Mirror exposure in women with bulimic symptoms: how do thoughts and emotions change in body image treatment?

Mirror exposure is an efficient treatment for body image problems in eating disorders. Although habituation processes and cognitive changes are postulated to be underlying mechanisms, evidence is scarce, especially during repeated mirror exposure treatment. Fourteen participants with eating disorders not otherwise specified (EDNOS) and five with bulimia nervosa (BN) composed the bulimic group (BG), and 19 healthy women without any mental disorder composed the healthy controls group (HC). The participants were treated by four standardized mirror exposure sessions. Subjective distress was assess

Written disclosure treatment for posttraumatic stress disorder in substance use disorder inpatients.

Comprehensive exposure-based approaches to treating posttraumatic stress disorder (PTSD) are effective, but they are time intensive and not widely used because of factors such as client noncompliance and fears of iatrogenic effects. Exposure by writing disclosure (WD), modeled after Pennebaker's brief stress-reduction procedure, may circumvent these obstacles. WD treatment reduces PTSD symptoms in trauma victims but has rarely been tested in diagnosed PTSD participants and never in substance use disorder (SUD) populations-despite high comorbidity rates. The authors applied a standard Pennebake

Imagery rescripting and exposure group treatment of posttraumatic nightmares in Veterans with PTSD.

This study details results of an open trial of a group psychological treatment for Veterans with posttraumatic stress disorder (PTSD) and chronic posttraumatic nightmares called "Imagery Rescripting and Exposure Therapy" (IRET). IRET is a variant of a successful imagery rescripting treatment for civilian trauma-related nightmares that was modified to address the needs of the Veteran population. Thirty-seven male U.S. Veterans with PTSD and nightmares attended 6 multicomponent group sessions. Findings indicated that the intervention significantly reduced frequency of nightmares and PTSD severit

Pavlovian fear reversal learning predicts outcome of exposure-based cognitive behavioral therapy for adult obsessive-compulsive disorder.

Extinction of conditioned fear responses is assumed to relate to beneficial effects of exposure-based treatment, but few studies investigated the role of fear conditioning for exposure techniques in clinical settings. Specifically, no study addressed the relationship between psychophysiological indicators of extinction and outcome of exposure and response prevention (ERP) in adult obsessive-compulsive disorder (OCD). This study investigated whether skin conductance responses (SCRs) to threat and safety signals in a pre-treatment Pavlovian reversal learning experiment predicted the outcome of i

The predictive value of neural reward processing on exposure therapy outcome: Results from a randomized controlled trial.

BACKGROUND: Exposure is the gold standard treatment for phobic anxiety and is thought to represent the clinical application of extinction learning. Reward sensitivity might however also represent a predictive factor for exposure therapy outcome, as this therapy promotes positive experiences and involves positive comments by the therapist. We hypothesized that high reward sensitivity, as expressed by elevated reward expectancy and reward value, can be associated with better outcome to exposure therapy specifically. METHODS: Forty-four participants with a specific phobia for spiders were includ

Separate and combined effects of genetic variants and pre-treatment whole blood gene expression on response to exposure-based cognitive behavioural therapy for anxiety disorders.

OBJECTIVES: Exposure-based cognitive behavioural therapy (eCBT) is an effective treatment for anxiety disorders. Response varies between individuals. Gene expression integrates genetic and environmental influences. We analysed the effect of gene expression and genetic markers separately and together on treatment response. METHODS: Adult participants (n ≤ 181) diagnosed with panic disorder or a specific phobia underwent eCBT as part of standard care. Percentage decrease in the Clinical Global Impression severity rating was assessed across treatment, and between baseline and a 6-month follow-up

Mediators and moderators of outcome in the behavioral treatment of childhood social phobia.

OBJECTIVE: The current study examined mediators and moderators of treatment response among children and adolescents (ages 7-17 years) with a primary diagnosis of social phobia. METHOD: Participants were 88 youths participating in one of two randomized controlled treatment trials of Social Effectiveness Therapy for Children. Potential mediators included changes in observer-rated social skill and child-reported loneliness after 12 weeks of Social Effectiveness Therapy for Children. Age and depressive symptoms were examined as potential moderators. RESULTS: Loneliness scores and social effectiv

Effects of internal and external distraction and focus during exposure to blood-injury-injection stimuli.

The present study examined the effects of attentional focus on fear reduction during exposure. Participants were randomly assigned to experimental conditions: exposure plus internal focus, exposure plus external focus, exposure plus internal distraction, exposure plus external distraction or exposure alone. Fifty blood-injury-injection fearful participants received 3 weekly exposure sessions. Participants in the distraction group reported the greatest fear reduction, with most notable reductions occurring for the external distraction condition. The distraction group also achieved a greater num

Mind the gap: a nested randomised pilot study of culturally inclusive, internet-delivered prolonged exposure for PTSD among immigrants.

Background: PTSD is disproportionately prevalent among immigrants in Sweden, yet access to evidence-based treatments remains limited, especially for those facing language barriers. Internet-delivered prolonged exposure (I-PE) may help overcome these challenges, but its feasibility and effectiveness in diverse populations remain underexplored. Objective: This study evaluated the feasibility, acceptability, and preliminary effects of a culturally inclusive, therapist-guided I-PE programme in easy-to-read English for immigrants with PTSD.Method: A nested pilot study within an ongoing RCT randomise

Preliminary efficacy of prolonged exposure therapy to reduce suicide risk in co-occurring bipolar disorder & trauma symptoms.

Bipolar disorder (BD) and trauma symptoms commonly co-occur and present a high rate of suicidality. Prolonged exposure (PE) is a well-established and efficacious treatment for symptoms of PTSD, untested in patients with co-occurring BD but promising in its effectiveness for reducing suicidal thoughts and behaviors. The current study evaluates the preliminary efficacy of PE in reducing suicide risk in patients with co-occurring BD and trauma symptoms. Participants (N = 32) were enrolled in a 10-session, weekly PE program and completed assessments of trauma symptoms, suicidality, state and trait

Written Exposure Therapy for Posttraumatic Stress Disorder and Integration of a Mindfulness-Based App in China: A Pilot Randomized Controlled Trial.

Despite posttraumatic stress disorder's (PTSD) significant burden in China, accessible and effective treatments remain limited. This pilot randomized controlled trial examined the feasibility, acceptability, and preliminary efficacy of Written Exposure Therapy (WET) for PTSD in China and explored whether adding a mindfulness-based app (MBA) improved insomnia outcomes. Forty-six adults meeting Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5) criteria for PTSD or subthreshold PTSD with associated insomnia were randomly assigned to WET (n = 15), WET + MBA (n = 15), or

Too little, too much, or just right? Does the amount of distraction make a difference during contamination-related exposure?

BACKGROUND AND OBJECTIVES: The extant literature has shown mixed results regarding the impact of distraction use on exposure outcome; however, a wide variety of distraction tasks have been utilized across studies. In order to better understand these discrepant findings, we aimed to evaluate the impact of differing levels of distraction on exposure outcome. Additionally, treatment acceptability and changes in self-efficacy were assessed to evaluate how these may differ as a function of distraction use. METHODS: In Experiment 1 (N = 176 participants tested), distraction tasks were experimentall

Exposure therapy triggers lasting reorganization of neural fear processing.

A single session of exposure therapy can eliminate recalcitrant and disabling fear of phobogenic objects or situations. We studied neural mechanisms of this remarkable outcome by monitoring changes in brain activity as a result of successful 2-h treatment. Before treatment, phobogenic images excited activity in a network of regions, including amygdala, insula, and cingulate cortex, relative to neutral images. Successful therapy dampened responsiveness in this fear-sensitive network while concomitantly heightening prefrontal involvement. Six months later, dampened fear-network activity persiste

Emotional variability and sustained arousal during exposure.

BACKGROUND AND OBJECTIVES: In traditional exposure therapy for phobias and anxiety disorders, reduction of fear responding is used as an index of learning. However, recent evidence in animal models suggests that sustained arousal and enhanced fear responding throughout exposure may actually predict better long-term outcomes (Rescorla, 2000). METHODS: The effects of sustained arousal during exposure were investigated in a clinical analog sample of 59 participants fearful of public speaking. Participants completed exposure with or without the presence of additional excitatory stimuli which were

Cost-effectiveness of evidence-based psychotherapies for PTSD: An examination of different treatment delivery modalities.

The goal of this study was to examine the cost-effectiveness of evidence-based psychotherapies for posttraumatic stress disorder (PTSD), including prolonged exposure and cognitive processing therapy, by comparing four delivery modalities: home-based telehealth (patient in home), office-based telehealth (patient in one office, provider in another), in-home in-person, and in-office in-person. Pooled data from 268 veterans enrolled in two harmonized PTSD clinical trials were used to examine costs related to personnel, travel mileage, and office space. Cost-effectiveness was calculated using an in

Long-Term Follow-up of Cognitive-Behavioral Therapy for Obsessive-Compulsive Disorder: Symptom Severity and the Role of Exposure 8-10 Years After Inpatient Treatment.

Obsessive-compulsive disorder (OCD) can be effectively treated by cognitive behavioral therapy (CBT) with exposure and response prevention (ERP). Yet, little is known about the long-term effects of inpatient CBT up to one decade after treatment. Thirty patients who had been treated with 12 weeks of intensive inpatient CBT with ERP were examined 8-10 years after their stay in hospital with regard to obsessive-compulsive symptoms, secondary outcomes, and use of healthcare services. Significant (p < .001) improvements in OC symptoms with medium and large effects compared to baseline on the Yale-B

A randomized controlled study of power posing before public speaking exposure for social anxiety disorder: No evidence for augmentative effects.

This manuscript details a randomized controlled study designed to test the efficacy of power posing (i.e., briefly holding postures associated with dominance and power) as an augmentative strategy for exposure therapy for social anxiety disorder (SAD). Seventy-three individuals diagnosed with SAD were assigned to one of three conditions: power posing, submissive posing, or rest (no posing) prior to participating in an exposure therapy session. Participants were assessed for between-group differences in pre- and post-manipulation salivary hormone levels, within-session subjective experiences of
