Treatment Strategies for Infected Wounds.

The treatment of skin wounds is a key research domain owing to the important functional and aesthetic role of this tissue. When the skin is impaired, bacteria can soon infiltrate into underlying tissues which can lead to life-threatening infections. Consequently, effective treatments are necessary to deal with such pathological conditions. Recently, wound dressings loaded with antimicrobial agents have emerged as viable options to reduce wound bacterial colonization and infection, in order to improve the healing process. In this paper, we present an overview of the most prominent antibiotic-em

Perineal wound infections: an audit.

An audit was completed by an NHS trust to determine the rate of perineal trauma amongst vaginal births and to assess the rate of perineal wound infections. The audit results confirmed a higher than average rate of perineal wound infections amongst women who had an instrumental birth. The trust decided to separate the contents of the delivery packs into two separate packs--one pack for birth and one pack for suturing--and developed a back-to-basics update session that was delivered to staff working within the maternity setting. A re-audit the following year confirmed that these measures had wor

Skin and wound infections: an overview.

Skin infections are common and may be caused by bacteria, fungi or viruses. Breaks in the skin integrity, particularly those that inoculate pathogens into the dermis, frequently cause or exacerbate skin infections. Bacterial skin infections caused by corynebacteria include erythrasma, trichomycosis axillaris and pitted keratolysis. Staphylococci may cause impetigo, ecthyma and folliculitis. Streptococcal skin infections include impetigo and erysipelas. Human papillomavirus skin infections present as several different types of warts, depending on the surface infected and its relative moisture,

Anaerobes in burn-wound infections.

Anaerobic bacteria are not routinely considered in discussions of the etiology of burn-wound infection. However, this review of the literature reveals that there are numerous case reports that document clinically significant anaerobic burn-wound infection and some prospective data that suggest a possible role of anaerobes in the clinical setting of burn-wound sepsis. Further studies are needed to answer the questions of how often anaerobes colonize burn wounds and how often anaerobes invade the wounds to cause clinically significant burn-wound sepsis.

Hydrogel-Based Sequential Photodynamic Therapy Promotes Wound Healing by Targeting Wound Infection and Inflammation.

Impaired wound healing remains a significant clinical challenge, often exacerbated by bacterial infections and persistent inflammation. Photodynamic therapy (PDT) has demonstrated efficacy in combating bacterial infections; however, its utility in wound healing is limited due to its inability to address inflammation. To overcome this limitation, we have developed a PDT-based sequential therapy. Our approach effectively eliminates bacteria by generating sufficient reactive oxygen species and nitric oxide (NO). It subsequently mitigates inflammation through sustained NO release and oxygen delive

Post-Operative Wound Infection by Multiple Zoonotic Organisms.

Rhodococcus hoagii (formerly Rhodococcus equi), originally isolated from equine species, is primarily transmitted through inhalation of contaminated aerosols such as dust and even breath of infected livestock. Pasteurella multocida is a common cause of cellulitis secondary to canine and feline bites. These are 2 zoonotic microbes that can present with potentially life-threatening, systematic illness in immunocompromised patients. In this brief report, we delineate the clinical course of a 42-year-old post-operative patient (status post-elective exploratory laparotomy, extensive adhesiolysis, an

Deep Sternal Wound Infection: Diagnosis, Treatment and Prevention.

Deep sternal wound infection (DSWI) is a rare but potentially devastating complication of median sternotomy performed in cardiac surgery. The incidence of DSWI is reported to be between 0.2% and 3%. Identifying high-risk patients and strategies to optimize risk factors plays an important role in reducing the incidence of DSWI. Management of DSWI can be complex and may require a multidisciplinary team approach involving infectious disease specialists, microbiologists, as well as cardiothoracic and plastic surgeons. Early detection, appropriate antibiotic treatment, aggressive surgical debrideme

Raoultella planticola: A Rare Cause of Wound Infection.

Raoultella planticola is a gram-negative, aerobic, rod bacterium commonly found in the environment, particularly in water, soil, and fish. This organism has been found to cause a variety of infections, such as necrotizing fasciitis, cystitis, cholecystitis, pancreatitis, hepatic disease, and soft tissue infections. The authors report the case of a 73-year-old-woman who presented with a R planticola wound infection of her left lower extremity after a dog scratch. She was treated successfully with local wound care using dehydrated human amnion/chorion and oral levofloxacin. Her wound healed comp

The measurement of wound infection after breast surgery.

Infection in breast wounds often occurs in the form of cellulitis, but the conventional criteria for wound infection are the presence of either pus or a serous discharge containing pathogens. Wound scoring systems may offer a more quantitative and clinically relevant approach when evaluating the morbidity caused by infection in wounds. The aim of this study was to develop a wound scoring system for patients undergoing breast surgery. The components of previously described wound scoring systems were measured in 218 women undergoing nonreconstructive breast surgery. Using conventional criteria,

Postoperative deep wound infection in instrumented spinal surgery.

This study was designed to analyze the frequency, nature and risk factors of postoperative deep wound infection in instrumented spinal surgery. The infection rate of 174 instrumented spinal operations was documented. The patients were investigated in terms of risk factors. Infections occurring after 12 months were considered as late. Eight (4.6%) acute infections occurred and were treated by early surgical debridement. Three (1.7%) late reactions were noted; they were treated by implant removal. These late reactions were not bacterial infections, but rather foreign body reactions around the im

Microorganisms associated with wound infection in Ekpoma, Nigeria.

Bacteria associated with wound infection in Ekpoma, Nigeria, and their antimicrobial susceptibility profile was investigated by standard microbiological methods, using hospital as well as non-hospital patients. Of 40 patients seen, 25 (62.5%) were males, while the rest were females. Those aged 30 years and above accounted for 63% of the patients, and post-operative sepsis was the most frequently encountered wound infection. Of the organisms encountered, Staphylococcus aureus was the most frequently occurring organism (39%), followed by coliform bacilli (24%), which was the most prevalent organ

Nocardia otitidiscaviarum infection of a traumatic skin wound.

A case of primary Nocardia otitidiscaviarum infection of a skin wound over an open fracture in a previously healthy adult who suffered multiple trauma in a car accident is reported. The organism was identified in cultures of pus specimens from the infected wound. The case demonstrated the difficulties of testing susceptibility of nocardiae in vitro and the necessity of prolonged antibiotic treatment. The prevalence of nocardial infections is underestimated, highlighting the need for adequate documentation of such infections and compilation of the information by public health authorities.

Risk factors for wound infection following elective cholecystectomy.

A study of risk factors for wound infection among patients undergoing elective cholecystectomy was undertaken. Over a 2-Year period 177 patients who underwent elective cholecystectomy for symptomatic gall stone disease were randomized into groups, one receiving antibiotics (96 patients) and the other not receiving antibiotics (81 patients). Gall bladder bile and wound swab were cultured to detect bacterial growth. Duration of preoperative hospital stay, type of skin incision and operating time were noted for each patient. Postoperatively wound infection developed in 22/177 (12%) patients. The
