Elbow joint luxation in a 1-month-old foal.

This paper reports on luxation of the elbow joint without concomitant fracture in a 1-month-old foal. Conservative treatment, with closed reduction and full-limb bandaging, including caudal and lateral splints, seemed successful initially, however, failed to provide enough stability and luxation recurred, and open reduction and surgical placement of prosthetic collateral ligaments was required. Luxation of the elbow joint should be considered when acute non-weight bearing forelimb lameness occurs associated with pain and swelling in the area of the elbow in young foals. Closed reduction failed

Cast immobilization after total elbow arthroplasty. A safe cost-effective method of initial postoperative care.

Seventeen nonconstrained total elbow replacements were inserted in 12 patients. One patient died prior to the one-year follow-up examination, and two others required revision in the immediate postoperative period. The remaining 14 primary total elbow arthroplasties in 11 patients were included in this study. There were eight women and three men with an average age of 58.1 years. The diagnosis was rheumatoid arthritis in 12 patients and posttraumatic arthritis and juvenile rheumatoid arthritis each in one patient. Postoperatively, patients were immobilized in a long arm cast. The mean hospital

Semiconstrained elbow replacement for distal humeral nonunion.

We have reviewed 36 of 39 consecutive patients with an average age of 68 years who had semiconstrained elbow replacement for distal humeral nonunion at an average follow-up of 50.4 months (24 to 127). Of these, 31 (86%) had satisfactory results, three (8%) had fair, and two (6%) had poor results; 32 patients (88%) had moderate or severe pain before and 91% had no or only mild discomfort after the procedure. Motion had improved from a mean arc of 29 degrees to 103 degrees before operation to 16 degrees to 127 degrees after surgery. All five flail extremities were stable at last assessment. Ther

Short-term to medium-term outcomes of cemented Vitallium radial head prostheses after early excision for radial head fractures.

Studies show that the radial head is a significant stabilizer to valgus and varus stresses and external rotatory stability. We review the outcomes of patients who had radial head replacement in our institution. Six patients with Mason-Johnston type III or IV radial head fractures underwent radial head replacement and were evaluated by radiologic and clinical assessment. The American Shoulder and Elbow Surgeons score, DASH (Disabilities Arm, Shoulder and Hand) score, and Broberg and Morrey Performance Index were calculated. Average follow-up was 29.7 months. The Broberg and Morrey score was exc

Long-term outcomes of radial head arthroplasty for complex radial head fractures.

PURPOSE: Radial head arthroplasty is widely used for unreconstructable radial head fractures, particularly in complex elbow trauma, yet long-term outcomes remain poorly documented. This study aimed to assess implant survival and clinical outcomes after radial head arthroplasty performed for traumatic fractures. METHODS: This retrospective monocentric study included patients who underwent radial head arthroplasty between 1997 and 2017 with a minimum follow-up of seven years. The primary outcome was implant survival, defined as absence of revision or implant removal. Secondary outcomes included

Long-term survival of Acumed anatomical radial head implant for Mason type III-IV fractures: a 15-year follow-up.

BACKGROUND: Radial head fractures account for 1.7% to 5.4% of all skeletal fractures and approximately one-third of elbow fractures. These injuries are often associated with other concurrent injuries, such as fractures of the coronoid, ligamentous injuries of the elbow, or trauma to other regions of the upper limb. Intra-articular fractures, especially Mason type III and IV, often require anatomical reduction to restore the articular surface, but such procedures can yield suboptimal results, highlighting the role of radial head implants in complex cases. MATERIALS AND METHODS: This retrospect

Limb salvage and complication management after (sub-) total humerus resection for primary malignant bone tumors in early childhood.

Limb-salvage after (sub-) total humerus resection in skeletally immature patients, aged less than 10 years, remains the domain of individual case decisions due to a small number of affected patients. Seven children aged ≤ 10 years at the time of (sub-) total bone sarcoma resection of the humerus between 2018 and 2023 were included in this retrospective study. Limb salvage with an intact motor function of the lower arm was achieved in 100% of cases at a mean follow-up of 89.8 months. Reconstruction survival was 28.6%. 61.8% (n = 8/13) of complications necessitated revision operations (n = 7). D

Learning from demonstration: Teaching a myoelectric prosthesis with an intact limb via reinforcement learning.

Prosthetic arms should restore and extend the capabilities of someone with an amputation. They should move naturally and be able to perform elegant, coordinated movements that approximate those of a biological arm. Despite these objectives, the control of modern-day prostheses is often nonintuitive and taxing. Existing devices and control approaches do not yet give users the ability to effect highly synergistic movements during their daily-life control of a prosthetic device. As a step towards improving the control of prosthetic arms and hands, we introduce an intuitive approach to training a

Monteggia-like lesions - treatment strategies and one-year results.

INTRODUCTION: The eponym "Monteggia fracture" includes various patterns of complex fracture-dislocations of the proximal ulna and radius, which are not well defined yet. They are frequently described as Monteggia-like lesions or Monteggia equivalent injuries. Until today, these injury patterns have been reported rarely. The objective of this retrospective study was to better define patterns of injury and to document the short-term results of treatment with current fixation techniques. METHODS: Ten patients with a Monteggia-like lesion were included in this study and clinical and radiological

[Effect of the rotator cuff condition on the results of shoulder arthroplasty].

PURPOSE OF THE STUDY: The purpose of this report was to study the repercussions of rotator cuff condition on unconstrained shoulder arthroplasty results. METHODS: Between 1986 and 1993, 40 unconstrained shoulder prostheses were performed (24 total and 16 hemiarthroplasties). At the time of the operation, rotator cuff condition was reported to be normal in 15 shoulders, atrophic in 10, scarred in 3, torn in 12. Clinical and radiographical results were analyzed and supplemented with a rotator cuff echography. Postoperative follow-up averaged 42.25 months (12 to 97 months). RESULTS: When the ro

Development of the national priority assistive product list in Malawi.

PURPOSE: In 2016 WHO launched the priority assistive products list (APL) consisting of 50 products and recommended that using this as a reference, countries should develop their own contextually relevant national APLs. This paper describes the development of Malawi's APL. METHODS: Two hundred and ninety-six persons with disabilities participated in a rapid Assistive Technology Assessment (rATA) survey. Six focus group discussions (FGDs) with people with various types of disabilities were conducted. The rATA questionnaire and FGDs collected data on assistive products (APs) participants used, A

Management of irreparable massive rotator cuff tears: a systematic review and meta-analysis of patient-reported outcomes, reoperation rates, and treatment response.

BACKGROUND: There is no consensus on the treatment of irreparable massive rotator cuff tears. The goal of this systematic review and meta-analysis was to (1) compare patient-reported outcome scores, (2) define failure and reoperation rates, and (3) quantify the magnitude of patient response across treatment strategies. METHODS: The MEDLINE, Embase, CENTRAL (Cochrane Central Register of Controlled Trials), and Scopus databases were searched for studies including physical therapy and operative treatment of massive rotator cuff tears. The criteria of the Methodological Index for Non-randomized S

The role of cultured Schwann cell grafts in the repair of gaps within the peripheral nervous system of primates.

With recent advances in cell culture techniques it is possible to isolate human SCs from adult peripheral nerves, expand and purify their number in cell culture, and construct a cellular prosthesis from the cultured cells. The current study was designed to ascertain whether these techniques could be used to repair nonhuman primate nerve injuries. In 12 adult female cynomologous monkeys, the musculocutaneous (msk) nerve was divided and prevented from regenerating and the brachioradialis nerve (brach) was exposed bilaterally (n = 24 nerves) and injured so that a 15-mm gap existed within the nerv

Beyond Implant Design: Surgical Principles and Early Outcomes of Patient-Specific Scapular Reconstruction After Total Scapulectomy.

Background/Objectives: Total scapulectomy for malignant scapular tumors remains a major reconstructive challenge because resection removes the osseous framework, muscular insertions, and biomechanical support required for shoulder function. Patient-specific three-dimensional (3D)-printed implants may improve anatomical reconstruction, but the indications and surgical principles underlying successful total scapular replacement remain poorly defined. Methods: This retrospective single-center case series included the first three consecutive patients who underwent total scapulectomy followed by im

Lateralized glenosphere reverse shoulder arthroplasty: inlay and onlay designs have similar clinical outcomes in patients with glenohumeral osteoarthritis.

BACKGROUND: Whether or how the position of the humeral tray (inlay or onlay) in reverse shoulder arthroplasty (RSA) affects outcomes is unclear. Our goal was to compare the clinical and radiographic results of RSA systems with inlay vs. onlay designs but with similar neck shaft angles (NSAs) and lateralized glenospheres. METHODS: We screened the institutional database at our tertiary academic center for patients who underwent primary RSA (with a lateralized glenosphere and a 135° NSA) from 2009 through 2017. The indication for surgery was glenohumeral osteoarthritis with glenoid bone loss (Wa

Comminuted fractures of the radial head. The role of silicone-implant replacement arthroplasty.

Radial head resection is the accepted treatment of comminuted radial-head fractures in adults, but the results are not always satisfactory. This study examines the results of silicone replacement arthroplasty of the radial head in acute fractures and as a salvage procedure after failed resections of the radial head. The first group of patients was evaluated both before and after silicone replacement arthroplasty had been done as a salvage procedure following a previous radial head fracture. This group comprised twelve patients with an average follow-up of 3.8 years. Improvement was found in a

A Meta-Analysis of Polymerase Chain Reaction for Prosthetic Joint Infection Diagnosis.

BACKGROUND: One of the most dangerous side effects of joint replacement for the hip, knee, shoulder, and elbow is prosthesis joint infection (PJI). Polymerase chain reaction (PCR) has been considered a promising method for PJI diagnosis due to its short diagnostic time and high sensitivity. Although several PCR methods such as multiplex PCR and broad-range PCR are useful diagnostic methods for detecting microorganisms causing PJI, values of different PCR methods for the diagnosis of PJI remain unclear. Thus, the objective of this study was to perform a meta-analysis of different PCR methods in

Functional Outcomes and Health-Related Quality of Life of Adults With Congenital Below-Elbow Amputation in North America.

PURPOSE: Congenital below-elbow amputation (BEA) is a common upper-extremity anomaly and generally encompasses 2 diagnoses, symbrachydactyly and transverse deficiency. Little is known about the physical, mental, and social well-being of adults with congenital BEA. A deeper understanding of longitudinal outcomes within this population may help guide family conversations and counseling for patients with congenital BEA. METHODS: The Shriners Hospitals for Children Health Outcomes Network was queried to identify all patients currently >18 years of age who had been seen as a child between 1975 and

Are We Justified Doing Salvage or Amputation Procedure Based on Mangled Extremity Severity Score in Mangled Upper Extremity Injury.

INTRODUCTION: Mangled upper limb injuries are at surge because of industrialization, modernization, and severe motor vehicle accidents. The utility of various scoring systems are meant for decision making in mangled lower limb injuries, and the same have been extrapolated for mangled upper limb injuries to make a decision of salvage or amputation for the lack of separate scoring system. MATERIALS AND METHODS: We applied mangled extremity severity score (MESS) and mangled extremity syndrome index (MESI) scoring systems to 10 cases of mangled upper limb injuries during the period of November 20
