Severe Rh(D) immunization: anti-D quantitation and treatment possibilities during pregnancy and after birth.

An extremely aggressive Rh(D), (C) and Kell alloimmunization during pregnancy is reported. Exceptionally high concentrations of anti-D were observed in the mother, in the fetus and in the amniotic fluid, indicating an active transport across the placenta and a passive excretion into the amniotic fluid. Treatment during pregnancy included maternal plasmapheresis and high-dose intravenous immunoglobulin. Intravascular transfusions were given to the fetus. Postpartum the newborn was given immunoglobulin, one exchange transfusion and four top-up transfusions. In the newborn the elimination rate of

Intrauterine intravascular transfusion for fetal haemolytic anaemia: the Western Australian experience.

OBJECTIVE: To report the first four years' clinical experience with fetal intravascular blood transfusion for the treatment of fetal haemolytic anaemia in Western Australia. SETTING: King Edward Memorial Hospital, Perth, which is the sole tertiary level perinatal centre in Western Australia with a referral base of approximately 25,000 pregnancies each year. METHODS: Transfusion was by injection of packed cells from Rh-negative donors into the fetal umbilical vein near the site of insertion into the placenta. Fetal haemoglobin levels were measured before and after each transfusion. In most ca

[The surface characteristics of the erythrocytes in hemolytic disease of the newborn].

Assessment of structural state of the erythrocyte (Er) surface was done in accordance with the capability of overmembranous glycoproteins and glycolipids of sorbing the vital stain Alcian blue (AB). The extent of AB sorbing by the Er surface of healthy infants is nearly 5.51.10(-13) g per cell, whereas in children with hemolytic disease of newborns (HDN) under average grave condition, and in children with HDN and concomitant diseases this index is seen to reduce significantly down to 82.4 and 70.8 per cent, respectively, which may be due presumably to the masking of glycocalyx with Er antibodi

Cordocentesis for rapid karyotyping.

Pure fetal blood was obtained by cordocentesis in 101 fetuses of 96 patients at 15 to 38 weeks' gestation. Rapid karyotype was obtained within 2 to 4 days by fetal lymphocyte culture. Chromosomal abnormality was detected in 12 (11.9%) fetuses. Abnormal karyotype was found in 5 of 44 fetuses with structural malformations, 3 of 13 fetuses with intrauterine growth retardation or oligohydramnios, 1 of 3 fetuses with nonimmune hydrops fetalis, 2 (one monozygotic set) of 10 discordant twins, 1 of 12 isoimmunized gestations, none of 8 cases with advanced gestational-maternal age, and none of 6 immune

[Ante-partum administration of preventive treatment of Rh-D immunization in rhesus-negative women. Parallel evaluation of transplacental passage of fetal blood cells. Results of a multicenter study carried out in the Paris region].

1,969 non immunized rhesus negative primiparous women were followed up in 23 maternity units in the geographical region of Paris. 1,882 could be retained to study antepartum protection and 1,884 to study transplacental passage of fetal blood cells. Two groups were defined according to whether they were born in even or uneven years, so that: 955 were the "control" group who delivered 590 rhesus D positive infants, and 927 were the "treated" group who delivered 599 rhesus D positive infants. The "control" group were used as controls at the 28th and 34th weeks of pregnancy, while the "treated" gr

Detection of fetomaternal hemorrhage.

The prevention of Rhesus D alloimmunization through Rh immune globulin (RhIg) administration is the major indication for the accurate detection and quantification of fetomaternal hemorrhage (FMH). In the setting of D incompatibility, D-positive fetal cells can sensitize the D-negative mother, resulting in maternal anti-D alloantibody production. These anti-D alloantibodies may lead to undesirable sequelae such as hemolytic disease of the newborn (HDN). Since the widespread adoption of FMH screening and RhIg immunoprophylaxis, the overall risk of Rh alloimmunization and infant mortality from HD

Three cases of massive fetomaternal hemorrhage presenting without clinical suspicion.

Fetomaternal hemorrhage (FMH) is a common obstetrical occurrence most often associated with small volumes of blood transferred across the placenta. Fetomaternal hemorrhage leads to alloimmunization of Rh D-negative mothers, resulting in an increased risk of hemolytic disease of the newborn. Massive FMH involving volumes of blood greater than 30 mL can cause substantial fetal morbidity and mortality. Massive FMH may present with signs and symptoms such as decreased movement, sinusoidal heart rhythms, or fetal anomalies. We present 3 cases of clinically unexpected massive FMH of 206, 88, and 155

Rh genotyping: avoiding false-negative and false-positive results among individuals of African ancestry.

High homology, variant alleles, and silent alleles have made the development of completely reliable genotyping assays for the RHD and RHC alleles difficult. An RHD pseudogene (RHDPsi) possessing a 37-bp insertion within exon 4 is common among serologically RhD-negative individuals of African descent and generates false-positive results in previously reported RhD genotyping assays. Genotyping RhC is problematic due to exon 2 homology between RHD and RHC; however, an RHC-specific 109-bp insertion within intron 2 has been reported useful for genotyping. Primers flanking the exon 4 insertion point

Management of rhesus alloimmunization in pregnancy.

Hemolytic disease of the newborn secondary to rhesus alloimmunization was once a major contributor to perinatal morbidity and mortality. Today, rhesus immune globulin has markedly decreased the prevalence of this disease so that only one to six cases occur in every 1000 live births. The rarity of this condition warrants consideration of consultation or referral to a maternal-fetal medicine specialist. Once sensitization occurs, rhesus immune globulin is no longer effective. Evaluation for the presence of maternal anti-D antibody should be undertaken at the first prenatal visit. First-time sens

The rate of decline in fetal hemoglobin following intrauterine blood transfusion in the management of red cell alloimmunization.

OBJECTIVE: Hemolytic disease of the fetus and newborn is characterized by fetal anemia, secondary to maternal alloantibody-mediated fetal erythrocyte destruction. Despite our reliance on intrauterine blood transfusion (IUT) to maintain severely affected pregnancies, it remains difficult to predict the fetal response to an infusion of donor blood. Our objective was to determine the daily rate of decline in fetal hemoglobin following one, two, and three transfusions. We also evaluated the relationship between the fetal hemoglobin level and the corresponding doppler measurement of the fetal middl

Prediction of the rate of decline in fetal hemoglobin levels between first and second transfusions in red cell alloimmune disease.

OBJECTIVE: To determine variables that predict the rate of decline in fetal hemoglobin levels in alloimmune disease. METHOD: Retrospective review of singleton pregnancies that underwent first and second intrauterine transfusions for treatment of fetal anemia because of maternal Rh alloimmunization in a tertiary referral center. RESULTS: Forty-one first intrauterine transfusions were performed at 26.1 weeks (standard deviation, SD, 4.6), mean volume of blood transfused was 44.4 mL (SD 23.5) and estimated feto-placental volume expansion was 51.3% (SD 14.5%). Between first and second transfusio

Cholestasis in neonates with red cell alloimmune hemolytic disease: incidence, risk factors and outcome.

BACKGROUND: Etiology of cholestatic liver disease in neonates with hemolytic disease of the newborn (HDN) has been associated with iron overload due to intrauterine red cell transfusions (IUTs). Data on the incidence and severity of cholestasis in neonates with HDN are scarce, and little is known about pathogenesis, risk factors, neonatal management and outcome. OBJECTIVE: To evaluate incidence, risk factors, management and outcome of cholestasis in neonates with red cell alloimmune hemolytic disease. METHODS: All (near-) term neonates with HDN due to red cell alloimmunization admitted to ou

[Hemolytic disease of the newborn has not vanished from Finland--routine protection of RhD negative mothers during pregnancy is justifiable].

Prophylaxis of RhD negative mothers with anti-D immunoglobulin after childbirth is the most important procedure reducing the immunization of the mother and the risk of severe hemolytic disease of the newborn. In spite of this, anti-D antibodies having relevance to pregnancy are later detected in 1.8% of RhD negative mothers. Half of these cases could be prevented by routine anti-D prophylaxis given to the mothers during weeks 28 to 34 of pregnancy. Convincing evidence of the effectiveness of this measure has accumulated in the last few years, and application of the treatment is justified also

The emerging pattern of hydrops fetalis--incidence, aetiology and management.

OBJECTIVES: To analyse the incidence, aetiology and management of live born cases of hydrops fetalis in a Regional Perinatal Centre. METHODS: We reviewed 35 cases of hydrops delivered over a six year period. RESULTS: Non-immune hydrops accounted for 80% of the cases and the majority of babies required Level 1 intensive care. The mortality rate was 40%. CONCLUSION: The pattern of hydrops is changing. Most of these babies now have non-immune hydrops and approximately two thirds are surviving.

ACOG practice bulletin. Prevention of Rh D alloimmunization. Number 4, May 1999 (replaces educational bulletin Number 147, October 1990). Clinical management guidelines for obstetrician-gynecologists. American College of Obstetrics and Gynecology.

The reduction in the incidence of Rh D alloimmunization is a prototype for the effectiveness of preventive medicine. Some controversies remain, however, such as the use of anti-D immune globulin in patients with either threatened abortion or antenatal hemorrhage. Similarly, it may not be cost-effective either to screen all Rh D-negative patients with an indirect Coombs test at 24-28 weeks of gestation or to screen all postpartum patients for excessive fetomaternal hemorrhage.
