Hookworms and the species infecting man in Zambia.

The rate of hookworm infections, determined by stool examination over a 7-month period, was 16.56% in medical examinees and 19.91% in in-patients in an urban hospital population in Zambia. The egg-load though was low. Culture of ova to the filariform larval stage, examination of adult worms expelled after anthelminthic treatment and collected at autopsy confirmed that the dominant species was Necator americanus. However, we report, perhaps for the first time, the presence of Ancylostoma duodenale in man in Zambia. Such information is important in assessing the clinical significance of hookworm

Strongyloides fülleborni infections in man in Zambia.

Rhabditiform larvae, and in two cases eggs, of Strongyloides spp. identified in the stools of Zambian patients were cultured to the free-living adult stage. Free-living adults of S. fülleborni, a parasite common in primates in Africa and Asia, were present in cultures from 13 (9.9%) of 131 cases of strongyloidiasis. In three of these cases, both S. fülleborni and S. stercoralis free-living adults were found.

Observations on Trichomonas vaginalis infections in Zambia.

Microscopy and culture of high vaginal swabs from an ante-natal clinic showed that 38.5% of the women harboured Trichomonas vaginalis; the corresponding figure from a gynaecology clinic was 31.4%. The incorporation of the culture method together with direct microscopy as a diagnostic technique in a routine laboratory is emphasized. Urine microscopy in out-patients, in-patients and medical examinees indicates that T. vaginalis in both males and females is not a rare infestation but the prevalence rate of 4.8% in males is considered an underestimate. Asymptomatic and symptomatic trichomoniasis w

A case of hartmannellid amebic meningoencephalitis in Zambia.

A case of amebic meningoencephalitis recognized in an adult Zambian is described. This is the first authenticated case from Africa. The morphologic features of the organism, its ability to form cysts in tissue, and the granulomatous tissue response denote that the ameba is an hartmannellid rather than a Naegleria. Free-living amebas of the family Hartmannellidae have not been incriminated before as a cause of primary amebic meningoencephalitis in man. To our knowledge this is the only case where such an ameba was responsible for fulminating meningoencephalitis. The presence of the amebas in a

Some transfusion-induced parasitic infections in Zambia.

The risk of acquiring a transfusion-induced infection in Zambia was studied for the first time. Blood slide examination of donors, despite the insensitivity of the method, established malaria as the most serious hazard. The species involved was Plasmodium falciparum, the cause of cerebral malaria, and which could be rapidly fatal in a non-immune host visiting an endemic area. Microfilariae of Dipetalonema perstans and Wuchereria bancrofti were also found in donor populations. While no disease may be induced, allergic reactions due to the breakdown products of dead microfilariae may manifest th

Histiocytic medullary reticulosis (malignant histiocytosis) in Zambia.

Fifteen cases of histiocytic medulary reticulosis are reported from Zambia, Africa. The importance of liver biopsy in the diagnosis is stressed. Forty cases have now been described from East and Central Africa suggesting a much higher incidence than in other parts of the world. It is thought that this may be due to prolonged stimulation of the reticuloendothelisl system.

Bovine babesiosis (Babesia bovis infection) in Zambia.

Two cases of Babesia bovis, a parasite associated with the tick Boophilus microplus, are reported for the first time from the central part of Zambia. It is concluded that infected B. microplus ticks are occasionally introduced into central Zambia by tick-infested cattle from the north-eastern part of the country where B. bovis is endemic. The spread of B. microplus in Southern Africa in a westward direction is discussed and related to the epidemiology of bovine babesiosis in Zambia.

Reproductive mortality in Lusaka, Zambia, 1982-1983.

In this study, age- and parity-specific birth data were used to estimate maternal mortality rates for 1982-83 at University Teaching Hospital in Lusaka, Zambia. Overall, 60 maternal deaths occurred during pregnancy or within 42 days after pregnancy termination, and four pregnancy-related deaths occurred more than 42 days after pregnancy termination. Nine of the 60 maternal deaths were caused by induced abortion, one by spontaneous abortion, twelve by hypertensive disease of pregnancy, ten by hemorrhage, nine by puerperal sepsis, and four by ectopic pregnancy. Of the 12 women who died from nono

Assessing infant sleep practices and other risk factors of SIDS in Zambia: a cross-sectional survey of mothers in Lusaka, Zambia.

BACKGROUND: Having infants sleep with their parents and sleeping face down or on their sides are the two most proximate and modifiable risk factors for sudden infant death syndrome (SIDS). Little is known about the burden of SIDS or the prevalence of these risk factors in Africa. Our primary objective was to determine the prevalence of modifiable risk factors of SIDS in Lusaka, Zambia. METHODS: We conducted cross-sectional surveys with recent mothers of infants aged < 1 year across two busy urban clinic sites in Lusaka, Zambia. We used log-binomial regression analysis to identify factors pred

Patient compliance in the treatment of Burkitt's lymphoma in rural Zambia: a retrospective study on 80 Burkitt's lymphoma patients in Katete, Zambia.

BACKGROUND: In African settings the treatment results of Burkitt's lymphoma (BL) seem to be less favourable compared with Western settings. The aim of this retrospective study was to analyse some factors that affect the treatment of BL. PATIENTS AND METHODS: Over a 16 year period, data were extracted of 80 patients. RESULTS: Complete remission 5%, very good partial response 35%, partial response 16%, no response 10%, data missing 34%. Of all patients, 56% did have a positive response to treatment. However, 51% of this subgroup of patients did not finish treatment. There was no difference in

Antimicrobial usage in broiler poultry farms in Zambia.

Although Zambia and many other countries globally have very high rates of antimicrobial resistance, there is a surprisingly limited amount of quantitative data on antimicrobial use (AMU) to guide important control actions. Here, we used the VetCAb-ID [Veterinary Consumption of Antibiotics-International Documentation; ©Tierärztliche Hochschule (TiHo) Hannover, Germany] system to conduct a prospective longitudinal analysis of AMU in broiler poultry farms in the Lusaka district of Zambia to quantify the use by the treatment frequency (TF) metric. The overall TF was 7, and the median treatment fre

A high prevalence of dogs seropositive to Leishmania in Zambia.

Domestic dogs are the main reservoir of Leishmania infantum, a causative agent of visceral leishmaniasis in humans. Although the disease is widespread in the world, the burden of visceral or any other disease form of leishmaniasis is poorly documented in Zambia, largely due to lack of surveillance. Recently, three cases of autochthonous canine leishmaniasis (CanL) were reported in Zambia following decades of presumed disease absence. This finding intimated probable disease emergence, raising the need for the identification of infection foci. Thus, in this study, we conducted the first mass ser

Aedes albopictus is present in the lowlands of southern Zambia.

Identifying the current geographic range of disease vectors is a critical first step towards determining effective mechanisms for controlling and potentially eradicating them. This is particularly true given that historical vector ranges may expand due to changing climates and human activity. The Aedes subgenus Stegomyia contains over 100 species, and among them, Ae. aegypti and Ae. albopictus mosquitoes represent the largest concern for public health, spreading dengue, chikungunya, and zika viruses. While Ae. aegypti has been observed in the country of Zambia for decades, Ae. albopictus has n

Filarial infections in domestic dogs in Lusaka, Zambia.

Filariae are common parasites of dogs in many parts of the world, but little is known about the status of these infections in sub-Saharan Africa. A study was carried out to determine the occurrence and species of filariae among 272 dogs in Lusaka, Zambia. Giemsa stained blood smear and Knott's concentration methods revealed microfilariae in 16 (5.9%) of the dogs. PCR confirmed that most of these dogs had Acanthocheilonema reconditum infection. Ten (4.0%) of the examined dogs were positive for Dirofilaria immitis circulating antigen (by DiroCHEK(®) test), but D. immitis microfilariae were not i

Mapping the geographical distribution of lymphatic filariasis in Zambia.

BACKGROUND: Past case reports have indicated that lymphatic filariasis (LF) occurs in Zambia, but knowledge about its geographical distribution and prevalence pattern, and the underlying potential environmental drivers, has been limited. As a background for planning and implementation of control, a country-wide mapping survey was undertaken between 2003 and 2011. Here the mapping activities are outlined, the findings across the numerous survey sites are presented, and the ecological requirements of the LF distribution are explored. METHODOLOGY/PRINCIPAL FINDINGS: Approximately 10,000 adult vo

Cost-effectiveness of eye care services in Zambia.

OBJECTIVE: To estimate the cost-effectiveness of cataract surgery and refractive error/presbyopia correction in Zambia. METHODS: Primary data on costs and health related quality of life were collected in a prospective cohort study of 170 cataract and 113 refractive error/presbyopia patients recruited from three health facilities. Six months later, follow-up data were available from 77 and 41 patients who had received cataract surgery and spectacles, respectively. Costs were determined from patient interviews and micro-costing at the three health facilities. Utility values were gathered by adm

Clinical patterns of juvenile idiopathic arthritis in Zambia.

BACKGROUND: Juvenile idiopathic arthritis (JIA) is a heterogeneous group of disorders with different disease manifestations among various populations. There are few reports of JIA among indigenous Africans especially sub-Saharan Africa. We present herein the clinical patterns of JIA encountered at a tertiary hospital in Lusaka, Zambia. METHOD: Hospital records of patients with a diagnosis of chronic arthritis with onset at the age of 16 years or less presenting to University Teaching Hospital, Lusaka, Zambia for the periods 1994-98 and 2006-2010 were retrospectively reviewed and reclassified
