. The woman's history in conjunction with the finding situation suggested an act of suicide. No signs of soot inhalation or soot swallowing were found at autopsy. Other vital signs were absent. Severe cardiac disease was the most notable pre-existing medical condition. Although the concentration of COHb in heart blood was low (3%), the concentration of cyanide was found to be 4.3mg/l in heart blood and 1.9mg

Gait Medicine: an interdisciplinary field centered on walking as a vital sign.

Walking speed predicts survival, cognitive decline, and disability in older adults with a precision comparable to that of blood pressure-yet no single discipline organizes itself primarily around walking. The result is a clinical vacuum: when walking deteriorates from the convergence of weak feet, eroding confidence, and an unwalkable neighborhood, patients have no specialist to consult and clinicians have no integrative framework to apply. We propose Gait Medicine as an interdisciplinary field organized around a three-layer model-somatic (foot structure, neuromuscular function, sensory-percep

Childhood Obesity as a Pediatric Vital Sign: A Population Health Initiative.

The high prevalence of obesity; the disparities in the risk of obesity based on race, ethnicity, and socioeconomic status; and the serious short- and long-term effects of childhood obesity led to the selection of obesity as a Pediatric Vital Signs (PVS) indicator. This report describes the approach (multisector collaborations, guidance form the enhanced chronic model), interventions (multisector interventions to involve clinicians, community organizations, data systems, policy advocates, children and families), and measures (proportion of children with overweight and obesity seen in a health s

Continuous vital sign monitoring on surgical wards: The COSMOS pilot.

STUDY OBJECTIVES: Alerts for vital sign abnormalities seek to identify meaningful patient instability while limiting alarm fatigue. Optimal vital sign alarm settings for postoperative patients remain unknown, as is whether alerts lead to effective clinical responses reducing vital sign disturbances. We conducted a 2-phase pilot study to identify thresholds and delays and test the hypothesis that alerts from continuous monitoring reduce the duration of vital sign abnormalities. DESIGN: Two-phase pilot. PATIENTS: 250 adults having major non-cardiac surgery. SETTING: Surgical wards. INTERVENT

National Trends in Vital Sign Abnormalities at Arrival to the Emergency Department.

INTRODUCTION: Recent reports suggest rising intensity of emergency department (ED) billing practices, sparking concerns that this may represent up-coding. However, it may reflect increasing severity and complexity of care in the ED population. We hypothesize that this in part may be reflected in more severe manifestations of illness as indicated by vital sign abnormalities. METHODS: Using 18 years of data from the National Hospital Ambulatory Medical Care Survey, we conducted a retrospective secondary analysis of adults (>18 years). We assessed standard vital signs using weighted descriptive
