Evaluating factors that can influence spirography ratings in patients with essential tremor.

Spirograph drawings are used in most comprehensive assessments of essential tremor (ET). Nevertheless, several different paradigms are used and no effort has been made to compare these. We used two different cohorts to assess different aspects of spiral rating. In the first, we had subjects simulate different levels of effort by writing (1) 'normally', (2) 'slowly and carefully', (3) 'softly', and (4) 'rapidly' using both their dominant and non-dominant hands. In the second, subjects (1) drew in between the lines of two drawn spirals, (2) traced a previously drawn spirograph, and (3) drew free

The regulation of fine movements in patients with Charcot Marie Tooth, type Ia: some ideas about continuous adaptation.

The flexibility of the human motor system is remarkable. Even when parts of the system are damaged, the output often remains optimal or near optimal. The neuromotor system is designed to keep the output optimal by shifting between input sources. This capability is termed the principle of continuous adaptation. This article describes an experiment in which patients suffering from a hereditary motor and sensory neuropathy, type 1a (Charcot Marie Tooth disease, type 1a), had to perform fine motor movements. We examined whether they were able to regulate these movements in spite of the fact that t

Selective drawing disorders after right subcortical stroke: a neuropsychological premorbid and follow-up case study.

We report the case of a patient affected by a subcortical lesion of the right non-dominant hemisphere, and demonstrate that he had selective constructional disorders by comparing his post-stroke performances with those assessed 18 months before the stroke. A detailed analysis was made of the visuospatial, perceptual, representational and executive competences involved in drawing tasks at one, two and six months post-stroke. Neuropsychological follow-up revealed the progressive recovery of all visuospatial abilities. This study provides some interpretative elements for constructional disorders

Neurodevelopmental study of writing disorders in middle childhood.

This study investigated patterns of neurodevelopmental dysfunction in children with writing disorders (WD). Records of children, ages 9 to 15 years, referred to a school problems clinic were examined. Using teacher questionnaire information, including ratings of writing legibility, mechanics, rate, linguistic sophistication, and spelling, 99 cases of WD were found. Sixty-three children without WD served as clinic controls. A cluster analysis revealed four discrete subtypes of WD and two non-WD control clusters. The four WD clusters were found to have different patterns of writing and reading c

Alexia without agraphia: a study of a case of verbal alexia without accompanying colour-naming defect.

A 47-year-old man developed a persistent form of alexia without agraphia as the result of a haemorrhagic intracerebral lesion in the left inferior temporo-occipital region, due to the rupture of an arteriovenous malformation. Surgical evacuation of the haematoma and excision of the malformation did not produce any modification of the reading deficit, which remained unchanged during a 4-year follow-up. The reading deficit was restricted to words and the patient was able to read only letter-by-letter, so that the whole words were reconstructed from the auditory names of the letters. So far, the

Charge-regulated wood-based aerogel triboelectric nanogenerator for sustainable mechanical energy harvesting and self-powered sensing.

Triboelectric nanogenerators (TENGs) have attracted increasing attention as promising technologies for sustainable mechanical energy harvesting and self-powered sensing. However, the electrical output of wood-aerogel-based TENGs is still limited by insufficient charge generation and severe charge dissipation during continuous operation. Herein, we report a high-performance TENG based on a carbon-nanotube-modified electrified wood aerogel (CNT/WA), in which the output performance is synergistically optimized through coupled regulation of porous structure and charge transport-dissipation behavio

The effect of task-oriented circuit training-based telerehabilitation on upper extremity motor functions in patients with Parkinson's disease: A randomized controlled trial.

INTRODUCTION: Upper extremity functions are impaired in patients with Parkinson's disease (PwPD), which leads to difficulties in activities of daily living (ADL), such as reaching or handwriting. The aim of this study was to investigate the effectiveness of task-oriented circuit training-based telerehabilitation (TOCT-TR) on the upper extremity motor function in PwPD. METHODS: In this randomized controlled trial (RCT) 30 PwPD (aged 45-70 years, Hoehn & Yahr stage I-III) were recruited and randomly allocated into two groups. The TOCT-TR group received home training through video sessions three

Differentiation of alcoholics. Childhood history of minimal brain dysfunction, family history, and drinking pattern.

Alcoholics were differentiated into two subgroups on the basis of drinking patterns and subjective response to alcohol. Severe drinkers (primary alcoholics) retrospectively reported more symptoms of childhood minimal brain dysfunction than less severe drinkers (secondary alcoholics), psychiatric patients, and normals. The alcoholics as a group reported a greater incidence of familial alcohol abuse than the psychiatric subjects, but a difference on this factor was not observed between the primary and secondary subgroups. In terms of clinical status, the primary alcoholics presented Minnesota Mu

Immunotherapy-associated dysgraphia as an early neurocognitive manifestation of immune effector cell-associated neurotoxicity syndrome: clinical characteristics, mechanistic insights, and assessment challenges.

BACKGROUND: Immune effector cell therapies, including chimeric antigen receptor T-cell (CAR-T) therapy and T-cell-engaging antibodies, are associated with immune effector cell-associated neurotoxicity syndrome (ICANS), a clinically recognized immune-mediated neurotoxicity. Among its neurological manifestations, writing impairment (dysgraphia), including paligraphia and other forms of writing disturbance, has emerged as a potentially sensitive clinical marker of higher-order cortical dysfunction. OBJECTIVE: This review synthesizes current clinical evidence on ICANS-associated dysgraphia, chara

Processing of self-related kinematic information embedded in static handwritten characters.

Handwritten characters are generated by our own motor actions, and previous studies have shown that the manner in which such characters are perceived and generated is related. However, the temporal course of the neural activation involved in the processing of self-related kinematic information embedded in static handwritten characters remains to be identified. We applied event-related potential (ERP) recording while participants judged whether handwritten characters were self- or non-self-generated. To test the effects of the self-related kinematic characteristics of static handwritten charact

A preliminary study of the neuroanatomical correlates of primary writing tremor: role of cerebellum.

INTRODUCTION: To explore the neuroanatomical correlates of primary writing tremor (PWT) and the role of cerebellum, using advanced structural neuroimaging. Till date, there are no studies exploring the gray and white matter changes using voxel-based morphometry (VBM) and diffusion tensor imaging (DTI) in PWT. METHODS: Ten male patients with PWT were evaluated clinically and with magnetic resonance imaging. VBM and DTI images of patients were compared with that of 10 healthy male subjects. Spatially unbiased infra-tentorial template (SUIT) analysis was done to investigate the alterations of ce

Is there information contained within the sentence-writing component of the mini mental state examination? A retrospective study of community dwelling older people.

OBJECTIVE: To investigate the relationship between features of the MMSE written sentence and cognitive function, depression and disability. METHODS: MMSE sentences from 191 community dwelling individuals without dementia from the Lothian Birth Cohort 1921 (LBC1921) study were: (a) photocopied and (b) typed as written. Sentences were rated for objective criteria: word number and frequency, first person usage, time orientation, and letter case. Twenty healthy raters (50% male, age 20-26 years), blind to all other data, rated each handwritten and typed sentence for subjective criteria: legibilit

Pure misallocation of "0" in number transcoding: a new symptom of right cerebral dysfunction.

To account for the mechanism of number transcoding, many authors have proposed various models, for example, semantic-abstract model, lexical-semantic model, triple-code model, and so on. However, almost all of them are based on the symptoms of patients with left cerebral damage. Previously, I reported two Japanese patients with right posterior cerebral infarction showing pure misallocation of "0" (omission: "40,265"-->"4,265," addition: "107"-->"1,007," transposition: "4,072"-->"4,702") both in writing and oral reading of Arabic numerals. To examine whether the pure misallocation of "0" is com

Regional cerebral blood flow correlates of the severity of writer's cramp symptoms.

UNLABELLED: Writer's cramp is a type of idiopathic focal dystonia with incompletely understood pathophysiology. Recent studies provide evidence that one element might be a sensory processing defect. We performed a PET study with O(15) H(2)O to find out in which brain areas activity correlates with the severity of writer's cramp symptoms. METHODS: We studied 10 patients with writer's cramp and 10 age- and gender-matched control subjects. There were seven conditions, each repeated twice: rest, writing, tapping with index finger for 2, 3, 4, and 5 min. For each scan, we obtained EMG recordings f

Acquired agraphia caused by focal brain damage.

Motor and linguistic aspects of writing were evaluated in 31 subjects with focal damage in 1 of 3 regions of the left hemisphere: (1) dorsolateral frontal lobe sparing primary motor cortex (group FL), (2) parietal lobe (group PL), or (3) temporal lobe (group TL). A standard procedure was used to evaluate writing for grapheme formation, spatial arrangement, spelling, word selection, grammar, and perseveration. It was predicted that agraphia would be observed in all 3 groups, and that the most severe impairments would be associated with frontal lobe damage, particularly in aspects of writing dep

A meaningful prediction of functional decline in amyotrophic lateral sclerosis based on multi-event survival analysis.

Amyotrophic lateral sclerosis (ALS) is a degenerative disorder of the motor neurons that causes progressive paralysis in patients. Current treatment options aim to prolong survival and improve quality of life. However, due to the heterogeneity of the disease, it is often difficult to determine the optimal time for potential therapies or medical interventions. In this study, we propose a novel method to predict the time until a patient with ALS experiences significant functional impairment (ALSFRS-R ≤ 2) for each of five common functions: speaking, swallowing, handwriting, walking, and breathin
