Intracardiac Wilms' tumor: diagnosis and management.

Complete excision of Wilms' tumor may require resection of adjacent organs and removal of intracaval tumor propagation. Extension of tumor to the right atrium can be determined preoperatively guiding a direct and safe approach to intracardiac tumor at the time of nephrectomy. Preoperative ultrasonography of two children with Wilms' tumor demonstrated caval and right atrial tumor. Laparotomy for nephrectomy and abdominal caval exposure was combined with cardiopulmonary bypass and atriotomy. In both patients, tumor contiguous with the renal pelvis extended from the iliac bifurcation into the rig

Bilateral Wilms Tumor: Multi-institutional Analysis of Nephron-Sparing Surgery at Centers with Focused Wilms Tumor Expertise.

PURPOSE: The proportion of children who undergo bilateral nephron-sparing surgery (BNSS) for bilateral Wilms tumor (BWT) is variably reported. We evaluated the frequency of BNSS versus unilateral radical nephrectomy with contralateral nephron-sparing surgery (NSS) at institutions with focused Wilms tumor expertise during the Children's Oncology Group (COG) AREN0534 era. We hypothesized that BNSS would be performed more frequently than the 35% frequency reported on AREN0534. METHODS: We identified the top 10 enrolling institutions (all renal tumors) on the COG Renal Tumor Banking Study (AREN03

Small bowel obstruction after nephrectomy for Wilms' tumor. A report of the National Wilms' Tumor Study-3.

OBJECTIVE: This study was undertaken to define the incidence and etiology of small bowel obstruction (SBO) after nephrectomy for Wilms' tumor. SUMMARY BACKGROUND DATA: Intestinal obstruction is one of the most common postoperative complications after nephrectomy for nephroblastoma. However, few reports have evaluated risk factors for SBO. Radiation therapy has been associated with increased intestinal complications in some adult cancer patients, but this has not been reported in children undergoing cancer surgery. METHODS: Postoperative SBO occurred in 131 of 1,910 children (6.9%) enrolled i

[Bilateral Wilm's tumor in the fetus].

The author reports a case of Wilms' tumor in a stillborn fetus and emphasizes the extreme rarity of this diagnosis in fetus and at birth despite monitoring pregnancy frequent sonography. There is no report of in utero ultrasound diagnosis of Wilms' tumor. Wilms' tumor develops from nephrogenic blastema rests which usually disappear after 34 weeks of gestation. Pediatric autopsies show that blastematous foci may persist after birth without necessarily forming nephroblastomas. Their frequency is one hundred times higher than that of nephroblastomas although they are often associated. Wilms' tumo

[Wilms' tumors and malformation complexes].

Wilms' tumor is an embryonal tumor which is derived from metanephric metanephric blastema. The occurrence of both sporadic and hereditary forms, along with various congenital abnormalities of Wilms' tumor suggest that the tumors develop when a predisposing germ line mutation is accompanied by a second mutation. The existence of both gross chromosomal abnormalities has led to the genetic characterization of a number of loci involved in the development of Wilms' tumor. A tumor suppressor gene for Wilms' tumor, WT1, has been isolated from the 11p13 region. The product of this gene is a transcript

The occurrence of Wilms tumor in horseshoe kidneys: a report from the National Wilms Tumor Study Group (NWTSG).

BACKGROUND/PURPOSE: An increased incidence of Wilms tumor has been noted in patients with a horseshoe kidney. These represent a difficult diagnostic and therapeutic challenge. The charts of all National Wilms Tumor Study Group (NWTSG) patients with Wilms tumor occurring in a horseshoe kidney were reviewed. METHODS: From 1969 to 1998, 8,617 patients were enrolled in the NWTSG. Forty-one patients were found to have a Wilms tumor arising in a horseshoe kidney for an incidence of 0.48%. Their records were reviewed retrospectively. RESULTS: Horseshoe kidney was not recognized preoperatively in 13

Imaging accuracy and incidence of Wilms' and non-Wilms' renal tumors in children.

PURPOSE: The purpose of this study is to determine the actual incidence, age distribution, and preoperative imaging accuracy of non-Wilms' tumors (nWT) in children with renal masses. METHODS: Pathologic reports from all tumor nephrectomies or open renal biopsies performed at a single institution from September 1999 to June 2005 were analyzed. Patient demographics, pathologic findings, specific imaging study descriptors, and differential diagnoses were tabulated. Accuracy of imaging studies in identifying specific tumors was calculated. RESULTS: Ninety-two patients were identified. Sixty-eigh

Botryoid Wilms' tumor: report of two cases.

BACKGROUND: Botryoid Wilms' tumor is a rare kind of Wilms' tumor. We report two cases of this tumor. METHODS: Case 1, a 2-year-old boy, was admitted with macrohematuria for 5 months. Case 2, a 19-month-old boy, was referred for a palpable abdominal mass. The two cases were checked by 64-row multi-slice spiral CT (MSCT) and scanned with the dynamic contrast enhancement. The masses were excised and pathologically confirmed. RESULTS: In case 1, the mass occurred in the renal pelvis and calyx bilaterally, with heterogeneous density and prominent calcification. By contrast enhanced CT scan, the m

Presacral extrarenal Wilms tumor in a child.

OBJECTIVES: Wilms tumor (WT) is one of the most common solid tumors in children. It also is the most frequent tumor found in the kidneys. In 5% of cases, it affects both kidneys simultaneously. Extrarenal WTs are extremely rare. The diagnosis of extrarenal WT is almost always made after surgical intervention. The tumor can be located in the retroperitoneum, uterus, cervix, testes, skin, and even the thorax. METHODS: We report on a 3-year-old girl with an extrarenal WT located in the presacral region. RESULTS: The exact mechanism whereby a WT occurs in extrarenal tissues is unknown. The prese

Late recurrence in children with Wilms' tumor.

We aimed in this study to evaluate the clinical and radiological features of the late recurrence of Wilms' tumor in children. Among 553 children diagnosed with Wilms' tumor between 1972 and 2004, four cases were determined to be late recurrences. Clinical, histopathological parameters, treatment details, and outcomes of the patients were evaluated retrospectively. The ages of the patients at the time of diagnosis were 2, 5, 5, and 9 years and the male/female ratio was 1/3. Two patients had stage II disease and two had stage IV characteristics. Histopathological examination showed favorable his

Pediatric renal masses: Wilms tumor and beyond.

A variety of pediatric renal masses may be differentiated from Wilms tumor on the basis of their clinical and imaging features. Wilms tumor is distinguished by vascular invasion and displacement of structures and is bilateral in approximately 10% of cases. Nephroblastomatosis occurs most often in neonates and is characterized by multiple bilateral subcapsular masses, often associated with Wilms tumors. Renal cell carcinoma is unusual in children except in association with von Hippel-Lindau syndrome and typically occurs in the 2nd decade. Mesoblastic nephroma is the primary consideration in a n

Surgical complications after nephrectomy for Wilms' tumor.

We reviewed the charts of 1,910 children enrolled in the Third National Wilms' Tumor Study who underwent primary nephrectomy. Four hundred and ninety-five surgical complications occurred in 379 children (19.8 percent). Patients with inoperable tumors, bilateral renal tumors, peroperative therapy and those who refused treatment were excluded from this review. The most common complication was intestinal obstruction, which occurred in 132 patients (6.9 percent). This was followed by extensive intraoperative hemorrhage (112 patients), defined as blood loss exceeding 50 milliliters per kilogram of
