Surgical anatomy and morphologic variations of umbilical structures.

The umbilicus is the main access route to the abdominal cavity in laparoscopic surgeries. However, its anatomical configuration is rarely studied in the surgical and anatomical literature. With introduction of laparoendoscopic single-site surgery and considering the significant number of primary and postoperative umbilical hernias, we felt the necessity to comprehensively study the umbilical structures and analyze their protective function against hernias. Twenty-four embalmed cadavers were studied in the anatomy laboratory of Case Western Reserve University. Round hepatic, median and medial l

A single volar incision fasciotomy will decompress all three forearm compartments: a cadaver study.

INTRODUCTION: Although there are three distinct compartments of the forearm, the typical approach for decompression of forearm compartment syndrome is a single incision fasciotomy of the volar compartment. This study had two purposes. The first purpose of this study was to investigate the compartment pressure changes within the forearm compartments following the creation of an acute compartment pressure using fresh-frozen cadaver forearm specimens. The second purpose was to compare the compartment pressure changes of the volar, dorsal, and mobile wad compartments at frequent intervals followin

Anatomical association between wrist extensor musculature and topographical pain sensitivity maps of the elbow area.

OBJECTIVE: High-density topographical sensitivity maps have been developed to visualize nonuniformity deep tissue pain sensitivity in, for example, lateral epicondylitis (LE). The aim of this cadaveric study was to determine the anatomical association between the topographical sensitivity maps over the elbow area and wrist extensor musculature. METHODS: A topographical pressure sensitivity map consisting of 12 points forming a 3 × 4 matrix: 4 points in the superior part, 4 points in the middle, and 4 points in the lower part around the lateral epicondyle was marker on a 50-year embalmed cadav

Clinical measurements of hard palate and implications for subepithelial connective tissue grafts with suggestions for palatal nomenclature.

PURPOSE: The objectives of the present study were to 1) identify a reliable measuring technique for a palatal graft, 2) observe the patterns of the neurovascular bundle, and 3) identify the morphology of the hard palate. Subepithelial connective tissue grafts are performed at an increasing rate to improve esthetics and oral health. Palatal graft techniques began in 1963, and today the subepithelial connective tissue graft is the most widely accepted technique. The greater palatine nerve and artery are critical neurovascular structures to identify. Their pattern and palate morphology are not we

Surgical anatomy of the 10th and 11th intercostal, and subcostal nerves: prevention of damage during lumbotomy.

PURPOSE: In a descriptive, inventorial anatomical study we mapped the course of the 10th and 11th intercostal nerves, and the subcostal nerve in the abdominal wall to determine a safe zone for lumbotomy. MATERIALS AND METHODS: We dissected 11 embalmed cadavers, of which 10 were analyzed. The 10th and 11th intercostal nerves, and the subcostal nerve were dissected from the intercostal space to the rectus sheath. Analysis was done using computer assisted surgical anatomy mapping. A safe zone and an incision line with a minimum of nerve crossings were determined. RESULTS: The 10th and 11th inte

Motion of the fibula relative to the tibia and its alterations with syndesmosis screws: A cadaver study.

BACKGROUND: The motion of the fibula in relation to the tibia is coupled on the motion of the talus in the ankle joint. Several authors investigated this motion with different methods. An injury of the elastic fixation of the fibula to the tibia and its treatment with the syndesmotic set screw has an impact on this motion. METHODS: The motion of the fibula relative to the tibia was measured in eight embalmed human above the knee amputated cadaver specimens using a 3D-motion analysis system. The relative motion was measured from 50° of plantar flexion to 30° of dorsiflexion. Experiments were p

Hepatic steatosis, fibrosis, and cancer in elderly cadavers.

The incidence of hepatic steatosis, fibrosis, and cancer in the elderly population remains unknown. Human cadavers used in anatomy teaching, which come largely from older adults, may provide liver tissue for examining their pathologies. Livers were obtained from 68 cadavers (mean age 82.1 ± 10.4 years) with diverse causes of death in the Anatomy course at Mount Sinai School of Medicine. Paraffin sections were stained with hematoxylin and eosin and Sirius red and evaluated for steatosis, fibrosis, cancer, and lipofuscin. Tissue preservation was graded as good in 38.2% of the embalmed livers, fa

Lateral pterygoid muscle: a three-dimensional analysis of neuromuscular partitioning.

The lateral pterygoid (LP) has been implicated in temporomandibular joint (TMJ) pathology. Few studies have examined muscle architecture of the superior (SLP) and inferior (ILP) heads of LP; moreover, the pattern of intramuscular innervation is poorly defined. The purpose of this study was to determine patterns of intramuscular innervation of LP using 3D modeling. The superior and lateral aspects of LP were exposed in 10 embalmed cadaveric specimens. Nerves entering the muscle, all branches of the mandibular nerve (V(3) ), were followed intramuscularly in short segments and sequentially digiti

Surgical approach for a new knee prosthesis concept (TSTP) retaining both cruciate ligaments.

The transversal support tibial plateau (TSTP) concept was developed to retain both cruciate ligaments during total knee arthroplasty. TSTP design consists of two individual joint surfaces, reinforced beneath the joint line by two joint surface supports and buttressed by a single transversal support (TS). This configuration is devised to provide good bony anchoring especially for the TS, and to ensure long-term alignment of the individual joint surfaces. TS insertion requires a small extra incision. This study assessed techniques to implant the TS into the tibia. Using a specially designed aimi

Is the high submandibular transmasseteric approach to the mandibular condyle safe for the inferior buccal branch?

PURPOSE: There are basically 3 main approaches for extra-articular mandibular condyle fractures: low cervical, retromandibular and preauricular. These include a risk of facial palsy affecting the marginal mandibular branch. We use a high submandibular transmasseteric approach featuring masseter section 10-20 mm above the mandibular basilar edge. Our null hypothesis was that both the marginal mandibular and the inferior buccal branches are not more at risk than in other surgical approaches. METHODS: This study was based on 20 parotidomasseteric dissections from 10 embalmed cadaveric heads. We

Ultrasound-guided blocks of the ilioinguinal and iliohypogastric nerve: accuracy of a selective new technique confirmed by anatomical dissection.

BACKGROUND: Ilioinguinal and iliohypogastric nerve blocks may be used in the diagnosis of chronic groin pain or for analgesia for hernia repair. This study describes a new ultrasound-guided approach to these nerves and determines its accuracy using anatomical dissection control. METHODS: After having tested the new method in a pilot cadaver, 10 additional embalmed cadavers were used to perform 37 ultrasound-guided blocks of the ilioinguinal and iliohypogastric nerve. After injection of 0.1 ml of dye the cadavers were dissected to evaluate needle position and colouring of the nerves. RESULTS:

Innervation patterns of the inferior glenohumeral ligament: anatomical and biomechanical relevance.

Although the Inferior Glenohumeral Ligament (IGHL) has a well known mechanical and proprioceptive relevance in shoulder stability, the interrelation of the ligament's anatomical disposition/innervation has not actually been described previously. The purpose of the study was to determine the IGHL innervation patterns and relate them to dislocation. Forty-five embalmed and 16 fresh-frozen human cadaveric shoulders were studied. Masson's Trichrome staining detailed the intraligamentous nerve fiber arrangements. The effect on the articular nerves of an anteroinferior dislocation of the shoulder jo

Detection of herpes simplex virus type 1 in human ciliary ganglia.

PURPOSE: To determine whether herpes simplex virus type 1 (HSV-1) DNA is present in the ciliary ganglion (CG). METHODS: Fifty CG and 47 trigeminal ganglia (TG) were resected from 63 formalin-fixed cadavers between 56 and 98 years of age that had been embalmed within 12 hours of death. The donors had no known active HSV infection at the time of death. DNA was extracted from all ganglia by proteinase-K digestion (TG) or digestion by a mild lysis buffer (CG). DNA was amplified by polymerase chain reaction for sequences from human chromosome 18, D18S1259 (positive control), and from the HSV-1 DNA

Stability of open-book pelvic fractures using a new biomechanical model of single-limb stance.

OBJECTIVE: A new biomechanical model of single-limb stance was developed to test the stability of intact, injured, and internally fixed pelves. DESIGN: Single-limb stance was simulated by applying muscle forces and body mass loading to cadaver pelves. We created a rotationally unstable "open-book" pelvic injury in nine embalmed pelves by dividing the ligaments of the pubic symphysis, pelvic floor, and anterior and interosseus sacroiliac joint. All pelves were devoid of gross structural abnormalities. INTERVENTION: Two methods of internal fixation of the pubis symphysis were compared: (a) a c

[Strain distribution in the L1 vertebra under axial load and load transmitting behavior between the vertebral body and posterior elements].

This study investigated the stress distribution in the L1 vertebra under a vertical axial load. Five normal spinal units (T12-L2) were obtained from embalmed human cadavers, to measure the surface strain in the L1 vertebra. Stress screening using a brittle-coat was prepared beforehand, then fourteen sites on the lateral half of the surface on the L1 vertebra were carefully chosen to install rosette strain gauges. A vertical axial load was applied up to 1471 N in two degrees of flexion, in the neutral position, and in two degrees of extension of the intact spine, and in the neutral facetectomiz

Prophylactic cerclage: a method of preventing femur fracture in uncemented total hip arthroplasty.

The incidence of femur fracture in non-cemented hip arthroplasty has been reported to be between 4.1% and 27.8%. To quantitate the hoop stress generated during insertion of a femoral broach in total hip arthroplasty and determine the effect of cerclage with both braided cable and wire, we harvested 14 pairs of embalmed cadaver femurs. These were reamed and broached to duplicate the surgical technique of inserting a straight non-cemented femoral component. Group one consisted of eight matched cadaver femurs which were tested using a single 2.0 mm chrome-cobalt cable placed around the calcar of

Mechanical considerations for the syndesmosis screw. A cadaver study.

The purpose of this study was to examine the mechanical necessity of using a syndesmosis screw to supplement rigid internal fixation of the fibula and medial malleolus in the treatment of pronation-external rotation fractures. The legs of thirty embalmed and five fresh cadavera were dissected and mounted through the tibia to a frame so that multiple radiographs could be made with a constant relationship between the specimen and the x-ray apparatus. A standardized pronation-external rotation load was applied to the foot, and widening of the syndesmosis was studied on mortise radiographs that we

Ultrasound-guided injection of the rotator interval synovial lining: A cadaveric validation.

INTRODUCTION: Ultrasound-guided injections of the rotator interval (RI) are commonly performed in clinical practice targeting the anatomical interface between the long head of the biceps tendon (LHBT) and the biceps reflection pulley (BRP). Herewith, cadaveric validation of the exact placement of the injectate inside the synovial compartment of the RI is lacking in the pertinent literature. The purpose of this study was to investigate the correct placement and the spread patterns of different dye volumes within the synovial space of the RI of the shoulder in cadaveric samples. METHOD: The RI
