Suture materials and suture techniques used in tendon repair.

Immediately after a tendon repair, the tendon contributes nothing to the strength of repair. During that time, the suture itself and suture technique are the sole contributors to the strength of repair. Although stainless steel is the strongest material that can be used at the time of repair, it has serious disadvantages. It is difficult to work with and makes a bulky knot. Conversely, all absorbable sutures become too weak too soon to be of value. At this time, nonabsorbable, synthetic fibers that are relatively strong, such as Supramid or prolene, are the most desirable materials available.

Association of Bolster Duration With Uptake Rates of Fibula Donor Site Skin Grafts.

IMPORTANCE: The fibula free flap donor site is associated with both short-term and long-term morbidity. Split-thickness skin graft (STSG) loss can lead to long delays in donor site healing and is associated with significant adverse sequelae. Patients may experience initial good STSG uptake after bolster removal but may have subsequent partial or total loss related to contact pressure or shearing. OBJECTIVE: To determine if increased duration of bolster use is associated with increased STSG uptake rates following fibula free flap reconstruction. DESIGN, SETTING, AND PARTICIPANTS: This retrosp

Three-dimensional virtual operations can facilitate complicated surgical planning for the treatment of patients with jaw deformities associated with facial asymmetry: a case report.

This article describes a case we experienced in which good postsurgical facial profiles were obtained for a patient with jaw deformities associated with facial asymmetry, by implementing surgical planning with SimPlant OMS. Using this method, we conducted LF1 osteotomy, intraoral vertical ramus osteotomy (IVRO), sagittal split ramus osteotomy (SSRO), mandibular constriction and mandibular border genioplasty. Not only did we obtain a class I occlusal relationship, but the complicated surgery also improved the asymmetry of the frontal view, as well as of the profile view, of the patient. The vir

Proximal row carpectomy.

Proximal row carpectomy (PRC) is a motion-preserving treatment for the degenerated wrist. PRC provides painless wrist range of motion with few complications. PRC treats specifically scapholunate advanced collapse, scaphoid non-union advanced collapse, chronic perilunate dislocations, and Kienbock's disease. The best candidates are older than 35 with an intact capitate head and lunate facet of the distal radius. Proximal row carpectomy provides satisfactory postoperative wrist range of motion and grip strength with few complications, especially when there is no capitolunate arthrosis. Postopera

The diaphysial axis-metacarpal head angle in the management of fractures of the base of the proximal phalanx in children.

The diaphysial axis-metacarpal head angle (DHA) is the angle formed between the longitudinal axis of the diaphysis of the proximal phalanx and central point of the metacarpal head. The normal DHA ranges from 177.1° and 180.0°. There were no significant differences between DHA measurements when taken by the same observer at two separate occasions (P = 0.986) or when taken by two different observers (P = 0.948). We have put an algorithm of management of paediatric phalangeal base fractures incorporating the DHA in the decision making. A prospective study of 92 children (5-14 years) with phalange

Emerging therapies for the management of traumatized immature permanent incisors.

Early loss of immature permanent teeth due to pulpal necrosis secondary to trauma can have dire consequences for a child's growth and development. The treatment alternatives include surgical endodontics, traditional calcium hydroxide apexification, and mineral trioxide aggregate (MTA) apexification. These options pose potential complications, including: arrest of root development; weakened dentinal walls; and increased potential for fracture. Revascularization of the dentin-pulp complex is a new approach that involves disinfecting the root canal system followed by tissue repair and regeneratio

Volar fixed-angle plate osteosynthesis of unstable distal radius fractures: 12 months results.

BACKGROUND: With an incidence of about 2-4 per 1,000 residents per year, the distal radial fracture is the most common fracture in the human skeleton. The introduction of fixed-angle plate systems for extension fractures at the radius was evaluated in a prospective study performed at our hospital after selection and acquisition of a new system. The focus of our interest was whether a secondary loss of reduction can be avoided by this plating system. METHODS: We reviewed 80 patients treated for unstable distal radius fractures using a volar fixed-angle plate. Postoperative management included

The crooked nose: the use of medial iliac crest bone graft as a supporting framework.

The crooked nose is one that the vertical axis of the nose inclines from the midline. As a rule, a major septal deformity almost always accompanies to impairing breathing and aesthetic appearance. To achieve total straightening of the crooked nose, the septum must become the target of the treatment. To prevent redeviation, we used rigid bone grafts instead of flexible and relatively weak cartilage to maintain the support of the corrected deformity. Between February 2005 and July 2007, 9 patients (all male) with crooked noses underwent corrective surgery. The mean age of the patients was 21 yea

Implant replacement of congenitally missing lateral incisor: a case report.

The therapeutic goal of reconstruction dentistry is to provide our patients with a dentition that is in harmony with the patient's musculature and temporomandibular joints. Malocclusion can contribute to both muscular and joint dysfunction. This case report demonstrates the reconstruction of a malocclusion caused by a congenitally missing lateral incisor utilizing a combination of orthodontic treatment and a dental implant to replace the congenitally missing tooth.

Treatment of developmental dysplasia of the hip with the Pavlik harness: factors for predicting unsuccessful reduction.

To evaluate the risk factors for unsuccessful reduction, we reviewed 100 hips with developmental dysplasia of the hip treated with the Pavlik harness. We divided them into a successful reduction group (reduced without avascular necrosis) and an unsuccessful reduction group (reduced followed by avascular necrosis or unreduced). We compared Yamamuro's distance A, the acetabular angle and age at initial application of the Pavlik harness between the two groups. Our data suggest that a Yamamuro's distance A of 7 mm or smaller, an acetabular angle of 36 degrees or greater, and an age at initial appl

Effect of occlusal appliances and clenching on the internally deranged TMJ space.

AIMS: Stabilization appliances and mandibular anterior repositioning appliances have been used to treat patients with internal derangement of the temporomandibular joint (TMJ) based on the assumption that these appliances work by decompressing the TMJ. The purpose of this study was to indirectly test this assumption. METHODS: Bilateral TMJ tomograms of 7 subjects with unilateral anterior disc displacement without reduction (ADDwor) were taken during comfortable closure and during maximum clenching in maximum intercuspation; tomograms were also taken with the 2 types of occlusal appliances in

Immobilisation versus early active motion following primary flexor pollicis longus tendon zones 1-2 repair: protocol for a registry-based multicentre randomised controlled trial in Sweden.

INTRODUCTION: Flexor pollicis longus (FPL) tendon injuries in the thumb are surgically challenging and require careful postoperative rehabilitation to avoid complications such as tendon rupture, stiffness and impaired hand function. Rupture rates after FPL repairs are substantially higher than those reported for finger flexor tendons. Although controlled early active motion (EAM) is widely used following FPL repairs, rehabilitation protocols developed for finger flexor tendons may not be directly transferable to the thumb because of differences in anatomy, biomechanics and functional demands.

Temporomandibular disorders in women with psoriatic arthritis by history of psoriasis: a cross-sectional study.

Temporomandibular joint involvement in psoriatic arthritis may result in impaired jaw function and reduced oral health-related quality of life. Patients with PsA have generally been analyzed as a single group regardless of psoriasis history, despite a reported association between psoriasis and TMJ involvement. To assess TMD symptoms, patient-reported outcome measures, and self-reported beneficial effects of systemic therapies and TMD-related treatment modalities in women with psoriatic arthritis, stratified by psoriasis history. A digital questionnaire was sent to members aged ≥ 18 years of th

When Is a Discrepancy in Crestal Bone Height Level for Adjacent Dental Implants Significant? A Retrospective Study with a Minimum Follow-up of 1 Year.

PURPOSE: To investigate the influence of a discrepancy in crestal bone height between two neighboring implants and how it affects MBL over time. MATERIALS AND METHODS: Medical and dental records and periapical radiographs of individuals with at least two adjacent implants were retrieved and analyzed. Records were divided into two groups according to the prosthetic part: (1) nonsplinted (NS) and (2) splinted (S) groups. Furthermore, groups were categorized by crestal bone height level (CBHL) differences between the two adjacent implant platforms: (1) ≤ 0.75 mm, (2) 0.76 to 1.25 mm, (3) 1.26 to

Challenging the Role of Routine Thromboprophylaxis in Foot and Ankle Surgery: A Comparative Study of Aspirin, Enoxaparin, and No Prophylaxis.

BACKGROUND: Deep vein thrombosis (DVT) is a common postoperative complication in orthopedic surgery and may result in life-threatening events such as pulmonary thromboembolism (PTE). However, evidence on the optimal prophylactic strategy for DVT following foot and ankle surgery remains limited. This study aimed to assess the effect of pharmacologic thromboprophylaxis in patients undergoing foot and ankle surgery with postoperative immobilization. METHODS: A retrospective review was conducted of 451 patients who underwent foot and ankle surgery followed by short-leg splint immobilization with

When the Nerve Lies Between the Fragments: A Mid-Shaft Humeral Fracture Mimicking a Holstein-Lewis Injury.

INTRODUCTION: Radial nerve palsy is a recognized complication of humeral shaft fractures, often associated with distal-third spiral patterns described by Holstein and Lewis. However, authentic mechanical entrapment of the nerve at the mid-shaft level is rather rare. These events test the limits of normal anatomy and show how important it is to look into things right once when fracture geometry suggests possible nerve entrapment. CASE REPORT: A 28-year-old right-handed female sustained a humeral fracture at the mid-shaft (AO 12-A3) following a collision with a two-wheeled vehicle. She showed e
