What are the sources of contraceptives for married and unmarried adolescents: Health services or friends? Analysis of 59 low- and middle-income countries.

BACKGROUND: Despite the efforts to promote universal coverage for family planning, inequalities are still high in several countries. Our aim was to identify which sources of contraceptives women mostly rely on in low- and middle-income countries (LMICs). We also explored the different sources according to age and marital status. METHODS: We used data from national health surveys carried out in 59 LMICs since 2010. Among all sexually active women at reproductive age, we explored inequalities in demand for family planning satisfied by modern methods (mDFPS) and in the source of modern contracep

Ethical challenges faced by French military doctors deployed in the Sahel (Operation Barkhane): a qualitative study.

BACKGROUND: French military doctors are currently deployed in the Sahel to support the armed forces of Operation Barkhane, in medical or surgical units. As well as supporting French soldiers, their other missions are diverse and complex: medical assistance to civilians and persons under control (PUC), advice to commanding officers. These tasks can create ethical dilemmas when decisions are forced upon doctors that may be in conflict with medical values or fundamental principles. Little is known about the specific dilemmas experienced by French military doctors in overseas operations. We theref

Percutaneous left atrial appendage closure versus non-vitamin K oral anticoagulants in patients with non-valvular atrial fibrillation and high bleeding risk.

AIMS: A significant number of patients with non-valvular atrial fibrillation (NVAF) are ineligible for non-vitamin K oral anticoagulants (NOACs) due to previous major bleeding or because they are at high bleeding risk (HBR). In this setting the indication for percutaneous left atrial appendage closure (LAAO) is a valuable alternative. We aimed to evaluate the efficacy and safety of NOACs versus LAAO indication in NVAF patients at HBR (HAS-BLED ≥3). METHODS AND RESULTS: All consecutive patients who underwent successful LAAO (n=193) and those treated with NOACs (n=189) (dabigatran, apixaban or

Validation of a Rapid Rabies Diagnostic Tool for Field Surveillance in Developing Countries.

BACKGROUND: One root cause of the neglect of rabies is the lack of adequate diagnostic tests in the context of low income countries. A rapid, performance friendly and low cost method to detect rabies virus (RABV) in brain samples will contribute positively to surveillance and consequently to accurate data reporting, which is presently missing in the majority of rabies endemic countries. METHODOLOGY/PRINCIPAL FINDINGS: We evaluated a rapid immunodiagnostic test (RIDT) in comparison with the standard fluorescent antibody test (FAT) and confirmed the detection of the viral RNA by real time rever

An indirect comparison of dabigatran, rivaroxaban and apixaban for atrial fibrillation.

New oral anticoagulant drugs are emerging as alternatives to warfarin for the prevention of stroke in patients with non-valvular atrial fibrillation. Two agents are direct factor Xa inhibitors (rivaroxaban and apixaban), and the third is a direct thrombin inhibitor (dabigatran). They have been separately compared to warfarin in large randomised trials. Our objective was to indirectly compare the three agents to each other for major efficacy and safety outcomes. Studies were assessed for comparability and the odds ratios of selected outcomes for each anticoagulant versus one another were estima

Regional status, epidemiology and impact of Taenia solium cysticercosis in Western and Central Africa.

In West Africa, Taenia solium cysticercosis in both pigs and man has been reported in Benin, Burkina-Faso, Ghana, Ivory Coast, Senegal and Togo, and although official data are lacking, T. solium is anticipated to be present in most of the pig-raising regions of other West African countries as well. In some regions of Nigeria, the prevalence of porcine cysticercosis and human taeniosis is quite high (20.5 and 8.6%, respectively). Surprisingly, however, no cases of human cysticercosis have been reported, although epilepsy is very common. Large epidemiological surveys have only been carried out i

A machine learning-based predictive model for radiosensitivity in nasopharyngeal carcinoma utilizing serum proteomics.

BACKGROUND: Nasopharyngeal carcinoma (NPC) remains highly sensitive to radiotherapy; however, radioresistance in a subset of patients leads to local recurrence and distant metastasis. Serum proteomics provides a minimally invasive approach to capturing dynamic physiological changes, and machine learning enables efficient construction of predictive models. This study aimed to develop and validate a serum proteomics–based machine-learning model for predicting radiotherapy sensitivity in nasopharyngeal carcinoma (NPC). METHODS: Pretreatment serum samples from newly diagnosed NPC patients were ana

Assessing WHO prioritisation criteria for children 6-59 months treated for moderate wasting in a MUAC-based protocol: a multicountry analysis in West and Central Africa.

BACKGROUND: In 2023, the WHO recommended prioritising treatment for moderately wasted children with higher risk characteristics, including mid-upper arm circumference (MUAC) 115-119 mm, weight-for-age z-score (WAZ) <-3, age <24 months or clinical and social vulnerabilities. However, applying multiple criteria is operationally challenging. The OptiMA (Optimising Acute Malnutrition Treatment) protocol treats children with MUAC <125 mm or oedema using reduced doses of ready-to-use therapeutic food. This study examined the distribution and programmatic relevance of WHO prioritisation criteria amon

Spatial variations and determinants of modern contraceptive use among postpartum women in Sub-Saharan Africa: analysis of recent DHS data (2015-2023).

The low uptake of postpartum family planning contributes to around 80 million unplanned pregnancies globally each year. Despite the postpartum period being crucial for preventing unintended pregnancies, modern contraceptive use remains low in Sub-Saharan Africa, with notable regional differences. This study aims to assess the pooled prevalence of modern contraceptive use, its spatial variation, and associated factors among postpartum women in Sub-Saharan Africa, utilizing data from recent Demographic Health Surveys (2015-2023). A cross-sectional analysis was conducted using data from 27 Sub-Sa

Assessing the Global, Regional, and National Burden of Childhood Lower Respiratory Infections From Nonexclusive Breastfeeding, 1990-2021: Global Burden of Disease, Injuries, and Risk Factors Study 2021 Analysis and Implications for Public Health Strategies.

BACKGROUND: Nonexclusive breastfeeding is a major risk factor for childhood lower respiratory infections, yet its global burden remains unquantified. Although exclusive breastfeeding is widely recommended, the attributable burden of early infancy lower respiratory infections linked to nonexclusive breastfeeding and cross-country performance gaps have not been systematically assessed. This analysis fills that gap by combining burden estimates with frontier benchmarking. METHODS: Using the Global Burden of Diseases, Injuries, and Risk Factors Study 2021 database, we estimated deaths, disability

Geographic disparities in hepatitis B vaccine coverage across Africa: Implications for targeted interventions and 2030 goals.

BACKGROUND & AIMS: Hepatitis B virus (HBV) remains a leading cause of liver-related morbidity and mortality in Africa. Reaching the World Health Organization (WHO) target of ≥90% third-dose (HepB3) coverage by 2030 demands precise, subnational intelligence. We mapped hepatitis B vaccination coverage, dropout, and projected trends. METHODS: We analyzed 245,168 child vaccination records from 89 nationally representative household surveys (2010-2022) covering 51 African countries. Bayesian geostatistical models were used to produce 5 × 5 km estimates of first-dose (HepB1) and HepB3 coverage, as

From poverty to health: intraurban inequalities in child health indicators in low-income and middle-income countries during the SDG era.

INTRODUCTION: Urban areas are generally associated with better health outcomes. However, in many low-income and middle-income countries (LMICs), rapid urbanisation has led to significant health disparities. These inequalities, especially those affecting the urban poor, are often overlooked in studies focused solely on rural-urban differences. This study aimed to assess the overall and country-specific intraurban inequalities in under-5 and neonatal mortality rates, immunisation coverage, care-seeking for acute respiratory infections (ARIs), coverage of diarrhoea treatment, and stunting, wastin

Prevalence and factors associated with contraceptive use among sexually active adolescent girls in 25 sub-Saharan African countries.

INTRODUCTION: Various countries in sub-Saharan Africa have taken divergent steps toward achieving the Sustainable Development Goal's target of universal access to sexual and reproductive health-care services by 2030, particularly among sexually active adolescent girls who are at risk of unplanned pregnancies and sexually transmitted infections. However, because contraceptive use among sexually active adolescents remains unexplored in sub-Saharan Africa, the researchers intended to examine the prevalence and factors associated with contraceptive use among adolescent girls who had been sexually

Tracking the geographical origin of Plasmodium falciparum causing a rare severe case of malaria imported into Palestine, a zero-indigenous case area.

BACKGROUND: Malaria cases in non-endemic zero-indigenous case areas are most likely to have been imported whatever of the route of importation. In countries recently declared malaria-free and now without local transmission, imported cases remain a threat to re-introduction of the disease and a burden on the health system. Unequal access and use of contraceptives among parenting adolescent girls in sub-Saharan Africa: a cross-sectional analysis of demographic and health surveys.

OBJECTIVE: We examined the divergent patterns, prevalence and correlates of contraceptive use among parenting adolescents in sub-Saharan Africa using the Demographic and Health Survey datasets of 17 countries. DESIGN: We included a weighted sample of 9488 parenting adolescent girls in our analysis. Current contraceptive use was defined as the use of any methods to delay or avoid getting pregnant at the survey time. We reported the prevalence of any contraceptive use for all countries and used multilevel binary logistic regression analysis to examine the individual and contextual factors assoc

Extracellular matrix disruption is an early event in the pathogenesis of skeletal disease in mucopolysaccharidosis I.

Progressive skeletal and connective tissue disease represents a significant clinical burden in all of the mucopolysaccharidoses. Despite the introduction of enzyme replacement strategies for many of the mucopolysaccharidoses, symptomatology related to bone and joint disease appears to be recalcitrant to current therapies. In order to address these unmet medical needs a clearer understanding of skeletal and connective tissue disease pathogenesis is required. Historically the pathogenesis of the mucopolysaccharidoses has been assumed to directly relate to progressive storage of glycosaminoglycan

The Efficacy of Artificial Intelligence in the Detection and Management of Atrial Fibrillation.

Источник пометил публикацию как отозванную

The integration of artificial intelligence (AI) into medicine offers transformative potential, particularly in the detection and management of atrial fibrillation (AF). However, the intersection of AI and AF has not been comprehensively evaluated. This systematic review focuses specifically on the applications of AI in AF risk prediction, monitoring, and management. Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, a comprehensive search was conducted in PubMed and Google Scholar using terms such as artificial intelligence, deep learning, mac
