Checking Wilderness Through Conservation Genetics: The Case of the European Wildcat in the Central Apennines, Italy.

The European wildcat (Felis silvestris silvestris, EWC) is threatened by habitat fragmentation, hybridisation with domestic cats, and climate change, yet its distribution and genetic status in Italy remain poorly resolved. Here, we developed a species distribution model (SDM) based on 69 occurrence records compiled from published sources to identify suitable habitats across the Italian peninsula and characterised 23 newly collected occurrences from the Central-Southern Apennines, which included associated biological samples. The model identified forest and grassland cover and elevation as the

Psychological First Aid for Wilderness Trauma: Interventions for Expedition or Search and Rescue Team Members.

When exposed to actual or threatened death or serious injury in austere settings, expedition members are at risk of acute stress reactions, as are search and rescue members involved with extricating the patient. Acute stress reactions are a normal response to significant trauma and commonly resolve on their own. If they do not, they can lead to post-traumatic stress disorder (PTSD), a set of persistent symptoms that cause significant effects on the person's life. Medication has a limited preventive role in the field for treatment of stress partly because so few are trained to administer it. Co

Wilderness Medical Society Clinical Practice Guidelines for the Treatment and Prevention of Drowning: 2019 Update.

The Wilderness Medical Society convened a panel to review available evidence supporting practices for acute management and treatment of drowning in out-of-hospital and emergency medical care settings. Literature about definitions and terminology, epidemiology, rescue, resuscitation, acute clinical management, disposition, and drowning prevention was reviewed. The panel graded available evidence supporting practices according to the American College of Chest Physicians criteria and then made recommendations based on that evidence. Recommendations were based on the panel's collective clinical ex

[Experience with endemic dermatological diseases in the Peruvian wilderness: mucocutaneous leishmaniasis and Brazilian foliaceous pemphigus].

With the setting up of a rural-medical post in the region of Pachitea-Pichis river, affluent of the Alto Ucayali, in the year 1973, the authors made the diagnosis of 120 cases of pigmentary Leishmaniasis (1,2 % of the total population). 55 % of this series corresponded to pure cutaneous forms and 65 were of the muco-cutaneous type, 21 of the latter being mutilating. Sixty per cent were young males working as wood labourers or farmers. The treatment with Repodral (Etilofen) was useful only in the initial froms. Brezilian Pemhigus foliaceous which is considered endemic in the same geographical a

Columbia Icefield Bus Rollover: A Case Study of Wilderness Mass Casualty Triage, Treatment, and Transport.

Mass casualty incidents (MCIs) are rare in wilderness and mountain settings. Few case studies have reported the response of such events within jurisdictions with well-developed trauma and emergency medical services systems (EMS). Here we explore a MCI in a wilderness setting on the Columbia Icefield inside the Jasper National Park within the Canadian Rocky Mountains. An all-terrain bus was involved that had rolled over while transporting tourists to explore the glacier. The bus rolled multiple times down the slope adjacent to the road, leading to 3 deceased and 21 patients requiring transport.

Regional Rodent-Borne Infectious Diseases in North America: What Wilderness Medicine Providers Need to Know.

Rodents can transmit infectious diseases directly to humans and other animals via bites and exposure to infectious salivary aerosols and excreta. Arthropods infected while blood-feeding on rodents can also transmit rodent-borne pathogens indirectly to humans and animals. Environmental events, such as wet winters, cooler summers, heavy rains, and flooding, have precipitated regional rodent-borne infectious disease outbreaks; these outbreaks are now increasing with climate change. The objectives of this review are to inform wilderness medicine providers about the environmental conditions that ca

Prolonged Exposure Dermatosis: Reporting High Incidence of an Undiagnosed Facial Dermatosis on a Winter Wilderness Expedition.

OBJECTIVE: Previously unclassified inflammatory skin lesions referred to as sun bumps have been observed throughout the year on participants of wilderness trips; however, the underlying cause and diagnosis remain unclear. The purpose of this prospective observational study was to document the incidence, characteristics, and risk factors associated with these skin lesions as they occurred on a winter wilderness expedition. METHODS: For this study, the lesions were defined as pruritic or erythematous skin lesions occurring while in the wilderness. Seventy-four participants in a wilderness ski t

A Man Living in the Wilderness Presents With a Unique Case of Erysipelothrix rhusiopathiae Causing Primary CNS Infection.

We report the first published case of Erysipelothrix rhusiopathiae causing subdural empyema. This 66-year-old male patient had relevant exposure history including living in a tent in the woods and having direct contact with wildlife. His main symptom which triggered his presentation was diplopia with exam findings consistent with a left partial oculomotor nerve palsy. Initial attempts at less invasive source control via burr holes alone failed. He was treated successfully with craniectomy and aqueous penicillin G with a duration of 6 weeks following surgery. CNS infection as the primary manife

Rescue Blankets as Multifunctional Rescue Equipment in Alpine and Wilderness Emergencies-A Narrative Review and Clinical Implications.

The utilization of rescue blankets in pre-hospital emergency medicine exceeds protection from hypothermia and enhanced visibility by far. In this narrative review, we focus on emphasizing the alternative applications of these fascinating multifunctional tools in the pre-hospital setting. A literature search in PubMed® and Web of ScienceTM yielded 100 results (last update was on 8 July 2022), a total number of 26 of which were included in this narrative review. Nine articles assessing alternative functions of rescue blanket were further evaluated and described in more detail. In addition, we pe

Wild Boar-Related Injuries in Wilderness Environments: Field-Oriented Management Principles and Evidence-Informed Clinical Recommendations.

Wild boar-related trauma is uncommon but may result in severe injuries that pose diagnostic and therapeutic challenges, particularly in wilderness or resource-limited environments. Most scientific literature consists of isolated case reports, regional series, and forensic descriptions; clinical guidance is therefore fragmented. This study synthesizes two complementary evidence sources: a structured observational cohort documenting 101 injuries sustained by boar hunters during organized hunts in Germany, and a comprehensive systematic review of all globally published wild boar- and feral pig-re

Enhancing Knowledge and Confidence in Wilderness Medicine Through Virtual Education: Lessons From a National Boot Camp.

Wilderness medicine (WM) education is an important aspect of training for sports medicine fellows, preparing healthcare providers to address medical challenges in remote and austere environments. It is traditionally taught in immersive, in-person settings. The COVID-19 pandemic prompted a rapid shift toward virtual education platforms, challenging traditional teaching methods. This study evaluates the efficacy of a virtual WM boot camp to teach WM concepts, converting an established in-person program into a national, virtual educational opportunity. Participants included Primary Care Sports Me

Where the wild things are: examining the reflexive affectivity of wilderness on product marketing within industrialised systems.

While our relationships with eating and drinking are thought to have undergone a period of disenchantment during industrialisation, some sociologists suggest a re-enchantment is taking hold. This paper explores the reflexive affectivity of wilderness on food and beverage marketing in consideration of persistent cultural tensions such as planetary decline, risk, freedom, and time pressures. Using social practice theories and semiotics, we examined significations of wilderness within a combined dataset of children's snack and alcohol product labels (n = 78). We describe icons and symbols used to

Simulated Patient as a Learner: Medical Volunteers Gain Knowledge by Participating in a Wilderness Medicine Training Session.

IntroductionA multifaceted approach to wilderness medicine education and training is necessary to provide a high-quality learning experience, often requiring innovative instructional techniques. Using volunteers to act as patients in medical education is a well-established practice that helps teach crucial skills. However, more is needed to know if there is potential knowledge acquisition through participation. This study examined the educational benefits for medical students serving as a simulated patient (SP), hypothesizing that these individuals will improve self-assessed knowledge and conf

Atmospheric inputs and nitrogen saturation status in and adjacent to Class I wilderness areas of the northeastern US.

Atmospheric inputs of N and S in bulk deposition (open collectors) and throughfall (beneath canopy collectors) were measured in and adjacent to two Class 1 wilderness areas of the northeastern US. In general, atmospheric S inputs followed our expectations with throughfall S fluxes increasing with elevation in the White Mountains, New Hampshire and throughfall S fluxes being greater in coniferous than deciduous stands in both sites. In contrast, throughfall N fluxes decreased significantly with elevation. Throughfall NO3 (-) fluxes were greater in coniferous than deciduous stands of Lye Brook,

AUDIENCE: This game is appropriate for medical students, interns, junior and senior residents. INTRODUCTION: The COVID-19 pandemic has forced a transition from in-person to virtual learning, and educators must innovate and adapt to keep learners engaged. One way to achieve this is through gamification.1 The authors employed a novel approach to gamification of virtual learning which engaged not only learners' computers but also their mobile phones. Learners worked in teams communicating by text message to answer ABEM board-style questions and occupy sites on the virtual board. EDUCATIONAL OBJ

Sex-specific energy expenditure during the Alaska mountain wilderness ski classic; insights from an Arctic winter expedition.

PURPOSE: The Alaska Mountain Wilderness Ski Classic (AMWSC) is a self-supported and self-oriented winter expedition that occurs in the remote North American Brooks Range, ∼200 km north of the Arctic Circle. Few investigations have evaluated sex-specific physiological responses under extreme cold and isolated circumstances. Our study examined sex-specific differences in total energy expenditure (TEE), water turnover (WT), and changes in body composition during the expedition. METHODS: Twenty adult participants (8 females, age: 41 ± 6 years, body mass index: 22.8 ± 1.9 kg/m2 and 12 males, age:
