The Journal of foot and ankle surgery : official publication of the American College of Foot and Ankle Surgeons2011eng
Clawing of the digits is a deformity seen both in patients with and without rheumatoid arthritis, resulting in pain and deformity in the forefoot. After failure of conservative treatment, the Stainsby procedure is one surgical option for severe clawing and metatarsalgia in both rheumatoid and nonrheumatoid feet. Results from the originating authors (G.D. Stainsby and P.J. Briggs) are consistent and reliable; however, there is little material outside of the originating center. This article reviews our experience in the Western Sussex Hospitals NHS Trust. Sixteen consecutive patients who underwe

BACKGROUND: Metatarsalgia with plantar hyperkeratosis can be a difficult problem to treat. Many different techniques have been reported with the Weil osteotomy very popular currently. Despite good results complications include metatarsophalangeal stiffness, hyperextension of the toe, prehensile deficit and a reduction in strength. We evaluated our results with a distal diaphyseal osteotomy.
MATERIALS AND METHODS: From 1999 to 2005, we performed 66 distal midshaft subtraction osteotomies in 62 patients suffering from metatarsal overload without metatarsophalangeal subluxation. Fixation was obt

BACKGROUND: While some studies have demonstrated that metatarsus primus elevatus in hallux rigidus is an uncommon secondary change rather than a causative factor, we believe it is, in fact, more common. We advocate surgical treatment if the first ray is elevated beyond normal range, thereby acting as a decompression realignment osteotomy.
MATERIALS AND METHODS: We analyzed patients operated between June 1994 and December 1996. Oblique distal osteotomy of the first metatarsal was performed in 20 patients. A retrospective review of clinical and radiological outcomes was performed including an A

A distal metatarsal osteotomy with soft tissue correction is a frequently performed operation to correct mild to moderate hallux valgus deformity. This is a prospective study of 28 feet in 25 patients who underwent spike osteotomy of the first metatarsal with medial capsulorraphy for symptomatic hallux valgus. The osteotomy is a distal metatarsal osteotomy with a spike fashioned in the plantar and lateral quarter of the proximal fragment and impacted into the trough created in the center of the distal fragment, providing lateral and plantar shift of the distal fragment. The American Orthopaedi

BACKGROUND: There are indications that the diagnosis and management of common foot problems vary widely in general practice.
OBJECTIVES: Our aim was to explore the variation of GPs' diagnosis and management of common foot problems and the possible correlation between GPs' characteristics and their competence to diagnose correctly.
METHODS: In a cross-sectional design, 90 GPs in The Netherlands were invited to complete a questionnaire regarding seven vignettes with common foot problems (hallux valgus, hallux rigidus, fasciitis plantaris, tarsal tunnel syndrome, metatarsalgia, corns and callus

Sixty-six feet operated on for hallux valgus in 1984 were reviewed. Keller-Brandes' operation was performed in all cases, together with Lelièvre's "fibrous cerclage" and flexor hallucis longus tendon anchoring to the sesamoid pad, as described by A. Viladot. In most cases, satisfactory clinical correction was achieved and the patients could wear standard shoes. More than half of the cases presented partial limitation of movement at the first metatarsophalangeal joint; however, flexion power of the big toe was preserved, and the range of motion was wide enough to allow a painless and limp-free

OBJECTIVE: Foot pathologies among older people have increased considerably, especially as life expectancy has increased, becoming a common reason for podiatric and medical foot care. The purpose of this investigation was to determine the variances in the quality of life (QoL) linked to overall foot health in older population with presence or absence of foot problems.
METHODS: A retrospective case-control investigation was performed in an outpatient health center, where a senior clinician recorded information linked to clinical and sociodemographic features. Besides, self-reported information

BACKGROUND: Brachymetatarsia is a shortening of one or more metatarsal bones. This condition can result in pedal complaints of metatarsalgia, calluses, toe deformities, and shoe irritation. When non-surgical treatments fail, surgical options include acute one-stage lengthening with a bone graft or gradual callus distraction with an external fixator. Acute correction is preferred for lengthening under 14 mm, while gradual methods are favored for corrections over 15 mm due to concerns of neurovascular injury and achieving incomplete correction. Gradual correction, however, can be associated with

BACKGROUND AND OBJECTIVE: The aim of this study was to explore the effectiveness of arch support functional insoles to prevent metatarsalgia.
METHOD: Twenty-five healthy females participated in the study. A Vicon motion capture system was used to collect kinematics data of the lower limb. An AMTI force plate was used to record the vertical ground reaction force (GRF), and the Novel Pedar-X System was used to measure foot pressure while subjects wore normal insoles or functional insoles with an arch support during walking and jogging.
RESULTS: With the arch support functional insoles, the fir

BACKGROUNDS: There has long been a consensus that shortening of the first metatarsal during hallux valgus reconstruction could lead to postoperative transfer metatarsalgia. However, appropriate shortening is sometimes beneficial for correcting severe deformities or relieving stiff joints. This study is to investigate, from the biomechanical perspective, whether and how much shortening of the first metatarsal could be allowed.
METHODS: A finite element model of the human foot simulating the push-off phase of the gait was established. Progressive shortening of the first metatarsal from 2 to 8 m

BACKGROUND: Minimally invasive surgery has a shorter surgical time, and in this study we focus on minimally invasive distal metatarsal metaphyseal osteotomy (DMMO). The operation seems to be less complex but requires a high learning curve. We report on our first patients to underline the need for extensive training and great awareness for the risks in the early learning stages.
METHODS: We evaluated 27 patients (mean age = 60.9 years) with a mean follow-up time of 7.2 months. Indication was metatarsalgia, intractable plantar keratosis, and the (sub-)luxation of the metatarsophalangeal joint.

BACKGROUND: Interdigital neuroma (IN), otherwise known as Morton's neuroma, is a common cause of metatarsalgia presenting to the elective foot and ankle clinic. Surgical excision of the IN in patients who fail to respond to nonoperative measures is considered to be the gold standard of care in many centers. The Royal College of Pathologists UK recommend that all excised interdigital Morton's neuromas are sent for formal histopathological analysis. We present a study correlating clinical and radiological diagnosis with histopathologic appearances of IN following surgical excision, and question

BACKGROUND: Hallux valgus is a common foot disorder often experienced with secondary callosities and metatarsalgia. Many factors including improper shoes might be responsible in the pathophysiology of the problem. Hallux valgus deformity has been shown to alter the biomechanics of the whole foot rather than affecting only the great toe. Due to changes in the biomechanical functioning of the first ray, other regions of the forefoot area have been shown to bear abnormal loads with increased vertical loading on medial, central and lateral forefoot regions. The purpose of this study was to investi

This article is a retrospective study comparing the efficacy of Regnauld arthroplasty to first ray osteotomies for the treatment of hallux valgus. One hundred consecutive cases of Regnauld arthroplasties were compared with 100 consecutive first ray osteotomies. One hundred fourty-one patients were available for follow-up, and based on clinical/radiographic examinations, 72 were treated with the osteotomy protocol (group A) and 69 with Regnauld arthroplasty (group B). Age at surgery, clinical symptoms, and preoperative radiologic findings were similar for the 2 groups; there was a preponderance

A retrospective analysis of the distal "L" osteotomy with bunionectomy (Reverdin-Laird procedure) for correction of hallux abducto valgus was performed. Sixty-nine cases meeting strict eligibility and exclusion criteria underwent evaluation an average of 33.51 months after the procedure. The evaluation included radiographs, physical examination, patient survey, and medical record review. The average decreases in radiographic angles were as follows: Hallux abductus: 19.97 degrees, relative intermetatarsal angle: 7.49 degrees, proximal articular set angle: 13.9 degrees. Twenty-seven cases demons

The importance of correction of the high proximal articular set angle with a subluxed joint in hallux abducto valgus for long-term correction is cited. I have shown that if the laterally deviated facet is not corrected, pronatory forces continue to act on the first ray. When force is applied to a still laterally tilted first metatarsophalangeal joint, further side-slipping of the joint will occur even though modification of the Silver, McBride, or osteotomy procedures of the first metatarsal have been performed successfully. A review of the standard procedures for the correction of a high prox

BACKGROUND: Patients with rheumatoid arthritis (RA) may continue to experience foot-related disability despite clinical remission. Foot involvement is associated with self-reported disability and objective gait alterations. Foot involvement is heterogeneous; studying patients with localised forefoot pain may help clarify which structural and inflammatory factors are associated with functional impairment.
OBJECTIVES: To identify clinical, structural and imaging factors associated with disability and gait performance in patients with RA in clinical remission with metatarsal-region forefoot pain

BACKGROUND: The aim of our study was to compare the clinical versus radiological diagnosis of patients suffering from a Morton's neuroma.
METHODS: Clinical assessments and pre-operative radiological imaging of patients who had excision of a Morton's neuromas were retrospectively compared.
RESULTS: 43 neuromas were excised from 36 patients over 68 months. The commonest clinical symptoms were tenderness on direct palpation (100%), pain on weight bearing (91%) which was relieved by rest (81%). The most sensitive clinical sign was a Mulder's click. Clinical assessment had a sensitivity of 98% (4

BACKGROUND: A modified Scarf osteotomy can not only correct hallux valgus deformity but also relieve lateral metatarsalgia by lowering the head of the first metatarsal. This effect is achieved by inclining the longitudinal osteotomy plane from dorsomedial to plantar-lateral. However, with the increase in the inclined angle, the correction ability for the intermetatarsal angle will weaken, which is critical for hallux valgus correction. The balance between intermetatarsal angle correction(IMAC) and the lowering of the first metatarsal head is crucial for good clinical benefits, and the inclined
