Precision forensic toxicology: Leveraging Toxicogenomics, multi-omics, and artificial intelligence for next-generation poisoning investigations.

For much of its history, forensic toxicology was built on a core analytical question: what poison or drug is present, and at what concentration? That question still matters, but modern casework increasingly shows that it is no longer sufficient on its own. The field grew from the nineteenth-century chemical toxicology of Mathieu Orfila and the arsenic-detecting Marsh test into a discipline central to medicolegal death investigation, overdose certification, and exposure reconstruction. Yet contemporary practice must now contend with novel psychoactive substances, highly potent synthetic opioids

Socioeconomic status and outcomes following spine stereotactic body radiation therapy.

PURPOSE: The Area Deprivation Index (ADI) ranks neighborhoods by adverse social exposome, considering factors related to income, education, employment, and housing quality. This study evaluates the association between socioeconomic status and outcomes following spine stereotactic body radiation therapy (SBRT). METHODS: A prospectively maintained database of consecutive patients treated with spine SBRT at a quaternary referral center from 2001 to 2024 was analyzed. Patient addresses were matched to the 2020 ADI to define most advantaged (ADI ≤ 50, 140 patients, 227 metastases) and least advant

The PSC scientific community resource: an asset for multi-omics interrogation of primary sclerosing cholangitis.

BACKGROUND: Primary sclerosing cholangitis (PSC) is a rare, chronic cholestatic liver disease that often progresses to end-stage liver disease and/or the development of hepatobiliary neoplasia. Lack of prognostic tools and treatment options for PSC is driven in part by our poor understanding of its pathogenesis, which is thought to be complex, the interaction of genetic variants, environmental influences and biological response throughout the course of disease. The PSC Scientific Community Resource (PSC-SCR) seeks to overcome previous shortcomings by facilitating novel research in PSC with the

Childhood adversity correlates with stable changes in DNA methylation trajectories in children and converges with epigenetic signatures of prenatal stress.

Childhood maltreatment (CM) is an established major risk factor for a number of negative health outcomes later in life. While epigenetic mechanisms, such as DNA methylation (DNAm), have been proposed as a means of embedding this environmental risk factor, little is known about its timing and trajectory, especially in very young children. It is also not clear whether additional environmental adversities, often experienced by these children, converge on similar DNAm changes. Here, we calculated a cumulative adversity score, which additionally to CM includes socioeconomic status (SES), other life

Understanding environmental contributions to autism: Causal concepts and the state of science.

UNLABELLED: The complexity of neurodevelopment, the rapidity of early neurogenesis, and over 100 years of research identifying environmental influences on neurodevelopment serve as backdrop to understanding factors that influence risk and severity of autism spectrum disorder (ASD). This Keynote Lecture, delivered at the May 2016 annual meeting of the International Society for Autism Research, describes concepts of causation, outlines the trajectory of research on nongenetic factors beginning in the 1960s, and briefly reviews the current state of this science. Causal concepts are introduced, in

Early-Onset Colorectal Cancer: From Epidemiologic Shift to Life-Course Carcinogenesis and Age-Attuned Care.

PURPOSE OF REVIEW: Early-onset colorectal cancer (EOCRC), conventionally defined as colorectal cancer diagnosed before the age of 50 years, has become one of the most important and unsettling epidemiologic shifts in gastrointestinal oncology. This review critically synthesizes recent evidence on EOCRC epidemiology, risk architecture, life-course carcinogenesis, molecular and microbiome-associated mechanisms, diagnostic delay, screening limitations, treatment considerations, and survivorship needs, with the aim of reframing EOCRC as an age-attuned clinical and biological challenge rather than a

The built environment and adverse cardiovascular events in US veterans with cardiovascular disease.

QUESTION: Can AI-extracted interpretable built environment features predict major adverse cardiovascular events (MACE) in a national veteran population? FINDINGS: In this cohort study of 770,990 U.S. veterans, seven built environment features were significantly associated with an increased risk of MACE. Two features, old/dilapidated buildings and visible wire, were associated with a decreased risk. Greenery was only linked to increased risk in higher social deprivation index quartiles. MEANING: Built environment features can serve as predictors of MACE, highlighting the potential role of nei

Computational histology reveals that concomitant application of insect repellent with sunscreen impairs UV protection in an ex vivo human skin model.

BACKGROUND: Histological alterations such as nuclear abnormalities are sensitive biomarkers associated with diseases, tissue injury and environmental insults. While visual inspection and human interpretation of histology images are useful for initial characterization, such low-throughput procedures suffer from inherent limitations in terms of reliability, objectivity and reproducibility. Artificial intelligence and digital morphometry offer unprecedented opportunities to quickly and accurately assess nuclear morphotypes in relation to tissue damage including skin injury. METHODS: In this work

Methodology for Biological Sample Collection, Processing, and Storage in the Newcastle 1000 Pregnancy Cohort: Protocol for a Longitudinal, Prospective Population-Based Study in Australia.

BACKGROUND: Research in the developmental origins of health and disease provides compelling evidence that adverse events during the first 1000 days of life from conception can impact life course health. Despite many decades of research, we still lack a complete understanding of the mechanisms underlying some of these associations. The Newcastle 1000 Study (NEW1000) is a comprehensive, prospective population-based pregnancy cohort study based in Newcastle, New South Wales, Australia, that will recruit pregnant women and their partners at 11-14 weeks' gestation, with assessments at 20, 28, and 3

Influence of perinatal and childhood exposure to tobacco and mercury in children's gut microbiota.

BACKGROUND: Early life determinants of the development of gut microbiome composition in infants have been widely investigated; however, if early life pollutant exposures, such as tobacco or mercury, have a persistent influence on the gut microbial community, its stabilization at later childhood remains largely unknown. OBJECTIVE: In this exposome-wide study, we aimed at identifying the contribution of exposure to tobacco and mercury from the prenatal period to childhood, to individual differences in the fecal microbiome composition of 7-year-old children, considering co-exposure to a width of

Longitudinal biomonitoring of mycotoxin exposure during pregnancy in the Yale Pregnancy Outcome Prediction Study.

Mycotoxins are fungal toxins that may trigger adverse health effects in pregnant women and their unborn children. Yet, data is scarce on the dynamic exposure patterns of mycotoxins in pregnant women, especially in the United States. This study assessed mycotoxin exposure profiles in women (n = 50) from the Yale Pregnancy Outcome Prediction Study (YPOPS) cohort at four distinct time points. Multi-analyte human biomonitoring assays based on liquid chromatography tandem mass spectrometry (LC-MS/MS), were developed for human serum and plasma matrices. The serum method was applied, together with an

White matter hyperintensities and their impact in brain structure and function in alzheimer's disease and behavioral variant frontotemporal dementia across Latin America and the United States: a cross-sectional study.

BACKGROUND: White matter hyperintensities (WMHs) are a core manifestation of normal and pathological aging and are potentially linked to geographical differences in social and physical exposomes. Previous studies have not examined the impact of WMHs burden on neurodegeneration and cognition in healthy controls (HCs) and patients with Alzheimer's disease (AD) and behavioral variant frontotemporal dementia (bvFTD) across geographic regions. This study addressed this gap by assessing the impact of WMHs burden on participants with and without dementia from Latin America (LA) and the United States

Prenatal exposure to environmental contaminants and cord serum metabolite profiles in future immune-mediated diseases.

BACKGROUND: Prenatal exposure to environmental contaminants is a significant health concern because it has the potential to interfere with host metabolism, leading to adverse health effects in early childhood and later in life. Growing evidence suggests that genetic and environmental factors, as well as their interactions, play a significant role in the development of autoimmune diseases. OBJECTIVE: In this study, we hypothesized that prenatal exposure to environmental contaminants impacts cord serum metabolome and contributes to the development of autoimmune diseases. METHODS: We selected c

Centenarians exposed to the Spanish flu in their early life better survived to COVID-19.

BACKGROUND: Although it is known that mortality due to COVID-19 increases progressively with age, the probability of dying from this serious infection among the oldest-old population is little known, and controversial data are found in literature. METHODS: We examine the mortality by year and month of birth of Belgians who had turned 100 during the current COVID-19 pandemic and whose birth fell on the years around the end the First World War and the outbreak of the H1N1 "Spanish flu" pandemic. FINDINGS: The COVID-19 mortality of the "older" centenarians is significantly lower than that of "y

Multi-omics analysis reveals that co-exposure to phthalates and metals disturbs urea cycle and choline metabolism.

This study aimed to evaluate the response of HepaRG cells after co-exposure to phthalates and heavy metals, using a high-dimensional biology paradigm (HDB). Liver is the main metabolism site for the majority of xenobiotics. For this reason, the HepaRG cell line was used as an in vitro model, and cells were exposed to two characteristic mixtures of phthalates and heavy metals containing phthalates (DEHP, DiNP, BBzP) and metals (lead, methylmercury, total mercury) in a concentration-dependent manner. The applied chemical mixtures were selected as the most abundant pollutants in the REPRO_PL and

Refining health indicators in French Guiana: Avoidable mortality through prevention and care (2001-2023).

INTRODUCTION: In French Guiana, life expectancy lags 2 to 3 years behind mainland France. Beyond widespread poverty, structural underdevelopment, and low physician density, the Guianese exposome differs radically from that of mainland France. Avoidable mortality - deaths that could have been prevented through prevention or care - had never been quantified in detail. This study aims to measure its extent, structure, and social cost over the 2001-2023 period. METHODS: This study analyses avoidable premature mortality below age 75 in French Guiana and mainland France over the period 2001-2023, u

Resilience mechanisms in Parkinson's disease: a domain-specific conceptual framework.

While nigrostriatal dopaminergic cell loss is the pathological hallmark of Parkinson's disease, it is well-known that dopamine deficiency cannot fully explain the clinical severity or disease progression. Emerging data suggest that resilience, i.e. the brain's capacity to maintain function despite pathology, plays a key role in shaping clinical variability of both motor and non-motor symptoms. However, different contributing terms have been proposed, including brain compensation and reserve, and it remains unclear how these terms can be delineated both conceptually and mechanistically. Therefo
