INTRODUCTION: Tongue-tie (ankyloglossia) can cause significant feeding difficulties in infants. A quality improvement project conducted in 2015-2016 at our centre led to the introduction of a referral pathway that standardised frenulotomy (tongue-tie release) services. This project completes a second-cycle audit to evaluate the appropriateness and effectiveness of the tongue-tie referral pathway for patients under three months old.
METHODS: A retrospective case-note review was conducted on 310 patients referred for tongue-tie assessment between January 2019 and December 2023. Data were collec

PURPOSE: The tongue is a complex organ that can provide insight into oral and systemic conditions. A variety of disorders, such as geographic tongue, hairy tongue, fissured tongue, macroglossia, microglossia, and others, can be observed. The objective of this cross-sectional study, conducted on a cohort of dental patients, was to examine the prevalence of tongue disorders and their relationship with factors such as demographic data, oral hygiene practices, history of systemic diseases, and bad habits.
METHOD: The tongues of 772 dental outpatients, comprising 355 males and 417 females, were ex

UNLABELLED: Ankyloglossia, or tongue-tie, is characterized by a short or thickened lingual frenulum; this can be associated with impaired breastfeeding, speech, and dentofacial growth. The indications for performing frenotomy, frenuloplasty, or other operative interventions are unclear.
METHODS: A meta-analysis was performed to identify the extent of the benefit from frenotomy in breastfeeding measures, degree of tongue-tie, and maternal pain during feeding in randomized controlled trials. A structured literature review analyzed the optimal type and timing of repair. An algorithm was develope

The aetiology of nonsyndromic cleft lip with or without cleft palate (NSCLP) is complex and involves both genetic and environmental risk factors. Classical research has shown that growth and patterning of the developing palatal shelves depend on epithelial-mesenchymal interactions. Expression of several signalling molecules and transcription factors in the anterior palate during early palate development has been documented. TBX22 encodes a T-box containing transcription factor and mutations in this gene are responsible for X-linked cleft palate and ankyloglossia. In the present study, we analy

BACKGROUND: In the past 10-15 years, there has been increased concern about ankyloglossia and its effect on infant breastfeeding. This has been associated with increased performance of frenotomy. Physicians and other healthcare professionals with expertise in breastfeeding have voiced concerns about complications related to the performance of infant frenotomy. Reviews of this topic have reported no significant complications after frenotomy. Other data on complications consist of case reports.
METHODS: An online survey was developed by physicians with expertise in breastfeeding and e-mailed to

The T-box transcription factor TBX22 is essential for normal craniofacial development, as demonstrated by the finding of nonsense, frameshift, splice-site, or missense mutations in patients with X-linked cleft palate (CPX) and ankyloglossia. To better understand the function of TBX22, we studied 10 different naturally occurring missense mutations that are phenotypically equivalent to loss-of-function alleles. Since all missense mutations are located in the DNA-binding T-box domain, we first investigated the preferred recognition sequence for TBX22. Typical of T-box proteins, the resulting sequ

BACKGROUND: The effectiveness of orofacial myofunctional therapy (OMT) has yet to be confirmed in the literature. This scoping review aimed to answer the question, "What evidence exists to support the effectiveness of OMT in treating/managing orofacial myofunctional disorders (OMDs) affecting orofacial structures' function and oral habits?"
METHODS: A librarian at the University of Alberta, Canada, developed a comprehensive search strategy and applied it to 6 databases and grey literature. The reference lists of included studies were cross-checked. Two independent reviewers screened the retri

BACKGROUND: Recent data suggest an increased risk of congenital anomalies with prenatal exposure to opioid analgesics. We sought to further quantify the risk of anomalies after opioid analgesic exposure during the first trimester in a population-based cohort study.
METHODS: Using administrative health data from Ontario, we followed 599 579 gestational parent-infant pairs from singleton pregnancies without opioid use disorder. We identified opioid analgesics dispensed in the first trimester and congenital anomalies diagnosed during the first year of life. We estimated propensity score-adjusted

Background: Inherited epidermolysis bullosa (EB) includes a group of rare genetic disorders affecting the skin and mucous membranes. These disorders are characterized by extreme fragility and blister formation after minimal or no trauma. Oral and systemic manifestations vary by subtype; the more severe forms often present with extensive intra-oral blistering, scarring, microstomia, vestibular obliteration, ankyloglossia, and-in some cases-oral cancer. This study aims to collect data on oral-health practices and challenges in people with EB to inform preventive strategies and dental care. Metho

AIM: To produce a simple tool with good transferability to provide a consistent assessment of tongue appearance and function in infants with tongue-tie.
METHODS: The Bristol Tongue Assessment Tool (BTAT) was developed based on clinical practice and with reference to the Hazelbaker Assessment Tool for Lingual Frenulum Function (ATLFF). This paper documents 224 tongue assessments using the BTAT. There were 126 tongue assessments recorded using the BTAT and ATLFF tools to facilitate comparisons between them. Paired BTAT assessments were obtained from eight midwives who were using the new assessm

We aimed to evaluate the safety of maternal Tdap; thus, we assessed health events by examining the difference in birth and hospital-related outcomes of infants with and without fetal exposure to Tdap. This was a retrospective cohort study using linked administrative datasets. The study population were all live-born infants in New Zealand (NZ) weighing at least 400 g at delivery and born to women who were eligible for the government funded, national-level vaccination program in 2013. Infants were followed from birth up to one year of age. There were a total of 69,389 eligible infants in the coh

BACKGROUND: Nipple pain is associated with early cessation of breastfeeding and may be caused by high intra-oral vacuum. However identification of high intra-oral vacuum is typically restricted to the research setting. This is the first reported case of an infant with high intra-oral vacuum that was clinically identified through a specific pattern of nipple trauma associated with nipple shield use. Knowledge of clinical signs associated with high intra-oral vacuum may facilitate early recognition of this unusual breastfeeding challenge.

BACKGROUND: Women who enter pregnancy overweight or obese tend to have poorer breastfeeding outcomes compared to non-overweight women. Women's experiences of specific breastfeeding-related problems and reasons for use of formula have not been systematically investigated according to pre-pregnancy BMI. The aim of this study was to compare self-reported breastfeeding problems in non-overweight and overweight women and identify the main reasons for use of infant formula during the first month postpartum.
METHODS: The present study involved a cross-sectional secondary analysis of data collected a

INTRODUCTION: Objective assessment of infant suckling biomechanics remains limited despite decades of pressure-based measurement research, and tongue-tie (ankyloglossia) is still largely evaluated using semi-quantitative anatomical scoring. We developed a hydraulic differential-pressure device - the electronic baby bottle (EBB) - designed to record real-time net intraoral mechanical load during nutritive suckling through a fluid-filled sensing cavity with a controlled air inclusion coupled to a differential pressure transducer. This study aimed to: (1) describe the EBB measurement principle, (

INTRODUCTION: As the number of frenotomy procedures increases worldwide, the recognition and management of potential complications has become increasingly relevant. Postprocedural scarring following laser release may severely restrict tongue mobility in infants and occasionally requires surgical revision.
CASE PRESENTATION: We report the case of a 3-month-old infant referred for persistent feeding difficulties and poor weight gain (5th percentile) after an unsuccessful laser frenotomy performed elsewhere. Clinical examination revealed severe ankyloglossia secondary to a dense restrictive scar
