Joint preserving procedures and endoprostheses in hemophiliacs - indications and long term results.

Hemophilic arthropathy is the result of recurrent joint bleedings in patients with severe haemophilia A or B and von Willebrand Syndrome. Conservative orthopaedic treatment is preferred in every stage of the arthropathy. Synovectomy is indicated only after failure of the conservative regimen after 3 to 6 months in order to control synovitis and recurrent bleeding. This paper presents the indications, methods, and results of different joint preserving operations. Especially synovectomy of the elbow joint with or without radial head resection shows very good long term results. Radiosynoviorthesi

Sonication improves microbiologic diagnosis of periprosthetic elbow infection.

BACKGROUND: Periprosthetic joint infection (PJI) is a relatively frequent and oftentimes devastating complication after total elbow arthroplasty (TEA). Its microbiologic diagnosis is usually based on periprosthetic tissue culture (hereafter referred to as tissue culture), but the sensitivity of tissue culture is variable. Although implant sonication culture has been shown to be superior to tissue culture for the diagnosis of hip and knee PJI, only a single small study (of fewer than 10 infected implants) has assessed sonication for PJI diagnosis after elbow arthroplasty. METHODS: We retrospec

Estimation of distal arm joint angles from EMG and shoulder orientation for transhumeral prostheses.

In this paper, we quantify the extent to which shoulder orientation, upper-arm electromyography (EMG), and forearm EMG are predictors of distal arm joint angles during reaching in eight subjects without disability as well as three subjects with a unilateral transhumeral amputation and targeted reinnervation. Prior studies have shown that shoulder orientation and upper-arm EMG, taken separately, are predictors of both elbow flexion/extension and forearm pronation/supination. We show that, for eight subjects without disability, shoulder orientation and upper-arm EMG together are a significantly

Intraoperative monitoring of ulnar nerve function during replacement of the rheumatoid elbow via the lateral approach.

Neurography of the ulnar nerve was performed pre-, intra- and postoperatively in 8 arms of 7 patients with rheumatoid arthritis operated on with total elbow replacement via the lateral approach. Ulnar nerve decompression was performed in 4 elbows before implantation. A reduction in the amplitude of compound muscle action potential (CMAP) recorded from the abductor digiti minimi on stimulation of the ulnar nerve in the axilla, was observed during elbow dislocation at surgery in all patients, in 5 cases transiently and in 3 cases until the end of surgery. The ulnar nerve had been decompressed in

Autogenous arteriovenous fistulas for haemodialysis: a review.

The last 40 years has witnessed revolutionary changes in vascular access provision for haemodialysis. Autogenous arteriovenous (AV) fistula is the best access modality and should be considered first when planning vascular access. Education is required to ensure preservation of the cephalic veins in patients at potential risk for the development of end stage renal disease (ESRD). The best access procedure should be performed first and the AV fistula allowed to mature before use. Autogenous AV fistula have a cumulative patency of 85-90% and 60-85% at 1 and 3 years respectively. Increased use of

Intraosseous radial nerve entrapment complicating total elbow revision.

A 43-year-old man underwent revision of a loose total elbow arthroplasty in 1995. The arthroplasty had been implanted 20 years previously for post-traumatic osteoarthritis after a gunshot wound complicated by permanent ulnar nerve palsy. The patient suffered a minimally displaced periprosthetic fracture 4 years after implantation that was treated closed. The patient subsequently developed severe loosening with bony dilation. During revision surgery, while grasping and removing the periprosthetic membrane from within the humeral medullary canal with a Kerrison rongeur, dorsiflexion of the wrist

The effect of radial head implant shape on radiocapitellar kinematics during in vitro forearm rotation.

BACKGROUND: A number of radial head implants are in clinical use for the management of radial head fractures and their sequelae. However, the optimal shape of a radial head implant to ensure proper tracking relative to the capitellum has not been established. This in vitro biomechanical study compared radiocapitellar joint kinematics for 3 radial head implant designs as well as the native head. METHODS: Eight cadaveric upper extremities were tested using a forearm rotation simulator with the elbow at 90° of flexion. Motion of the radius relative to the capitellum was optically tracked. A stem

Total elbow arthroplasty for primary osteoarthritis.

BACKGROUND: Primary osteoarthritis of the elbow is a less common indication for total elbow arthroplasty (TEA). Higher complication rates in younger, active patients may offset short-term improvements in pain and function. The purpose of this study was to determine pain relief, functional outcomes, complications, and survival of TEA in this population. METHODS: Between 1984 and 2011, 20 consecutive TEAs were performed for primary elbow osteoarthritis. Two patients died before the 2-year follow-up. Mean age at surgery was 68 years (range, 51-85 years). Outcome measures included pain, motion, M

[Application of compressing mini-screws in treatment of patients with intra-articular fractures].

The authors present an experience with the application of compressing headless mini-screws (Herbert screws) in treatment of intra-articular fractures in 37 patients. The mini-screws were used both singly and in combination with other metallic constructions. Long-term results of the treatment were followed up in 21 patients (6 months), in 14 of them excellent results were noted, in 5--good, in 1--satisfactory and in 1--unsatisfactory results. One complication was notes as the formation of a false articulation of the radius neck. An analysis of the results showed that the application of mini-scr

[Treatment of isolated fractures of the olecranon: percutaneous double-screw fixation versus conventional tension band wiring].

BACKGROUND: Isolated fractures of the olecranon account for 7-10% of all osseous injuries in adults. These fractures are usually treated surgically by conventional tension band wiring. The percutaneous double-screw fixation is an optional treatment in patients with two fragment olecranon fractures with some postoperative advantages. PATIENTS AND METHODS: A total of 13 patients with isolated fractures of the olecranon (Schatzker-Schmelling type A) treated by percutaneous double-screw fixation were included in this prospective study. The mean age of patients was 43.6 ± 11.0 years. Intraoperativ

Treatment of Complex Proximal Humeral Fractures in the Elderly with Reverse Shoulder Arthroplasty.

OBJECTIVE: To assess the short-term efficacy of reverse shoulder arthroplasty in the treatment of complex proximal humeral fractures in the elderly. METHODS: Forty-three elderly patients treated operatively for complex proximal humeral fractures with reverse shoulder arthroplasty from July 2017 to January 2019 were retrospectively reviewed. To be specific, 12 males and 31 females were reviewed with an average age of 72.0 years (range, 66-78 years). All fractures were attributed to trauma and treated for 8.0 days on average (range, 6-11 days). As suggested from Neer classification, 21 cases (4

A low-cost transhumeral prosthesis operated via an ML-assisted EEG-head gesture control system.

Objective.Key challenges in upper limb prosthetics include a lack of effective control systems, the often invasive surgical requirements of brain-controlled limbs, and prohibitive costs. As a result, disuse rates remain high despite potential for increased quality of life. To address these concerns, this project developed a low cost, noninvasive transhumeral neuroprosthesis-operated via a combination of electroencephalography (EEG) signals and head gestures.Approach.To address the shortcomings of current noninvasive neural monitoring techniques-namely, single-channel EEG-we leveraged machine l

Early dislocation after reverse total shoulder arthroplasty.

BACKGROUND: Although instability can occur after reverse total shoulder arthroplasty (RTSA), the risk factors, the treatment, and ultimate fate of the implant in these patients remains poorly understood. METHODS: Demographics, acute treatment, and the need for revision were evaluated in all patients with RTSAs who sustained a subsequent dislocation within the first 3 months. Standardized outcome scores were collected preoperatively and at the final follow-up. RESULTS: Atraumatic instability occurred in 11 patients (incidence, 2.9%) treated with RTSA early (before 3 months postsurgery). The m

Fitting of electronic elbow on an elbow disarticulated patient by means of a new surgical technique.

Rehabilitation of trans-humeral amputees represents a challenge. One of the difficulties to overcome is prosthetic suspension. In the case of elbow disarticulation the presence of humeral condyles permits better suspension, but there is no room for elbow joints, particularly electronic ones, and the only available mechanisms are provided by external hinges. The purpose of this report is to present a case of a primarily elbow disarticulated patient, with indication for surgical revision due to bad skin coverage, neuroma and the wish to improve prosthetic fitting. The surgical plan outlined was

Elbow dislocation with intra-articular fracture: the results of operative treatment without repair of the medial collateral ligament.

PURPOSE: To determine the effectiveness of a protocol for the treatment of fracture-dislocations of the elbow based on the concept that, if dislocation of the elbow with associated fractures can be made to resemble a simple elbow dislocation by repairing or reconstructing the fractured structures, repair of the medial collateral ligament (MCL) will not be necessary. METHODS: Over a 5-year period, a single surgeon operated on 34 patients with a posterior dislocation of the elbow associated with one or more intra-articular fractures. The mean age of these 19 men and 15 women was 48 years. Assoc

Patient and healthcare professional perspectives on which potential prognostic factors for failure of total elbow replacement should be investigated.

BACKGROUND: Total elbow replacement (TER) is an established treatment for the painful arthritic elbow; however, TER has higher failure rates than other joint replacements, such as hip and knee replacement. Understanding the prognostic factors associated with failure of TER is essential for informed decision-making between patients and clinicians, patient selection, and service planning. The aim of this study is to explore the views of patients and healthcare professionals on which potential prognostic factors should be investigated in relation to TER failure. METHODS: This evaluation comprise
