A filamentary soft robotic probe for multimodal in utero monitoring of fetal health.

Fetal surgery offers valuable opportunities to address severe congenital disabilities, yet accurate evaluation of fetal physiological changes during in utero procedures to mitigate the risk of operative complications remains an unmet need. Conventional unimodal approaches lack predictive value, specificity and compatibility with minimally invasive interventions. Here we present a bioelectronic system featuring a multimodal, steerable filamentary probe that interfaces directly with the fetus in utero, enabling reliable and minimally invasive monitoring of various physiological parameters. Radial Expansion of Chorioamniotic Membranes After Perforation Causes Less Damage Than Fixed Diameter Instruments.

OBJECTIVE: Iatrogenic preterm premature rupture of membranes (iPPROM) complicates up to 40% of fetoscopic surgeries, driven by the limited healing potential of the chorioamniotic membrane after perforation. While radial expansion reduces tissue damage in other surgical fields, its application in fetal surgery remains unexplored. METHODS: We compared perforations in fresh chorioamniotic membranes created by three devices: standard 2 mm (6Fr) and 4 mm (12Fr) sheathed trocars, and an angioplasty balloon that radially expands from 2 to 4 mm. Perforation dimensions (height, width, area) and shape

Intrathoracic Kidney Associated with Congenital Diaphragmatic Hernia: Prenatal Detection and Outcomes.

INTRODUCTION: Intrathoracic kidney (ITK) is a rare renal ectopia often diagnosed incidentally. It is infrequently detected prenatally and frequently coexists with congenital diaphragmatic hernia, a rare clinical combination with limited reported cases. METHODS: A retrospective case series was conducted of prenatally diagnosed ITK with congenital diaphragmatic hernia at a tertiary fetal medicine center over 6 years. Data included prenatal imaging, genetic studies, fetal interventions, delivery, surgical management, and long-term outcomes. RESULTS: Four male fetuses with ITK and diaphragmatic

In utero Partial Exchange Transfusion Combined with in utero Blood Transfusion for Prenatal Management of Twin Anemia-Polycythemia Sequence.

Monochorionic twin pregnancies are at risk of unique complications due to placental sharing and vascular connections between placental territories assigned for each twin. Twin anemia-polycythemia sequence (TAPS) is an infrequent but potentially dangerous complication of abnormal placental vascular connections. TAPS occurs due to very-small-caliber (< 1 mm) abnormal placental vascular connections which lead to chronic anemia in the donor twin and polycythemia in the recipient twin. TAPS may occur spontaneously or following fetoscopic laser photocoagulation of communicating placental vessels for

Fetal cardiovascular hemodynamics in twin-twin transfusion syndrome.

Twin-twin transfusion syndrome (TTTS) complicates 10-15% of monochorionic-diamniotic (MCDA) pregnancies. It originates from unbalanced transfer of fluid and vasoactive mediators from one twin to its co-twin via placental anastomoses. This results in hypovolemia in the donor and hypervolemia and vasoconstriction in the recipient twin. Consequently, the recipient demonstrates cardiovascular alterations including atrioventricular valve regurgitation, diastolic dysfunction, and pulmonary stenosis/atresia that do not necessarily correlate with Quintero-stages. Selective fetoscopic laser photocoagul

Twin anemia-polycythemia sequence in a case of monoamniotic twins.

Twin anemia-polycythemia sequence (TAPS) complicates up to 6% of monochorionic diamniotic twin pregnancies, typically in the late second or third trimester. The presence of only a few and very small arteriovenous vascular anastomoses characterizes the underlying angioarchitecture at the chorionic plate in cases of TAPS. In monoamniotic twins, large vascular anastomoses can usually be seen at the placental vascular equator, and therefore one would not expect the development of TAPS in monoamniotic twins. We report a case of TAPS in a monoamniotic pregnancy at 26 + 5 weeks' gestation which respo

Elective laparoscopic fetal laser photocoagulation in twin-twin transfusion syndrome: a case report.

Twin-twin transfusion syndrome (TTTS) is a complication of multiple gestation in which arteriovenous vascular communications occur in a shared placenta. Due to these communications, there is an imbalance of blood flow between the developing fetuses. This results in 1 twin becoming the donor and the other becoming the recipient. In severe cases, the recipient may experience polycythemia, polyhydramnios, and hydrops fetalis, while in the donor oligohydramnios and severe anemia may develop. It has been reported that fetal mortality can reach as high as 60% to 80% if TTTS develops before 26 weeks'

Changes in the recipient fetal Tei index in the peri-operative period after laser photocoagulation of placental anastomoses for twin-twin transfusion syndrome.

OBJECTIVE: To assess serial changes in the Tei index of the recipient fetus secondary to fetoscopic laser photocoagulation of placental anastomoses (FLP) for twin-twin transfusion syndrome (TTTS) during the peri-operative period. STUDY DESIGN: A prospective study of cases that underwent FLP at the Texas Children's Fetal Center was performed. Mean and individual changes in the Tei index measurements were obtained during the pre-operative, intra-operative and the post-operative (12-h and 24-h) periods after surgery and were compared using paired t-test. A p-value of < 0.05 was considered as sig

Specific complications of monochorionic twin pregnancies: twin-twin transfusion syndrome and twin reversed arterial perfusion sequence.

Monochorionic twins are subjected to specific complications which originate in either imbalance or abnormality of the single placenta serving two twins. This unequal placental sharing can cause complications including twin-twin transfusion syndrome (TTTS), twin anemia-polycythemia sequence (TAPS), selective intrauterine growth restriction or twin reversed arterial perfusion sequence (TRAP). Monochorionicity also makes the management of these specific complications as well as that of a severe malformation in one twin hazardous since the spontaneous death of one twin exposes the co-twin to a ris

Fetal surgery for congenital diaphragmatic hernia: the North American experience.

The role for fetal surgery in treating fetuses with congenital diaphragmatic hernia (CDH) is unclear. Two decades of investigation have improved our understanding of the prenatal natural history, pathophysiology, and outcomes of these patients. During this same period, there have been advances in fetal surgery techniques including improvements in fetal monitoring, maternal-fetal anesthesia, tocolysis, and improved instrumentation to permit increased application of videoscopic approaches. Because of technical challenges, open fetal repair of CDH has been abandoned. Fetal tracheal ligation has s

Demonstration of placental vascular anatomy in monochorionic twin gestations.

Invasive treatment modalities for severe chronic twin-to-twin transfusion syndrome (TTTS), such as fetoscopic laser coagulation of communicating vessels, have revived the need for detailed studies of placental angioarchitecture. We describe a practical placental vascular injection technique using alcohol-resistant tissue-staining dyes. Injection of color-coded gelatin-dye mixtures effectively delineated the intertwin vasculature, and allowed unequivocal macroscopic classification of vascular communications as artery-to-artery, vein-to-vein, or deep artery-to-vein anastomoses. The existence of

Fetal laser surgery in genetic polydactyly mice.

The first digital ray of the hindlimb plate in Slc:ICR mouse fetus was irradiated with 2 watts argon laser beam for 0.3 sec after releasing from the abdominal cavity and envelop of uterine myometrium on day 13 of gestation, and then the fetuses were allowed to develop in the abdominal cavity contacting with the uterus via the placenta exo utero until term. ICR mouse fetuses which received fetal laser surgery lost their first digits completely, resulting in 4-digit hindfoot on day 18 of gestation. The homozygous Polydactyly Nagoya (Pdn/Pdn) mice exhibit 1-3 extra digits both in the fore- and hi

Drugs in feto maternal medicine.

The study of drug distribution in pregnancy was limited by ethical, technical and medico-legal considerations as samples of body fluids could only be taken at delivery. In recent times fetal blood samples have been taken with the fetoscope and will provide a new tool to monitor fetal concentrations and metabolic pathways. The advanced technology of ultrasound allows non invasive study of the fetal circulation and early experience of sympathomimetic drugs administered to the mother will be discussed. Auto immune disorders carry high perinatal wastage. New drugs have made reproductive life possi

Maternal outcomes following prenatal spina bifida repair: A systematic review and meta-analysis.

OBJECTIVE: Prenatal surgical interventions for neural tube defects have been shown to improve fetal outcomes but are associated with previously poorly quantified maternal risk, which is inherently dependent on the surgical approach used. We hypothesized that a meta-analysis would enable objective quantitation of maternal risk and safety, stratified by which antenatal neural tube defect surgical approaches were employed. STUDY DESIGN: We conducted a meta-analysis by searching PubMed, Scopus, Embase, and Web of Science databases. We included all studies that performed prenatal intrauterine neur

In-Utero Myelomeningocele Repair: Historical Evolution, Technical Refinements, and Global Impact of the Vanderbilt Experience.

Myelomeningocele (MMC) is a severe neural tube defect resulting from incomplete closure of the spinal column during early embryogenesis, often leading to lifelong neurological, urological, and orthopedic impairments. The "two-hit hypothesis" describes both the primary embryologic malformation and progressive in-utero neural injury as contributors to clinical severity. While postnatal repair prevents further exposure of neural tissue, it does not reverse the intrauterine damage already sustained. This manuscript provides a comprehensive review of the historical evolution, surgical techniques, a

Advances in Fetal Surgery: A Narrative Review of Therapeutic Interventions and Future Directions.

Fetal surgery has emerged as a viable option for the management of selected congenital anomalies that result in severe or lethal outcomes if left untreated until birth. Conditions such as spina bifida, urinary tract obstruction, congenital cystic adenomatoid malformation, diaphragmatic hernia, sacrococcygeal teratoma, and twin-twin transfusion syndrome have shown improved prognosis after in utero intervention, open, or fetoscopically. Despite significant advances in surgical methods and anesthesia, preterm labor remains a primary concern. Stem cell transplantation and in utero gene therapy are

Fetal anemia in monochorionic twins: a review on diagnosis, management, and outcome.

INTRODUCTION: Monochorionic twins may develop fetal anemia when blood is unequally distributed via the placental vascular anastomoses. This review focuses on the causes of fetal anemia in complicated monochorionic twins and highlights the differences in management and outcome. AREAS COVERED: Fetal anemia can occur in the context of twin anemia polycythemia sequence (TAPS), chronic twin-twin transfusion syndrome (TTTS) and acute peripartum TTTS, and in cotwins after single fetal demise. Diagnosis of fetal anemia is based on abnormal Doppler ultrasound measurements. Management options include f

Myelomeningocele Surgery over the 10 Years Following the MOMS Trial: A Systematic Review of Outcomes in Prenatal versus Postnatal Surgical Repair.

Background and Objectives: Myelomeningocele is the most severe form of spina bifida, a congenital neural tube defect arising from an incomplete neural tube closure during early development with damage worsening with advancing gestational age. The Management of Myelomeningocele Study (MOMS) Trial proved that surgery performed before 26 weeks of gestation significantly improved the prognosis, significantly changing treatment paradigms. This article aims to provide a review of the changes and updates in spina bifida repair over the 10-year period following the MOMS Trial. Material and methods: We

Retinoic acid and tracheal occlusion for diaphragmatic hernia treatment in rabbit fetuses.

INTRODUCTION: Lung hypoplasia and pulmonary arterial hypertension in congenital diaphragmatic hernia lead to a high perinatal mortality. Although sustained fetoscopic tracheal occlusion (TO) improves lung development, a major side effect is abnormal pneumocyte differentiation. This study evaluated the potential ability of intratracheal retinoic acid (RA) administration to reduce adverse effects of sustained TO in a rabbit model of diaphragmatic hernia. METHODS: A left diaphragmatic defect was created on day 23 in time-dated pregnant rabbits. On day 28, the same rabbits underwent sham surgery
