Eosinophilic effusions: A clinicocytologic study of 12 cases.

OBJECTIVES: Our aim is to study the prevalence of eosinophilic effusions (EEs) in our institution, to highlight their cytologic patterns, potential interpretation pitfalls, diagnostic importance, and to investigate their possible causes and clinical associations. MATERIALS AND METHODS: We conducted a retrospective review study for over 13 years. We retrieved all of the cytology reports of pleural, pericardial, and peritoneal effusions with eosinophils. We reviewed and screened the cytology slides looking for eosinophils that constitute >10% of the cells. We extracted the clinical, radiologic,

Role of endoscopic third ventriculostomy in tuberculous meningitis with hydrocephalus.

Hydrocephalus is one of the commonest complications of tuberculous meningitis (TBM). It can be purely obstructive, purely communicating, or due to combinations of obstruction in addition to defective absorption of cerebrospinal fluid (CSF). Endoscopic third ventriculostomy (ETV) as an alternative to shunt procedures is an established treatment for obstructive hydrocephalus in TBM. ETV in TBM hydrocephalus can be technically very difficult, especially in acute stage of disease due to inflamed, thick, and opaque third ventricle floor. Water jet dissection can be helpful in thick and opaque ventr

Peritoneal and nodal abdominal tuberculosis presenting as intestinal obstruction mimicking malignancy: a case report.

BACKGROUND: Abdominal tuberculosis (TB) is an uncommon form of extra pulmonary TB that often presents with nonspecific symptoms and poses significant diagnostic challenges. Its clinical and radiologic features can closely mimic malignancy and other abdominal pathologies, frequently leading to delayed diagnosis and management, especially in resource-limited settings. CASE PRESENTATION: We report a 20-year-old Eritrean male presenting with a 1-month history of abdominal pain, fever, weight loss, and later respiratory symptoms. Initial evaluation suggested pneumonia; however, persistent symptoms

Genital and peritoneal tuberculosis: potential role of laparoscopy in diagnosis and management.

Genital and peritoneal tuberculosis are rare in developed countries and can mimic ovarian cancer. We report two different cases that came to our attention, both in a month. The first patient was referred to us for an asymptomatic pelvic mass. The second patient came to us because of weight loss, abdominal swelling, and ascites. Unfortunately, the imaging findings were misleading, and the laboratory analyses were not helpful. Only the laparoscopic approach was useful for final diagnosis. We conclude that the symptoms of abdominal tuberculosis vary greatly, and laparoscopy can be essential for i

[Role of computed tomography in extrapulmonary tuberculosis].

A retrospective evaluation of 65 computed tomographic investigations on 27 patients with proven disseminated extrapulmonary tuberculosis showed no CT changes characteristic for tuberculosis, except for a higher density of tubercular abscesses. In the majority of cases the extent of tubercular manifestations could only be exactly established after the CT investigation. With the aid of this information together with the clinical course of the disease, the timing, type and extent of operative measures could be determined. Finally, in comparison with conventional X-ray and sonography, computed tom

A case report of racemose pattern of intracranial tuberculoma with brain stem and hypophyseal involvement developing paradoxically during treatment.

Tuberculosis involving the central nervous system, a source of considerable morbidity and mortality, forms 5-10% of the disease burden associated with tuberculosis. Central nervous system tuberculosis may present as meningitis, tuberculoma, abscesses, cerebritis or miliary tuberculosis. The most common site of tuberculoma has been reported to be at the grey-white matter junction and the periventricular region. They may even be found in the epidural, subdural and subarachnoid spaces, and the brain stem, with the rarer sites of involvement being the cavernous sinus, sella turcica, hypophysis, hy

CT findings of abdominal tuberculosis in 12 patients.

Our purpose was to evaluate the Computed Tomography (CT) findings of the abdominal tuberculosis (TBC) retrospectively which was diagnosed histopatologically. This study included 12 patients. All patients were evaluated by abdominal CT study. Most findings of CT studies were mesenteric calcified or noncalcified lymphadenopathies, ascites, thickened intestinal wall located on the right lower quadrant of abdomen, thickening of peritoneum, mottled soft-tissue densities in omentum and mesenterium. In addition, one of the patients had bilateral calcified adrenal glands and one of them had calcified

Tuberculosis of the small bowel with perforation and hematogenous spread in a renal transplant recipient.

A 41-year-old male was admitted because of acute abdomen. A flat plate of the abdomen suggested pneumoperitoneum and a chest X-ray an infiltrate in the right upper lobe. The patient was a renal allograft recipient and was on immunosuppressive therapy with azathioprine, cyclosporine and steroids. At laparatomy inflammatory thickening of the bowel wall was found in the terminal ileum with necrotic areas and two sites of perforation. The involved terminal ileum was removed together with a right hemicolectomy. The resected segment showed exudative ileal tuberculosis and fibrinous and purulent peri

A deceiving disease in women for clinicians: peritoneal tuberculosis.

INTRODUCTION: Peritoneal tuberculosis (TB) is uncommon in developed countries, although there is an increase in incidence due to the patients with acquired immunodeficiency syndrome and in immigrants from countries with tuberculosis. The aim of the study was to identify characteristic features of peritoneal tuberculosis (TB), which may be useful for the clinical differential diagnosis and management of this deceiving disease. MATERIALS AND METHODS: For this retrospective study, 18 patients, who were diagnosed with peritoneal TB were identified after surgery. RESULTS: Initial presentation con

Pelvic-peritoneal tuberculosis with elevated serum and peritoneal fluid Ca-125 levels. A report of two cases.

We report 2 patients with pelvic-peritoneal tuberculosis and elevated serum and peritoneal fluid levels of Ca-125. The first was a young and infertile women who had cul-de-sac nodularity and dysmenorrhea. The other was postmenopausal and presented with weight loss and ascites. While a preoperative diagnosis of endometriosis was made in the former, intraperitoneal malignancy was considered in the latter. The diagnosis of pelvic-peritoneal tuberculosis was reached by laparoscopic-directed biopsy in both patients. Serum levels of Ca-125 returned to normal limits following antituberculous drug tre

Does CSF composition predict shunt malfunction in tuberculous meningitis?

BACKGROUND: Hydrocephalus secondary to tuberculous meningitis (TBM) continues to be a challenging condition to treat for neurosurgeons in developing countries. Shunt complications are reportedly more frequent in patients undergoing ventriculo-peritoneal shunt in patients with TBM than in those undergoing shunt surgeries for other causes. AIM: The aim of this study was to evaluate the relationship of cerebrospinal fluid (CSF) composition on shunt malfunction. METHODS: We compared the CSF composition of 53 patients who had shunt malfunction during a five year period with that of 137 matched co

CT-guided core-needle biopsy in omental pathology.

PURPOSE: To assess the accuracy and clinical usefulness of CT-guided core-needle biopsy in the diagnosis of omental pathology. MATERIAL AND METHODS: We retrospectively reviewed the results of CT-guided percutaneous core biopsies in 25 patients with focal (n = 2) or diffuse (n = 23) omental pathology. These results were compared to the final diagnoses as determined by laparotomy (n = 15), laparoscopic biopsy (n = 3), endoscopic biopsy (n = 1), or by the results of percutaneous biopsy and clinical-radiological and bacteriological modalities (n = 6). The final diagnoses showed 4 patients with is

Disseminated tuberculosis after pregnancy progressed to paradoxical response to the treatment: report of two cases.

BACKGROUND: Early postpartum women are more likely to develop tuberculosis than nonpregnant women mainly due to immune reconstitution after delivery. Paradoxical response (PR) during antituberculosis treatment also arises via recovery from immunosuppression. However, no study focused on PR during antituberculosis treatment in a postpartum patient has been reported. CASE PRESENTATION: We present two sequential cases (Patient 1: 26-year-old; Patient 2: 29-year-old) of postpartum tuberculosis with pulmonary and extrapulmonary lesions (Patient 1: peritonitis; Patient 2: psoas abscess secondary to

[Role of laparoscopy in the diagnosis of peritoneal tuberculosis in endemic areas].

The aim of our study is to demonstrate the difficulties of diagnosis of peritoneal tuberculosis in endemic areas and the value of laparoscopy for diagnostic confirmation. We report a retrospective study of 60 cases of peritoneal tuberculosis, collected from January 2004 to December 2009. The patients' median age was 41 years, and the sex ratio 0.4. The dominant reasons for consultation were pain and abdominal distension. Abdominal ultrasound found ascites in all patients. Computed tomography (CT) was performed in 30 patients and confirmed the ultrasound data. Paracentesis of the ascites took p
