TASP1-Related Suleiman-El-Hattab Syndrome: The First Genetically Confirmed Case in Iran Accompanied by a Comprehensive Literature Review.

Suleiman–El-Hattab syndrome (SULEHS; OMIM #618674) is an ultra-rare autosomal recessive multisystem neurodevelopmental disorder caused by biallelic loss-of-function variants in TASP1, with a characteristic combination of global developmental delay/intellectual disability, distinctive craniofacial dysmorphism, recurrent infections, congenital anomalies, and a typically cheerful affect. Because fewer than ten molecularly confirmed individuals have been reported, the phenotypic spectrum and neuroradiologic variability remain incompletely defined. We describe the first genetically confirmed Irania

Liddle syndrome caused by SCNN1B mutation presenting with severe early-onset hypertension and hypokalemia in an adolescent: a case report.

BACKGROUND: Liddle syndrome is a rare autosomal dominant form of monogenic hypertension caused by gain-of-function mutations in genes encoding the epithelial sodium channel (ENaC). The disease is characterized by early-onset hypertension, hypokalemia, metabolic alkalosis, and suppression of the renin-angiotensin-aldosterone system (RAAS). Early diagnosis is essential because targeted therapy with ENaC inhibitors can effectively control blood pressure and prevent end-organ damage, whereas mineralocorticoid receptor antagonists are ineffective. CASE PRESENTATION: We report a 15-year-old Chinese

Occipitoparietal epilepsy, hippocampal atrophy, and congenital developmental abnormalities.

PURPOSE: Diagnostic uncertainty may arise in patients with occipitoparietal epilepsy when there is neuroimaging evidence of a posterior quadrant lesion and coexistent hippocampal abnormalities ("dual pathology"). It is not known whether hippocampal atrophy (HA) in these patients results from seizure propagation to temporolimbic structures or whether it is part of the pathological process underlying the occipitoparietal epilepsy. Clarification of this issue may have a significant bearing on the management of these patients. METHODS: We studied 20 patients with occipitoparietal epilepsy and neu

Cognitive impairment in epilepsy patients and its correlations.

OBJECTIVE: Epilepsy is a severe disease in which seizures play the leading role. Striking clinical manifestations of the attacks take most of the attention of healthcare professionals. Apart from epilepsy itself, it is well known that epilepsy patients may also have psychiatric comorbidities. These disorders, such as anxiety and depression, are mostly thought to be related to epileptic seizures or antiepileptic medications. In clinical practice, cognitive impairment is another disrupted area of interest in epileptic patients. Our study aimed to detect this deterioration and its correlations wi

Lesion-induced pseudo-dominance at functional magnetic resonance imaging: implications for preoperative assessments.

OBJECTIVE: To illustrate how lesion-induced neurovascular uncoupling at functional magnetic resonance imaging (fMRI) can mimic hemispheric dominance opposite the side of a lesion preoperatively. METHODS: We retrospectively reviewed preoperative fMRI mapping data from 50 patients with focal brain abnormalities to establish patterns of hemispheric dominance of language, speech, visual, or motor system functions. Abnormalities included gliomas (31 patients), arteriovenous malformations (AVMs) (11 patients), other congenital lesions (4 patients), encephalomalacia (3 patients), and tumefactive enc

Developmental follow-up of breastfed term and near-term infants with marked hyperbilirubinemia.

OBJECTIVE: In recent years, the increased prevalence of breastfeeding in conjunction with early discharge practices has increased the risk for marked hyperbilirubinemia in neonates. This has resulted in the potential for bilirubin brain injury in affected infants. The purpose of this study was to identify all infants >/=36 weeks' gestational age with bilirubin levels >25 mg/dL and evaluate them for early and late evidence of bilirubin brain injury. METHODS: We reviewed the charts of all infants (from 1993-1996) >/=36 weeks' gestational age who were readmitted to the hospital during the first

Right hemisphere atrophy rate associates with naming recovery after left hemisphere ischaemic stroke.

Numerous large-scale epidemiological studies investigating the trajectory of brain function after ischaemic stroke have presented data suggesting, on average, a persistent, accelerated decline in cognitive domains, including language, after accounting for the immediate impact of the stroke. We sought to further examine this trend, speculating that the average persistent decline may be a reflection of two subgroups with vastly different prognoses: (i) a minority experiencing decline secondary to neurodegenerative processes such as vascular dementia and Alzheimer's disease and (ii) a majority wi

Complex cranial base trauma resulting from recreational fireworks injury: case reports and review of the literature.

Two patients who sustained complex skull base trauma secondary to recreational fireworks injuries are reported. Initial assessment and management included axial and coronal computerized tomography, control of hemorrhage, debridement of wound and brain, isolation of brain from external environment, and reconstruction of the cranial base floor. Secondary orbital and facial reconstruction used available bone fragments and iliac bone graft in one patient and vascularized free tissue transfer in the other. In both patients, reconstruction of both the intracranial and extracranial compartments was s

Nocardial brain abscess.

Nocardial infections, although rare, are challenging for clinicians to treat. The associated mortality rate remains high; such infections usually occur in immunocompromised patients who have predisposing factors such as malignancy, diabetes mellitus, malnutrition and uremia. However, there have been increasing reports of nocardial infections being observed in immunocompetent patients. Nocardial organisms are mostly isolated from plants and soil, and infection occurs most often as a result of inhalation or direct skin inoculation. Nocardial infections disseminate hematogenously from the primary

Necrotizing encephalomyelopathy of Leigh. Neuropathological findings in 8 cases.

Eight patients with necrotizing encephalomyelopathy are reported. Lesions were found only in the central nervous system. The principal sites affected were the caudate nuclei, putamina, periaqueductal tissues, tegmentum, and structures in the floor of the IVth ventricle. Lesions were also found in the centrum ovale, cerebellum, and the grey and white matter in the spinal cord. The histological findings were of spongy rarefaction, scarring, and excessive vascularity. The pattern of inheritance of necrotizing encephalomyelopathy suggests that it is due to an autosomal recessive gene. Though the d

Fetal brain injury in survivors of twin pregnancies complicated by demise of one twin as assessed by in utero MR imaging.

OBJECTIVE: The aim of this study was to quantify the risk of brain abnormalities in single-twin demise of monochorionic pregnancies and to describe the type of brain injuries using ultrasound and in utero magnetic resonance (iuMR) imaging. METHODS: Monochorionic twin pregnancies complicated by the demise of one twin referred between 2004 and 2013 were reviewed. Ultrasound was performed in a tertiary centre prior to iuMR. The cases were subdivided into those associated with co-twin loss following fetoscopic laser ablative treatment for twin-twin transfusion syndrome (TTTS) and those associated

Association Between Antemortem FLAIR White Matter Hyperintensities and Neuropathology in Brain Donors Exposed to Repetitive Head Impacts.

BACKGROUND AND OBJECTIVES: Late neuropathologies of repetitive head impacts from contact sports can include chronic traumatic encephalopathy (CTE) and white matter degeneration. White matter hyperintensities (WMH) on fluid-attenuated inversion recovery (FLAIR) MRI scans are often viewed as microvascular disease from vascular risk, but might have unique underlying pathologies and risk factors in the setting of repetitive head impacts. We investigated the neuropathologic correlates of antemortem WMH in brain donors exposed to repetitive head impacts. The association between WMH and repetitive he
