One-Stage Repair of Alveolar Cleft and Nasal Deformities Using Grafts From Nasal Septum: Application of Vomer, Ethmoid, and Septal Cartilage.

BACKGROUND: Patients with alveolar cleft unrepaired suffer from nasal deformities of different magnitude. Bone and cartilage grafts are harvested through several incisions. In this study, we present a method to simultaneously correct nasal deformities and repair alveolar cleft using grafts from the nasal septum. PATIENTS AND METHODS: All 6 patients with unilateral cleft lip and palate have alveolar cleft unrepaired combined with nasal deformity. Computed tomography scans and 3-dimensional-printed models of vomer and ethmoid bone were used for the purpose of preoperative design and for assessi

Anophthalmia, cleft lip/palate, absent vomer bone, nystagmus, and mental-motor retardation: a new syndrome or Fryns "anophthalmia-plus" syndrome?

OBJECTIVE: We report that a 4-year-old boy presented with right unilateral complete cleft lip and palate, right anophthalmos, left congenital nystagmus, absence of the vomer bone, mental-motor retardation, and normal lymphocyte karyotype (46, XY). METHODS: For reconstruction of the deformities, we performed cleft lip repair by Millard's rotation-advancement technique and planned cleft palate repair. CONCLUSIONS: This combination of cleft lip and palate, anophthalmos, congenital nystagmus, absent vomer bone, and mental-motor retardation has not, to our knowledge, previously been described. We

The Utility of the Ultrasound "Superimposed-Line" Sign at the Junction of the Vomer and Maxilla in First-Trimester Screening for Fetal Cleft Palate: A Case-Control Study.

INTRODUCTION: The current retrospective case-control study evaluates the diagnostic value of screening for a fetal cleft palate by using the ultra-sound-based observation of the "superimposed-line" sign appearing at the junction of the vomer and maxilla in the first trimester of pregnancy. METHODS: Retrospective analyses were performed of ultrasonographic images of nuchal translucency obtained during the first trimester of pregnancy (11-13+6 weeks) from 45 fetuses with a cleft palate later confirmed following parturition or induced labor (cases) and 4,500 normal fetuses confirmed after partur

Repair of cleft lip and simultaneous repair of cleft hard palate with vomer flap in unilateral complete cleft lip and palate: a comparative study.

UNLABELLED: Repair of anterior palate after repairing the cleft lip in complete cleft lip and palate patient is sometimes very difficult. It needs wide, extensive and difficult dissection, and has later chances of wound infection, wound dehiscence, complete wound disruption, recurrent oronasal fistula formation and also maxillary hypoplasia. In this study, we compare the outcome of simultaneous repair of cleft lip and cleft hard palate with vomer flap against cleft lip repair alone in patients with unilateral complete cleft lip and palate (UCLP). METHODS: A prospective study was carried out i

Dentofacial morphology in adolescent or early adult patients with cleft lip and palate after a treatment regimen that included vomer flap surgery and pushback palatal repair.

Dentofacial morphology was evaluated in 94 adolescent or early adult patients born with unilateral or bilateral cleft lip and palate. As well as lip closure, the primary treatment included vomer flap surgery and pushback palatal repair. Roentgencephalometric measurements as well as classification of the patients into different classes of dentofacial deformity indicated development of bimaxillary retrognathia with severe midfacial deficiency in about a quarter of the cases. Our results were similar to those reported by other teams who used similar surgical regimen.

A follow-up study of cleft children treated with vomer flap as part of a three-stage soft tissue surgical procedure. Facial morphology and dental occlusion.

The development of the face and jaws in cleft patients, treated with a three-stage surgical procedure including a single layer vomer flap, was studied by analysing cephalometric radiographs and dental casts. The material consisted of 13 patients with complete bilateral cleft lip and palate and 50 cases with complete unilateral cleft lip and palate, operated on 1964--1970. At the follow-up the average patient in both cleft categories demonstrated a maxillary retrognathia and in the unilateral cleft sample also a facial skeletal profile straighter than normal, though not as pronounced as we had

Vomopalatoplasty: a novel procedure to reduce velopharyngeal distance in cleft palate repair.

Despite numerous improvements in the palatoplasty procedure, speech dysfunction tends to develop in many patients, requiring another surgery. In addition, vomer flaps have been used in palatoplasty in various shapes and on purposes. Nonetheless, they have been used mostly to cover the defect in wide and complete type of cleft palate. We introduce the vomopalatoplasty procedure that uses a vomer flap to reduce the nasopharyngeal space in incomplete or submucous type of cleft palate patients. The mucoperiosteal flaps on the nasal and oral sides were elevated by the conventional palatoplasty proce

Hard palate-repair technique and facial growth in patients with cleft lip and palate: a systematic review.

The vomer flap technique for repair of the hard palate is assumed to improve maxillary growth because it causes less scarring in growth-sensitive areas of the palate. The aim of this systematic review was to investigate the effect of techniques using the vomer flap compared with the palatal flap on facial growth in patients with cleft lip and palate. All papers published before 21 July 2012 were sought in the databases PubMed and MEDLINE. Search terms included "facial growth", "cleft lip and palate", "palatal repair technique", and "vomer flap". Additional studies were identified by hand searc

[A computerized tomography study of morphological and positional features of basicranial bones in patients with different facial types].

BACKGROUND: There is a lack of a comprehensive theory about how cranium base influences the development of facial skull morphology. Due to up-to-date tomography technologies, it is possible to get more reliable information about position of basicranial bones. OBJECTIVE: Aim of this research is to analyze anatomical and morphological features of sphenoid and vomer and their interrelationship in patients with different facial types of the skull. MATERIAL AND METHODS: Cone beam computed tomography (CBCT or CT) results of 120 adult patients were examined. The sample was divided into three groups

Nasal Cavity CT Imaging Contribution to the Diagnosis and Treatment of Choanal Atresia.

Background and Objectives: Choanal atresia is the most common congenital malformation of the nose. Materials and Methods: We have evaluated 24 CT images of children with choanal atresia treated at the Department of Pediatric Otorhinolaryngology FM CU and the NICD Bratislava (Slovakia). In accordance with the methodology used by Slovis et al. (1985), we have measured parameters related to anomalous development in the nasal cavity: vomer width, the width of soft atresia and the width of the air space of unilaterally developed choana. Results: In the group of 24 patients, 11 (46%) were male and 1

Histologic Aspect of the Curved Vomerine Mucosa in Cleft Lip and Palate.

BACKGROUND: Common surgical techniques aim to turn the entire vomerine mucosa with vomer flaps either to the oral side or to the nasal side. The latter approach is widely performed due to the similarity in color to the nasal mucosa. However, we lack a histologic description of the curved vomerine mucosa in cleft lip and palate malformations. METHODS: We histologically examined an excess of curved vomerine mucosa in 8 patients using hematoxylin-eosin, periodic acid-Schiff, Elastin van Gieson, and Alcian blue stains. Tissue samples were obtained during surgery at 8 months of age. RESULTS: Our

An uncommon cleft subtype of unilateral cleft lip and palate.

The finding that the vomer plays a crucial role in maxillary growth suggests that the bilateral cleft configuration of unilateral cleft lip and palate (UCLP), in which the vomer is detached from the non-cleft-side secondary hard palate, negatively influences palatal development, and this hypothesis was tested. Sixty persons with complete UCLP, including those with the vomer detached from (n = 30, b-UCLP) and attached to (n = 30, u-UCLP) the secondary hard palate, were analyzed morphologically, with the use of cast models taken at 10 days, 3 mos, and 12 mos of age. The anterio-posterior palatal

Avoidance of the use of vomerine mucosa in primary surgical management of velopalatine clefts.

A brief review of normal maxillary growth and the dysfunctional pathoanatomy of velopalatine clefts is presented. A plan of management for patients with these deformities is proposed; this is based on the degree to which the deformity departs from normal. The authors stress the importance of early, complete reconstitution of the soft palate and avoidance of the use of vomer flaps in the closure of the hard palate. The influence of the vomer, nasal septum, and labionasal musculature on postnatal facial development is discussed.

Comparison of different nasal septal areas between patients with and without nasal obstruction.

OBJECTIVES: We intended to investigate the deviation of septal swell body, perpendicular plate, septal spur and vomer in patients with and without nasal obstruction. METHODS: We compared the deviation of these septal areas in computed tomography scans of patients scheduled for nasal surgical procedures (cases) and of patients without clinically relevant nasal obstruction (controls). RESULTS: Septal swell body was similarly deviated between 56 cases (median value: 6.5 mm) and 56 controls (6.4 mm; p > 0.2). Septal spur was more deviated in cases (5.6 mm) than in controls (4.7 mm; p < 0.001). T

Choanal atresia: precise CT evaluation.

Computed tomography facilitates the diagnosis of children with choanal atresia. This modality defines the full anatomic abnormality of bony choanal atresia: medial bowing and thickening of the lateral wall of the nasal cavity, enlargement of the vomer, and fusion of these elements. Bony atresia patients can be clearly separated from those with membranous choanal atresia. In membranous atresia, the lateral wall of the nasal cavity is minimally affected and the vomer is normal. This precise anatomic evaluation aids in determining the surgical approach.

Predictive Evaluation of Septal Cartilage-bone Complex for Rhinoplasty Using Cone Beam Computed Tomography.

BACKGROUND: Septal extension grafting (SEG) is commonly used for correcting Asian short noses. When septal cartilage is limited, septal bone can be included. This study evaluates the outcomes of SEG using a septal cartilage-septal bone complex (SCBC) and assesses preoperative cone beam computed tomography (CBCT) for predicting septal bone suitability. METHODS: A retrospective review was conducted of Korean women planned for SEG with SCBC from July 2021 to June 2022. Preoperative CBCT scans measured Hounsfield Unit (HU) values for septal cartilage, perpendicular plate of the ethmoid (PPE), and
