[Oral homeostasis imbalance under hypoxia].

Hypoxia disrupts oral homeostasis through multiple interconnected pathways, including interference with tooth germ development, impairment of salivary gland function and salivary buffering capacity, compromise of the oral mucosal barrier, imbalance in jawbone and alveolar bone remodeling, and alterations in the diversity and functionality of the oral microbiome. These disturbances collectively contribute to the onset and progression of oral diseases. Moreover, disruption of oral homeostasis may, in turn, affect systemic homeostasis, increasing the risk of disorders in other organ systems throu

Orthodontic Treatment for a Patient with Root Resorption of All Four Maxillary Incisors due to Bilaterally Impacted Canines.

Maxillary canines require the longest period from the generation of the tooth germ to the completion of eruption, and they have more difficulties in eruption than other teeth. The incisor roots are often resorbed due to malpositioned canines. An 11-year-old girl presented with an extremely rare case of root resorption of four maxillary incisors due to bilaterally impacted maxillary canines, which were located excessively mesially. The case was managed through oral surgery and orthodontic treatment over five years. After extracting the maxillary deciduous canines, the maxillary bilateral canine

Management of multiple and displaced mandibular fractures in a pediatric patient sans mandibular immobilization, sans open reduction and internal fixation.

Maxillofacial fractures in the pediatric population are generally managed by conservative approaches such as soft diet and medication or semi-invasive procedures namely inter-maxillary fixation or circum-mandibular wiring. These approaches are preferred over any invasive treatment to minimize injury to the growing skeleton and tooth germs. Displaced fractures that cause functional problems such as restricted mouth opening, malocclusion or impaired breathing, mandate open reduction and internal fixation. However, surgical management is associated with morbidity related to general anesthesia, ri

Dental treatment considerations for a pediatric patient with incontinentia pigmenti (Bloch-Sulzberger syndrome).

Incontinentia pigmenti (IP) is a uncommon gene disorder, heritage with X-linked dominant mode. IP patients have a characteristic dentition varying from marked hypodontia to delayed eruption and conical crowns on both dentitions. A 5½-year-old girl, whose mother and younger sister were also diagnosed with IP, has the whirling-like pigmented skin lesion over her trunk and four extremities. Four primary teeth and multiple permanent tooth germs were found to be congenital missing. Dental considerations of further treatment were discussed with her parents including the preservation of primary molar

Segmental alveolar process fracture involving primary incisors: treatment and 24-month follow up.

Alveolar process fractures are particularly complicated by the developing jaws, the presence of tooth germs and the eruption of permanent teeth. The present report describes the management and 24-month follow up of a segmental mandibular alveolar process fracture involving laterally luxated primary incisors. A 5-year-old boy was admitted to the clinic 3 h after a fall accident, which resulted in the fracture of the mandibular alveolar process with substantial dislodgement of the buccal cortical bone and right primary incisors. Emergency treatment consisted of fracture reduction in the dislodge

Changes in the tartrate-resistant acid phosphatase cell population in dental follicles and bony crypts of rat molars during tooth eruption.

It was the aim of this study to determine the cellular changes that occur in the enamel organ, dental follicle, and surrounding bony crypt of the rat molar prior to and during tooth eruption. By use of light microscope histochemistry to detect cells containing tartrate-resistant acid phosphatase (TRAP), it was seen that TRAP-positive mononuclear cells were present in the dental follicle prior to the onset of eruption (e.g., three days postnatal age) and then declined in number during eruption. Concurrently, TRAP-positive osteoclasts were initially present in large numbers on the surface of the

Appliance-Supported Decompression for Radicular Cysts Associated With Primary Molars: A Pediatric Case Series.

INTRODUCTION: Radicular cysts associated with primary teeth are uncommon in pediatric patients but may cause extensive bone destruction and displacement of developing permanent tooth germs. Conventional surgical enucleation may increase the risk of damage to adjacent structures; therefore, conservative treatment approaches such as marsupialization and decompression are preferred. Appliance-supported decompression systems have been increasingly used to maintain continuous drainage while preserving the eruption potential of permanent teeth. CASE SERIES: This case series presents three pediatric

The position of the mental foramen on panoramic radiographs of patients with neurofibromatosis type 1.

UNLABELLED: Neurofibromatosis type 1 (NF1) is an autosomal dominant tumor predisposition disorder and a disease with frequent osseous malformations. In the craniomaxillofacial region, bony malformations of NF1 patients are often associated with plexiform neurofibromas (PNF). PNF can manifest early in life and often have a topographical relationship to the branches of the trigeminal nerve. It is unclear when these nerve sheath tumors interact with the skeleton. Since tooth and bone development can be correlated with stages of prenatal development, a fixed point of mandibular ossification, the t

Recent advances in metal nanoparticles to treat periodontitis.

The gradual deterioration of the supporting periodontal tissues caused by periodontitis, a chronic multifactorial inflammatory disease, is thought to be triggered by the colonization of dysbiotic plaque biofilms in a vulnerable host. One of the most prevalent dental conditions in the world, periodontitis is now the leading factor in adult tooth loss. When periodontitis does develop, it is treated by scraping the mineralized deposits and dental biofilm off the tooth surfaces. Numerous studies have shown that non-surgical treatment significantly improves clinical and microbiological indices in i

Cell dynamics in cervical loop epithelium during transition from crown to root: implications for Hertwig's epithelial root sheath formation.

BACKGROUND AND OBJECTIVE: Some clinical cases of hypoplastic tooth root are congenital. Because the formation of Hertwig's epithelial root sheath (HERS) is an important event for root development and growth, we have considered that understanding the HERS developmental mechanism contributes to elucidate the causal factors of the disease. To find integrant factors and phenomenon for HERS development and growth, we studied the proliferation and mobility of the cervical loop (CL). MATERIAL AND METHODS: We observed the cell movement of CL by the DiI labeling and organ culture system. To examine ce

Efficiency of a pendulum appliance for molar distalization related to second and third molar eruption stage.

A modified pendulum appliance, including a distal screw and special preactivated pendulum springs (built-in straightening activation and toe-in bending), was used for bilateral maxillary molar distalization in 36 adolescent patients in various stages of the molar dentition. The patients were divided into 3 groups (PG 1-3) according to the stage of eruption of their second and third molars. In PG 1 (18 patients), eruption of the second molars had either not yet taken place or was not complete. In PG 2 (15 patients), the second molars had already developed as far as the occlusal plane, with the

[Prevalence of hypodontia at the department of pedodontics and orthodontics of Semmelweis University].

Examination of hypodontia as one of the most frequent developmental disorder receives high importance in the national as well as in the international literature. As the disorder can affect the dental and mental development of the children therefore early diagnosis and treatment are necessary. The authors analyzed the OP radiograms of patients aged 6 to 18 who were registered between 2006 and 2011 at the Department of Pedodontics and Orthodontics of Semmelweis University, excluding children suffering from general diseases or other syndromes. The aim of the examination was to determine the occur

Management and subsequent 13-year progress of a mandibular fracture with malocclusion in a child--case report.

Under acute conditions, maxillofacial injuries may be treated without the opportunity for an assessment of occlusal irregularities, even when there are mandibular fractures, because life-threatening injuries have priority over occlusion. Consequently, mandibular fractures may result in post-trauma malocclusion and facial deformity. The case history reported is of a male patient who had been involved in a traffic accident in childhood and suffered mandibular fractures. The initial incomplete management resulted in persistent deformation of the mandible, disturbance of dental occlusion and diffi

The landscape of cell regulatory and communication networks in the human dental follicle.

INTRODUCTION: The dental follicle localizes the surrounding enamel organ and dental papilla of the developing tooth germ during the embryonic stage. It can differentiate and develop to form the periodontal ligament, cementum, and alveolar bone tissues. Postnatally, the dental follicle gradually degenerates, but some parts of the dental follicle remain around the impacted tooth. However, the specific cellular components and the intricate regulatory mechanisms governing the postnatal development and biological function of the dental follicle have not been completely understood. METHODS: We anal

Decompression of a Mandibular Dentigerous Cyst Using a Dual-Purpose Removable Appliance in a Pediatric Patient: A Case Report.

Dentigerous cysts (DCs) are among the most common odontogenic lesions in mixed dentition. Management strategies vary from radical enucleation-often necessitating the removal of associated tooth germs-to more conservative decompression techniques. In pediatric patients, decompression is preferred as it preserves developing permanent teeth and supports normal eruption. In January 2023, an 8.5-year-old male presented to the Department of Pediatric Dentistry, Akdeniz University, with swelling in the right mandibular region. Clinical and radiographic examination revealed a unilocular radiolucent le

Abnormal Enamel Mineralization and Hypercalcemia due to GNA11 Variants.

Abnormal enamel mineralization has been reported in genetic conditions associated with hypocalcemia but never with hypercalcemia. In the present study, we report 10 patients from 2 unrelated families in whom hypomineralized enamel co-segregated with hypercalcemia due to a complete deletion or frameshifting premature truncation (p.(Ser18Argfs*2)) of the guanine nucleotide-binding protein (G-protein) subunit α11 (GNA11) gene. Both GNA11 variants were heterozygous and predicted to cause a loss of function of Gα11. Multiproxy imaging analyses of naturally exfoliated primary teeth and clinically ex

TMEM119 (c.G143A, p.S48L) Mutation Is Involved in Primary Failure of Eruption by Attenuating Glycolysis-Mediated Osteogenesis.

Primary failure of eruption (PFE) is a rare oral disease with an incidence rate of 0.06%. It is characterized by abnormal eruption mechanisms that disrupt tooth eruption. The underlying pathogenic genetic variant and mechanism of PFE remain largely unknown. The purpose of this study was to explore the role of a novel transmembrane protein 119 (TMEM119) mutation in two PFE patients in a Chinese family. Information collection was performed on the family with a diagnosis of PFE, and blood samples from patients and healthy family members were extracted. Whole-exome sequencing was performed. Bioinf
