Reverse sural artery flap for reconstruction of blast injuries of the foot and ankle.

BACKGROUND: The reverse sural artery fasciocutaneous flap is a versatile soft tissue coverage procedure for traumatic soft tissue defects of the distal tibia, ankle, and foot. War-related blast injuries represent a unique injury mechanism. There are no reports on use of this reconstructive flap in treating highly contaminated war-related musculoskeletal trauma of the foot and ankle. The current study presents results using this soft tissue coverage procedure in a subacute fashion to treat a population of war-related blast injuries. MATERIALS AND METHODS: A retrospective chart study was perfor

Musculoskeletal injuries in the Afghan war.

Among the 1274 patients admitted to a Pakistan border hospital from 1985 to 1987, the distribution and outcome of musculoskeletal war injuries differed from those seen in other conflicts. Serious complications from injuries were found in approximately 50 per cent of patients, of which most were wound infections, chronic osteomyelitis, and restriction of joint motion. Guerrillas in the Afghan war had no access to acute medical treatment in the field. Many patients died before reaching the hospital, as reflected in the low proportion of paraxial injuries; very high complication rates were noted

The disease profile of poverty: morbidity and mortality in northern Uganda in the context of war, population displacement and HIV/AIDS.

The population of Gulu District (northern Uganda) has been severely incapacitated by war, epidemics and social disruption. This study is aimed at describing disease patterns and trends in this area through a retrospective analysis of discharge records for 155205 in-patients of Lacor Hospital in the period 1992-2002. The burden of infectious diseases in childhood is overwhelming, with malaria accounting for the steepest increase in admissions. Admissions for war-related injuries and malnutrition fluctuated with the intensity of the war and the severity of famine. Emerging and re-emerging infect

War's youngest victims: a descriptive cross-sectional study on injury distributions, severity patterns, and outcomes among paediatric trauma patients in Kharkiv, Ukraine.

BACKGROUND: Paediatric trauma in conflict zones is a major public health concern, with children being highly susceptible to both immediate injuries and long-term disabilities. The Russian Invasion of Ukraine in February 2022 has significantly affected the civilian population, particularly children. However, despite reports highlighting the extent of paediatric casualties, empirical data on injury patterns among children in the ongoing conflict remains scarce. OBJECTIVE: Describe the injury distributions, severity and outcomes of paediatric war-related trauma patients during the Russian invasi

Lower Extremity Limb Salvage, Functional Restoration, and Regenerative Surgery-Lessons Learned Across Recent Military Conflicts.

Plastic and Reconstructive Surgery originated from surgical specialists who evolved techniques to improve the treatments for patients who suffered devastating war-related injuries. Furthermore, additional refinements and advancements in plastic and reconstructive surgical techniques tend to parallel advancing military weaponry which has expanded injury patterns suffered by those injured in war. The evolution of combat munitions, coupled with enhanced military and civilian trauma care abilities, has lead to an increase not only in the proportion of military personnel and enemy combatants who su

Psychiatric Casualties during the Korean War: Focusing on American and Common Wealth Soldiers.

A crucial gap in the medical history of the Korean War is the history of psychiatry during the Korean War. War puts those who participate in it through physical and mental extremes, inflicting not only physical injuries but also psychological trauma and damage. However, studies of the medical aspects of the Korean War have been limited to topics related to physical injuries and their treatment, and there are no studies that systematically summarize the traumatic effects on the human mind thrown into the midst of the war, the consequences of these effects, and the medical efforts made to deal w

U.S. War Costs: Two Parts Temporary, One Part Permanent.

Military spending, fatalities, and the destruction of capital, all of which are immediately felt and are often large, are the most overt costs of war. They are also relatively short-lived. But the costs of war borne by combatants and their caretakers, which includes families, communities, and the modern welfare state, tend instead to be lifelong. In this paper I show that a significant component of the budgetary costs associated with U.S. wars is long-lived. One third to one half of the total present value of historical war costs are benefits distributed over the remaining life spans of vetera

Neglected war injuries: Reconstruction versus amputation.

INTRODUCTION: War injuries are remarkably different situations with profound severity and significant contamination compared to civilian injuries. Delayed definitive management makes the situation much more complicated. This study aimed to report the outcomes of limb reconstruction of neglected war injuries with a previous decision of amputation. PATIENTS AND METHODS: From 2010 to 2021, 1104 patients with war-related fractures from conflict-zone countries were referred to our center including 589 open tibial fractures. Of these tibial fractures, amputation had already been decided in 54 cases

The complex dynamics of maxillofacial trauma in blast and fragmentation injuries: Current understanding and gaps in scientific evidence.

Recent epidemiological data demonstrate a significant rise in blast-induced maxillofacial trauma incidences in both military and civilian settings. This study aimed to comprehensively analyze maxillofacial trauma caused by heavy weaponry, examining blast dynamics, ballistic forces, and fragmentation effects. A comprehensive literature review was conducted using PubMed, SciSpace, and Google Scholar, incorporating studies on blast injuries, ballistic trauma, and war-related maxillofacial damage. Keywords included "maxillofacial injuries", "blast injuries", "ballistic trauma", "high-energy trauma

Penetrating injuries of the heart and the great vessels.

An account of a personal experience with 18 cases of penetrating injuries of the heart and/or the great vessels due to shrapnels or bullets during the civil war in Lebanon is presented. Six of 18 patients were children. A survey of the literature did not disclose a description of these injuries in the pediatric group. Seven patients had signs of cardiac tamponade preoperatively. Fifteen patients had concomitant injuries to other organs. Emergency thoracotomy was used in patients within two hours after arrival to the hospital and was not preceded by pericardiocentesis. Cardio-pulmonary bypass w

An Inventory of the Combat Medics' Aid Bag.

INTRODUCTION: Tactical Combat Casualty Care (TCCC) recommends life-saving interventions; however, these interventions can only be implemented if military prehospital providers carry the necessary equipment to the injured casualty. Combat medics primarily use aid bags to transport medical materiels forward on the battlefield. We seek to assess combat medic materiel preparedness to employ TCCC-recommended interventions by inventorying active duty, combat medic aid bags. METHODS: We sought combat medics organic to combat arms units stationed at Joint Base Lewis McChord. Medics volunteered to com

Penile transplantation: an emerging option for genitourinary reconstruction.

Penile transplantation is an emerging option for patients with severe genital defects not amenable to traditional reconstructive options. In this article, we discuss the burgeoning problem of severe male genitourinary trauma in the military, the limitations of traditional reconstructive options in addressing these problems, and the potential for penile transplantation to provide improved outcomes. We also review the preclinical research and limited worldwide experience with penile transplantation to date, including lessons learned, and discuss the many important technical, logistical, and ethi

Reconstruction of firearm and blast injuries in Syrian war refugees.

BACKGROUND: War injuries differ from other injuries owing to the large tissue defects they cause and their high risk of contamination. As fragments scattered by high-energy firearms and explosives cause serious composite tissue damage, repair of such injuries is difficult and requires a long treatment period. We discuss the treatment methods used for injured Syrian War refugees admitted to our clinic and present the most effective repair methods for war-related tissue defects for each region of the body. METHODS: A total of 61 patients treated between June 2012 and April 2015 were retrospecti
