Differential Cortical Responses of Functional and Sensory Electrical Stimulation in Closed-Loop Tremor Suppression for Parkinson's Disease.

Functional electrical stimulation (FES) and sensory electrical stimulation (SES) are widely used in tremor suppression for Parkinson's disease (PD), however, their therapeutic efficacy varies significantly across individuals. This study investigated the differential cortical effects of FES and SES during closed-loop tremor suppression in PD patient, aiming to identify neurophysiological biomarkers for guiding personalized neuro modulation strategies. We developed an inertial based closed-loop tremor suppression system that delivers out-of-phase FES and continuous SES based on real-time tremor

Clinical Characteristics and Pathophysiology of Fecal Incontinence Mixed With Constipation: An Underrecognized Problem.

INTRODUCTION: Fecal incontinence (FI) and chronic constipation (CC) are diagnosed as distinct problems, although many suffer with both problems. The phenotypical features and underlying mechanisms of FI mixed with CC (FI-MC) is unclear. We investigated the clinical and pathophysiological characteristics of FI-MC and compared with FI alone and healthy controls. METHODS: In a retrospective study, patients with FI were categorized as having FI-MC or FI alone. For comparison, we recruited healthy controls. All subjects completed bowel symptom questionnaire, anorectal manometry, balloon expulsion,

The fNIRS evaluation of frontal and temporal lobe cortical activation in Chinese first-episode medication-naïve and recurrent depression during a verbal fluency task.

BACKGROUND: Functional near-infrared spectroscopy (fNIRS) identifies neurophysiological differences between psychiatric disorders by assessing cortical hemodynamic function. Few trials have studied differences in brain functional activity between first-episode medication-naïve depression patients (FMD) and recurrent major depression (RMD). We aimed to determine the differences between FMD and RMD in oxygenated hemoglobin concentration ([oxy-Hb]), and to investigate the correlation between frontotemporal cortex activation and clinical symptoms. METHODS: We recruited 40 patients with FMD, 53 wi

EEG Power Spectral Density in NREM Sleep is Associated with the Degree of Hypoxia in Patients with Obstructive Sleep Apnea.

PURPOSE: Obstructive sleep apnea (OSA) is a prevalent sleep-related breathing disorder. Research conducted on patients with OSA using electroencephalography (EEG) has revealed a noticeable shift in the overnight polysomnography (PSG) power spectrum. To better quantify the effects of OSA on brain function and to identify the most reliable predictors of pathological cortical activation, this study quantified the PSG power and its association with the degree of hypoxia in OSA patients. PATIENTS AND METHODS: This retrospective study recruited 93 patients with OSA. OSA patients were divided into t

Persistent Brain Connectivity Changes in Healthy Volunteers Following Nitrous Oxide Inhalation.

BACKGROUND: Nitrous oxide holds promise in the treatment of major depressive disorder. Its psychotropic effects and NMDA receptor antagonism have led to comparisons with ketamine. Despite longstanding use, persistent effects of nitrous oxide on the brain have not been characterized. METHODS: Sixteen healthy volunteers were recruited in a double-blind crossover study. In randomized order, individuals underwent a 1-hour inhalation of either 50% nitrous oxide/oxygen or air/oxygen mixtures. At least two 7.5-minute echo-planar resting-state functional magnetic resonance imaging scans were obtained

Feasibility of EEG Phase-Amplitude Coupling to Stratify Encephalopathy Severity in Neonatal HIE Using Short Time Window.

Goal: It is challenging to clinically discern the severity of neonatal hypoxic ischemic encephalopathy (HIE) within hours after birth in time for therapeutic decision-making for hypothermia. The goal of this study was to determine the shortest duration of the EEG based PAC index to provide real-time guidance for clinical decision-making for neonates with HIE. Methods: Neonates were recruited from a single-center Level III NICU between 2017 and 2019. A time-dependent, PAC-frequency-averaged index, tPACm, was calculated to characterize intrinsic coupling between the amplitudes of 12−30 Hz and th

Morphological analysis of the brain subcortical gray structures in restless legs syndrome.

BACKGROUND: Although several studies have shown the involvement of specific structures of the central nervous system, the dopaminergic system, and iron metabolism in restless legs syndrome (RLS), the exact location and extent of its anatomical substrate is not yet known. The scope of this new study was to investigate the brain subcortical gray structures, by means of structural magnetic resonance imaging (MRI) studies, in RLS patients in order to assess the presence of any volume or shape abnormalities involving these structures. METHODS: Thirty-three normal controls (24 females and nine male

Effects of Diacutaneous Fibrolysis on Mechanosensitivity, Disability, and Nerve Conduction Studies in Mild to Moderate Carpal Tunnel Syndrome: Secondary Analysis of a Randomized Controlled Trial.

OBJECTIVE: People diagnosed with carpal tunnel syndrome (CTS) have fibrosis between the soft, connective, and neural tissues that could worsen the compression of the median nerve. The diacutaneous fibrolysis (DF) technique may release tissue adhesions and increase the mobility of connective tissues. The purpose of this study was to compare the outcomes of DF in people with mild to moderate CTS on mechanosensitivity, disability, and nerve conduction studies. METHODS: This was a secondary analysis of a double-blinded, randomized, placebo-controlled trial. Patients were recruited between April a

Multicentre prospective randomised single-blind controlled study protocol of the effect of an additional parent-administered sensorimotor stimulation on neurological development of preterm infants: Primebrain.

INTRODUCTION: Preterm and very low birthweight infants are at increased risk for neurodevelopmental disorders, including cerebral palsy, sensory impairment and intellectual disability. Several early intervention approaches have been designed in the hope of optimising neurological development in this context. It seems important that the intervention takes into account parental mental health, focuses on parent-child interactions and lasts sufficiently long. This study aims to evaluate the effects of a stimulation programme administered by parents until 6 months post-term on motor and neurophysio

The Danish 22q11 research initiative.

BACKGROUND: Neurodevelopmental brain disorders such as schizophrenia, autism and attention deficit hyperactivity disorder are complex disorders with heterogeneous etiologies. Schizophrenia and autism are difficult to treat and often cause major individual suffering largely owing to our limited understanding of the disease biology. Thus our understanding of the biological pathogenesis needs to be substantiated to enable development of more targeted treatment options with improved efficacy. Insights into the pre-morbid disease dynamics, the morbid condition and the underlying biological disease

Sleep polygraphic study of children and adolescents with narcolepsy/cataplexy.

The alterations of the Cyclic Alternating Pattern (CAP) recently found in narcoleptic adult patients suggest the presence of an impaired modulation of the fluctuations of the arousal level during their non-rapid eye movement (NREM) sleep, possibly because of the persistence of neurophysiological mechanisms typical of rapid eye movement (REM) sleep. The same mechanism might play a role in the occurrence of leg movement (LM) activity during sleep characterized by low levels of periodicity. The aim of this study was to evaluate CAP and sleep LM activity in a group of children and adolescents with

Effects of device-based and manual sensory acuity training on nonspecific chronic low back pain: a three-arm randomized controlled trial protocol.

BACKGROUND: Nonspecific chronic low back pain (NCLBP) is a prevalent and disabling condition often associated with impaired sensory discrimination and altered cortical representation. Sensory acuity training (SAT) has emerged as a promising strategy to target these neurophysiological mechanisms; however, the comparative effectiveness of technology-based versus manual sensory acuity interventions remains unclear. This protocol comprises a three-arm, randomized controlled trial designed to evaluate the effects of technology-based and manual SAT, in addition to conventional physiotherapy, compare

Connectomic Mapping of Chronic Musculoskeletal Pain: Neural Circuitries Identified Through a Systematic Review and ALE Meta-Analysis.

Functional neuroimaging of the encephalon of humans with chronic musculoskeletal pain (CMP) has consistently demonstrated functional alterations in the neurophysiological properties of cortical and subcortical circuits. Nevertheless, the current knowledge on specific neural circuitries that may occur in different CMP subgroups is limited, which in turn limits the understanding of the encephalon mechanisms associated with persistent pain and clinical heterogeneity. This systematic review (CRD42022382309) and activation likelihood estimation (ALE) meta-analysis of observational functional magnet
