Assessing processing speed and its neural correlates in the three variants of primary progressive aphasia with a non-verbal tablet-based task.

Prior research has revealed distinctive patterns of impaired language abilities across the three variants of Primary Progressive Aphasia (PPA): nonfluent/agrammatic (nfvPPA), logopenic (lvPPA) and semantic (svPPA). However, little is known about whether, and to what extent, non-verbal cognitive abilities, such as processing speed, are impacted in PPA patients. This is because neuropsychological tests typically contain linguistic stimuli and require spoken output, being therefore sensitive to verbal deficits in aphasic patients. The aim of this study is to investigate potential differences in p

Selective familiarity deficits after left anterior temporal-lobe removal with hippocampal sparing are material specific.

Research has firmly established a link between recognition memory and the functional integrity of the medial temporal lobes (MTL). Dual-process models of MTL organization maintain that there is a division of labour within the MTL, with the hippocampus (HC) supporting recollective processes and perirhinal cortex (PRc) supporting familiarity assessment. An older neuropsychological literature suggested a different type of division of labour within the MTL, with left-sided structures playing a critical role in memory for verbal materials and right-sided structures being differentially involved in

[Frontotemporal dementia: a review].

CLINICAL CHARACTERISTICS: Frontotemporal dementia (FTD) is a neurological disorder characterised by the progressive degeneration of the frontal and anterior temporal cortex. FTD, as well as nonfluent progressive aphasia and semantic dementia, belongs to the more generic entity of frontotemporal lobe degeneration. Considering the involvement of the frontal lobe, the initial clinical presentation of FTD may be psychiatric, such as changes in personality or behavioural disorders. Psychiatrists, therefore, have to establish the differential diagnosis with late-onset schizophrenia or affective diso

Sniffin' sticks smell identification test: exploring measurement properties in an adult Brazilian healthcare professionals sample.

INTRODUCTION: Olfactory evaluation has gained significant attention in both neurological and otorhinolaryngological assessments, particularly following the COVID-19 pandemic where SARS-CoV-2 infection emerged as a frequent cause of chemosensory dysfunction. The Sniffin' Sticks identification test is a widely used screening tool for olfactory function. This study aimed to investigate the psychometric properties, specifically validity evidence based on internal structure (unidimensionality) and relations to other variables (demographic effects), of the 16-item Sniffin' Sticks odor identification

ISIT: a frame-clustering implicit measure of situated experience.

INTRODUCTION: The Implicit Situation Intensity Test (ISIT) is a method for converting participants' verbal reports about an experience into a quantitative, per-participant implicit measure of how strongly cognitive, emotional, behavioral, and value-laden frames have been activated by that experience. METHOD: ISIT operates by clustering frames extracted from the verbal report's extended knowledge graph (XKG)-the structured representation produced by the POLANYI++ neurosymbolic pipeline-into a Frame Ontology (FO) and scoring each frame's salience for each participant on a normalized scale. Frontotemporal dementia and its subtypes: a genome-wide association study.

BACKGROUND: Frontotemporal dementia (FTD) is a complex disorder characterised by a broad range of clinical manifestations, differential pathological signatures, and genetic variability. Mutations in three genes-MAPT, GRN, and C9orf72--have been associated with FTD. We sought to identify novel genetic risk loci associated with the disorder. METHODS: We did a two-stage genome-wide association study on clinical FTD, analysing samples from 3526 patients with FTD and 9402 healthy controls. To reduce genetic heterogeneity, all participants were of European ancestry. In the discovery phase (samples

[Neuropsychological and scintigraphic aspects of frontotemporal dementia preceding amyotrophic lateral sclerosis].

Between 1993 and 2001, we observed fifteen patients (ten men and five women, mean age 63 years) with frontotemporal dementia (FTD) which preceded signs of amyotrophic lateral sclerosis (ALS) which developed 21 months later. Mean disease duration in the fourteen deceased patients was 38 months. FTD associated with ALS is characterized by rapid course, predominance of disinhibited forms (orbito-basal), presence of aphasia with neologisms, and semantic memory disorders. Performed in all patients, single-photon emission computed tomography demonstrated a bifrontal pattern of low uptake, sometimes

Disorders of visual recognition.

Agnosias are disorders of recognition, specific to one sensory channel, that affect either the perceptual analysis of the stimulus or the recognition of its meaning. In the visual modality, objects, faces, and colors can be separately disrupted. Apperceptive object agnosia refers to failure to achieve a structured description of the shape of the object. Associative agnosia refers to inability to attribute a meaning to a correctly perceived stimulus. It must be differentiated from optic aphasia, in which the object is recognized but cannot be named in the visual modality. Associative agnosia an

Integrative systematic review and transcriptomic -machine learning analysis of molecular signatures in metaplastic breast cancer.

BACKGROUND: Metaplastic breast cancer (MpBC) is a rare and aggressive breast cancer subtype characterised by marked histological heterogeneity, therapeutic resistance and poor clinical outcomes. Despite increasing molecular research, existing evidence remains fragmented, heterogeneous and poorly integrated, limiting clinical translation and biomarker validation. METHODS: We developed an integrative analytical framework combining systematic review, quantitative meta-analysis, transcriptomic profiling and interpretable machine learning to identify and prioritise molecular markers in MpBC. Epigenome-wide DNA methylation patterns associated with disease activity in systemic lupus erythematosus.

Systemic lupus erythematosus (SLE) is a complex autoimmune disease with fluctuating disease activity and heterogeneous clinical manifestations. While DNA methylation changes have been implicated in SLE pathogenesis, their relationship to disease activity remains unclear. This study aimed to identify epigenetic correlates of disease activity in women with SLE using genome-wide DNA methylation profiling. Whole blood DNA from 48 women with established SLE was analyzed using Illumina EPIC arrays. Disease activity was assessed using the Systemic Lupus Erythematosus Disease Activity Index 2000 (SLED

DOME: Directional medical embedding vectors from Electronic Health Records.

MOTIVATION: The increasing availability of Electronic Health Record (EHR) systems has created enormous potential for translational research. Recent developments in representation learning techniques have led to effective large-scale representations of EHR concepts along with knowledge graphs that empower downstream EHR studies. However, most existing methods require training with patient-level data, limiting their abilities to expand the training with multi-institutional EHR data. On the other hand, scalable approaches that only require summary-level data do not incorporate temporal dependenci

Visuospatial functioning is associated with sleep disturbance and hallucinations in nondemented patients with Parkinson's disease.

Introduction: Cognitive impairment is a common symptom of Parkinson's disease (PD) associated with reduced quality of life and a more severe disease state. Previous research has shown an association between visuospatial dysfunction and worse disease course; however, it is not clear whether this is separable from executive dysfunction and/or dementia. This study sought to determine whether distinct cognitive factors could be measured in a large PD cohort, and if those factors were differentially associated with other PD-related features, specifically to provide insight into visuospatial dysfunc

Compositionality and the angular gyrus: A multi-voxel similarity analysis of the semantic composition of nouns and verbs.

The cognitive and neural systems that enable conceptual processing must support the ability to combine (and recombine) concepts to form an infinite number of ideas. Two candidate neural systems for conceptual combination-the left anterior temporal lobe (ATL) and the left angular gyrus (AG)-have been characterized as "semantic hubs" due to both functional and anatomical properties; however, these two regions likely support different aspects of composition. Here we consider two hypotheses for the role of AG in conceptual combination, both of which differ from a putative role for the ATL in "feat

[Clinical types of FTLD: progressive nonfluent aphasia; comparative discussions on the associated clinical presentations].

Progressive nonfluent aphasia (PNFA) is one of the 3 clinical presentations of frontotemporal lobar degeneration (FTLD), the other 2 being frontotemporal dementia and semantic dementia (SD). PNFA and SD, both representing relentlessly progressive language impairment in the realm of FTLD, may share a large part with primary progressive aphasia (PPA). A salient distinction between PPA and PNFA or SD is that PPA includes another clinical type, namely, logopenic/phonemic aphasia (LPA), which is not represented in FTLD. This is primarily because LPA is usually caused by Alzheimer's disease (AD) and

Transcriptomic signature of frontotemporal lobar degeneration with TDP-43 type C pathology.

Semantic variant of primary progressive aphasia is a clinical subtype of frontotemporal lobar degeneration and is marked by TDP-43 subtype C pathology (FTLD-TDP C). It is a sporadic disease, yet has a strikingly homogeneous clinicopathological presentation, suggesting a common pathophysiology. The aim of this study was to discover dysregulated pathways in FTLD-TDP C through transcriptomics of the temporal cortex, its most affected region. Bulk RNA sequencing was conducted on temporal cortices of a post-mortem cohort of 18 FTLD-TDP C patients and 23 sex- and age-matched controls. Differential e

Two terms--one meaning: the conundrum of contemporary nomenclature in autism.

TOPIC: Autism as a diagnostic construct in the last decade has undergone a process of rapid evolution in the way that it is operationalized. A by-product of the important conceptual shift is that we have been left with two terms, autistic disorder and Asperger's disorder, that point to the same thing. The existence of two terms that cannot be clearly delineated results in confusion and an added element in an already anxiety-laden mix for all those trying to come to terms with the concept of autism. PURPOSE: This paper moves beyond contemporary arguments of whether autistic disorder and Asperg

Transcriptomic profiling of the middle temporal gyrus reveals differential glial/neuronal dysregulation across Alzheimer's disease and aging.

Alzheimer's disease (AD), the most common cause of dementia, is characterized by amyloid-β plaques, neurofibrillary tangles, and widespread neuronal dysfunction. Aging, the strongest risk factor for AD, is also associated with some overlapping processes, such as neuronal cell transcriptional downregulation and glial cell activation. The middle temporal gyrus (MTG) is a brain region that supports semantic processing and default-mode connectivity and shows early vulnerability in both aging and AD. Here we profile bulk RNA-seq from 606 postmortem MTG samples with the goal of understanding the tra

Clinicoradiological and neuropathological evaluation of primary progressive aphasia.

BACKGROUND: Primary progressive aphasia (PPA) defines a group of neurodegenerative disorders characterised by language decline. Three PPA variants correlate with distinct underlying pathologies: semantic variant PPA (svPPA) with transactive response DNA-binding protein of 43 kD (TDP-43) proteinopathy, agrammatic variant PPA (agPPA) with tau deposition and logopenic variant PPA (lvPPA) with Alzheimer's disease (AD). Our objectives were to differentiate PPA variants using clinical and neuroimaging features, assess progression and evaluate structural MRI and a novel 18-F fluorodeoxyglucose positr
