Distal radioulnar joint (DRUJ) instability following distal radius fracture (DRF) is an increasingly recognised entity, particularly with recent advances in the management of DRF. This review aims to provide a comprehensive appraisal of DRUJ anatomy, methods of assessing instability and current treatment strategies. Both non-operative and operative modalities are considered. In the acute setting, surgical management primarily involves open reduction and internal fixation (ORIF) of the distal radius, which can restore DRUJ stability through correction of translation and distraction of the dista

In this article we will do an overview of the general and specific complications that occur after the most common wrist and hand surgeries. Knowledge of the different surgical techniques is essential for postoperative imaging evaluation. General complications include infection, complex regional pain syndrome, problems related with the surgical approach (open or arthroscopic) and bone healing problems. The most frequent fractures of the wrist with specific complications are distal radius fractures and scaphoid fractures as associated with tendon ruptures secondary to friction, nonunion or secon

Background Treatment algorithm for disruption of the triangular fibrocartilage complex (TFCC) from the ulnar fovea includes direct TFCC repair, tendon reconstruction of the radioulnar ligaments, or a salvage procedure in cases with painful distal radioulnar joint (DRUJ) degeneration. Case Description We describe our surgical technique for reconstruction of the distal oblique bundle (DOB), to attain DRUJ stability in a young man, after failed attempts of direct TFCC reinsertion and radioulnar ligament reconstruction with the Adams procedure. Literature Review Reconstruction of the central ba

Bilateral bipartite lunate is a very rare congenital anomaly of the lunate. A 36-year-old military European male was referred to our service diagnosed with a lunate fracture. Symptoms began 3 months before our encounter, after falling on his outstretched left hand. The patient was misdiagnosed with a lunate fracture, therefore treated with a cast and then transitioned to a removable splint over 2 months in total; When the patient presented to our facility, on physical examination, he referred pain over the dorso-ulnar side of the wrist, especially the ulnar snuff. Tenderness to palpation over

PURPOSE: To determine whether ulnar positive variance (UPV) and relative ulnar positive variance (RUPV) affect clinical outcomes following arthroscopic transosseous repair of triangular fibrocartilage complex (TFCC) foveal tears.
METHODS: This retrospective comparative study included patients who underwent arthroscopic transosseous repair for isolated Palmer type 1B TFCC foveal tears between April 2021 and June 2024, with a minimum follow-up of one year. Patients with concomitant fractures or those undergoing concurrent ulnar shortening osteotomy were excluded. Ulnar variance (UV) was measure

OBJECTIVE: Arthroscopic decompression (wafer procedure) of the ulnocarpal compartment in patients with ulnar impaction syndrome.
INDICATIONS: The arthroscopic wafer procedure is recommended in ulnar-plus situations with up to 3 mm length-excess. To perform this procedure the ulnar head needs to be accessible for the burr through a pre-existing, impaction-related, centroradial lesion of the triangular fibrocartilage complex (TFCC). The additional presence of a distal radioulnar joint (DRUJ) type C confirms the indication.
CONTRAINDICATIONS: The wafer procedure is contraindicated if there is n

PURPOSE: The diagnostic sensitivity, specificity, and accuracy of 1.5 Tesla (T) and of 3.0T magnetic resonance imaging (MRI) are correlated with wrist arthroscopy findings in patients presenting with ulnar-sided wrist pain.
METHODS: The records and diagnostic MRI scans of 102 patients who presented between 1997 and 2006 with ulnar-sided wrist pain were evaluated. Preoperative MRI scans at 1.5T (n = 70) and 3.0T (n = 32) were evaluated by 2 experienced musculoskeletal radiologists with different levels of experience who were blinded to the arthroscopic findings. Pseudogout, also known as calcium pyrophosphate deposition disease, is a rheumatological condition arising from accumulation of calcium pyrophosphate dihydrate crystals in connective tissues. We present a case of a 56-year-old Bangladeshi woman who underwent focused right inferior parathyroidectomy for primary hyperparathyroidism from a right inferior parathyroid adenoma. On the first post-operative day, she complained of left elbow painful swelling with redness and warmth. Arthrocentesis of left elbow was done due to suspicion of septic arthritis. Two weeks prior to this surgery, she had sudd

Various authors have documented wrist ligament injuries in patients with distal radius fractures (DRFs). We conducted a study to determine whether scapholunate interosseous ligament (SLIL), triangular fibrocartilage complex (TFCC), or chondral injuries directly assessed with arthroscopy predict DRF outcomes. Forty-two patients who underwent open reduction and internal fixation of DRFs were enrolled in the study. At time of fracture surgery, patients were arthroscopically evaluated for SLIL and TFCC injuries and chondral surface damage. The Disabilities of the Arm, Shoulder, and Hand (DASH) que

OBJECTIVE: The purpose of this study is to develop an automated model to assist in the detection of substructural injuries of the triangular fibrocartilage complex (TFCC), thereby reducing the reliance on subjective assessment.
METHODS: This retrospective study utilized 330 TFCC injured patients and 273 healthy controls from two institutions, only analyzing 2821 coronal fat-saturated T2-weighted imaging slices. From 483 samples (267 injured, 216 normal), 2292 images were processed: 1834 for training, 458 for validation, with an internal test set of 209 images from 47 samples (26 injured, 21 n

PURPOSE: Ulnar shortening osteotomy (USO) is a standard treatment for ulnar impaction syndrome and triangular fibrocartilage complex (TFCC) injuries. However, complications such as delayed union, nonunion, and refracture after implant removal remain controversial issues. This study aimed to evaluate the clinical outcomes of modified step-cut USO using an osteotomy guide and a specialized ulnar shortening plate designed to maximize bone contact, enhance fixation stability, and reduce the invasiveness of the procedure.
METHODS: A retrospective case series involving 23 consecutive patients (23 w

UNLABELLED: We examined the medium- to long-term outcome of the original reconstruction technique of the radioulnar ligament using a half-slip of the extensor carpi ulnaris tendon for chronic foveal avulsion of the triangular fibrocartilage complex. We treated 37 wrists in 36 patients with a minimum follow-up of 5 years. Of 36 patients, 12 had more than 10 years follow-up. At final follow-up, 30 patients (31 wrists) reported no pain, five patients (five wrists) reported mild occasional pain and one patient reported severe persistent pain. Two patients had lost 30° of supination. Complete stabi

INTRODUCTION: Blood flow restriction training (BFRT) has enhanced physiological function and performance in lower limb injuries. However, its application in upper limb injuries, particularly at the wrist, remains underexplored. This case report aims to evaluate the effects of BFRT on wrist injury rehabilitation.
CLINICAL FINDINGS AND DIAGNOSIS: The athlete presented with tenderness, reduced range of motion, and decreased strength at the wrist joint. Clinical examination indicated involvement of the triangular fibrocartilage complex (TFCC) and extensor carpi ulnaris (ECU).
METHODOLOGY: BFRT w

BACKGROUND: An institutional review board-approved study of the functional outcomes of patients after surgical treatment of hamate arthrosis lunotriquetral ligament tear (HALT) lesions was conducted.
METHODS: In all, 21 wrists in 19 patients underwent arthroscopic, open, or combined treatment of HALT lesions. Seven patients underwent isolated hamate debridement and 14 had concomitant procedures to address lunotriquetral pathology. Nineteen wrists underwent procedures to address additional pathology, including triangular fibrocartilage complex, ulnotriquetral ligament split, and scapholunate l

BACKGROUND: Magnetic resonance imaging (MRI) is being increasingly utilized to define pathology and guide treatment in patients presenting with wrist pain. The clinical relevance of MRI identified or confirmed pathology has not been established, and the prevalence of asymptomatic MRI pathology is not known.
METHODS: Twenty volunteers with no previous wrist injury or symptoms underwent bilateral MRI wrist studies in this exploratory diagnostic study. The scans were reported by an experienced musculoskeletal radiologist and an experienced wrist surgeon, with a consensus reached on each report.

Twenty-eight extensor carpi ulnaris lesions at the wrist were treated surgically between 1990 and 2002. Fifteen patients had an isolated extensor carpi ulnaris tenosynovitis or tendinopathy, five had extensor carpi ulnaris dislocation, four had an extensor carpi ulnaris subluxation and four had an extensor carpi ulnaris rupture. Seventeen patients first developed their symptoms while playing sports. At a mean follow-up of 23 months, twenty-two patients had returned to their previous activities. Seven of the 27 patients had lost more than 30% of their grip strength and five had restricted wrist

Glomus tumors are benign neoplasms typically found in the fingers and usually present with localized pain, swelling, and cold sensitivity. Extradigital glomus tumors are rare, and their diagnosis is often delayed due to atypical location and lack of cold sensitivity. In this article, we report a unique case of an extradigital glomangioma, a subset of glomus tumors, masquerading as more common ulnar-sided wrist pathology, such as a triangular fibrocartilage complex tear or extensor carpi ulnaris tendinitis. The patient presented with a two-year history of gradually worsening ulnar-sided wrist p

Pain in the dorsoulnar region of the wrist is a common complaint and may be caused by a variety of pathologies. Longitudinal tears of the extensor carpi ulnaris (ECU) tendon are among them, and it has recently been noted that these lesions are more frequent than previously thought. Most ECU injuries can be managed conservatively. Surgical treatment is indicated in cases of tendon rupture, especially when associated with instability or injury to the triangular fibrocartilage complex (TFCC). In such cases, the procedure should include tendon debridement with resection of inflammatory tissue, deb

We reviewed retrospectively a series of 128 consecutive patients who had wrist arthroscopy at our institution between January 1988 and July 1991. The aim of the procedure was to establish a diagnosis in 27 patients, to challenge a pre-operative diagnosis in 95 patients and to evaluate joint surfaces in six patients. For the 27 patients with a previously unknown diagnosis, the diagnostic sensitivity was 67% and the specificity 100%. Arthrotomy or other intervention was avoided in 18. For the 95 patients with an established pre-operative diagnosis, arthrotomy was avoided in 45: in 26 the desired
