Can transcutaneous bilirubinometry safely guide phototherapy treatment of neonatal jaundice in Malawi?

OBJECTIVES: To assess the correlation between total serum bilirubin (TSB) and transcutaneous bilirubin (TcB) values in Malawian newborn infants, and to investigate whether TcB can be used safely to guide phototherapy treatment in the absence of TSB results. METHODS: 128 newborn jaundiced infants were studied in the neonatal nursery at Queen Elizabeth Central Hospital, Blantyre. Paired TSB and TcB measurements (from forehead and sternum) were compared using the linear regression and Bland-Altman methods. Clinical decisions based on TcB results were compared with those based on 'gold standard'

Immersion in water in the first stage of labor: a randomized controlled trial.

BACKGROUND: Current forms of analgesia often have significant side effects for women in labor. Bathing in warm water during labor has been reported to increase a woman's comfort level and cause a reduction in painful contractions. The objective of this trial was to compare immersion in warm water during labor with traditional pain management for a range of clinical and psychological outcomes. METHODS: A prospective randomized controlled trial of 274 pregnant women, who were free from medical and obstetric complications and expecting a singleton pregnancy at term, was conducted at the Women's

Is it time to reassess the risk for the growth-retarded fetus with normal Doppler velocimetry of the umbilical artery?

OBJECTIVE: Previous studies demonstrate an association between abnormal umbilical artery Doppler velocimetry and the birth of a small-for-gestational-age infant and between abnormal result and adverse neonatal outcome. The hypothesis is that preterm growth-retarded infants with normal antenatal velocimetry have outcomes similar to other preterm infants, whereas preterm small-for-gestational-age infants with abnormal Doppler results define a subgroup with increased morbidity. STUDY DESIGN: For 100 live-born infants, at risk for fetal growth retardation and undergoing antenatal Doppler and targ

Human infections due to Malassezia spp.

The genus Malassezia contains three member species: Malassezia furfur and Malassezia sympodialis, both obligatory lipophilic, skin flora yeasts of humans, and Malassezia pachydermatis, a nonobligatory lipophilic, skin flora yeast of other warm-blooded animals. Several characteristics suggest the basidiomycetous nature of these yeasts, although a perfect stage has not been identified. Classically, these organisms are associated with superficial infections of the skin and associated structures, including pityriasis versicolor and folliculitis. Recently, however, they have been reported as agents

A survey of the management of neonatal hypoglycaemia within the Australian and New Zealand Neonatal Network.

BACKGROUND: Neonatal hypoglycaemia is a common problem linked to both brain damage and death. There is controversy regarding both the definition of and best treatment for neonatal hypoglycaemia. AIM: To determine current management of neonatal hypoglycaemia within the Australian and New Zealand Neonatal Network (ANZNN). METHODS: Four questionnaires were sent to the Director of each of the 45 nurseries within the ANZNN. The Director was asked to complete one questionnaire and give the remaining three to other doctors involved with the management of babies with hypoglycaemia in the nursery. R

Molecular epidemiology of an outbreak of multiresistant Klebsiella pneumoniae in a Tunisian neonatal ward.

During the first quarter of 1996, a major outbreak of clinical infection caused by multiresistant Klebsiella pneumoniae (MRKP) occurred in the neonatal ward of the 'Maternité Wassila Bourguiba' in Tunis, Tunisia. In total, 32 isolates of MRKP, comprising 23 clinical isolates and nine surveillance isolates, were recovered during this period and analysed for epidemiological relatedness. The isolates were compared with 17 other isolates of MRKP that were recovered during 1995. Macrorestriction profiles of total genomic DNA following XbaI restriction endonuclease digestion were analysed by PFGE; t

Caregivers' views on childcare staff's role in promoting children's oral health.

BACKGROUND: The primary reason children in the UK are referred to hospitals for a general anaesthetic is to have decayed teeth removed. Tooth decay is a non-communicable disease which is preventable through healthy behaviours such as brushing at least twice a day with fluoride toothpaste, reducing the frequency of sugar consumption throughout the day and regular dental check-ups. Recently, oral health became a mandatory component of the England Early Years Settings' (EYS) framework. Successful implementation of this public health strategy necessitates involvement from parents and carers, howev
