A generalised scan statistic test for the detection of clusters.

The scan statistic can be used for the testing of clusters of disease in time. However, it is not valid in situations in which there is some change in the population at risk or in the detection rate of the disorder. Furthermore, it cannot distinguish between a cluster due to the changing pattern of known risk factors for the disease and one due to other causes. A generalised scan statistic is proposed which overcomes these limitations. Its use is illustrated with previously published birth defects data.

Hepatocellular carcinoma in the context of non-alcoholic steatohepatitis (NASH): recent advances in the pathogenic mechanisms.

Hepatocellular carcinoma (HCC) is the most common type of liver cancer. HCC is particularly aggressive and is one of the leading causes of cancer mortality. In recent decades, the epidemiological landscape of HCC has undergone significant changes. While chronic viral hepatitis and excessive alcohol consumption have long been identified as the main risk factors for HCC, non-alcoholic steatohepatitis (NASH), paralleling the worldwide epidemic of obesity and type 2 diabetes, has become a growing cause of HCC in the US and Europe. Here, we review the recent advances in epidemiological, genetic, ep

Dynamics of a network-based SIR epidemic model with double impulsive controls.

A network-based susceptible-infected-recovered (SIR) epidemic model is proposed to study the spread of infectious diseases, thereby incorporating vertical transmission and a saturated incidence rate. Additionally, it introduces double impulsive control strategies, including pulse vaccination and screening with treatment at distinct fixed intervals. Dynamical behaviors of the formulated model are analyzed using two types of transmission thresholds, namely $ R^0_{k_i} $ and $ \widehat{R^0_{k_i}} $. By applying the Floquet theorem and comparison principle, the system admits a disease-free periodi

On the simultaneous inference of susceptibility distributions and intervention effects from epidemic curves.

Susceptible-Exposed-Infectious-Recovered (SEIR) models with inter-individual variation in susceptibility or exposure to infection were proposed early in the COVID-19 pandemic as a potential element of the mathematical/statistical toolset available to policy development. In comparison with other models employed at the time, those designed to fully estimate the effects of such heterogeneity tended to predict small epidemic waves and hence require less containment to achieve the same outcomes. However, these models never made it to mainstream COVID-19 policy making due to lack of prior validation

The role of multiscale and delayed dynamics in tuberculosis Transmission and control: a mathematical approach.

Transmission of tuberculosis (TB) among human population depends on an individual's infectiousness, which is further determined by the concentration of Mycobacterium tuberculosis (Mtb) in the body. Additionally, Mtb is resistant to dryness, cold, acidic, and alkaline environments and can survive in acidic and alkaline environments for 4-5 years. Mtb in the environment plays a significant role in TB transmission and should not be overlooked. To investigate the epidemiologic relationships among pathogens, hosts, and the environment, we first develop a multiscale TB model that includes multiple t

An Agent-Based Modelling Approach to Investigate the Impact of Sex and Gender on Tuberculosis Transmission: A Case Study of Kampala, Uganda.

Tuberculosis (TB) is an airborne disease caused by the pathogen Mycobacterium tuberculosis. In 2023, it returned to being the leading cause of death from an infectious agent globally, replacing COVID-19. More than 10 million people are diagnosed with TB every year. The majority of cases in adults occur in males (62.5% of all global adult cases in 2023, compared to 37.5% in females). The main reasons for males suffering from a higher burden of global TB cases, compared to females, may be in large part due to population-scale factors, such as employment type, the quantity and type of social cont

Modelling with SPEED: a Stochastic Predictor of Early Epidemic Detection.

The frequency of emerging infectious disease outbreaks continues to rise, necessitating predictive frameworks for public health decision-making. This study introduces the Stochastic Predictor of Early Epidemic Detection (SPEED) model, an adaptation of the classic Susceptible-Infected-Recovered model, employing a Gillespie-like algorithm to simulate early-stage stochastic disease transmission. SPEED incorporates individual-level detection probabilities based on the infection time and the lag from GP consultation to lab confirmation. The model dynamically adjusts to public health responses by en

Modeling sparse Rift Valley fever incidence data: a Bayesian perspective on zero-inflated self-exciting and autoregressive models.

BACKGROUND: Rift Valley fever (RVF) is a mosquito-borne zoonotic disease for which predictive modeling is often hindered by sparse data, particularly the high frequency of zero counts in both human and livestock surveillance systems. While zero-inflated models are commonly used for sparse data, several temporal count modelling frameworks exist, including less common self-exciting models that assume an initial case increases the likelihood of subsequent cases. METHODS: This study compares three zero-inflated Bayesian models: the negative binomial (ZINB) with autoregressive temporal random effe

Sensitivity analysis for unmeasured confounding in estimating the difference in restricted mean survival time.

The difference in restricted mean survival time has been increasingly used as an alternative measure to the hazard ratio in survival analysis. Although some statistical methods have been developed for estimating the difference in restricted mean survival time adjusted for measured confounders in observational studies, the impact of unmeasured confounding on the estimate has rarely been assessed. We develop a novel sensitivity analysis for the estimate of the difference in restricted mean survival time with respect to unmeasured confounding. After formulating the sensitivity analysis problem as

Threshold dynamics of a reaction-advection-diffusion schistosomiasis epidemic model with seasonality and spatial heterogeneity.

Most water-borne disease models ignore the advection of water flows in order to simplify the mathematical analysis and numerical computation. However, advection can play an important role in determining the disease transmission dynamics. In this paper, we investigate the long-term dynamics of a periodic reaction-advection-diffusion schistosomiasis model and explore the joint impact of advection, seasonality and spatial heterogeneity on the transmission of the disease. We derive the basic reproduction number R 0 and show that the disease-free periodic solution is globally attractive when R 0

Review of methods for handling confounding by cluster and informative cluster size in clustered data.

Clustered data are common in medical research. Typically, one is interested in a regression model for the association between an outcome and covariates. Two complications that can arise when analysing clustered data are informative cluster size (ICS) and confounding by cluster (CBC). ICS and CBC mean that the outcome of a member given its covariates is associated with, respectively, the number of members in the cluster and the covariate values of other members in the cluster. Standard generalised linear mixed models for cluster-specific inference and standard generalised estimating equations f

Seeing the Small to Prevent the Big: The Role of Cerebral Microvessels in Ischemic Stroke Etiology and Prevention.

Despite decades of progress in its management, ischemic stroke remains a leading cause of death and disability worldwide, with incidence rising in younger populations. Although traditional stroke risk assessment focuses on macrovascular pathology, the cerebral microcirculation plays an important but less-recognized role. This review synthesizes evidence on microvascular involvement in the pathogenesis of ischemic stroke and evaluates existing and emerging methods for microvascular assessment of ischemic stroke risk (MvASR). Epidemiological studies show that cerebral small vessel disease as mea

PM10 Impairs CD56dim NK Cell Cytotoxicity via FNBP1 Suppression to Exacerbate Rheumatoid Arthritis: Insights from Multimodal Multi-Omics.

Air pollution (AP), intensified by industrialization and urbanization, is a key environmental factor linked to rheumatoid arthritis (RA). However, its molecular and immunological impact on RA remains unclear. This study integrates epidemiological data, bioinformatics, single-cell transcriptomics, and animal models to investigate how AP contributes to the development of RA. Global epidemiological analysis shows rising RA prevalence in over 95% of countries. Mendelian randomization analysis indicated a positive correlation between PM10 exposure and the risk of RA. Integrative modelling of reported case numbers and seroprevalence reveals time-dependent test efficiency and infectious contacts.

Mathematical models have been widely used during the ongoing SARS-CoV-2 pandemic for data interpretation, forecasting, and policy making. However, most models are based on officially reported case numbers, which depend on test availability and test strategies. The time dependence of these factors renders interpretation difficult and might even result in estimation biases. Here, we present a computational modelling framework that allows for the integration of reported case numbers with seroprevalence estimates obtained from representative population cohorts. To account for the time dependence o

Escherichia coli O104:H4 outbreak in Germany--clarification of the origin of the epidemic.

BACKGROUND: In 2011, Germany was hit by one of its largest outbreaks of acute gastroenteritis and haemolytic uraemic syndrome caused by a new emerging enterohaemorrhagic Escherichia coli O104:H4 strain. The German Haemolytic Uraemic Syndrome/Enterohaemorrhagic E. coli (GHUSEC) outbreak had unusual microbiological, infectiological and epidemiological features and its origin is still only partially solved. The aim of this article is to contribute to the clarification of the origin of the epidemic. METHODS: To retrospectively assess whether the GHUSEC outbreak was natural, accidental or a delibe

Dosimetric calculations for uranium miners for epidemiological studies.

Epidemiological studies on uranium miners are being carried out to quantify the risk of cancer based on organ dose calculations. Mathematical models have been applied to calculate the annual absorbed doses to regions of the lung, red bone marrow, liver, kidney and stomach for each individual miner arising from exposure to radon gas, radon progeny and long-lived radionuclides (LLR) present in the uranium ore dust and to external gamma radiation. The methodology and dosimetric models used to calculate these organ doses are described and the resulting doses for unit exposure to each source (radon

Structural Design and Data Requirements for Simulation Modelling in HIV/AIDS: A Narrative Review.

Born out of a necessity for fiscal sustainability, simulation modeling is playing an increasingly prominent role in setting priorities for combination implementation strategies for HIV treatment and prevention globally. The design of a model and the data inputted into it are central factors in ensuring credible inferences. We executed a narrative review of a set of dynamic HIV transmission models to comprehensively synthesize and compare the structural design and the quality of evidence used to support each model. We included 19 models representing both generalized and concentrated epidemics,
