Hydrogen Sulfide Metabolite, Sodium Thiosulfate: Clinical Applications and Underlying Molecular Mechanisms.

Thiosulfate in the form of sodium thiosulfate (STS) is a major oxidation product of hydrogen sulfide (H2S), an endogenous signaling molecule and the third member of the gasotransmitter family. STS is currently used in the clinical treatment of acute cyanide poisoning, cisplatin toxicities in cancer therapy, and calciphylaxis in dialysis patients. Burgeoning evidence show that STS has antioxidant and anti-inflammatory properties, making it a potential therapeutic candidate molecule that can target multiple molecular pathways in various diseases and drug-induced toxicities. This review discusses

Palliative Care in End-Stage Renal Disease

Patients with end-stage renal disease (ESRD) often experience a multitude of physical and psychosocial symptoms. The life expectancy of patients on dialysis is approximately 20% of that of age-matched individuals without renal disease. Of note, uremic calciphylaxis (or calcific uremic arteriopathy) can cause painful skin lesions and portends a poor prognosis. Frequent assessments, early intervention, and involvement of the interdisciplinary team are key to providing care for these patients. Decision-making for the initiation and withdrawal of dialysis is a complicated process. The prognosis af

Calcinosis cutis universalis in a patient with systemic lupus erythematosus.

Deposition of calcium salts in the skin and subcutaneous tissue occurs in a variety of rheumatic diseases, being most commonly associated with scleroderma, CREST (calcinosis, Raynaud's phenomenon, esophageal dysfunction, sclerodactyly, and telangiectasia), dermatomyositis, and overlap syndromes but is a rare complication of systemic lupus erythematosus (SLE). Calcinosis is classified into four subsets: dystrophic, metastatic, idiopathic, or calciphylaxis/iatrogenic. The pathophysiology of calcinosis cutis remains unclear. Our patient developed extensive areas of calcifications in the trunk and

[A POEMS syndrome revealed by a blue toe syndrome].

INTRODUCTION: Arterial and venous thromboses occur in almost one in five patients with POEMS syndrome and usually in macrocirculation. CASE REPORT: We report a 67-year-old male with a POEMS syndrome who presented initially with a blue toe syndrome. He complained of Raynaud's syndrome and left foot paresthesia. Physical examination showed gynecomastia, lymphadenopathies and skin lesions. Cardiovascular investigations excluded atrial fibrillation, unstable atherosclerotic lesions and vascular calcifications. Imaging studies showed diffuse osteosclerotic lesions. Monoclonal protein with lambda l

Severe anion gap acidosis associated with intravenous sodium thiosulfate administration.

INTRODUCTION: Severe anion gap (AG) acidosis associated with intravenous sodium thiosulfate (STS) administration has not been previously described in nondialysis chronic kidney disease (CKD) patients. CASE REPORT: We present a CKD patient with a baseline creatinine 1.8 mg/dL (eGFR 28 ml/min/1.73 m²) who developed sustained and life-threatening AG acidosis associated with intravenous STS treatment for calciphylaxis. DISCUSSION: Although marketed as a safe drug, STS can cause life-threatening acidosis as illustrated in this case. STS-induced AG acidosis should be considered in the differential

Sodium thiosulfate in the treatment of calcific uremic arteriolopathy.

Calcific uremic arteriolopathy (CUA; also known as calciphylaxis) is a life-threatening condition observed mostly in patients on dialysis. The key histopathologic features of CUA include media calcification of small arteries, associated with endovascular fibrosis and thrombosis. Several risk factors for CUA are related to disturbances in bone and mineral metabolism; current treatments largely aim to normalize these disturbances by lowering serum calcium phosphate concentration and thereby preventing, or even reversing, calcium phosphate oversaturation, precipitation and, finally, calcification

Calcific uremic arteriolopathy in a transplanted kidney.

Calcific uremic arteriolopathy is a severe and life-threatening condition that develops in patients with kidney disease. Most commonly, it presents with skin rash and painful subcutaneous nodules. However, other organs may be involved. We report a case of a young man who developed renal failure in the transplanted kidney. Renal biopsy demonstrated myointimal calcification in the renal arteries consistent with this diagnosis.

Cutaneous gangrene due to hyperparathyroidism secondary to chronic renal failure (uraemic gangrene syndrome).

Cutaneous and subcutaneous gangrene are serious sequelae of secondary or tertiary hyperparathyroidism which may accompany chronic renal failure. Based on analysis of the present typical case and a retrospective survey of similar cases in the literature, we propose the term 'uraemic gangrene syndrome' for this association. These skin lesions in chronic renal failure patients represent the most serious clinical manifestation of calciphylaxis, a condition originally described by Selye. The appearance of early skin lesions should be regarded as an indication for subtotal parathyroidectomy. The ass

A novel treatment for ulcerative calcinosis cutis.

BACKGROUND: Calcinosis cutis is a term used to describe a group of disorders which result in calcium deposits in the skin. These disorders can be separated based on etiology. OBJECTIVE AND METHODS: Sodium thiosulfate has been used to systemically treat calciphylaxis with little to no adverse effects. We report two cases of ulcerative calcinosis cutis which were refractory to multiple topical treatments and did not improve with correction of underlying electrolyte abnormalities. RESULTS: Both cases showed an excellent response to topical 25% sodium thiosulfate compounded in zinc oxide. LIMIT

Clinical management of nonuremic calcific arteriolopathy: a report of three cases.

INTRODUCTION: Calcific arteriolopathy involves the deposition of insoluble calcium salts in the vasculature and soft tissues, leading to ischemia, thrombosis, and cutaneous necrosis. Calcific arteriolopathy is commonly associated with ESRD but can also occur outside of ESRD, known as NUC. OBJECTIVE: This article reviews the clinical management and outcomes of 3 cases of non-uremic calcific arteriolopathy. MATERIALS AND METHODS: This case series describes the clinical presentation and successful treatment of NUC in a community hospital-based outpatient wound center in southeastern Wisconsin.

Multidisciplinary approach to calcific uremic arteriolopathy.

PURPOSE OF REVIEW: Calcific uremic arteriolopathy (CUA), as known as calciphylaxis, is a rare and poorly understood disease seen predominantly in end stage renal disease patients. A collaborative multidisciplinary approach to develop and implement treatment and prevention methods is described. RECENT FINDINGS: Overall, the scientific literature on CUA is largely restricted to case reports and case series. Recent reports indicate that the incidence of CUA may be on the rise and emphasize an association with vitamin K antagonist therapy, obesity, and diabetes mellitus. Serum calcium, phosphorou

Oxalosis and livedo reticularis.

Oxalosis is a disease caused by the deposition of calcium oxalate in extrarenal tissues, most commonly bone, myocardium, retina, blood vessels, and skin, causing the clinical manifestations of the disease. Involvement of the blood vessels of the skin can give rise to livedo reticularis, acrocyanosis, ulcers, and gangrene. We present the case of a 60-year-old woman with a history of recurrent renal lithiasis that had led to terminal renal failure requiring hemodialysis and, subsequently, peritoneal dialysis. The patient developed tender red-violaceous skin discoloration of sudden onset, consist

Diffuse Idiopathic calcinosis cutis: a case report in a 13-year-old Syrian boy.

Calcinosis cutis is characterized by the deposition of calcium salts in the skin and subcutaneous tissue. It is divided into the following subtypes: dystrophic, metastatic, iatrogenic, idiopathic and calciphylaxis. In this case, we report a 13-year-old Syrian boy with idiopathic calcinosis cutis, the lesions were unusually widespread, unlike the common condition which is usually localized to one area. The case was unrelated to any systemic or local disorders, and the patient had no complications, so no treatment was planned. As the best of our knowledge, there are very few cases of diffuse or

A 52-Year-Old Female with Multiple Swellings in Both Hands: Idiopathic Calcinosis Cutis.

Calcinosis cutis is a rare disorder characterized by deposition of insoluble calcium salts in skin and subcutaneous tissue. Depending upon the aetiology, there are five subtypes of calcinosis cutis described in the literature; dystrophic, metastatic, idiopathic, iatrogenic, and calciphylaxis. Idiopathic calcinosis cutis is rarely described in the literature. We herein report a 52-year-old female presented with calcinosis cutis in both hands. After ruling out metabolic and systemic causes of abnormal calcium deposition, the patient was diagnosed with idiopathic calcinosis cutis. The patient was

Foot alterations in the hemodialyzed patient.

On the basis of their experience, the authors refer to complications observable at the foot level in chronic uremic patients in long-term hemodialytic treatment. These complications, termed renal osteodystrophy, are attributed to alterations in the phosphocalcium metabolism and to hyperparathyroidism. From an anatomopathologic viewpoint, they are characterized by alteration in bone tissue (osteomalacia and/or fibrous osteitis) and by soft tissue alterations. Alterations in the foot are the same as those observed in other osteoarticular regions. They consist of bone subperiosteal reabsorption o
