Pharmacologic Characterization of LTGO-33, a Selective Small Molecule Inhibitor of the Voltage-Gated Sodium Channel NaV1.8 with a Unique Mechanism of Action.

Discovery and development of new molecules directed against validated pain targets is required to advance the treatment of pain disorders. Voltage-gated sodium channels (NaVs) are responsible for action potential initiation and transmission of pain signals. NaV1.8 is specifically expressed in peripheral nociceptors and has been genetically and pharmacologically validated as a human pain target. Selective inhibition of NaV1.8 can ameliorate pain while minimizing effects on other NaV isoforms essential for cardiac, respiratory, and central nervous system physiology. Here we present the pharmacol

Pharmacogenetics of myotonic hNav1.4 sodium channel variants situated near the fast inactivation gate.

Sodium channel myotonia and paramyotonia congenita are caused by gain-of-function mutations in the skeletal muscle voltage-gated sodium channel hNav1.4. The first-line drug is the sodium channel blocker mexiletine; however, some patients show side effects or limited responses. We previously showed that two hNav1.4 mutations, p.G1306E and p.P1158L, reduce mexiletine potency in vitro, whereas another sodium channel blocker, flecainide, is less sensitive to mutation-induced gating defects. This observation was successfully translated to p.G1306E and p.P1158L carriers. Thus, the aim of this study

A designer saxitoxin-engineered sodium channel pair as a general tool for subtype-selective inhibition.

We have used small molecule chemical design and protein engineering to fashion an orthogonal ligand-channel pair that enables acute and selective inhibition of a genetically encoded voltage-gated sodium channel (NaV). A modified ligand based on the nerve toxin, saxitoxin (STX), binds potently and reversibly to a NaV bearing two unique amino acid mutations in the outer mouth of the channel (Site 1). The same ligand is ineffective at blocking wild-type NaVs and does not disrupt action potential signals in neuronal cells or brain tissue at working concentrations. The versatility of this tool deri

Ligand-based pharmacophore modeling for the discovery of NaV1.7 inhibitors.

Voltage-gated sodium channel NaV1.7 is a key mediator of electrical excitability and signal transmission in peripheral nociceptors and has emerged as a highly attractive therapeutic target for the development of novel analgesic agents. However, the development of selective NaV1.7 inhibitors has been characterized by significant challenges, with repeated failures in clinical trials despite encouraging preclinical data. In this study, we developed and validated a series of ligand-based pharmacophore models (LBPMs) that can be useful for the discovery of novel NaV1.7 inhibitors with improved sele

Structural basis for inhibition of the voltage-gated sodium channel NaV1.7 by the tarantula toxin HWTX-I.

The voltage-gated sodium channel NaV1.7 is a pivotal therapeutic target for the developing novel, potent, and specific analgesics. HWTX-I, a peptide isolated from the venom of the tarantula Ornithoctonus huwena, is known to inhibit N-type calcium channels and has also been demonstrated to block tetrodotoxin-sensitive NaV channels in dorsal root ganglion neurons. In this study, we show that HWTX-I potently inhibits NaV1.7 currents with an IC50 value of 40.6 ± 16.1 nM, but it does not significantly affect the voltage dependence of NaV1.7 steady-state activation and inactivation. Evaluation of an indirect NaV1.7 inhibitor as adjunctive analgesic in burn-related neuropathic pain in a cat.

Burn-related neuropathic pain (BRNP) can arise following burn-induced nerve damage, affects approximately 6% of burned human patients and can result in chronic pain. Although widely studied in humans, data on BRNP or its treatment in animals is lacking. A 4-year-old domestic shorthair cat was presented with an infected, non-healing wound suspected to be a caustic burn. Initial treatments included surgical debridement, antimicrobials, and corticosteroids, but the cat developed persistent pruritus leading to self-inflicted trauma. Despite various interventions, including prednisone, chlorampheni

Discovery of pyridyl urea sulfonamide inhibitors of NaV1.7.

NaV1.7 is an actively pursued, genetically validated, target for pain. Recently reported quinolinone sulfonamide inhibitors displayed promising selectivity profiles as well as efficacy in preclinical pain models; however, concerns about off-target liabilities associated with this series resulted in an effort to reduce the lipophilicity of these compounds. Successful prosecution of this strategy was challenging due to the opposing requirement for lipophilic inhibitors for NaV1.7 potency and in vivo clearance (CL). Deconstruction of the heterocyclic core of the quinolinone series and utilization

A hydrophobic loop of the spider-venom peptide Tl1a drives activity at NaV1.8.

Voltage-gated sodium (NaVs) channels are pore-forming transmembrane proteins that regulate the influx of sodium ions across cell membranes. Spider venoms are a rich source of NaV-modulating peptides with high selectivity and potency, making them important tools for understanding NaV structure and function. NaV1.8 is tetrodotoxin-resistant, expressed in the peripheral nervous system and contributes to the propagation of action potentials in nociceptive neurons, making it a potential therapeutic target for pain. We identified Tl1a, a 36 amino acid residue peptide isolated from the crude venom of

Block of Voltage-Gated Sodium Channels by Aripiprazole in a State-Dependent Manner.

Aripiprazole is an atypical antipsychotic drug, which is prescribed for many psychiatric diseases such as schizophrenia and mania in bipolar disorder. It primarily acts as an agonist of dopaminergic and other G-protein coupled receptors. So far, an interaction with ligand- or voltage-gated ion channels has been classified as weak. Meanwhile, we identified aripiprazole in a preliminary test as a potent blocker of voltage-gated sodium channels. Here, we present a detailed analysis about the interaction of aripiprazole with the dominant voltage-gated sodium channel of heart muscle (hNav1.5). Elec

3'-Methoxydaidzein exerts analgesic activity by inhibiting voltage-gated sodium channels.

Isoflavones are widely consumed by people around the world in the form of soy products, dietary supplements and drugs. Many isoflavones or related crude extracts have been reported to exert pain-relief activities, but the mechanism remains unclear. Voltage-gated sodium channels (VGSCs) play important roles in excitability of pain sensing neurons and many of them are important nociceptors. Here, we report that several isoflavones including 3'-methoxydaidzein (3MOD), genistein (GEN) and daidzein (DAI) show abilities to block VGSCs and thus to attenuate chemicals and heat induced acute pain or ch

Nuciferine analogs block voltage-gated sodium, calcium and potassium channels to regulate the action potential and treat arrhythmia.

Dysfunction of the Nav1.5, Cav1.2, and Kv channels could interfere with the AP and result in arrhythmias and even heart failure. We herein present a novel library of nuciferine analogs that target ion channels for the treatment of arrhythmias. Patch clamp measurements of ventricular myocytes revealed that 6a dramatically blocked both the INa and ICa without altering the currentvoltage relationship (including the activation potential and peak potential), accelerated the inactivation of Nav and Cav channels and delayed the resurrection of these channels after inactivation. Additionally, 6a signi

The insecticidal neurotoxin Aps III is an atypical knottin peptide that potently blocks insect voltage-gated sodium channels.

One of the most potent insecticidal venom peptides described to date is Aps III from the venom of the trapdoor spider Apomastus schlingeri. Aps III is highly neurotoxic to lepidopteran crop pests, making it a promising candidate for bioinsecticide development. However, its disulfide-connectivity, three-dimensional structure, and mode of action have not been determined. Here we show that recombinant Aps III (rAps III) is an atypical knottin peptide; three of the disulfide bridges form a classical inhibitor cystine knot motif while the fourth disulfide acts as a molecular staple that restricts t

Novel paradigms on scorpion toxins that affects the activating mechanism of sodium channels.

Scorpion toxins classified as beta-class are reviewed using a new paradigm. Four distinct sub types are recognized: "classical", "Tsgamma-like", "excitatory" and "depressant"beta-scorpion toxins. Recent experimental data have made possible to identify the interacting interfaces of the Na(+) channel-receptor site 4 with some of these toxins. The voltage-sensor trapping mechanism proposed for the action of these toxic peptides is analyzed in the context of what causes a modification of the activating mechanism of Na(+) channels. A cartoon model is presented with the purpose of summarizing the mo

Evaluation of the antinociceptive activities of several sodium channel blockers using veratrine test in mice.

An important role of voltage-gated sodium channels (VGSCs) in many different pain states has been established in animal models and humans wherein sodium channel blockers partially ameliorate pain. However, behavioral tests for screening analgesics that exhibit pharmacologic action by acting on VGSCs are rarely reported, and there are no studies on antinociception using veratrine as a nociceptive agent. The aim of the present study was to examine the amount of nociceptive behavior evoked by subcutaneous administration of veratrine into the hind paw and investigate whether veratrine can be used

Isoform-Specific NaV Modulation in Painful Chemotherapy-Induced Neuropathy: Promise, Limitations, and a Clinical Agenda.

Painful chemotherapy-induced peripheral neuropathy (CIPN) is highly prevalent and functionally disabling yet remains largely refractory to current treatments. Isoform-specific modulation of voltage-gated sodium channels (NaV), particularly NaV1.8, has emerged as a promising strategy to reduce nociceptor hyperexcitability. The recent approval of the NaV1.8-selective inhibitor suzetrigine for acute postoperative pain demonstrates the feasibility of highly selective peripheral sodium-channel inhibition, although its indication is currently restricted to acute pain and its relevance for chronic ne

The major component of Heteractis magnifica sea anemone venom, RpIII, exhibits strong subtype selectivity for insects over mammalian voltage-gated sodium channels.

Voltage-gated sodium channels (NaV) are molecular targets for the development of drugs for the treatment of diseases such as epilepsy, neuropathic pain, long QT syndrome, etc., as well as for insecticides. Therefore, the search for novel selective NaV channel ligands is relevant. Using amplicon deep sequencing of tentacle cDNA libraries from sea anemones Heteractis magnifica, 36 transcripts related to RpIII neurotoxin, a NaV channel modulators, were revealed. The recombinant RpIII was moderately toxic for mice (LD50 0.030 ± 0.004 mg/kg) but did not demonstrate any activity towards NaV in human

1-O-Acetylgeopyxin A, a derivative of a fungal metabolite, blocks tetrodotoxin-sensitive voltage-gated sodium, calcium channels and neuronal excitability which correlates with inhibition of neuropathic pain.

Chronic pain can be the result of an underlying disease or condition, medical treatment, inflammation, or injury. The number of persons experiencing this type of pain is substantial, affecting upwards of 50 million adults in the United States. Pharmacotherapy of most of the severe chronic pain patients includes drugs such as gabapentinoids, re-uptake blockers and opioids. Unfortunately, gabapentinoids are not effective in up to two-thirds of this population and although opioids can be initially effective, their long-term use is associated with multiple side effects. Therefore, there is a great

[Erythromelalgia: Diagnosis and therapeutic approach].

Erythromelalgia is a rare intermittent vascular acrosyndrome characterized by the combination of recurrent burning pain, warmth and redness of the extremities. It is considered in its primary form as an autosomal dominant neuropathy related to mutations of SCN9A, the encoding gene of a voltage-gated sodium channel subtype Nav1.7. Secondary erythromelalgia is associated with myeloproliferative disorders, drugs (bromocriptine, calcium channel blockers), or clinical conditions such as rheumatic diseases or viral infection. Primary familial erythromelalgia include genetics and sporadic forms assoc

Recent advances in small molecule Nav 1.7 inhibitors for cancer pain management.

The dorsal root ganglion (DRG) is the primary neuron responsible for transmitting peripheral pain signals to the central nervous system and plays a crucial role in pain transduction. Modulation of DRG excitability is considered a viable approach for pain management. Neuronal excitability is intricately linked to the ion channels on the neurons. The small and medium-sized DRG neurons are chiefly engaged in pain conduction and have high levels of TTX-S sodium channels, with Nav1.7 accounting for approximately 80% of the current. Voltage-gated sodium channel (VGSC or Nav) blockers are vital targe

Flecainide binding dynamics in voltage-gated sodium channels: A comparative isoform analysis of the state-dependent block.

BACKGROUND AND PURPOSE: Local anaesthetics and class I anti-arrhythmic drugs block voltage-gated Na+ channels, but the molecular basis for the isoform differences in drug sensitivity remains unclear. Understanding these mechanisms is essential for optimizing therapeutic efficacy and safety. Flecainide, a class Ic anti-arrhythmic agent, is also used for non-dystrophic myotonias but lacks use in epilepsy. This study aimed to elucidate the mechanisms underlying the isoform differences in the state-dependent block of flecainide. EXPERIMENTAL APPROACH: We employed site-directed mutagenesis, electr
